[Multiphasic screening in a large industrial city. 2].

By the programme for detection of patients suffering from hypertension as appendage to the electronic data processing project "early detection of carcinoma of cervix uteri" in 54,020 examinations 10,469 women with hypertension were discovered and sent to competent specialists for continued observation and treatment. The portion of unknown hypertensions was 58.3%. In the examined female population of Karl-Marx-Stadt aged 20 to 65 years the prevalence of hypertension was 19.4% and corresponded to the results of national and international studies. - 16,714 women in the mainly fertile age were on hormonal contraceptives. The blood pressure of these women and of a comparable group without hormonal treatment was examined, the results were discussed. The histories did not allow to make conclusions for a certain disposition caused by the diagnosed hypertension. Moreover, our dates could not answer, if and which ranges the blood pressure (systolic and/or diastolic) increased in particular case.

Experiences with multiphasic screening for cancer.

This study reports on our experiences with a multiphasic cancer screening clinic for the detection of early-stage cancers. The clinic under review is the Prevention-Detection Clinic (PDC) housed at Roswell Park Memorial Institute in Buffalo. The PDC offers both free cancer screening examinations and risk-reduction education and counseling. Since its inception in May 1979 through 1984, over 13,000 individuals have been screened and counseled. Names of screenees were recently matched against the files of the Western New York Tumor Registry, a population-based cancer registry, to identify screenees subsequently diagnosed with cancer. Matching identified a total of 332 individuals with a cancer diagnosis. Of these, 98 individuals were found to have been diagnosed prior to screening. Of the remaining 234 individuals, 135 were diagnosed after referral from the PDC, 23 went undetected at screening, and 76 were either diagnosed several years after screening or with recurrent disease. The majority of cancers (67%) were detected with in situ/localized stages of disease.

Multiphasic screening: an idea whose time has passed.

The theory of screening for preclinical disease requires that early diagnosis and treatment will lead to an outcome which is more favorable than that resulting when treatment is initiated after symptoms appear. In practice, this requirement is met for a surprisingly small number of diseases. Early detection components of "health packages" must be developed, therefore, strictly on the basis of proven effectiveness. Existing programs should be revised in accordance with international guidelines, and adapted as needed to the local incidence and prevalence of disease and anticipated population compliance, so that the maximum benefit of early detection can be realized and the numerous hazards of screening avoided.
