Integration of STI services into FP/MCH services: health service and social contexts in rural Ghana.

Ghana, like many countries, has taken up the call to integrate STI management with MCH/FP services. Since 1994 a range of policies has been developed on safe motherhood and family planning, as well as syndromic STI management guidelines for 'mid-level' staff and an all-encompassing 'reproductive health' policy. The success of these policy initiatives depends to a large extent on the processes and contexts of implementation, yet analysis of this has rarely been undertaken. This paper analyses the rural context of policy implementation in the rural Upper East Region of northern Ghana and suggests that a 'blanket' policy to integrate STI and FP/MCH services may be inappropriate in particular contexts. It illustrates how the implementation of health policies is influenced--and often impeded--not only by local service contexts, economic and epidemiological factors but also by culturally defined social attitudes and behaviours. These can influence not only whether a policy is accepted by a population but also how it is implemented by health staff. Future reproductive health policy needs to address these issues in order to develop goals which can realistically be implemented in specific settings. Enhancing at district level the voice of nurses working at community level and promoting collaborative, culturally-specific and community-based initiatives could facilitate this.

[Utilization of health services].

Health services utilization has been associated to several socio-demographic factors, as age, gender, or socio-economic status. It has also been associated to the organizational structure and finance of health care, and to health status of the individuals. In the present article main results of studies of health services utilization, are reviewed, with special emphasis on primary health care. The main factor associated with health services utilization is health services need as measured by individual's health status. The rest of variables have a minor role when jointly analyzed. When utilization of services is caused for a preventive visit, variables related with health status are less explicit. Methodological limitations of these studies and problems when analyzing equity in utilization of health services are also discussed.

[Present problems in health services and health care].

In order to deal with and resolve contemporary problems of the health services and health care it is important to outline their concept, contents and definition as well as those of their fundamental constituents--the material, technical basis, workers and their activities, which determine the state health policy. The author analyzes from this aspect the hitherto accomplished development with regard to all basic constituents and discusses the problem of the relationship of the intensity and extensity of activities of the health services and health care. A foremost part is played by medical is played by medical science--medicine, its state, knowledge assembled by students during their undergraduate studies and medical graduates during their postgraduate training. A substantial and decisive component of the health services as regards therapeutic and preventive care is formed by health workers and the quality of their work which depends on objective as well as subjective factors which are analyzed. Health services do not comprise only practical activities but social medicine and the organization of health services are very important scientific disciplines which participate in the quality of activities. This scientific discipline was founded by prominent personalities. It must be stated that in practical medical work leading authorities do not always respect the scientific character of the discipline and thus very complicated problems arise as regards scientific control. All these problems from the aspect of the department of social medicine and organization of health services as well as subjective and scientific control must be resolved from the aspect of reconstruction of society.

Implications for health services.

Health services for older people in the NHS have developed pragmatically, and reflect the nature of disease in later life and the need to agree objectives of care with patients. Although services are likely to be able to cope with the immediate future, the growth of the elderly population anticipated from 2030 calls for long-term planning and research. The issue of funding requires immediate political thought and action. Scientifically the focus needs to be on maximizing the efficiency of services by health services research and reducing the incidence of disability in later life through research on its biological and social determinants. Senescence is a progressive loss of adaptability due to an interaction between intrinsic (genetic) processes with extrinsic factors in environment and lifestyle. There are grounds for postulating that a policy of postponement of the onset of disability, by modifications of lifestyle and environment, could reduce the average duration of disability before death. The new political structures of Europe offer under exploited-unexploited opportunities for the necessary research.

Health policy and the coloring of an American male crisis: a perspective on community-based health services.

Health services at the community level are organized and financed in such a way that men need access but encounter barriers to care such as poor service design, lack of insurance, and the absence of health literacy. Community health delivery systems may not be appropriate, effective, fit, or able to meet the needs they are charged to fill. Community-based health services, including health departments, are underfunded, understaffed, and unable to carry out their mission in a way that protects the health of the community. The current design for funding and delivering health care services excludes poor men, particularly men of color. Improving the health of men requires modifications in the way health care is financed, delivered, and managed.
