Polymorphism of N-acetyltransferase 2 (NAT2) gene polymorphism in shanghai population: occupational and non-occupational bladder cancer patient groups.

OBJECTIVE: Arylamine N-acetyltransferases (NATs) are involved in the detoxification of aromatic amines and hydrazine. In order to explore the possible association of NAT2 polymorphism with bladder cancer risk in benzidine exposed or non-exposed Chinese individuals, healthy subjects, subjects with bladder cancer of a former benzidine exposed cohort in Shanghai dyestuff industry and a group of bladder cancer patients without known occupational exposure to aromatic amines were genotyped for NAT2 gene polymorphism. METHODS: NAT2 genotyping was performed with a set of RFLP procedures at seven major polymorphic loci of gene coding area: G191A, C282T, T341C, C481T, G590A, A803G and G857A. RESULTS: The wild allele NAT2 *4 was the most prevalent allele (59%) in healthy individuals. The alleles NAT2*6A and NAT2*7B were also frequently observed (21% and 17%, respectively). In contrast to Caucasians, the percentage of slow acetylators was lower (12% in Chinese vs. 58% in Caucasians, P < 0.001). No relevant differences were observed for homogenous rapid, heterogeneous rapid/slow and homogeneous slow acetylation genotypes between the healthy subjects and both groups of bladder cancer patients. CONCLUSION: The present work did not support the association of slow acetylating genotypes of NAT2 gene with elevated risk of bladder cancer in Chinese whereas it was documented as an important genetically determined risk factor in Caucasians. Different mechanisms might play a role in individual susceptibility to bladder cancer related with aromatic amine exposure in various races or ethnic groups.

[Acrylates as a significant source of occupational and non-occupational contact allergy].

The authors review the literature on the contact allergy to acrylate plastics used in many branches of industry, construction, dental prosthetics, conservative dentistry, orthopedics and also in the production of glue, binding materials, and lacquers, especially printer's ink. The most frequent sources of non-occupational allergy are presented. In addition, the authors draw attention to new allergy-producing chemical compounds, difficulties in the interpretation of skin test results, mostly because of polyvalence of allergy to acrylates, and to characteristic clinical picture of skin changes.

[Descriptive statistics of occupational accidents during 2005 in the area of competency of the Occupational Health Unit FG/1].

During 2005, the Unit of Occupational Health of the Local Health Centre FG/1 started to record accident at work. The results confirme "Industry" as the sector with the highest risk of accidents at work. Many accidents at work seem to be in relation with the insufficient use of D.P.I. or with the wrong use of D.P.I. The results point out the necessity of a better model to record data and in particular the data that describe what/how has the accident happened and the sector/subsector of workers. In order to continue this activity during the next years, it will be useful to control the trend.

Collection of occupational epidemiologic data: the use of surrogate respondents to provide occupational histories.

When information about subjects in a study is unavailable from other sources, surrogate respondents, such as family members, have provided information about the subject. This chapter focuses on the quality of occupational histories provided by surrogates. The author addresses the issues of (1) nonresponse of the surrogate, (2) surrogate-subject and surrogate-record agreement, and (3) the impact of misclassification on estimates of relative risk.

[Return to work after occupational accidents and occupational diseases].

The key formula "by all suitable means" is a structural support for successful occupational reintegration. In this respect, the special system of accident insurance administered primarily through the "Berufsgenossenschaften" (BG), differs from the other branches of social security. This system's roots in civil law do not allow any comparison with the principle of public law insurance underlying the other branches of social security in Germany. Under the statutory accident insurance scheme, the employer, i.e., the liable party within the employment relationship, has to compensate any health damage incurred by an employee, subject to the principles of compensation for damages under civil law, in a manner enabling the employee to return to his position in worklife and in society as unscathed as possible. From this general mandate by the legislator, the social partners involved within the self-management of the BGs draw their strength and their dynamics in arranging for organizational methods within the-administrative offices and in cooperation with the health care providers. This control of rehabilitation, i.e., the accident insurance system's share of the responsibility for successful rehabilitation, contributes a great deal to reentergrating insured persons into work and career, returns valuable workers to the employers, thus having a favourable effect for the gross national product of a society, and last but not least provides a support for the social state.

[Occupational asthma due to isocyanates: current research orientation in occupational hygiene].

The replacement of the HDI monomer by less volatile prepolymers is not accompanied by a decrease of the incidence of occupational asthma in Switzerland. Field measurements show that the prepolymer concentration levels may be high although the monomer is hardly detectable or complies with the norm. Our researches are focused on the improvement of generation and measurement techniques (international quality control), on the characterization of the prepolymers to study their transformation in biological media. Field surveys will be conducted to establish the prepolymer concentration profiles. We recommend for the introduction in Switzerland of threshold limit values for HDI-biuret and isocyanurate.

Guidelines for the clinical evaluation of occupational asthma due to high molecular weight (HMW) allergens. Report of the Subcommittee on the Clinical Evaluation of Occupational Asthma due to HMW Allergens.

Sensitization to HMW allergens is a relatively common immunopathogenic factor in occupational asthma. The mechanism of sensitization is an IgE-mediated, type I reaction. High molecular weight allergen refers to proteins and polymers of organic compounds over 5 kd and usually in the 20 to 50 kd range. In most cases diagnosis requires: 1. A high index of suspicion of job-related asthma. 2. Exposure to HMW compounds and clinical findings associated with such an exposure. 3. Confirmation of sensitization by appropriate in vitro or in vivo tests. 4. Confirmation of a pathogenic role by physiologic measurements in either a natural setting or during laboratory-controlled challenges.

The risks of occupational exposure and infection by human immunodeficiency virus, hepatitis B virus, and hepatitis C virus in the dialysis setting. Italian Multicenter Study on Nosocomial and Occupational Risk of Infections in Dialysis.

BACKGROUND: The dialysis setting has been recognized as a high-risk environment for transmission to both patients and health care personnel of blood-borne infections, such as hepatitis B virus, hepatitis C virus, and HIV. METHODS: A seroprevalence survey of HIV, hepatitis B virus, and hepatitis C virus infection among 1002 patients and a subsequent 1-year surveillance study of percutaneous injuries and skin and mucous membrane contaminations were carried out among 527 health care workers in nine Italian dialysis units. The risks of occupational acquisition of HIV, hepatitis B virus, and hepatitis C virus infections among health care workers were calculated according to a deterministic model. RESULTS: HIV antibody, hepatitis B surface antigen, and hepatitis C antibody prevalences among patients were 0.1%, 5.1%, and 39.4%, respectively. A total of 67 percutaneous injuries, 29 mucous membrane contaminations, and 271 skin contaminations were reported by health care workers. The risk of acquiring infection was calculated to be 4000 and 8000 times lower for HIV than for hepatitis B and C, respectively. CONCLUSIONS: The risks of infection with HIV, hepatitis B, and hepatitis C for health care workers at dialysis units differ greatly and depend on the demographic profile and medical history of patients undergoing dialysis. To minimize the risk of exposure to HIV and other blood-borne pathogens, efforts must continue to increase compliance with universal precautions. Needle designs incorporating safety features and improvements in dialysis equipment design are also needed to avoid potential exposure.

Congenital malformation and maternal occupational exposure to glycol ethers. Occupational Exposure and Congenital Malformations Working Group.

Glycol ethers are found in a wide range of domestic and industrial products, many of which are used in women's work environments. Motivated by concern about their potential reproductive toxicity, we have evaluated the risk of congenital malformations related to glycol ether exposure during pregnancy as part of a multicenter case-control study, conducted in six regions in Europe. The study comprised 984 cases of major congenital malformations and 1,134 controls matched for place and date of birth. Interviews of the mothers provided information about occupation during pregnancy, sociodemographic variables, and other potential risk factors (medical history, tobacco, alcohol, drugs). A chemist specializing in glycol ethers evaluated exposure during pregnancy, using the job description given by the mother, without knowledge of case or control status. We classified malformations into 22 subgroups. The overall odds ratio (OR) of congenital malformation associated with glycol ether exposure was 1.44 [95% confidence interval (CI) = 1.10-1.90], after adjustment for several potential confounders. The association with exposure to glycol ethers appeared particularly strong in three subgroups: neural tube defects (OR = 1.94; 95% CI = 1.16-3.24), multiple anomalies (OR = 2.00; 95% CI = 1.24-3.23), and cleft lip (OR = 2.03; 95% CI = 1.11-3.73). In this last subgroup, risk, especially of an isolated defect, tended to increase with level of exposure.

Occupational low-back injury in a hospital employee population: an epidemiologic analysis of multiple risk factors of a high-risk occupational group.

A case-control study was carried out using 84 cases of employee back injuries and 168 controls (matched triplets) at Children's Hospital and Health Center, San Diego. The objective was to examine the impact of multiple individual and work-related risk factors for low-back injury from hospital employee health records. Hospital workers include occupational groups historically regarded as being at high risk, particularly nurses and others involved in patient care. In terms of traditional risk factors, significant associations were found for history of low-back pain or "slipped disc" by self-report and for history of previous back injury. Working the day shift also was significant (odds ratio [OR] = 2.23, P less than 0.005). Marital status (single) approached significance (OR = 1.65, confidence interval [CI] = 0.091, 2.99), as did low body weight (OR = 1.47, Cl 0.70, 3.10). No significant association was found between cigarette smoking and low-back injury. Possible work site health promotion interventions to lower the risk of low-back injury in this population are suggested.

Prevalence of occupational asthma among automobile and furniture painters in the center of Eskisehir (Turkey): the effects of atopy and smoking habits on occupational asthma.

BACKGROUND: Occupational asthma (OA) is a respiratory disorder characterized by airway hyperreactivity caused by agents present in the workplace. For determination of the prevalence of OA among car and furniture painters exposed to isocyanate in the center of Eskisehir, Turkey, a clinical and epidemiologic prospective study in three phases was done, incorporating 312 (89.4%) of the painters. METHODS: Of these subjects, 190 (61%) were furniture painters and 122 (39%) automobile painters. In the first phase of the study, a modified questionnaire and pulmonary function test (PFT) were done. During the second phase, peak expiratory flow rate (PEFR) was monitored in 52 subjects whose complaints were confirmed and who agreed to a month of such monitoring. In the third phase, nonspecific bronchial provocation tests (NSBPT) with histamine were done on 23 of the PEFR-monitored workers. RESULTS: Finally, through questionnaire, typical history, PFT, PEFR monitoring, and NSBPT, 30 workers (9.6%) were diagnosed as having OA. Smoking habits and atopy in the OA-diagnosed workers were found to be statistically significantly high in comparison to the other workers. CONCLUSIONS: It was concluded that OA is a common disorder among automobile and furniture painters, and smoking habits and atopy were seen to have a significant effect on OA occurrence.

Occupational and non-occupational hepatitis B virus infection among hospital employees in Jerusalem: a basis for immunisation strategy.

The present study was designed to assess the risk of hepatitis B virus (HBV) infection among hospital employees, who often contract the infection before the beginning of their employment, and to suggest a prevention strategy. The study population consisted of 2518 subjects working or studying at the two Hadassah University hospitals, on Mount Scopus and at Ein Kerem in Jerusalem. The total prevalence for anti-HBc positivity as an indicator for past or present HBV infection was 17.6%. Several variables, including country of birth, age, and duration of employment significantly affected the rate of anti-HBc positivity. The highest rates for anti-HBc+ were found in personnel of selected departments such as haemodialysis (31.8%), haematology/oncology (28.3%), and the blood bank (24.0%), after adjustment for country of birth, age, and sex. Specific occupations in the hospital were associated with an increased rate of anti-HBc positivity. Thus the highest rate of HBV infection (after adjustment for country of birth, age, and sex) was shown for housekeepers (32.4%) and departmental secretaries (23.6%), who take care of waste products containing blood, or who transfer vials containing blood to the hospital laboratories. By comparison, anti-HBc was positive in 17.2% of nurses, 15.6% of physicians, and only 7.8% of administrative clerks. Israel is a country of immigration, and anti-HBc rates were four times higher in employees born in countries where HBV is more endemic--for example, in north Africa and Mediterranean countries--than in employees born in western Europe or the United States. However, rate of anti-HBc + increased significantly with age as well as duration of employment in the hospital, irrespective of country of birth. These data indicate that although HBV infection often occurs in Israel before commencement of employment in the hospital, hospital employees are at significant risk for contracting HBV infection during their professional lifetime regardless of where they were born. Moreover, paramedical personnel such as housekeepers and departmental secretaries are in the highest risk group for contracting HBV. Finally, as a result of the high background of anti-HBC positivity in selected ethnic groups, mandatory screening for anti-HBc before employment in medical institutions in Israel is recommended for them, then active vaccination against HBV for employees at risk. Employees who immigrated from western Europe and the United States should be immunised without pre-vaccination screening for HBV.

Fate of occupational asthma. A follow-up study of patients with occupational asthma due to Western Red Cedar (Thuja plicata).

Thirty-eight patients with red cedar asthma proved by inhalation provocation test were studied after they had left exposure for more than 6 months. Twenty-seven patients became asymptomatic, with normal lung function (group A). Three patients had persistent chronic bronchitis with a moderate degree of airway obstruction, probably as a result of cigarette smoking (group B1). Eight patients continued to have recurrent attacks of asthma that decreased in severity after cessation of exposure, and their symptoms were probably due to previous exposure (group B2). The effect of breathing helium on maximal expiratory flow at 50 per cent of the vital capacity was studied. All except one patient in group A were responders (change in maximal expiratory flow at 50 per cent of vital capacity greater than 30 per cent). Two patients in group B1 and 2 in group B2 were nonresponders, suggesting obstruction in the small airways. All patients with red cedar asthma demonstrated bronchial hyperreactivity to methacholine to the same extent as patients with nonoccupational asthma. This hyperreactivity persisted after they left exposure, irrespective of symptoms. It is not known at present whether bronchial hyperreactivity is the predisposing factor in occupational asthma or is the result of the disease.

A multicentre randomized controlled trial of leisure therapy and conventional occupational therapy after stroke. TOTAL Study Group. Trial of Occupational Therapy and Leisure.

OBJECTIVE: To evaluate the effects of leisure therapy and conventional occupational therapy (OT) on the mood, leisure participation and independence in activities of daily living (ADL) of stroke patients 6 and 12 months after hospital discharge. DESIGN: Multicentre randomized controlled trial. SETTING AND PARTICIPANTS: Four hundred and sixty-six stroke patients from five UK centres. MAIN OUTCOME MEASURES: The General Health Questionnaire (12 item), the Nottingham Extended ADL Scale and the Nottingham Leisure Questionnaire, assessed by post, with telephone clarification. RESULTS: Four hundred and forty (94%) and 426 (91%) subjects were alive at 6 and 12 months, respectively. Three hundred and seventy-four (85% of survivors) and 311 (78% of survivors) responded at 6 and 12 month follow-up respectively. At six months and compared to the control group, those allocated to leisure therapy had nonsignificantly better GHQ scores (-1.2: 95% CI -2.9, +0.5), leisure scores (+0.7, 95% CI -1.1, +2.5) and Extended ADL scores (+0.4: 95% CI -3.8, +4.5): the ADL group had nonsignificantly better GHQ scores (-0.1: 95% CI -1.8, +1.7) and Extended ADL scores (+1.4: 95% CI -2.9, +5.6) and nonsignificantly worse leisure scores (-0.3: 95% CI -2.1, +1.6). The results at 12 months were similar. CONCLUSION: In contrast to the findings of previous smaller trials, neither of the additional OT treatments showed a clear beneficial effect on mood, leisure activity or independence in ADL measured at 6 or 12 months.
