The paper deals with the description of the regions of the use of automatized processing of medical informations; i.e. the basic processes of the information processing referred to patients in the hospital, the information and documentation referred to patients, the information relations between the place demading efficiencies and bringing efficiencies in the medical institution including systems organized in the territory as well as the finding of decisions in medicine. With the description of the individual regions of use are connected considerations of the utility, issuing from the project of the electronic data processing realised at the Medical Academy Dresden. The regions of application described make clear that the tasks of the automatized processing of medical informations extend to the following: Taking over of functions of the description of information and the storage of information, taking over of functions of the comparison of informations and the control of information, taking over of functions of the organisational preparation and the planning of diagnostic and therapeutic courses as well as the taking over of functions of the information integration. It is clearly to be established that the electronic data processing shall not restrict the turning of the physician to the patient, but shall improve and enlarge it. The electronic data processing is a modern means of work which with progressing scientific and technical development medicine needs just as much as other technical procedures and methods.
During the last 2 years, a personal computer in the special diabetes department of Innsbruck University Eye Hospital has not merely facilitated statistical analysis; without the PC such analysis would not have been possible. Logistically, the most important function is data acquisition, because the system only works if all data are entered. In addition to data processing, word processing, statistical analyses, and chart processing using graphics software are all done on the PC. The system is currently being developed further to include on-line access to literature databases via Datex-P and additional workstations. After 3 years of experience with the PC it is difficult to imagine doing scientific work without it.
The present paper describes several possibilities of electronic data processing (EDP) in clinical evaluation of new diagnostic reagents for coagulation. In order to evaluate two novel assays for determination of prothrombin time (PT) new statistical and graphical methods were developed and tested for their practicability. These methods are especially useful for establishing reference curves, for checking the characteristics of new PT reagents, for determining the "International Sensitivity Index" of each lot of reagents and of the specific therapeutic range for oral anticoagulation as well as for testing the comparability with other PT reagents. The results show that the statistical and graphical methods have proved their usefulness for clinical evaluation of new PT methods and that there might be further applications for such methods in comparison tests of similar coagulation assays.
Electronic data processing is taking over in the nursing domain in German hospitals. This is due, not only to the fact that computer hard- and software are getting better and cheaper, but also due to the legal demands of the new health legislation. Neither nurse-managers nor the nurses themselves are adequately prepared for this technical innovation or its implications. The dominant influence of management-interests leads to the danger that criteria relevant to the content of nursing fail to be taken into account. Consequently the quality of care may suffer. The Ministry for environment, energy, youth, family and health has commissioned the Agnes Karll Institute for Nursing Research to identify software features specific for the requirement of nurses. The study focuses on rostering programs for nursing staff allocation, in relation to the workload and on conditions which have to be met to make software nurse-friendly. Selected, important results are reported.

An epidemic spread of Acinetobacter calcoaceticus var. anitratus in two neurosurgical wards is described retrospectively and prospectively using electronic data processing. Isolation of the species from sputum preceded the isolation from CSF by 1/2-1 year. Control measures directed against spread by air and indirect contact controlled the epidemic. Reexamination of 20 selected strains from the epidemic revealed two distinct resistance patterns.
The primary precondition for the application of electronic data processing (EDP) are organisational measures including adequate personnel who are needed for preparatory work. Special documentation and coding systems must be introduced for the purpose of data preparation, whereby the size of the EDP equipment (hardware) depends on the problem to be tackled. The EDP programms (software) should be designed in such a way that every doctor is able to operate the EDP equipment even with little knowledge of data processing. Equipment ranges from minicomputers and centralized computer systems to integrated information systems. Whereas today well-designed application models for minicomputers already exist, the integrated information system is still in a development stage.
Electronic data processing is more used also in the analysis, control and surveillance of human nutrition. The present paper deals with a method that permits to evaluate the nutritional structure of all-day canteen feeding by means of a computer program based on the dishes actually delivered by the canteen. Apart from the energy value, the values for animal and vegetable proteins, fats, carbohydrates, calcium, vitamin A and vitamin C have been adopted as criteria. The evaluation was based on a specific scoring system. The results obtained are made available to the respoective institutions in the form of graphs. The qualitative and quantitative effects produced by this method of evaluation have been studied in 10 selected canteens during a period of 3 years.
Experiences in a two years application of electronic data processing in medical routine microbiology are reported. Optical mark reader forms serve as request forms as well as workprotocol. On the left two thirds of these forms (Fig. 1 and 2) the patients data including clinical statements, and the kind of specimen to be investigated (sputum, smear etc.) as well as results not essential for the printout of laboratory reports are filled in. On the right third of the optical mark reader form, in which area the Bell & Howell document reader can read pencil marks, the patients identification number, the tests requested and all results are marked right at the work bench. As shown in the flow diagram (Fig. 3) after completion of the investigation the date on the mark reader forms are read mechanically and the test results are printed (Fig. 5) by means of the computer. In addition, the data processing system, which proved to be very flexible, performs all necessary types of administrative work as filing of laboratory data in a coded form on cards for permanent storage (Fig. 7) statistics and the printout of bills (Fig. 6). The processing of routine microbiology date is achieved by a Siemens DVA 404/3 (64 kbt) connected for magnetic disc device (2993 Mio bytes), and a line printer (60,000 lines per hour) and dialog display monitors, which are used simultaneously for the on line and off line handling of all the data in clinical chemistry-, serology- and hematology-departments of the same institute.
