Long-term disease-free survival in a child with refractory metastatic malignant germ cell tumor treated by high-dose chemotherapy with autologous bone marrow rescues.

We report the case of a 8-month-old girl who presented with a metastatic malignant germ cell tumor (MGCT) that proved to be resistant to chemotherapy, including salvage platinum-based combination therapy, surgery, and metastatectomy. The child was then treated with high-dose carboplatin, etoposide, and cyclophosphamide with autologous bone marrow rescue (ABMR) for two courses, since the first course was well tolerated and resulted in serum alpha-f etoprotein normalisation. The child remains in complete remission 3 years post-ABMR and 4 years post-diagnosis, with no detectable treatment-related sequelae. Thus, as already reported in adults, dose-intensive chemotherapy with ABMR may have curative potential in children with refractory MGCT.

Prognostic factors affecting long-term outcomes in patients with resected stage IIIA pN2 non-small-cell lung cancer: 5-year follow-up of a phase II study.

The aim was to investigate the efficacy of neoadjuvant docetaxel-cisplatin and identify prognostic factors for outcome in locally advanced stage IIIA (pN2 by mediastinoscopy) non-small-cell lung cancer (NSCLC) patients. In all, 75 patients (from 90 enrolled) underwent tumour resection after three 3-week cycles of docetaxel 85 mg m-2 (day 1) plus cisplatin 40 or 50 mg m-2 (days 1 and 2). Therapy was well tolerated (overall grade 3 toxicity occurred in 48% patients; no grade 4 nonhaematological toxicity was reported), with no observed late toxicities. Median overall survival (OS) and event-free survival (EFS) times were 35 and 15 months, respectively, in the 75 patients who underwent surgery; corresponding figures for all 90 patients enrolled were 28 and 12 months. At 3 years after initiating trial therapy, 27 out of 75 patients (36%) were alive and tumour free. At 5-year follow-up, 60 and 65% of patients had local relapse and distant metastases, respectively. The most common sites of distant metastases were the lung (24%) and brain (17%). Factors associated with OS, EFS and risk of local relapse and distant metastases were complete tumour resection and chemotherapy activity (clinical response, pathologic response, mediastinal downstaging). Neoadjuvant docetaxel-cisplatin was effective and tolerable in stage IIIA pN2 NSCLC, with chemotherapy contributing significantly to outcomes.

CT-guided intraarterial chemotherapy in locally advanced tumors.

In a retrospective study, 123 patients with tumors (the majority were recurrent pelvic or breast neoplasms) underwent 376 cycles of intraarterial chemotherapy. Contrast material-enhanced computed tomography was performed to check the position of the catheter during 221 cycles. On the basis of findings, the catheter was repositioned 46 (20.8%) times because of weak contrast enhancement in the tumor region (n=24[10.9%]), involvement of neighboring healthy tissue (n=15[6.8%]), or both (n=7[3.2%]).

The value of a follow-up programme after radical surgery for colorectal carcinoma.

To evaluate the effectiveness of a follow-up programme after curative surgery for colorectal carcinoma, a 10-year series of 402 patients was surveyed for the detection rate of potentially curative recurrences and metachronous neoplasms. There were 120 recurrences (30%), and 100 of them (83%) were detected at scheduled check ups. Initial suspicion of recurrence was most often based on physical examination, carcinoembryonic antigen assay, or sigmoidoscopy. Reoperation was undertaken in 62 patients, in 26 cases (22%) for cure. The 5-year survival was 48% after curative reoperations. Metachronous adenomas and carcinomas occurred in 38 and 11 patients, respectively, giving corresponding cumulative 5-year incidences of 13% and 3.8%. Altogether, 37 patients (9.2%) had a curative reoperation for recurrent or metachronous carcinoma, and a further 38 patients (9.5%) had adenomas removed by polypectomy. It is concluded that regular follow-up is useful for the patients, and the follow-up schedule must be planned to detect both early recurrences and metachronous neoplasms.

The bladder tumor antigen (BTA) test compared to voided urine cytology in the detection of bladder neoplasms.

PURPOSE: Tests to detect recurrent bladder neoplasms are limited and none is consistently accurate. Recent studies suggest that the bladder tumor antigen (BTA) test, an agglutination reaction for basement membrane complexes, is superior to voided urine cytology in clinical practice. We compared BTA and voided urine cytology to bladder washings and cystoscopy, emphasizing diagnostic yield among patients with causes of basement membrane complexes other than bladder cancer. MATERIALS AND METHODS: Random voided urine specimens from 67 patients with a history of bladder neoplasms were collected before cystoscopy and bladder washing. Urine also was obtained from 34 patients with inflammatory bladder conditions including 5 with a history of prostate cancer. Each urine was tested for BTA according to a commercial kit. Positive results were indicated by yellow on a test pad. Blinded to all other results, each urine and each bladder washing were examined microscopically, and a positive test had malignant/suspicious cells. Bladder biopsies were performed when endoscopic lesions were seen. Specimens were grouped into 4 categories: group 1--biopsy proved bladder neoplasm, group 2--history of bladder cancer but not biopsy proved, group 3--history of prostate cancer and group 4--no history of urological cancer. RESULTS: Voided urine cytology was positive in 54% of specimens from patients with biopsy proved bladder neoplasms compared to 29% for BTA. Relative yield for voided urine cytology versus BTA was not changed if all group 2 cases having a positive bladder washing and positive cystoscopy were assumed to have bladder cancer, nor was relative yield altered by subsequent short-term followup. Of voided urine specimens 14% from group 1 patients and 41% from group 2 patients had scant cells. Overall diagnostic yield was superior for bladder washing. False-positive BTA occurred in 7 of 34 patients with no history of urological or prostate cancer. There were no false-positive voided urine cytology interpretations in these groups. CONCLUSIONS: BTA is not superior to voided urine cytology in detecting bladder neoplasms and may be limited by false-positive reactions in patients with other causes of basement membrane complexes in urine. Voided urine samples may be limited by high frequency of hypocellularity. Of 34 patients with a hypocellular urine specimen 4 had biopsy proved bladder cancer. Bladder washing yields best results but requires instrumentation. No test, including cystoscopy, is accurate always.

Autoradiographic evidence of a loss of iodination within hormone-dependent GR mouse mammary tumors as they progress to independence.

The purpose of this study was to determine by use of light- and electron-microscope autoradiography whether or not iodination occurred in mammary tumors of female GR mice. Of the sixty tumors studied it was found that pregnancy-dependent and hormone-induced tumors possessed iodinating ability. Although mammary glands from nonpregnant GR mice lacked the ability to iodinate, by the 16th day of pregnancy in response to hormonal stimulation the glands readily iodinated casein, and some epithelial cells contained ultrastructural cytochemical evidence of mammary peroxidase. Preneoplastic mammary gland lesions known as hyperplastic alveolar nodules were also able to iodinate, as were plaques, the disc-shaped lesions which give rise to the hormone-responsive mammary tumors in this strain. Plaques also contained epithelial cells with mammary peroxidase activity. When hormone-induced mammary tumors were transplanted into syngeneic mice they retained the ability to iodinate for several generations. However, as the tumors progressed to hormone independence, the ability to iodinate was gradually lost. Hormone-independent mammary tumors from GR mice lacked both iodinating ability and cytochemical evidence of mammary peroxidase. These findings suggest that iodination depends upon hormone-responsive cells within the mammary tumors and that as these cells become hormone unresponsive, the ability to iodinate is lost.

Tumour-associated tissue eosinophilia as a prognostic factor in oral squamous cell carcinomas.

AIMS: Tumour-associated tissue eosinophilia has been described in many sites, including head and neck. The mechanism of eosinophil recruitment and its role in tumours has not yet been defined, and its presence has been related to a favourable as well as unfavourable prognosis. The aim of this study was to evaluate the influence of tumour-associated tissue eosinophilia on the prognosis of 125 oral squamous cell carcinoma patients. METHODS AND RESULTS: The number of eosinophils was obtained by morphometric analysis and ranged from 0 to 392 per mm2. Tumour-associated tissue eosinophilia was classified according to intensity as mild, moderate, or intense and correlated statistically to the intensity of the mononuclear inflammatory infiltrate as well as to the location of the eosinophilic inflammatory infiltrate. The multivariate analysis demonstrated that intense tumour-associated tissue eosinophilia is an independent favourable prognostic factor for oral squamous cell carcinomas. CONCLUSION: These findings suggest an anti-tumoral role of eosinophils not as yet well understood that should be better investigated.
