The clinical application of the nursing process in selected acute care settings: a professional mirage.

The nursing process is the problem solving framework purported to be used in a multiplicity of health care settings. Although its use is widespread in educational and clinical settings, some nurse clinicians display negative attitudes towards its use and state that it is incongruent with nursing practice. To date, there has been no study cited that has examined it use within clinical settings to determine the substance of these claims. Using grounded theory methodology, this study examined the clinical application of the nursing process in acute care hospital settings. Data were obtained from semi-structured interviews with predominantly nurse clinicians (n = 27), participant field observations of nurse clinicians, and in-depth audits of patient records. Textual data were managed using NUD-IST and analysed using constant comparative method. Data generation and analysis proceeded simultaneously using open coding, theoretical coding, and selective coding techniques until saturation was achieved. Nurses in this study experienced the basic social problem of being in a state of "Unknowing" that was linked to a number of factors, such as, the existence of a fragmented and inconsistent method of determining and communicating patient care and work conditions of immense change and uncertainty. The findings revealed several problems with the clinical application of the nursing process and illustrated that the espoused theory was unable to be clinically applied.

To describe or prescribe: assumptions underlying a prescriptive nursing process approach to spiritual care.

Increasing attention is being paid to spirituality in nursing practice. Much of the literature on spiritual care uses the nursing process to describe this aspect of care. However, the use of the nursing process in the area of spirituality may be problematic, depending upon the understandings of the nature and intent of this process. Is it primarily a descriptive process meant to make visible the nursing actions to provide spiritual support, or is it a prescriptive process meant to guide nursing actions for intervening in the spirituality of patients? A prescriptive nursing process approach implies influencing, and in some cases reframing, the spirituality of patients and thereby extends beyond general notions of spiritual support. In this paper we discuss four problematic assumptions that form the basis for a prescriptive approach to spiritual care. We conclude that this approach extends the nursing role beyond appropriate professional boundaries, making it ethically problematic.

Nursing process: teaching elderly clients.

1. Nurses responsible for teaching older persons must adjust traditional approaches to address special age-related physical and psychosocial changes. 2. In order to develop effective teaching strategies, nurses should use the nursing process as a guide for identifying key knowledge deficits, establishing realistic goals, incorporating appropriate content, and evaluating learning. 3. The teaching environment, the actual presentation, and the presenter of information all influence the older adult's ability to learn important health-related information.

Orem and the nursing process: new directions for the 1990s.

The fourth edition of Orem's book Nursing: Concepts of Practice (1) offers many new insights and applications of her model which will be particularly welcomed by nurses in this country. While there will still be complaints about the 'jargon' the model requires nurses to contend with, Orem has made a serious attempt at making her model more accessible to practitioners. Careful attention has been given to the nurse's role in health promotion and the care of dependent others, in particular children. There has also been the introduction of 'green' issues, such as the impact of toxic substances in the atmosphere. Yet the most important reformulation of Orem's ideas involve the nursing process. The explanation of Orem's ideas in terms identifiable as assessment, planning, implementation and evaluation will assist both student nurses and qualified staff to tackle the 'practical' aspects of Orem's model with more familiarity and ease. This article presents an overview of Orem's current thinking about the nursing process, and identifies similarities with the nursing process as generally understood in this country.

Computer support for the nursing process.

Due to the increased complexity of nursing care in hospitals, new tools are being sought to aid the nursing process. One such tool is the computer. This article explores the various types of computer systems that can better support the nursing process in hospitals.

Orlando's deliberative nursing process theory: a practice application in an extended care facility.

Ida J. Orlando was one of the first nursing theorists to write about the nursing process based on her own research. Her Deliberative Nursing Process Theory focuses on the interaction between the nurse and patient, perception validation, and the use of the nursing process to produce positive outcomes or patient improvement. Orlando's key focus was to define the function of nursing. This author's purpose is to acquaint nurses with Orlando's theory and to encourage the use of the deliberative process to bring about patient improvement. This article provides an analysis of Orlando's theory and a demonstration of its successful use in clinical practice in an extended care setting.

[Optimizing the nursing process by new management instruments. The Northern Germany Nursing Research Network].

The Northern Germany Nursing Research Network (Pflegeforschungsverbund Nord) has dedicated itself to developing management instruments intended to optimize the nursing process. It is coordinated by the administrative office of the Institute of Public Health and Nursing Research (Institut für Public Health und Pflegeforschung IPP, director: Prof. Dr. Stefan Görres), Department of "Interdisziplinäre Alterns- und Pflegeforschung", University of Bremen. Four partial projects are currently being worked on in two groups: Group A--"Deficient areas of nursing action": The projects belonging to this group are "Individual care-patterns of elderly people and their determinants" (University of Bremen) and "Mobility Restrictions in Nursing Homes-Multicentre Observational Study (MORIN)" (University of Hamburg). The purpose of these studies is to examine the relevance of quantitatively significant risk combinations from a nursing science perspective. Group B--"The generation and testing of nursing action management instruments": The projects in this area are "Nursing rounds as instruments of quality assurance in home-based care" (University of Applied Sciences, Bremen) and "Predicting the risk of falls, efficiency of standard scales in opposition to nursing assessment-Cluster-Randomised Controlled Trial (PROFESSION)" (University of Hamburg). These studies concentrate in a unique manner on the generation and testing of management/control instruments suitable for application to nursing action.

Applying the nursing process to patient care in the operating room.

Using the nursing process in operating room nursing helps to assure personalized, continuous, and integrated care for the surgical patient. The steps in the nursing process clearly demonstrate the role and functions of the O.R. nurse as a professional member of the surgical team and as a component of the health care system. By this means the patient is assured of protection and quality care.

[A new documentation system to simplify a patient-centered nursing process in ambulatory care and in long-term care in a nursing facility].

In 1995/96, the new care insurance was introduced in Germany, regulating longterm nursing care in the community and in nursing homes. In order to implement the new legislative regulations the Federal Ministry of Labour and Social Affairs commissioned the AKI Institute to develop adequate patient centered quality standards for care planning and documentation. On the results of a multimethod survey a new documentation system was developed that enables a new patient centred approach, putting the client's habits and his or her view of health problems in the centre of nursing action. Traditional formats were altered to facilitate the documentation of the nursing process. The results show clearly that care is now more orientated towards a patient's autonomy and wishes. On the whole, the documentation is clearer and more systematic.

Coming of age in nursing. Rethinking the philosophies behind holism and nursing process.

No one is going to like everything that Kobert and Folan say in this article, but it offers a rich bouquet of ideas for discussion and debate. Are they right to assert that holism and nursing process are incompatible? That nursing process is associated with an immature level of professional development? That holism is the answer for nursing? That holism equates with feminism while nursing process represents a masculine reductionism?

[Implanting the nursing process].

INTRODUCTION: One of the basic events which has made an impact on the advance of the Nursing profession has been the introduction of the Nursing Process as a scientific method; however, this topic is still very controversial. The authors have carried out a study throughout Spain which compiles data regarding the number of Primary Health Centers which use the Nursing Process as well as how many nurses use this method. METHODOLOGY: An analytic study using Primary Heath Centers throughout Spain chosen by means of a systematic sampling method having a 95% confidence level on a total of 500 health centers. RESULTS: 43% of these centers apply this nursing method. 33% of nurses apply this process. COMMENTS: An important number of health centers and nurses who work in primary health care use this nursing process, but this same data indicate that an even higher number of nurses do not use this method. Another statistic to bear in mind is that in those centers where professional development classes in this methodology have been set up, almost double the number of nurses use this process than in centers where there have been no such classes.

[Significance of teaching the Nursing Process for the faculty].

This paper aimed to analyze the significance of teaching the Nursing Process (NP) for faculty, through the Representational Theory of Significance. Ten professors from Guanajuato University participated in the investigation. Data were collected through semi-structured interview and questionnaire. The analysis was done from the perspective of the mentioned theory. All faculty members know, teach and apply the NP. They also refer to the importance of its teaching and practice by professionals. In conclusion, regarding the significance of the nursing process, faculty members tend to recognize it as a scientific method and an important instrument in professional nursing activities; as for the meaning of teaching the nursing process, it is considered as a well-founded, indispensable and modern instruction for the discipline, as long as it covers the five stages it consists of.

Documenting nursing process in the perioperative setting. Continuity of care, patient evaluation.

Using our Perioperative Nursing Process form in addition to intraoperative and PACU flow sheets allows us to document the nursing process in a manner that satisfies the goals for documentation established by our committee. Because we place all forms in the patients' medical records, they are available for review by the JCAHO to prove that we comply with their standards and the standards of AORN and ANA. The nurses have been satisfied with the documentation system because it can be used efficiently and does not require redundant documentation as the patient transfers into each perioperative area.

[The nursing process: factors that facilitate and obstruct teaching].

The present study intends to identify how the teaching nursing process in the Graduation Schools of the São Paulo State are presented. The population was composed of 247 percents. As part of the methodology a questionnaire was elaborated. It was sent to 899 percents of the 32 public and private nursing schools of the São Paulo State. The results demonstrated that the factors that difficult the teaching of the nursing process were bigger in number than the factors that facilitate. However, the teaching of the nursing process, despite the presented difficulties, has been offered to the teachers of public and private schools of the São Paulo State.
