Oral health status and treatment need of 11-13-year-old urban children in Tibet, China.

OBJECTIVE: To describe the oral health status and treatment need of 11-13-year-old children in urban Tibet and to determine if there was a difference in oral health status between the Tibetan and Han children. SURVEY DESIGN AND SUBJECTS: The two largest primary schools in Lhasa, Tibet, were selected and all children aged between 11 and 13 years were surveyed. Each child was interviewed and clinically examined in the schools, using portable equipment, by one of four calibrated examiners. The examination procedures and diagnostic criteria used followed those recommended by the World Health Organization. RESULTS: 347 children (207 Tibetans and 140 Han) were surveyed. Over 90% of the children claimed to brush their teeth at least once a day. Three-quarters had never visited a dentist. Caries prevalence was 44% in the Tibetans and 24% in the Han (P<0.001). The mean DMFT scores of the Tibetans and Han were 0.8 and 0.4 respectively (P<0.001). Only 1% of the children in both ethnic groups had healthy gums (highest CPI score = 0) and about two-thirds of them had calculus. One-third of the children were in need of treatment for dental caries and most of the required treatment items were one-surface fillings but 10% of the children needed extraction. CONCLUSION: Dental caries and treatment need level of both Tibetan and Han children in Tibet was low but their periodontal health status was unsatisfactory.

[Fluoride in the environment and brick-tea-type fluorosis in Tibet].

To explore whether endemic fluorosis in Tibet is related to local factors, an epidemiological survey of fluorosis was conducted in 3 districts in Tibet. The survey used the WHO and FAO established "Guideline for the study of dietary intake of chemical contaminants" and the Horowitz suggested criteria for children dental fluorosis examination. The fluorine in the local environment of the 3 selected districts was studied in detail. The possible fluorine sources examined indicated that only the brick-tea carried a high content of fluoride; water, other food materials, and fuel contents were insignificant. The children daily fluorine intake was high, where Naqu was 8.03 mg, Lahsa was 6.93 mg, Dingri was 7.68 mg. All the dietary fluoride was come from the brick-tea processed foods: the buttered-tea and zamba, these two contributed 99.74%, 98.70% and 99.60% of their daily fluorine intake respectively. The dental fluorisis index indicated that Naqu and Dingri were severe epidemic and Lahsa was moderate epidemic. The endemic fluorosis that occurs in Tibet was essentially due to heavy consumption of foodstuffs prepared with brick-tea, however, the high altitude, harsh living conditions, and poor nutritional status aggravated the fluorosis. Brick-tea type fluorosis is a new type of fluorosis recently discovered in China. Compared with the drinking water type and coal combustion type of fluorosis, it covered extensive area, more covert, and difficult to control. Decrease the fluorine content of brick-tea might be the most promising measure.

[Secular changes of physical growth in students of Zang-tribe in Tibet during 1965-2004].

OBJECTIVE: To analyze the secular changes of physical growth in 7-17 year old students of Zang-tribe in Tibet during 1965-2004, and to make suggestions for further improvement. METHODS: The data collected by local government in 1965 and Chinese national survey on students' physical fitness and health in 1985, 1995 and 2004 were used to analyze and find out the differences from the students' stature, body weight, chest girth, and BMI in different years, and to compare the increasing rate per 10 years in different periods of time. RESULTS: Levels on stature, body weight and chest girth were increased significantly among 7-17 year-old students of Zang-tribe in Tibet during 1965-2004. From 1965 to 1985, the levels of stature and body weight were increased 11.4 cm and 5.0 kg respectively among male Zang-tribe students, 9.1 cm and 3.9 kg among the female. During 1985-2004, stature levels of Zang-tribe students was kept rising in much less extent, while average levels of body weight were reaching up remarkably, especially in 1995-2004, in which the average increasing rate per 10 years had achieved 14.8% and 13.4% among the male and female students aged 7-17 years old respectively. Levels of chest girth were increased rapidly in 1965-1985, showing a higher increasing rate in male students (2.7%) than in female students (1.9%). In 1985-2004, the increasing rates fell obviously both in male and female students, but there existed a higher rate in 1995-2004 than in 1995-1985, especially for females (4.0%). Upon the secular changes of stature and body weight, the levels of BMI were decreased in 1985 as comparing to those in 1965, then increased rapidly in 1985-2004, especially in 1995-2004, having an increasing rate up to 12.6% and 12.4% respectively in male and female students. CONCLUSION: Secular changes on physical growth among 7-17 year-old students of Zang-tribe in Tibet were paralleled with the economic progress in 1965-2004, giving active effects by economic progress and nutrition improvements to the physical growth of children and adolescents.

[An epidemiological analysis on malaria in Linzhi district of Tibet in 1986-2004].

OBJECTIVE: To investigate the epidemiological characters of malaria in Linzhi district of Tibet. METHODS: A retrospective analysis on the epidemiology of malaria was carried out using the data on malaria situation in Linzhi district of Tibet in 1986-2004, referring to the distribution of season, population and region. RESULTS: The accumulative number of malaria cases in the period of 1986-2004 was 2459. The annual incidence of malaria in the district was reduced from 2.44 per ten thousand in 1986 to 1.03 per ten thousand in 2004, declined by 57.8% in 17 years. 99.3% of the cases were reported from Motuo County which was a typical high endemic area of malaria. The peak of prevalence occurred in June-October and 66.7% of the total cases were in the age group of 15-59 years old. 81.0% of the cases were farmers and 90.0% were Menba nationality. CONCLUSION: Motuo County has been the major area of malaria endemic in Linzhi district of Tibet. Most malaria cases in other counties are imported from Motuo.

Oblique stepwise rise and growth of the Tibet plateau.

Two end member models of how the high elevations in Tibet formed are (i) continuous thickening and widespread viscous flow of the crust and mantle of the entire plateau and (ii) time-dependent, localized shear between coherent lithospheric blocks. Recent studies of Cenozoic deformation, magmatism, and seismic structure lend support to the latter. Since India collided with Asia approximately 55 million years ago, the rise of the high Tibetan plateau likely occurred in three main steps, by successive growth and uplift of 300- to 500-kilometer-wide crustal thrust-wedges. The crust thickened, while the mantle, decoupled beneath gently dipping shear zones, did not. Sediment infilling, bathtub-like, of dammed intermontane basins formed flat high plains at each step. The existence of magmatic belts younging northward implies that slabs of Asian mantle subducted one after another under ranges north of the Himalayas. Subduction was oblique and accompanied by extrusion along the left lateral strike-slip faults that slice Tibet's east side. These mechanisms, akin to plate tectonics hidden by thickening crust, with slip-partitioning, account for the dominant growth of the Tibet Plateau toward the east and northeast.

Seismic images of crust and upper mantle beneath Tibet: evidence for Eurasian plate subduction.

Seismic data from central Tibet have been combined to image the subsurface structure and understand the evolution of the collision of India and Eurasia. The 410- and 660-kilometer mantle discontinuities are sharply defined, implying a lack of a subducting slab beneath the plateau. The discontinuities appear slightly deeper beneath northern Tibet, implying that the average temperature of the mantle above the transition zone is about 300 degrees C hotter in the north than in the south. There is a prominent south-dipping converter in the uppermost mantle beneath northern Tibet that might represent the top of the Eurasian mantle lithosphere underthrusting the northern margin of the plateau.

Seismic imaging of the downwelling Indian lithosphere beneath central Tibet.

A tomographic image of the upper mantle beneath central Tibet from INDEPTH data has revealed a subvertical high-velocity zone from approximately 100- to approximately 400-kilometers depth, located approximately south of the Bangong-Nujiang Suture. We interpret this zone to be downwelling Indian mantle lithosphere. This additional lithosphere would account for the total amount of shortening in the Himalayas and Tibet. A consequence of this downwelling would be a deficit of asthenosphere, which should be balanced by an upwelling counterflow, and thus could explain the presence of warm mantle beneath north-central Tibet.

Mitochondrial cytochrome b sequence variation and phylogenetics of the highly specialized Schizothoracine fishes (Teleostei: Cyprinidae) in the Qinghai-Tibet plateau.

The complete 1140 bp mitochondrial cytochrome b sequences were obtained from 39 individuals representing five species of all four genera of highly specialized schizothoracine fishes distributed in the Qinghai-Tibet plateau. Sequence variation of the cytochrome b gene was surveyed among the 39 individuals as well as three primitive schizothoracines and one outgroup. Phylogenetic analysis suggested that the group assignment based on 1140 bp of the cytochrome b sequence is obviously different from previous assignments, and the highly specialized schizothoracine fishes (Schizopygopsis pylzovi, Gymnocypris przewalskii, G. eckloni, Chuanchia labiosa, and Platypharodon extremus) form a monophyletic group that is sister to the clade formed by the primitive schizothoracine fishes (Schizothorax prenanti, S. pseudaksaiensis, and S. argentatus). The haplotypes of Schizopygopsis pylzovi and G. przewalskii were paraphyletic based on cytochrome b data, which most likely reflected incomplete sorting of mitochondrial DNA lineages. The diploid chromosome numbers of Schizothoracinae were considered in phylogenetic analysis and provided a clear pattern of relationships. Molecular dating estimated for highly specialized schizothoracine fishes suggested that the highly specialized schizothoracine fishes diverged in the late Miocene Pliocene to Pleistocene (4.5 x 10(4)-4.05 x 10(6 ) years BP). The relationship between the cladogenesis of highly specialized schizothoracine fishes and geographical events of the Qinghai-Tibet plateau is discussed.

Mountain scorpions: a new genus and species from Tibet (China).

A new genus and species of mountain liochelid scorpion are described on the basis of a single specimen collected in Tibet (China). This is the first liochelid scorpion found in the high Himalayan Mountains of Tibet, and the first example of the family to be collected out of its typical tropical and subtropical areas of distribution. Some considerations on mountain scorpions are proposed.

Beyond species trees: pervasive gene flow limits phylogenomic resolution in the diversification of Juniperus from the Qinghai-Tibet Plateau.

Understanding how lineages diversify despite persistent ancestral polymorphism and recurrent gene flow remains a central challenge in evolutionary biology. Juniperus distributed across the Qinghai-Tibet Plateau provide an ideal system for addressing this question because repeated geological uplift and climatic oscillations have likely promoted cycles of lineage divergence, range shifts, and secondary contact. Here, we combined approximately 1.08 million genome-wide SNPs from 164 individuals representing thirteen Juniperus lineages with phylogenomic datasets comprising 3,381 nuclear single-copy genes and nearly complete plastomes. We detected extensive phylogenomic discordance and cytonuclear incongruence across genomic datasets. Topology weighting, coalescent simulations, quartet-based tests, and analyses of gene flow and reticulation collectively support the interpretation that these patterns were shaped by the combined effects of prolonged incomplete lineage sorting and gene flow during lineage diversification. Ecological niche analyses further provide a spatial and climatic context in which environmentally similar lineages may have had greater opportunities for secondary contact during historical range shifts. Collectively, our results reveal that the evolutionary history of Qinghai-Tibet Plateau Juniperus is characterized by reticulate diversification rather than strictly bifurcating evolution, and demonstrate how genome-wide discordance can provide biological insights into the evolutionary processes underlying lineage diversification.

Low back pain in rural Tibet.

In a baseline assessment of 30 rural villages surrounding Shigatse City, Tibet, many people, especially women, identified low back pain as a serious health problem. Consequently, we aimed to establish the prevalence of such pain and to develop appropriate interventions. We did a cross-sectional study of the prevalence of low back pain and related functional disability using two-stage random cluster sampling. We included 499 adults aged at least 15 years from 19 villages. The point prevalence of low back pain was 34.1% (95% CI 27.9-40.3% [170 people]); the 12-month prevalence was 41.9% (35.5-48.3% [209 people]). 100 (20%) villagers had substantial functional disability associated with low back pain. Low back pain is likely to be an important and under recognised problem in rural societies like Tibet.

Elevation-structured viral ecological strategies along glacier-fed rivers on the Qinghai-Tibet Plateau.

The Qinghai-Tibet Plateau, a climate-vulnerable source of Asia's major rivers, harbors underexplored viral communities critical to ecosystem functions. By integrating 597 metagenomes from the Yangtze, Yellow, Lancang, and Yarlung Tsangpo rivers with 85 public available glacial metagenomes (Tibetan Glacier Genome and Gene catalog), we built the Glacier-to-River Virome Catalogue, encompassing 36,358 vOTUs and 897,250 viral protein clusters, to decode viral adaptation and ecological influence across elevation gradients. Our results reveal that high-altitude conditions favor viruses with elevated Guanine-Cytosine content, larger genomes and more cold-adaptation genes. A central finding is a systematic viral lifestyle shift from temperate in glaciated regions to lytic viruses downstream, accompanied with decline of pathogens carrying antibiotic resistance genes along the glacier-to-river gradients. Further, viral auxiliary metabolic genes transition from glacier nutrient scavenging (e.g., nitrogen and sulfur transporters) to downstream mineralization processes (e.g., denitrification) in plains highlights their role in biogeochemical cycling. These findings position viruses as pivotal regulators of microbial community structural and functional dynamics to glacier-to-river gradient change and biogeochemistry in the Qinghai-Tibet Plateau, providing critical insights into climate response in vulnerable Asian water towers.

High-Resolution Chromosome-Level Genome Assembly and Annotation of Triplophysa stewarti, an Endemic Plateau Loach from the Qinghai-Tibet Plateau.

The bottom-dwelling fish Triplophysa stewarti, endemic to the Qinghai-Tibet Plateau, is a valuable model for studying high-altitude adaptation in aquatic ecosystems. However, the lack of a high-quality reference genome has hindered comparative genomic and evolutionary studies within this genus. Here, we present a chromosome-level genome assembly for T. stewarti, generated using PacBio HiFi long-read sequencing and Hi-C scaffolding. The 697.9 Mb assembly is highly continuous (scaffold N50 of 253.58 Mb) and encompasses 25 chromosomes, representing 92.65% of the genome. BUSCO analysis indicated a 98.4% completeness, supporting the high quality of the assembly. We annotated 28,009 protein-coding genes, with 97.04% being functionally assigned across multiple databases (NR, UniProt, KEGG, GO, Pfam and InterPro). Additionally, repetitive elements constituted 42.47% of the genome, and we identified 52,709 non-coding RNAs. This high-quality reference genome provides a fundamental resource for exploring the adaptive evolution, population structure, and conservation genetics of T. stewarti and related species on the Qinghai-Tibet Plateau.

Kashin-Beck osteoarthropathy in rural Tibet in relation to selenium and iodine status.

BACKGROUND AND METHODS: Kashin-Beck disease is a degenerative osteoarticular disorder that is endemic to certain areas of Tibet, where selenium deficiency is also endemic. Because selenium is involved in thyroid hormone metabolism, we studied the relation among the serum selenium concentration, thyroid function, and Kashin-Beck disease in 575 subjects 5 to 15 years of age in 12 villages around Lhasa, Tibet, including 1 control village in which no subject had Kashin-Beck disease. Clinical, radiologic, and biochemical data were collected. RESULTS: Among the 575 subjects, 280 (49 percent) had Kashin-Beck disease, 267 (46 percent) had goiter, and 7 (1 percent) had cretinism. Of the 557 subjects in whom urinary iodine was measured, 66 percent had a urinary iodine concentration of less than 2 microg per deciliter (157 nmol per liter; normal, 5 to 25 microg per deciliter [394 to 1968 nmol per liter]). The mean urinary iodine concentration was lower in subjects with Kashin-Beck disease than in control subjects (1.2 vs. 1.8 microg per deciliter [94 vs. 142 nmol per liter], P<0.001) and hypothyroidism was more frequent (23 percent vs. 4 percent, P=0.01). Severe selenium deficiency was documented in all villages; 38 percent of subjects had serum concentrations of less than 5 ng per milliliter (64 nmol per liter; normal, 60 to 105 ng per milliliter [762 to 1334 nmol per liter]). When age and sex were controlled for in a multivariate analysis, low urinary iodine, high serum thyrotropin, and low serum thyroxine-binding globulin values were associated with an increased risk of Kashin-Beck disease, but a low serum selenium concentration was not. CONCLUSIONS: In areas where severe selenium deficiency is endemic, iodine deficiency is a risk factor for Kashin-Beck disease.

Having a "safe delivery": conflicting views from Tibet.

In the Tibetan Autonomous Region (TAR) of the People's Republic of China (PRC) maternal mortality ratios remain among the highest in the world. Although traditional Tibetan medical theory, practice, and pharmacology include information on maternal and child health care, Tibet is one of the few societies in the world that does not have traditional birth attendants or midwives. Using ethnographic methods, we gathered data from individual interviews with rural Tibetan women (N=38) about their beliefs and behaviors surrounding pregnancy and childbirth. Additional data were gathered through interviews with prefecture, county, and township health care providers. These data were used to develop a culturally appropriate village birth attendant training program in rural Tibet. We describe Tibetan women's perspectives of "having a safe delivery" in relation to concepts about "safe delivery" according to evidence-based medicine in the West. Our work also provides an example of the benefits and challenges that arise when ethnographic research methods are used to design and implement health care interventions.

Simultaneous detection of Anaplasma marginale and a new Ehrlichia species closely related to Ehrlichia chaffeensis by sequence analyses of 16S ribosomal DNA in Boophilus microplus ticks from Tibet.

To identify ehrlichial agents in Boophilus microplus ticks, DNA samples of B. microplus collected from the Tibet Autonomous Region and Sichuan Province of China were screened by a nested PCR. Sixteen of 43 (37%) DNA samples of B. microplus from Tibet were positive in nested PCR analysis. All 27 samples from Sichuan were negative. The screen identified two ehrlichial agents based on different 16S rRNA genes that were found after amplifying and sequencing the 5'-end fragments of the 16S rRNA genes. One sequence was identical to that of the gene of Anaplasma marginale, an etiological agent of animal anaplasmosis. The other sequence was most similar to that of the gene of Ehrlichia chaffeensis, an etiological agent of human monocytic ehrlichiosis. The sequence of 1,501 bases from the novel ehrlichial agent was obtained and showed the greatest levels of sequence similarity (97 to 98%) to 16S rRNA gene sequences of the members of the E. canis group of the genus EHRLICHIA: Sequence comparison of the 16S rRNA gene with the members of the genus Ehrlichia reveals that the novel ehrlichial agent detected in B. microplus ticks is a new species of the genus Ehrlichia and is most closely related to E. chaffeensis.

Cataract surgical coverage and outcome in the Tibet Autonomous Region of China.

BACKGROUND: A recently published, population based survey of the Tibet Autonomous Region (TAR) of China reported on low vision, blindness, and blinding conditions. This paper presents detailed findings from that survey regarding cataract, including prevalence, cataract surgical coverage, surgical outcome, and barriers to use of services. METHODS: The Tibet Eye Care Assessment (TECA) was a prevalence survey of people from randomly selected households from three of the seven provinces of the TAR (Lhoka, Nakchu, and Lingzhr), representing its three main environmental regions. The survey, conducted in 1999 and 2000, assessed visual acuity, cause of vision loss, and eye care services. RESULTS: Among the 15,900 people enumerated, 12,644 were examined (79.6%). Cataract prevalence was 5.2% and 13.8%, for the total population, and those over age 50, respectively. Cataract surgical coverage (vision <6/60) for people age 50 and older (85-90% of cataract blind) was 56% overall, 70% for men and 47% for women. The most common barriers to use of cataract surgical services were distance and cost. In the 216 eyes with cataract surgery, 60% were aphakic and 40% were pseudophakic. Pseudophakic surgery left 19% of eyes blind (<6/60) and an additional 20% of eyes with poor vision (6/24-6/60). Aphakic surgery left 24% of eyes blind and an additional 21% of eyes with poor vision. Even though more women remained blind than men, 28% versus 18% respectively, the different was not statistically significant (p = 0.25). CONCLUSIONS: Cataract surgical coverage was remarkably high despite the difficulty of providing services to such an isolated and sparse population. Cataract surgical outcome was poor for both aphakic and pseudophakic surgery. Two main priorities are improving cataract surgical quality and cataract surgical coverage, particularly for women.

'It seems he is an enthusiast about Tibet': Lieutenant-Colonel James Guthrie, OBE (1906-71).

Of the more than 20 officers of the Indian Medical Service who served in Tibet during 1904-50, when British Indian diplomats were stationed in that Himalayan state, James Guthrie was perhaps the most successful both in gaining the goodwill of the Tibetans and in advancing the reputation of medicine there. A Scotsman, Guthrie served in various military hospitals in India before his posting to Gyantse in southern Tibet in 1934-36, and during World War II he rose to be Assistant Director of Medical Services at the 10th Army headquarters in Teheran and Baghdad. Guthrie preferred the more remote imperial postings, however, and in 1945 he was posted to the Tibetan capital of Lhasa as Medical Officer to the British mission there. With his wife, who had nursing experience, he remained there until 1949, enjoying the variety of medical challenges and displaying an ability to accommodate Tibetan cultural beliefs within the practice of medicine. After service in Kuwait he returned to the UK, where he practised in the Shetland Islands for five years before establishing his own practice near Lyme Regis, where he died in 1971.
