A study of twenty-nine patients suffering from post-traumatic anal incontinence is presented. Twenty-eight patients had reconstructive surgery with a 96% overall success rate. Careful patient selection, preoperative evaluation and preparation, meticulous surgical technique and careful postoperative management insure a successful therapeutic result.
The increasing acceptance of the transsexual patient by society as well as a willingness of insurance companies to compensate for associated surgical procedures will result in an increase in patients presenting for sex reassignment. Data from gender identity centers suggest that the multidisciplinary approach leads to the best therapeutic results. Significant advances in phalloplasty techniques are yielding genitalia with more normal function and appearance.

Underway in Virginia is a cost-conscious program designed by Blue Shield to discourage the use of inhospital procedures deemed by national specialty societies to be relatively ineffective or outmoded. The author describes the program and its early influence.
Public opinion maintains the myth that ambulatory surgery is a benign operation. Although it may not be the ideal moment for demonstrating exceptional technical skill, there are unpredictable factors and certain limitations. To date, no specific legal concept of medical appraisal applicable to ambulatory surgery has been developed in France. Nevertheless, the nation that the surgeon is required to use all possible means is keenly accentuated because in a very short delay of a few hours the patient no longer is under direct management of the surgeon or his health care team. The aim of this work was to summarize the mandatory techniques for quality care and safety adapted to the lesions and the circumstances and taking into account accepted procedure, the current state of the art and the socio-familial environment. Two specific problems are encountered in the medical appraisal: a) defining whether the complication is inherent to ambulatory surgery, b) deciding how the responsibility could be shared. The possibility of special insurance for demonstrated non-fault surgical risk would be an elegant solution to avoid legal bias and facilitate controlled development of ambulatory surgery units.
Because the price of orthopedic implants has soared to astronomical heights, and insurance companies are paying less for surgical procedures, institutions must find ways of decreasing their costs and reducing their overhead. In the operating room the orthopedic nurse manager can be instrumental in reducing the amount of implant inventory by collecting and reporting data concerning shelf life of implants and coordinating new product trials and evaluation periods to decrease the number of implant systems to one or two.
OBJECTIVE: The management of paediatric airway disease is an integral aspect of paediatric otolaryngology. Recently, paediatric tertiary referral centres have developed centres of excellence for various aspects of paediatric care. The Pediatric Otolaryngology Airway Clinic at The Children's Hospital of Alabama, was developed as a regional referral centre for the management of children with difficult airway problems. Primary participants include the paediatric otolaryngologist, the airway management nurse, and the speech pathologist, in addition to other health care personnel. Over an 18-month period, 195 patients were seen in the clinic. The purpose of this study was to assess both the demography and the airway pathology in this patient population-specifically evaluating presenting diagnoses, diagnostic evaluation techniques, surgical intervention, geographic and racial distribution, insurance coverage, and referral patterns. CONCLUSION: This multidisciplinary approach to the management of children with chronic tracheotomies and other conditions involving the airway provides a unique environment that optimizes care for these complex patients.
