Twenty-four cases of histologically confirmed congenital intraspinal lipoma of the lumbosacral region were studied by means of myelography with metrizamide. The findings were compared with intraoperative observations. Myelography with metrizamide clearly revealed the detailed intrathecal structures and allowed a classification of intraspinal lipomas into four types, in terms of their insertion into the conus medullaris: dorsal type, either with direct or indirect (via an intrathecal stalk) insertion of the extrathecal lipoma into the dorsal aspect of the conus medullaris; caudal type; combined type; and filar lipoma. Based on our surgical experience in untethering and decompression of the lesions, the classification was found to be useful in designing a safe and effective surgical procedure which minimized all possible trauma to the intrathecal neural structures.

Postmortem aortography was performed in 38 adults, aged 17 to 29 years, to elucidate the normal anatomy of the arteries supplying the lower thoracic and lumbosacral regions. The lowest 3 thoracic segmental arteries, examined in 32 cases, ran symmetrically in 27 cases. In 5 cases (16%) their course was asymmetric; on one side every segment was fed by an artery of its own, whereas on the other side a segmental artery fed 2 or more segments. The course, distribution and size of the 1st to 4th lumbar arteries were constant in every case except one, while the middle sacral artery and its branches, the 5th lumbar arteries, varied in size, being frequently compensated for, partly or wholly, by the iliolumbar arteries derived from the internal iliac arteries. In their course around the vertebral bodies, lumbar arteries frequently passed between and beneath fibrous attachments of the crura of the diaphragm and the psoas muscle.
True extrarenal Wilms' tumor is a rare malignant neoplasm most frequently presenting in the retroperitoneal or inguinal regions. We report an unusual subcutaneous lumbosacral (LS) region extrarenal Wilms' tumor without associated teratomatous tumor elements or associated neural tube defect in a 2 1/2-year-old girl. Pathologic review revealed features of true extrarenal Wilms' tumor, and the patient remains in complete remission following surgery and combination chemotherapy. This report illustrates the importance of early surgical intervention and pathologic examination of similar soft tissue masses in children.

An MRI tomograph was used to examine 396 patients with suspected disc pathology of the lumbosacral region. Forty two patients in whom disc herniation was diagnosed underwent surgery. MR proved to be a reliable and accurate method for the diagnosis of disc herniation, revealing its site and size as well as compression on the neural structures. The limits of MR are discussed and integrated diagnostic protocol in lumbosacral disc pathology is proposed.
Neuropeptides were first localized in the human spinal cord by immunocytochemistry and substance P has been shown, by the same method, to be reduced ipsilaterally in the dorsal horn after limb amputation and bilaterally in the Riley-Day syndrome. Several neuropeptides increasingly fulfil the criteria to establish them as neurotransmitters or neuromodulators, and they may also have trophic actions in the spinal cord. Using radioimmunoassay and immunocytochemistry, we present here for the first time a quantitative regional distribution and localization of vasoactive intestinal polypeptide (VIP), substance P, somatostatin, bombesin and cholecystokinin (CCK-8) in normal postmortem human spinal cord. A comparison of the distribution of these peptides reveals an exceptional pattern for VIP, with relatively much higher levels in the lumbosacral region. Immunocytochemical analysis shows a distinctive distribution of VIP-containing fibres and terminals at the lumbosacral segments. This VIP-containing system may have an important role in the spinal control of urogenital function in man.
We quantitatively analyzed several features of orthogradely labeled peripheral growth cones in the lumbosacral region of the chick embryo. We compared motoneuron growth cones in regions where they appear to express specific directional preferences (the plexus region and regions where muscle nerves diverge from main nerve trunks), which we operationally defined as "decision regions," to motoneuron growth cones in other pathway regions (the spinal nerve, nerve trunk, and muscle nerve pathways) which we termed, for contrast, "non-decision region." We found that motoneuron growth cones are larger, more lamellepodial, and have more complex trajectories in decision regions. Sensory growth cone populations, which are thought to be dependent upon motoneurons for outgrowth (Landmesser, L., and M. Honig (1982) Soc. Neurosci. Abstr. 8: 929), do not enlarge or become more lamellepodial in motoneuron decision regions, suggesting that this local environment does not affect all species of growth cones equally and that the alterations in motoneuron growth cones in these regions may be relevant to their specific guidance. In addition, the resemblance between the sensory population and other closely fasciculating growth cones lends support to the suggestion that sensory neurons utilize motoneuron neurites as a substratum. We suggest that the convoluted trajectories, enlarged size, and more lamellepodial morphology of motoneuron growth cones in decision regions is either related directly to the presence of specific cues that guide motoneurons or to some aspect of this environment that allows them to respond to specific cues.

In the past, several techniques for raising the gluteal mass have been described, all of which were based on dermolipectomies of major or minor extension. The main disadvantage of these dermolipectomies is the visible external scar. The author presents his technique for lipectomy of the ilio-lumbosacral region. It is proposed that multiplanar liposuction in this region lengthens the lumbar lordotic curve and results in upward traction on the buttocks, resulting in a more youthful contour of the region. The article describes a series of 98 patients with lipodystrophy of the ilio-lumbosacral area who underwent liposuction of the region and were highly satisfied with the results.

The results of studies performed by the authors show that time measurements of proprioceptive muscular reflexes (triceps surae and tibialis posterior reflexes) may be considered a valuable addition to methods of topical diagnosis of radicular disturbances in the lumbosacral region. The reflexes were tested in twenty normal subjects. There were found an intraindividual constancy of the respective reflex time with a maximum deviation of 2 ms as well as a lateral variability up to 1.3 ms. Radicular compressions resulted in an increase in reflex time by 2 to 66 ms or failure of the reflex potential to be recorded. The pathological cases were compared with clinical and neurological studies, positive myelographies using radiopaque substances, and results of surgical operations. Compared with the results of surgical operations, changes in reflex time indicated damage to the reflex arc in ninety percent of the cases.
