The effect of increased processivity on overall fidelity of human immunodeficiency virus type 1 reverse transcriptase.

We previously reported that two insertions of 15 amino acids in the beta3-beta4 hairpin loop of fingers subdomain of HIV-1(NL4-3) RT confer an increased polymerase processivity. The processivity of human immunodeficiency virus (HIV) reverse transcriptase (RT) is thought to influence the fidelity of HIV-1 RT, which tends to create errors at template sites with high termination probability. Employing the two insertion variants of HIV-1 RT (FE20 and FE103), we examined the relationship between processivity, overall fidelity and error specificity. Although the overall mutation rate was unaffected by increased processivity, one of the mutants, FE103, generated significantly fewer frameshift errors. The other mutant, FE20, generated errors at hotspots not previously observed for HIV-1 RT. Our results indicate that an increase in the polymerase processivity of HIV-1 RT does not necessarily result in a decreased mutation rate and confirm that changes in processivity alter the sequence context in which the errors are made. Furthermore, our results also reveal that the mutation frequency obtained via in vitro gap-filling reactions with wild-type HIV-1(NL4-3) RT is only 2-fold higher than that obtained via a single cycle infection assay using the same, wild-type HIV-1(NL4-3) RT sequence as part of the helper pol function [Mansky and Temin: J Virol 69:5087-5094;1995].

Elevated overall rates of transmethylation in cell lines from diverse human tumors.

In a study of a diverse set of human tumor cell lines previously shown to all have a defect in methionine metabolism (Stern, P. H., Wallace, C.D. and Hoffman, R.M., J. Cellular Physiology 119, 29-34, 1984), we demonstrate in this report that all have enhanced overall rates of transmethylation compared to normal human fibroblasts. Transmethylation rates were measured by blocking S-adenosylhomocysteine hydrolase and measuring the AdoHcy which accumulates as a result of transmethylation. The enhanced transmethylation rates may be the basis of the above-mentioned defects in methionine metabolism previously reported in human tumor cells, including the basis of the inability of the majority of the tumor cells to grow when methionine is replaced by homocysteine. The excess and unbalanced tRNA methylation observed for the last 25 years in many types of cancer may be at least in part explained by our results of elevated rates of overall transmethylation in cancer cells. The alteration of such a fundamental process as transmethylation in cancer may be indicative of its importance in the oncogenic process.

A risk adapted approach reduces the overall institutional incidence of postoperative nausea and vomiting.

PURPOSE: Routine prophylactic antiemetic treatment of surgical patients appears justified only in case of an increased risk of postoperative nausea and vomiting (PONV). The objective of this investigation was to assess the feasibility and efficacy of a dichotomized risk score adapted management of PONV based on ondansetron prophylaxis and treatment with respect to the overall institutional rate of PONV. METHODS: After estimating the individual PONV risk by a simplified score, 162 adult patients scheduled for elective surgery received either 4 mg ondansetron intravenously (two to four risk factors = high-risk) or no prophylaxis (zero to one risk factor = low-risk). For antiemetic treatment ondansetron was given intravenously and orally. Incidence of PONV was recorded during the first 24 hr after recovery. RESULTS: Data from 159 subjects were analyzed with 44 patients classified as low-risk and 115 patients classified as high-risk. Nine low-risk and 58 high-risk patients experienced PONV. The expected institutional PONV incidence of 47% was reduced to 36%. Treatment with ondansetron was necessary in seven low-risk and 37 high-risk patients with a complete response rate of 71% (low-risk) and 43% (high-risk). CONCLUSION: Providing antiemetic prophylaxis with ondansetron to high-risk patients strictly based on a simplified risk score can reduce the overall institutional rate of PONV. However, classifying patients into two groups while using ondansetron as the single antiemetic in the high-risk group appears to be of limited efficacy as the incidence of PONV in high-risk patients is still double that of low-risk patients.

A DANREF certified reference plastic for measurement of overall migration into the food simulant olive oil by single sided testing.

A reference material for the determination of overall migration from a plastic coextrudate into the fatty food simulant olive oil was produced and certified in an interlaboratory study. The analyses were carried out according to the ENV 1186 standard from the European Committee for Standardization (CEN) [ 1, 2, 3] with exposure of the coextrudate to olive oil for 10 days at 40 degrees C. After an initial preliminary interlaboratory study eight laboratories participated in the certification round, and two different methods were used to obtain single sided exposure of the plastic to the oil. The certified value was determined as the mean of laboratory mean values. No outliers were found. A reference value of 8.6 mg/dm2 +/- 1.4 mg/dm2 (+/- half width of the 95% confidence interval) was obtained which is within the range relevant for the regulatory limit (10 mg/ dm2), making this reference material suitable for laboratories measuring according to the EU overall migration limit [4]. The material has been found stable over 45 months.

Effect of human activities on overall trend of sedimentation in the lower Yellow River, China.

The Yellow River has been intensively affected by human activities, particularly in the past 50 years, including soil-water conservation in the upper and middle drainage basin, flood protection in the lower reaches, and flow regulation and water diversion in the whole drainage basin. All these changes may impact sedimentation process of the lower Yellow River in different ways. Assessing these impacts comprehensively is important for more effective environmental management of the drainage basin. Based on the data of annual river flow, sediment load, and channel sedimentation in the lower Yellow River between 1950 and 1997, the purpose of this paper is to analyze the overall trend of channel sedimentation rate at a time scale of 50 years, and its formative cause. It was found in this study that erosion control measures and water diversion have counteractive impacts on sedimentation rate in the lower Yellow River. Although both annual river flow and sediment decreased, there was no change in channel sedimentation rate. A regression analysis indicated that the sedimentation in the lower Yellow River decreased with the sediment input to the lower Yellow River but increased with the river flow input. In the past 30-40 years, the basin-wide practice of erosion and sediment control measures resulted in a decline in sediment supply to the Yellow River; at the same time, the human development of water resources that required river flow regulation and water diversion caused great reduction in river flow. The former may reduce the sedimentation in the lower Yellow River, but the reduction of river flow increased the sedimentation. When their effects counterbalanced each other, the overall trend of channel sedimentation in the lower Yellow River remained unchanged. This fact may help us to better understand the positive and negative effects of human activities in the Yellow River basin and to pay more attention to the negative effect of the development of water resources. The results of this study demonstrate that, if the overuse of river water cannot be controlled, the reduction of channel sedimentation in the lower Yellow River cannot be realized through the practice of erosion and sediment control measures.

Tei-Index in coronary artery disease--validation in patients with overall cardiac and isolated diastolic dysfunction.

BACKGROUND: The index "isovolumic contraction time and isovolumic relaxation time divided by ejection time" ("Tei-Index") has been demonstrated to provide useful information about disease severity and prognosis in patients with dilated cardiomyopathy and cardiac amyloidosis. In patients with coronary artery disease (CAD), the diagnostic utility of this index is unclear. We attempted to validate the Tei-Index in CAD patients with overall cardiac or isolated diastolic dysfunction. METHODS AND RESULTS: Sixty subjects were included who underwent left heart catheterization for invasive measurement of left ventricular end-diastolic pressure (LVEDP): 20 symptomatic CAD patients with overall cardiac dysfunction (defined by a LV ejection fraction (EF) < 45% (mean 27 +/- 8%) and a LVEDP > or = 16 mmHg, (mean 22 +/- 6 mmHg), NYHA class 2.7 +/- 0.4, OCD group), 29 symptomatic CAD patients with isolated diastolic dysfunction (defined by an EF > 45% (mean 55 +/- 8%), a normal end-diastolic diameter index (mean 2.8 +/- 0.4 cm/m2) and a LVEDP > or = 16 mmHg (mean 22 +/- 6 mmHg), NYHA class 2.3 +/- 0.4, IDD group) and 11 asymptomatic control subjects (EF 65 +/- 9%, LVEDP 11 +/- 4 mmHg, CON group). After conventional 2-D- and Doppler echocardiographic examination, the Tei-Index was obtained. The Tei-Index was easily and reproducibly measured in all study subjects. In the OCD group, isovolumic contraction time was prolonged and ejection time was shortened in comparison to the CON group, resulting in a significantly increased Tei-Index (0.71 +/- 0.28 vs 0.40 +/- 0.11, p < 0.01). In the IDD group, isovolumic relaxation time was prolonged and isovolumic contraction time was shortened in comparison to controls, resulting in a largely unchanged Tei-Index (0.45 +/- 0.14, p = ns). Receiver operating characteristic curve analysis for the Tei-Index yielded an area under the curve of 0.92 +/- 0.04 for separating patients with vs without OCD. Using a Tei-Index > 0.49 as a cut-off, OCD patients were identified with a sensitivity of 96% and a specificity of 86%. CONCLUSION: The Tei-Index is a valid and readily derived indicator of global cardiac dysfunction in CAD patients with impaired systolic and diastolic LV performance. The use of this index seems to be limited in CAD patients with primary diastolic dysfunction.

Postoperative interferon alpha treatment postponed recurrence and improved overall survival in patients after curative resection of HBV-related hepatocellular carcinoma: a randomized clinical trial.

BACKGROUND/AIMS: Recurrence after resection of hepatocellular carcinoma (HCC) is a frequent event. This study evaluated the effect of postoperative interferon alpha (IFN alpha) treatment on recurrence and survival in patients with hepatitis B virus (HBV)-related HCC. METHOD: Two hundred and thirty six patients were randomized after resection into IFN alpha treatment (5 micro i.m. tiw for 18 months) and control groups. Treatment was terminated if recurrence was diagnosed, and recurrence was managed the same way in both groups. Statistical analysis was based on the method of intent-to-treat. RESULTS: The two groups were comparable in all clinicopathological parameters. The median overall survival was 63.8 months in the treatment group and 38.8 months in the control group (P=0.0003); the median disease-free survival period was 31.2 versus 17.7 months (P=0.142). Fever, leucocytopenia, and thrombocytopenia were adverse effects in the treatment group, but were mostly manageable. CONCLUSIONS: IFN alpha treatment improved the overall survival of patients with HBV-related HCC after curative resection, probably by postponing recurrence.

Predictors for patterns of brain relapse and overall survival in patients with non-small cell lung cancer.

Our goal was to investigate prognostic factors for different patterns of brain relapse and overall survival so that treatments could be tailored and treatment outcomes improved. We studied 292 patients with non-small cell lung cancer (NSCLC) who had symptomatic, solitary, or multiple brain metastases (isolated or not isolated from extracranial metastases) that had developed early ( 6 months) from initial diagnosis. Factors affecting patterns of relapse and survival were analyzed by univariate and multivariate analyses. Good ECOG performance status (PS) at the time of NSCLC diagnosis was the most important factor that predicted late (rather than early) relapse and improved survival, and was the only factor that predicted isolated brain metastases. Patients whose lungs showed a complete response (CR) to treatment had a higher rate of late brain relapses than non-responders (NR) did (67.3% vs. 7.8%, P<0.001). CR patients also experienced a longer median overall survival than NR patients. Patients with late brain relapses showed better median survival times (18 months vs. 4 months, P<0.0001) than patients with early relapses, and this was an independent factor by Cox regression analysis. Our findings provide a justification for enrolling patients with good PS and controlled lung lesions into clinical trials for the prevention of early, non-isolated brain relapse. More aggressive therapeutic approaches should be applied to patients with late, isolated and solitary relapses to improve both quality and quantity of life.

Vitamin K (menaquinone) biosynthesis in bacteria: high-performance liquid chromatographic assay of the overall synthesis of o-succinylbenzoic acid and of 2-succinyl-6-hydroxy-2,4-cyclohexadiene-1-carboxylic acid synthase.

An early enzyme in menaquinone (vitamin K2) biosynthesis is the synthase forming 2-succinyl-6-hydroxy-2,4-cyclohexadiene-1-carboxylic acid (SHCHC) from isochorismic acid. In turn, SHCHC is aromatized to o-succinylbenzoic acid (OSB) by OSB synthase. An assay for the combined activity of these two enzymes ("overall OSB synthesis") has been developed using a high-performance liquid chromatographic method for the quantitation of OSB. The assay, which measures as little as 0.1 nmol of OSB, is vastly superior to the radiogas chromatographic method previously used to estimate overall OSB synthesis. To measure SHCHC synthase activity separately, the enzymatically formed SHCHC is converted nonenzymatically to OSB (heating to 80 degrees C, pH 10, 10 min), which is then quantitated by the HPLC assay. The preparation of the substrate, isochorismic acid, and its purification by preparative HPLC are also described.

Overall proteolysis in perfused and subfractionated chemically induced malignant hepatoma of rat: effects of amino acids.

Control livers and chemically induced hepatoma-bearing livers of nonstarved rats were perfused cyclically with and without the addition of amino acids (known to suppress proteolysis) to the perfusate. Morphologic analysis of the fractional cytoplasmic volume of the lysosomal apparatus (dense bodies and autophagic vacuoles) demonstrated that the addition of amino acids to the perfusion medium inhibited autophagic sequestration of cytoplasm in both tumor and control hepatocytes, although the inhibition was stronger in control than in tumor hepatocytes. The fractional cytoplasmic volume of autophagic vacuoles (AVs) was larger in hepatoma cells than in control hepatocytes regardless of whether amino acids were added or not. The transition (degradation of sequestered cytoplasm) of AVs into dense bodies seems to be prolonged in malignant hepatoma cells. Assessment of rates of protein degradation both in the perfusion medium and in isolated lysosomes disclosed that proteolysis was much lower in tumor liver than in control liver. This can be explained by lower lysosomal enzyme activities in tumor cells, as was evident from tissue homogenate and isolated lysosomes. The addition of amino acids to the perfusate reduced total proteolysis from 1.73 to 0.78% per hour in control hepatocytes and from 0.49 to 0.33% per hour in tumor hepatocytes, i.e., inhibitions of 55 and 33%, respectively. Proteolysis as estimated from isolated lysosomes was also inhibited by amino acids added to the perfusion medium but the inhibition was more conspicuous in control (from 14 to 7.4%) than in tumor cells (from 5.2 to 3.6%). In conclusion, the results show that the relative cytoplasmic volume of AVs is higher but overall proteolysis lower in malignant hepatoma tissue than in control liver. Amino acids in perfusion medium inhibit overall proteolysis and AVs sequestration in both tumor and control hepatocytes, although the inhibition is stronger in control hepatocytes. Thus, even highly neoplastic cells maintain their ability to respond to physiologic regulators.

Gastric motility is stimulated but overall blood flow is unaffected during cold restraint in the rat.

The relationship between cold restraint-induced lesion formation, gastric motility, and gastric mucosal blood flow was studied in rats. Both anesthetized and unanesthetized animals placed in cold restraint developed gastric mucosal lesions. Gastric (corpus) motility was measured using extraluminal force transducers. Animals placed in cold restraint developed persistent, high amplitude, prolonged duration contractions. Those rats in which such contractions lasted greater than 1 h developed gastric mucosal lesions, whereas those animals in which such contractions lasted less than or equal to 1 h had no lesions. Overall gastric mucosal blood flow was measured using the hydrogen gas clearance technique. There was no significant change in overall gastric mucosal blood flow measured after 1, 2, and 3 h of cold restraint. We conclude that (a) the physical effect of cold water immersion is by itself sufficient to cause cold "restraint" lesions and (b) such lesions are associated with high amplitude, prolonged duration contractions lasting greater than 1 h.

Glutaraldehyde fixation of the primary antibody-antigen complex on nitrocellulose paper increases the overall sensitivity of immunoblot assay.

A simple method to increase the overall sensitivity of immunoblot assays is described. The method is based on fixation of the primary monoclonal antibody-antigen complex on nitrocellulose paper by a divalent crosslinker, glutaraldehyde. The fixation of the antibody-antigen complex significantly increases the sensitivity of the assay. Unexpectedly, the glutaraldehyde treatment also improves the signal/noise ratio by decreasing non-specific binding of secondary antibodies and/or tertiary reagents. Thus, a significant overall improvement of immunoblot assays is achieved by this method.

The effects of the spontaneous ingestion of particular foods or beverages on the meal pattern and overall nutrient intake of humans.

The influence of the ingestion of particular beverages and foods on the overall nutrient intakes and meal patterns of humans was investigated by paying 323 adults to maintain 7-day diaries of everything they ingested, time of ingestion, and subjective and social conditions. Ingestion of noncaloric beverages, diet sodas, and coffee or tea, were associated with low overall intakes but were not found to influence the amount eaten over the course of the day or in individual meals. Fifteen different caloric containing drink or food types were found, in general, to add to the total calories ingested in meals or over the day without displacing calories ingested in other forms. The results indicate that individual foods or beverages are ingested independent of other constituents and that intake within meals or over the entire day is elastic and readily influenced by nonregulatory factors.

Overall and regional lung function in Andean natives after descent to low altitude.

Overall lung volumes, regional residual volume to total lung capacity ratio (RVr/TLCr), regional ventilation (V/V) and perfusion (Q/V) were measured at 670 m in six Quechua Indians on days 2 and 37 after leaving their high-altitude homes (3500-4500 m). On day 2 the lung volumes averaged between 124 and 137% of those predicted for low-altitude residents (LAR) and there were no significant changes on day 37. Although overall RV/TLC was not different from the predicted value for LAR, RVr/TLCr on day 2 was higher at the top and lower at the bottom of the lungs compared to LAR. Regional Q/V and V/V were not different from LAR on day 2, or on day 37. However, the ratio of Q/V at the bottom to Q/V at the top was 2.36 on day 2 and 2.84 on day 37 (P less than 0.05). On day 2 hemoglobin- and volume-corrected diffusing capacity was 145% of the value predicted for LAR and this fell to 135% predicted on day 37 (P less than 0.05). Natives of high altitude reportedly have more alveoli that LAR and this could explain the greater vital capacity toward the bottom of the lung if the alveolar proliferation is concentrated there. This might also lower pulmonary vascular resistance at the bottom which would explain the normal Q/V distribution, even though pulmonary artery pressure may be increased.

Longitudinal Gompertzian analysis of Parkinson's disease mortality in the U.S., 1955-1986: the dramatic increase in overall mortality since 1980 is the natural consequence of deterministic mortality dynamics.

Age-adjusted mortality rates for Parkinson's disease (PD) for men and women in the United States, from 1955 to 1986, were calculated and subjected to longitudinal Gompertzian analysis. Annual age-adjusted PD mortality rate distributions were determined by a common intersect point and a variable environmental factor. For men, the death rate at age 73.75 years was 19.15/100,000; for women, the death rate at age 78.99 years was 28.64/100,000 for each year from 1955 to 1986. The environmental factor declined (improved) 360.7-fold for men and 319.6-fold for women in 1986 compared to 1955. Despite this dramatic environmental improvement favoring survival in PD, there has been a 57.6% increase since 1980 in the annual crude PD mortality rate for men compared to the stable annual crude mortality rate from 1955 to 1979. A corresponding 37.9% increase in the annual crude PD mortality rate for women has also occurred since 1980. The increase in overall mortality is due entirely to rapidly increasing age-adjusted mortality rates at ages greater than the intersect points for men and women. The results suggest that overall mortality due to common 'degenerative' diseases may increase dramatically as has occurred in PD.

Evaluation of the overall degree of sensitization to airborne allergens by a single serologic test: implications for epidemiologic studies of allergy.

In most epidemiologic studies of respiratory allergy, the overall degree of sensitization is usually measured by indices based on skin prick test reactions to a panel of relevant airborne allergens. In the present study we used a single assay (CAP-Phadiatop, Pharmacia, Uppsala, Sweden), which is based on the reaction between serum-specific IgE and a mixture of locally relevant inhalant allergens to measure the degree of sensitization to inhalants in the sera of 1815 young Italian men, in whom a complete diagnostic workup for respiratory allergy was performed in parallel. The intensity of reaction in the CAP-Phadiatop assay was highly correlated with: (1) an allergy index (p < 0.005), based on skin prick test reactivity to seven relevant inhalant allergens (Dermatophagoides pteronyssinus, mixed grass pollens, cat epithelium, Parietaria judaica, Olea europaea, Artemisia vulgaris, Alternaria tenuis); (2) the concentration of specific IgE to the same allergens, measured by CAP-RAST (p < 0.001), in both monosensitized and polysensitized subjects; (3) the rate of respiratory allergic diseases (p < 0.0001). Therefore CAP-Phadiatop intensity of reaction is a reliable indicator of the overall degree of sensitization to inhalant allergens. Furthermore, as demonstrated by relative operating characteristic analysis, it has an acceptable detectability power in identifying subjects with symptomatic expression of the atopy status [P(A) = 0.948]. The evaluation of the degree of sensitization to inhalants by a single and semiautomated assay represents a step forward in the standardization of procedures for epidemiologic studies of inhalant allergic diseases and for mass screening programs, which are aimed at identifying predictive markers of disease susceptibility.

DNA repair in an active gene: removal of pyrimidine dimers from the DHFR gene of CHO cells is much more efficient than in the genome overall.

DNA repair was measured in the dihydrofolate reductase gene in Chinese hamster ovary cells, amplified for the gene, by quantitating pyrimidine dimers with a specific UV-endonuclease. More than two thirds of the dimers had been removed from a 14.1 kb restriction fragment of the gene by 26 hr after irradiation (20 J/m2), while little removal was detected in fragments upstream of the gene and only 15% were removed from the genome overall. This suggests that damage processing can vary according to function or activity of affected sequences, which has general implications for correlations of DNA repair with survival and mutagenesis. Perhaps preferential repair of vital sequences facilitates UV-resistance of these cells despite low overall repair levels.

Effect of the ATP level on the overall protein biosynthesis rate in a wheat germ cell-free system.

A sensitive assay which examines the effects of ATP level on the overall activity of a cell-free translation system in a protein synthesis is described. The translational activity of cell-free system was measured in terms of a rate of protein synthesis directed by the 'test' template. The test template encodes a photoluminescent protein, obelin accumulation was determined from the kinetic curves of obelin. The rate of obelin mRNA translation. Time-dependent nucleotide level measurements were conducted throughout the translation processes. It has been shown that the rate of translation decreases exponentially with the decrease of the ATP level. This fall in the overall translation rate is due in part to the mRNA becoming inactive in the translation process. This is not caused by degradation, this mRNA can be restored for translation in a fresh cell-free system by phenol treatment. The reported results provide evidence that the level of ATP unambiguously determines the translational activity of the system.
