The fibula pro tibia procedure in the treatment of nonunion of the tibia.

We reserve the fibula pro tibia procedure for infected nonunions or for cases that already have been subjected to one or more other methods of treatment. The fibula pro tibia procedure is a technically simple one-stage procedure. The site of the pseudarthrosis is not exposed; hence there is less risk of recurrent infection. Consolidation was achieved in 75% of our cases. The objective findings are positive: consolidation after an average of one year. Knee mobility is normal in most cases, and ankle mobility is only slightly decreased. In most cases the angular deformities are clinically acceptable. All infections have cleared.

Skeletal union following long bone reconstruction using vascularised fibula graft.

Thirteen patients had skeletal reconstruction using vascularised fibula graft following resection of the diseased bone. Eleven patients had reconstruction of the lower limbs and two patients of the upper limbs. Clinical and radiographical evidence union were achieved with the average time of 32 weeks (earliest eight weeks). Six out of 11 patients (54%) in lower limb reconstruction started weight bearing at the average of 27 weeks. Bony union in this study is comparable with other studies using vascularised fibula graft.

Free vascularized fibula graft for a cystic femoral head and neck lesion in the West Indies.

A case is presented of an 18-year-old athlete with fibrous dysplasia of the femoral neck and head. The approach was by joint plastic and orthopaedic teams, which minimized operating time and allowed the option of vascular bone grafting. The lesion was curetted through a greater trochanteric window and the defect reconstructed with a free vascularized fibula graft with excellent result. Weight bearing was achieved in six months and there was minimal donor site morbidity. We believe the free vascularized fibula graft to be a reconstructive option, in difficult orthopaedic problems, facilitated by microsurgery and there is immense benefit of a joint effort between the orthopaedic and plastic surgical teams.

Tuberculous pseudotumor of the proximal end of the fibula. A case report.

A 72-year-old asian woman who had immigrated from Vietnam 10 years ago presented with a soft tissue mass around the proximal fibula. Conventional radiographs and magnetic resonance imaging demonstrated a destructive lesion of the head of the fibula with an intact superior tibiofibular joint and an abscess surrounding the destroyed bone. Histopathology obtained by a CT-guided needle biopsy revealed necrotizing epithelioid granulomata without demonstrating acid-fast bacilli. However, culture grew Mycobacterium tuberculosis. When tuberculosis affects the end of a long tubular bone, the adjacent synovial joint is usually involved. Cases where joint involvement does not occur are extremely rare and unusual. We report such a case.

[Treatment of osteonecrosis of femoral head with free vascularized fibula grafting].

OBJECTIVE: To evaluate the effect of the treatment of necrosis of femoral head with the free vascularized fibula grafting. METHODS: From October 2000 to February 2002, 31 hips in 26 patients with ischemic necrosis of the femoral head were treated with free vascularized fibula graft. Among these patients, 21 patients (25 hips) were followed up for 6-18 months (12 months on average). According to Steinberg stage: II period, 5 hips; III period, 8 hips; IV period, 12 hips. RESULTS: Among 25 hips, their Harris Hip Score at all satges were improved during the follow-up. The symptom of pain diminished or disappeared after operation. The patient's ability to work and live was not limited or only slightly limited during the follow-up. Radiographic evaluation showed that most femoral heads improved (18 hips) or unchanged (6 hips) and only one worsened. CONCLUSION: The free vascularized fibular grafting is a valuable method for femoral head necrosis. With this method, we can prevent or delay the process of the disease.

Age ranges of epiphyseal fusion in the distal tibia and fibula of contemporary males and females.

The range of variation in epiphyseal fusion in North American populations has not been sufficiently established. This significant oversight can lead to exclusion of persons of interest in a forensic investigation. This study evaluates epiphyseal fusion of the distal tibia and fibula in 570 European-, African-, and Mexican-American children and young adults. Radiographs of 270 females aged 9 to 17 and 300 males aged 11 to 20 were analyzed to assess the range of variation of epiphyseal fusion at each age. Results indicate that complete fusion in females occurs as early as 12 years in the distal tibia and fibula. All females demonstrated complete fusion by 16 years with no significant differences between ancestral groups. Complete fusion in males occurs as early as 14 years in both epiphyses. All males demonstrated complete fusion by 19 years. Significant differences in the earliest age of complete fusion showed that African- and Mexican-American males demonstrate complete fusion as early as 14 years in both epiphyses while European-American males do not express complete fusion until 16 years.

The dorsal antiglide plate in the treatment of Danis-Weber type-B fractures of the distal fibula.

The results of 93 of 107 consecutive antiglide plate internal fixations of Danis-Weber Type-B fractures of the distal fibula are reported with a follow-up period of just over one year. Using the ankle evaluation scale of Weber, 66.7% excellent, 27.9% good, and only 5.4% poor results were found. The method of dorsal antiglide plate fixation is possible in most of the frequent Type-B ankle fractures. Stabilization is better with the antiglide plate, especially in older patients with osteoporotic bone. Because of these advantages and the biomechanically sound technique, the antiglide plate is recommended for stabilization of the Danis-Weber Type-B fracture of the distal fibula.

Radical resection for osteogenic sarcoma of fibula with preservation of the limb.

This is a report of a 15 year old girl with osteogenic sarcoma of the proximal fibula treated by en bloc radical excision without amputation of the limb. The anatomical individuality of the fibula, vascularization of the leg and effective adjuvant chemotherapy determined the feasibility of the procedure. There is no evidence of tumor recurrence 27 months after surgery.

[The fibular incisure of the tibia. The cross-sectional position of the fibula in distal syndesmosis].

The ligamentous structure of the distal tibiofibular syndesmosis has been described by numerous authors. Yet the bony part, i.e. the fibular incisure of the tibia and the relationship of the fibula to this joint surface have hardly been dealt with. For this reason, we decided to investigate and typify the bony structures and the surrounding areas of the syndesmosis on 25 frozen cadaver feet. We found these could be divided into three groups: about 75% had concave surfaces, 16% convex surfaces, and in 8% typification was not possible because of irregular forms. In addition, measurements were taken that reflect the position of the fibula in the distal tibiofibular syndesmosis. These data are useful on X-ray examination of the ankle and are therefore of practical clinical significance.

[Congenital defects of tibia and fibula].

This study comprised 7 patients with longitudinal anomalies of the knee and leg classified in accordance with the 'classification of congenital limb deficiencies' introduced by Frantz and O'Rahilly. The anomalies were paraxial hemimelia of the fibula (4 instances), paraxial hemimelia of the tibia (6 instances). In the absence of the proximal tibia, therapy consisted of knee reconstruction according to Brown. In the absence of the distal part of the tibia a proximal tibiofibular synostosis was performed. In one instance corrective osteotomy of the tibia was required in the absence of the fibula. Exarticulation according to Syme was resorted in order to cope with an incorrigible abnormal position of the foot and for leg length inequality. The therapy should aim at giving the child walking ability at a normal time. Exarticulation has to be preferred to amputation in the treatment of these patients.

Posteromedial bowing of the tibia and fibula: a literature review and case presentation.

Congenital bowing of the tibia and fibula is a serious orthopedic condition rarely encountered by a podiatrist. However, with the increased number of hospital based podiatrists, it becomes imperative that a thorough knowledge of this disorder and the possible treatments be well understood. Deformities of this type are not very common. Most of the literature describes anterior and anterolateral bowing with pseudoarthrosis. The authors have only been able to document 128 cases of posteromedial bowing in the literature with no mention of it in the podiatric literature. This report will present a review of congenital angulation of the tibia and fibula with its associated deformities. Concentration will be given primarily to posteromedial bowing, including a case presentation from the Scholl College of Podiatric Medicine, Pediatric Foot Clinic.

Fractures of the fibula proximal to the distal tibiofibular syndesmosis.

Study of thirty-six cases of fracture of the fibula at levels proximal to the distal tibiofibular syndesmosis established that there are three types, distinguished by the direction of the fracture line, which are produced by different mechanisms: supination-external rotation, pronation-abduction, and pronation-external rotation. Advanced lesions that were seen were severe injuries of the ankle which included rupture of the deltoid ligament or fracture of the medial malleolus and complete diastasis of the distal tibiofibular syndesmosis in addition to the fracture of the fibula. Surgical treatment is necessary in most advanced lesions.

[The fibula-pro-tibia operation in infected defect pseudarthroses of the lower leg].

Surgery to effect synostosis between fibula and tibia is to be considered if it is not possible to bridge a relatively large gap at the tibia directly at the site of the defect by autologous (cortico)-spongiosaplasty because of inflammatory complications. Various modifications of the procedure are possible--they are described in the article--but the authors are of the opinion that preference should be given to synostosis which leaves the fibula at its physiological site. Technical details are given and the pros and cons discussed. This article is based on the experience collected during the treatment of eight patients.

Mechanical properties of the electrically stimulated callus. An experiment with constant direct current in rabbit fibulae.

The investigation of the effect of direct current on the mechanical properties of experimental fractures was conducted by applying 7-10 microA of constant current to the osseous defect of the rabbits' fibulae. The fibulae were tested mechanically with the combined methods of nondestructive technique and breaking-strength measurement. The result was analyzed in relation to the volume of the newly formed callus, which was estimated by roentgenographic appearance and calculation. There is no accurate proof that an electrically stimulated callus is superior in mechanical strength. A more massive callus in a relatively earlier stage suggests that the stimulated callus may have some advantages over the nonstimulated callus.

Congenital posteromedial bowing of the tibia and fibula.

Patterns of growth and development are presented for 33 patients with congenital posteromedial bowing of the tibia and fibula. The bowing is accompanied by shortening of the tibia and fibula, an initial calcaneovalgus deformity of the foot, and a decrease in ankle motion that does not improve with age. In general, the greater the initial bowing, the greater the ultimate extremity length discrepancy. The proportionate length differences between the normal and the bowed tibiae remained stable after the age of 12 months. This allowed a projection of anticipated extremity length discrepancy at maturity and a determination of the proper treatment. At birth the bowing varied from approximately 25 to 70 degrees, and at maturity the absolute tibial length discrepancies varied from 3.3 to 6.9 cm. Both the posterior and the medial components of the bow became markedly reduced with increasing age, but a mild residual medial portion of the bow remained. Soft tissue enlargement was observed in the posterior aspect of the affected legs in early life and developed to a relative muscle atrophy in later years.

Fracture-dislocation of the ankle with posterior entrapment of the fibula behind the tibia.

Fracture-dislocation of the ankle with entrapment of the fibula behind the tibia is a rare condition. Prompt recognition and treatment are necessary to prevent permanent disability. In three of four patients, the injury was managed by closed reduction. If the injury is accurately diagnosed and if traction and medial rotation are applied to the foot while the fibular shaft is pushed laterally, a stable, satisfactory reduction may be achieved. If closed reduction fails, a lateral approach through a small vertical incision directly over the fractured fibular malleolus is sufficient to allow open reduction of the fibula.

[Treatment of bimalleolar fractures by centromedullary nailing of the fibula].

An osteosynthesis technique involving nailing of the fibula was used to treat 106 bimalleolar fractures. Details of the technique used are described and results analyzed with respect to clinical course and radiologic findings. A generally favorable outcome was obtained. Nailing of the fibula provides a solid foundation, particularly in low and comminuted fractures.

Posterior fracture-dislocation of the distal part of the fibula. Mechanism and staging of injury.

The Bosworth fracture, a fixed posterior fracture-dislocation of the distal part of the fibula due to external rotation of the supinated foot, is a rare injury. In this report we review the literature, present two new cases, and describe our cadaver studies, showing that the initial stages of the injury are identical to those of a Maisonneuve fracture. The final stages that are unique to this fracture-dislocation are the posterior dislocation and fracture of the fibula and the fracture of the medial malleolus or rupture of the deltoid ligament. The objectives of treatment are reduction of the dislocation, repair and stabilization of the tibiofibular syndesmosis, and restoration of the ankle mortise to its anatomical position.
