[Digestive cancers in France. Comparison of the incidence in 7 departments and estimation of incidence in the entire country of France].

Estimation of differences in the incidence of digestive cancers in France was made from incidence data coming from 7 French departments covered by population-based registries. For some localizations, such as cancer of the pancreas, incidence variations could be explained by differences in recording techniques. But geographic variations clearly appeared for digestive tract cancers: the incidence of esophageal cancer is 4 to 5 fold less in the departments of Tarn and Haute-Garonne than in the department of Calvados. Conversely, the incidence of colorectal cancer is lower in the department of Calvados than in the others. For the entire country, the estimated number of digestive cancers is 46,300 cases per year. This number represents a third of all cancers in males and a fourth in females. Colorectal cancer (25,700 cases per year) is the most frequent of all cancers in both sexes. Cancer of the stomach (8,200 cases per year) is the second digestive cancer, equal to esophageal cancer in males. The heavy burden of digestive cancers in public health is emphasized by our results. These results should lead our country to intensify research in the direction of primary and secondary prevention.

[COART France 2003 report on new socioeconomic data on osteoarthritis in France].

RISING COSTS: According to the first evaluation of the socioeconomic impact of osteoarthritis in France conducted in 1993, the estimated annual cost was slightly less than one billion euros. In 2003, the annual cost was approximately 1.8 billion euros. Overall costs for osteoarthritis have thus increased by more than 80%, i.e. 8% per year. COST OF OFFICE VISITS: In 2002, 13.4 million office visits for osteoarthritis yielded an overall cost of 270 million euros and led to 17 million drug prescriptions (1.5 per consultation) yielding an overall cost of 574 million euros. HOSPITAL COSTS: With 127,000 acute-care admissions, 175,000 hospital days for day care and/or rehabilitation, and implantation of 118,000 hip or knee prostheses, the overall hospital cost came to 820 million euros in 2002. INDIRECT COSTS: Five million days of sick leave in 2002, in 88% of the cases for peripheral osteoarthritis, led to 180 million euros in indemnities.

[Organ transplantation in France in 1984 (activity report of France-Transplant)].

Nine hundred and seventy four renal transplantations were performed in 1984, thus attaining a total 7927 grafts made in France at the end of 1984. The numbers of heart transplantations (77), to which 100 prospects in 1985 can be added, as well as liver grafts (13) plus 50 prospects in 1985, are showing a spectacular increase. Although the considerable improvement of heart graft and liver graft results can be attributed, essentially to the use of Cyclosporine, this trend can be dependent in the case of kidney transplantations also on transfusions before and on a better HLA-B DR compatibility which can in optimal situations represent a respective gain of 10% and 15% of graft survival.

[Veno-occlusive disease of the liver after bone marrow transplantation. Report of the symposium Autograft in France and the group of study of bone marrow transplantation. France Auto-Greffe et le Groupe d'Etude de la Greffe de Moelle osseuse].

Hepatic veno-occlusive disease is a frequent complication after high-dose chemo- or radiotherapy after bone marrow transplantation and is a major cause of mortality. During the 3 weeks following transplantation, acute major hepatic vascularization is observed together with portal hypertension and weight gain, ascitis and oedema of the lower limbs due to non-thrombotic obstruction of the centrilobular hepatic veins. This report summarizes the observations presented at a French symposium of France Autogreffe and Groupe d'Etude de la Greffe de Moelle osseuse. Different pathogenic processes are implicated including endothelial mechanisms due to toxic factor related to graft preparations and immunosuppressor treatments (methotrexate, cyclosporin), physical factors related to irradiation, immunological factors related to the expression of class II antigens on endothelial cells and viral factors, in particular cytomegalovirus infection. The incidence of veno-occlusive disease varies greatly from one series to another. Two French groups reported 5 and 3 cases among 1991 and 253 autologous grafts respectively (1.2 and 2.3%) while the Seattle group observed 53% among 355 consecutive grafts, although the same clinical criteria were used. Histological criteria include fibrosis around a non-thrombotic occlusion of the centrilobular veins, cytolysis and congestion of the sinuses. Other methods for diagnosis include transjugular biopsy, the suprahepatic pressure and imaging techniques. Prophylactic continuous infusion of low-dose heparin has been associated with a lower incidence. Trials using anti-tumour necrosis drugs and prostaglandin E1 have also been undertaken and show possible effects towards decreasing prevalence.(ABSTRACT TRUNCATED AT 250 WORDS)

A new influenza surveillance system in France: the Ile-de-France "GROG". 1. Principles and methodology.

The aims of influenza surveillance have changed over the years. The early detection of epidemics is essential for practical decisions on vaccine development, strategy of vaccine prophylaxis, chemoprophylaxis or chemotherapy. A new surveillance system is presented which combines the classical specific data related to the isolations, direct detection of virus as well as serological findings, with non-specific indices. The rationale for definition and elaboration of such indices is discussed: selected parameters deal with medical activity of a panel of sentinel doctors (general practitioners and pediatricians), notifications of diagnosed acute respiratory viral infections, measure of emergency doctors activity, absence from work (as seen by physicians, insurance agencies or industrial personnel), selected drug prescription and consumption, hospital admissions. The sentinel physicians also cooperate in taking samples from cases, thereby extending the field of specific surveillance since they have contact with standard cases of the disease and not only hospitalized patients. A weekly comparison of time variations of specific and non-specific indices should permit a reliable evaluation of the epidemic situation and therefore be of great help for practical decisions. The system has operated since 1984, and the initial hypothesis of the sensitivity of some of the indices has already been verified.

A new influenza surveillance system in France: the Ile-de-France "GROG". 2. Validity of indicators (1984-1989).

The evaluation of the validity of the GROG's indicators is based on the first five years of activity, four of which being epidemic ones. A virological criterion for identifying epidemics is used as the gold standard. For each non-specific indicator, the ratio of epidemic weeks to non-epidemic weeks is outlined. Therefore we can define four epidemic situations in relation with the strains circulating and the non-specific indicators. Influenza A epidemics, which are known as a mortality problem can also be described according to its impact upon morbidity. As a result, it is possible to set up a sensitive and specific surveillance system for early detection of influenza epidemics. This objective is achieved by combining both the weekly collection of virological data as well as the collection of other available indicators either in routine practice or as provided by a network of sentinel practitioners. Such detection can pave the way for an improvement in preventive measures against the harmful effects on health of influenza epidemics.

Screening for congenital hypothyroidism in France. Misdiagnosed cases: collaborative study of screening centres in France.

Misdiagnosed cases of congenital-hypothyroidism (CH) during the first 9 years of the French screening program were analysed. A total of 50 cases were missed (3% of total diagnosed) which represents a severe failure of the system. Failures were caused by technical errors of sample collection or TSH assay (n = 27) or due to normal TSH (n = 22) or T4 (n = 1) concentrations in the newborn blood specimens. We conclude that screening methods should be improved and that physicians should remain alert to clinical signs of hypothyroidism.

The biogenic volatile organic compounds emission inventory in France: application to plant ecosystems in the Berre-Marseilles area (France).

An inventory describing the fluxes of volatile organic compounds (VOCs), isoprene and monoterpenes, and other VOCs (OVOCs) from the biosphere to the atmosphere, has been constructed within the framework of the ESCOMPTE project (fiEld experimentS to COnstrain Models of atmospheric Pollution and Transport of Emissions). The area concerned, located around Berre-Marseilles, is a Mediterranean region frequently subject to high ozone concentrations. The inventory has been developed using a fine scale land use database for the year 1999, forest composition statistics, emission potentials from individual plant species, biomass distribution, temperature and light intensity. The seasonal variations in emission potentials and biomass were also taken into account. Hourly meteorological data for 1999 were calculated from ALADIN data and these were used to predict the hourly isoprene, monoterpene and OVOC fluxes for the area on a 1 kmx1 km spatial grid. Estimates of annual biogenic isoprene, monoterpene and OVOC fluxes for the reference year 1999 were 20.6, 38.9 and 13.3 kt, respectively, Quercus pubescens, Quercus ilex, Pinus halepensis and garrigue vegetation are the dominant emitting species of the area. VOC emissions from vegetation in this region contribute approximately 94% to the NMVOC (non-methane volatile organic compounds) of natural origin and are of the same order of magnitude as NMVOC emissions from anthropogenic sources. These results complete the global ESCOMPTE database needed to make an efficient strategy for tropospheric ozone reduction policy.

Prevalence of antibodies to Borrelia burgdorferi in forestry workers of Ile de France, France.

A cross-sectional sero-epidemiological study was conducted on forestry workers, a high risk population for Lyme borreliosis. The prevalence of seropositive forestry workers (indirect immuno fluorescence assay) is 15.2% (n = 211). Almost 70% of participants reported a history of tick bite. Among districts, high seroprevalences were observed in Fontainebleau and Rambouillet; two sites of high risk for Lyme borreliosis. However, clinical prevalence of the disease is very low. During the investigation no active Lyme borreliosis was observed. Thus, asymptomatic infection predominates.
