[Differential diagnostic problems in primary volvulus of the small intestine in adulthood].

Primary small bowel volvulus in the adult is a rare, life-threatening emergency. The preoperative diagnosis is difficult and in most of the cases incorrect. We present a case of primary small bowel volvulus in the adult. Preoperative diagnosis of intraabdominal bleeding following blunt abdominal trauma was erroneously made and the patient underwent laparotomy. Review of the literature is presented and discussed.

A case of primary volvulus of the small intestines in a dog is presented. This condition is described as rare in the dog. The surgical treatment and positive results are discussed.

[Primary volvulus of the small intestines in adults].

In our area, small bowel volvulus is a sporadic form of mechanical intestinal obstruction. Diagnostic problems may occur. The clinical presentation is that of an acute abdomen. The cause of symptoms may be due to narrowing of the bowel itself, or strangulation of the blood supply, or both. The types (primary and secondary) and incidence of small bowel volvulus are age-related and demonstrate astonishing geographical differences. Goals for treatment of small bowel volvulus should include physician awareness and accurate workup of this uncommon diagnosis.

Volvulus of the small intestine.

At the Mayo Clinic, six patients with primary volvulus and 51 with secondary volvulus were treated during a 10-year period. Volvulus of the small intestine must be considered when a patient presents with small-bowel obstruction, and early operative intervention should be undertaken to prevent vascular compromise.

[Volvulus of the small intestine, a rare complication during laxative period before colonoscopy].

HISTORY AND CLINICAL FINDINGS: A 69-year-old patient was hospitalized 16 h after investigation of the colon (screening colonoscopy), suffering from abdominal pain, which had started during the laxative period the day before colonoscopy. INVESTIGATIONS: Clinical and technical investigations showed signs of a peritonitis. TREATMENT AND COURSE: By laparotomy a volvulus of the small intestine was found, caused by adhesions after appendectomy years before. A resection of a part of the small intestine got necessary, there were no problems during the postoperative course. CONCLUSIONS: If pain appears during the laxative period before colonoscopy, rare complications like volvulus of the small intestine should be considered.

The fine structure of adult Onchocerca volvulus. II. The intestine and the reproductive system of the male worm.

The morphology of the gut and the genital duct of five male worms from Liberia and two male worms at least five years old, from Upper Volta are described. The lumen of the intestine can be distended vesicularly or it can be reduced to a cleft which is filled with microvilli. The intestinal epithelium contains concentric spherules or dense bodies with piles of lamellae. In parts of the intestine of a live worm the cells can be completely disintegrated. The wall of the testis consists of a squamous epithelium which contains numerous dense bodies on the luminal side. The wall of the vas deferens is a squamous or cuboid epithelium which contains glycogen and various particles. In the ejaculatory duct the epithelial cells are linked together by desmosomes at their luminal side. The brownish appearance of some organs under the light microscope is related to the accumulation of iron containing particles.
