Brain abscess by mycotic and bacterial infection in a diabetic patient: clinical report and review of literature.

This report presents a case of lethal invasive mucormycosis, a rare fungal infection, which predominantly affects immunocompromised patients, and is reported in a 57-year-old female who presented with cerebral abscess. The patient, who had undiagnosed diabetes mellitus, presented with extensive right hemifacial deficiency of the bones and soft tissues consequent to surgical resection of the ethmoid-spheno-maxillo-orbital district after mucormycosis. A reconstruction with a pectoral pedunculated flap was performed. The maxillary swelling extended to the contiguous area, involving the palate and homolateral orbital floor. Mucous and cutaneous samples showed the presence of Aspergillus fumigatus, and diagnosis of rhinocerebral mucormycosis was made. The patients also presented with a right hemiplegia consequent to a cerebral abscess by Eikenella corrodens. The authors decided to position an intraoral prosthesis to restore palatal integrity and masticatory function and inserted four titanium fixtures for the retention of the bone-anchored facial prosthesis.

[Fabricating facial prostheses using CAD/CAM and rapid prototyping technique].

At present, the treatments of hemifacial microsomia such as the missing of ear and eye still rely on the skill of technician to make the wax model of contralateral apparatus of patient in China. In this paper, CAD/CAM and rapid prototyping (RP) techniques are integrated to successfully create a silastic prosthesis by using the patient's data of CT or laser scanning. The clinical results suggest that a high accuracy has been achieved in shape, size, and protrusion of the facial prostheses, which indicates that the application of RP techniques in conjunction with CAD/CAM is a suitable approach for fabricating facial prosthesis.

Prosthesis serviceability for acquired jaw defects.

Patients with acquired jaw defects treated with prostheses present variable prosthesis longevity due to variable factors that influence the tissue foundation anatomically and physiologically supporting the prosthesis. Prosthesis serviceability is unpredictable or limited in longevity. Multiple prostheses and sequential prostheses interrelated are often indicated to effectively rehabilitate patients with acquired jaw defects.

New surgical obturator prosthesis for hemimaxillectomy patients.

Many materials have been used to make immediate and intermediate obturator prostheses for the hemimaxillectomy patient. The weight of the obturator prosthesis is a major hindrance. A new thermoplastic material, Polysar, is described to create a hollow obturator extension for immediate and intermediate lightweight obturator prostheses.

[Stereolithographic model construction based on 3-dimensional reconstructed CT sectional image sequences].

Techniques for 3D reconstruction of medical objects and production of models by CAM have been markedly improved. Milling tools have limited abilities to reproduce complex anatomical structures. Even if 5-axis milling systems are used, the problem of collisions between tool and object is not yet under control. An alternative is offered here by stereolithography. We performed a computed tomography (Somatom DRH, Siemens/Erlangen) of a child with extensive maxillary bone defect after surgical treatment of a congenital tumour. The bone defect was covered by an alloplastic implant. 3D reconstructions were performed by the aid of a conventional personal computer. Generated 3D volume data sets were transferred to a stereolithography system (3D Systems GmbH, Darmstadt/FRG). The produced model revealed high accuracy of the anatomical structures. Intraoperatively, the alloplastic prosthesis was removed and the shape of the new implant could be designed using the stereolithographic model.

[Application of prosthetic replacement for bilateral maxillectomy defect].

A 59-year-old woman with large hard palate cancer underwent surgical resection. We constructed a maxillary prosthesis to be connected to the patient's mandibular denture. Bilateral maxillectomy resulted in total loss of the hard palate and bilateral alveolar and partial loss of the soft palate. A obturator prosthesis was prepared beforehand to fit the surgical cavity. The patient recovered functional speech and swallowing at a relatively early stage, achieving satisfactory cosmetic results.

The effects of a maxillary speech-aid prosthesis for the combined tongue and mandibular resection patient.

The purpose of this investigation was to develop a protocol for the fabrication of a prosthesis that would improve speech in individuals who have undergone complete removal of the tongue and mandible. A 60-year-old man was suffering from severe xerostomia and was unable to produce intelligible speech. Speech analysis without the prosthesis revealed a profound articulatory disorder. With the prosthesis, xerostomia was eliminated and the subject had fewer articulatory errors of severity. Improvement in speech intelligibility was significant at p less than 0.001.

[The chronological changes in the size of maxillary defect].

Use of a maxillary prosthesis is now considered one of the good methods to ameliorate the crippling caused by the defect in the maxillary palate area after an ablative procedure for a maxillary tumor and it consumes no physical resources from the patient. There has been debate on whether the prosthesis should be made in the very early postoperative days or after the wound is stabilized. In this study, changes in the area of the defect in the palate after surgery were investigated by using plaster models from clinical cases we have experienced. Twenty-one cases were investigated according to the following features: "maxillary sinus cancer "+" more than 1.5 years of follow-up for maxillofacial prosthetics "+" presence of more than two casts of the case". These casts were photographed from a distance of 1.5 m, and the photographs were enlarged to the original size of the cast, then the defect area was measured with a Planimeter (Uchida Co. Ltd). The area of the defect had shrunk in 16 cases. It took over 1 year to get stabilized in 80% of the cases. It is believed to take around 3.5 years for all cases to become stable. This tendency had no relationship with the pattern of the defect, the stroke of the mouth opening, the pattern of the rest of the teeth, or the defect area itself. These findings suggest that it may be advisable to start the prosthetic work as early as possible rather than waiting for stability of the defect wound, to improve the patients's QOL.

[Dermatologic effects of two facial prosthesis materials in combination with glues].

Erythema is frequently observed in the region of contact between facial prostheses and the skin. In a large-scale study on the cutaneous tolerance of facial prosthesis materials, two prosthetic materials (glue and the solvent for the glue) were tested in the allergy test after Magnusson and Kligman (OECD). Only the solvent showed a moderately pronounced allergenic potential in the macroscopic assessment.

A mechanically retained, four part oral and facial prosthesis: a clinical report.

The design of a prosthesis depends not only on anatomical and physiological factors, but also needs to encompass patient's socio-economic limitations and psychological needs. A four-part prosthesis for a patient with a maxillary and orbital defect was designed to be easy for the patient to insert, remove and clean, retentive without requiring sophisticated attachments or the use of adhesives, and made from economically viable materials. The denture was attached to an acrylic resin core which in turn clipped into a silicone glove obturator. The facial section fastened onto the ocular "button" on a larger, separate core, which then connected into the orbital opening of the obturator. The entire assembly was self-retentive, yet each component could also be worn separately if the need arose. This provided a functional prosthesis as well as psychological benefits to the patient who had no access to medical facilities or supplies in her rural home town.

Restoration of a total maxillectomy patient using endosseous implants.

A vehicular accident resulted in a total maxillectomy for the patient compromising deglutition, mastication, speech, and appearance. Bone grafting provided a site for the placement of endosseous implants. The implants were used as stabilization for a prosthesis that resolved these problems. The prosthesis consisted of a multipart substructure attached to the implants. The superstructure was in intimate contact with the substructure, obturated the defect, and provided occlusal contacts, palatal form, and lip support.
