The results of a study on the prevalence of mental diseases and abnormalities among a geographically well-defined group of 70-year-olds, all living in nine suburbs of Copenhagen, are submitted and discussed. The total psychiatric morbidity was found to be 15.5%; 6.4% suffered from psychoses, males preponderating, and 7.4% classified as "neuroses + personality disorders", females preponderating. Only 15% of the pschotic group were institutionalised. Of the entire material, 5% were demented, and this included 1.l% patients who were severley demented. There was a definite preponderance of males. Of the population, 2.6% were living in their homes with senile or arteriosclerotic dementia. About 1% of the population were looked after in their homes despite severe dementia of varying geneses. Hidden morbidity, meaning that the proband had neither been institutionalised nor had consulted a doctor for mental illness within the past 5 years, made up of 15% of the psychotic group and 19.2% of the group "neuroses + personality disorders + alcoholisms". The total hidden morbidity constituted 2.6% of the population or 16.5% of the demonstrated psychiatric morbidity. The prevalence findings are on the whole on a level with those reported from other studies, most of which have been concerned with population groups over 65 years of age.
A retrospective case-control study was conducted to estimate the possible association between risk of multiple sclerosis (MS) and the number of changes in residence before the age 20 years. Forty-one patients and 82 controls were interviewed. Using the Mantel-Haenszel estimate of the odds ratio, no association was found between the number of moves and MS.
In order to compare the food habits and defecation tendencies by region, a questionnaire survey was conducted of 1722 college women (18-20 years of age) living in Sapporo (146), Sendai (183), Osaka (1214), and Kumamoto (179). (1) When questioned about food habits, less than 58.5% of the respondents in the four groups considered their food intake to be sufficient to maintain their health. In regard to knowledge about their food intake requirements, the positive response was the highest in Sendai (85.8%), followed by Kumamoto (58.2%) and Osaka (50.9%) with the lowest in Sapporo (41.1%) (p < 0.01, among the four groups). (2) The students in Sendai, Osaka, Kumamoto and Sapporo, 96.2%, 93.5%, 92.9% and 92.5%, respectively, considered that constipation could affect their health (p < 0.05, among four groups). The percentage of those with regular bowel movements every day, tended to decrease in the order of Osaka (22.2%), Sapporo (21.2%) and Sendai (20.8%), with the lowest in Kumamoto (12.3%). Bowel movements of less than three times a week were reported by 24.7% in Sapporo, 24.0% in Sendai, 23.2% in Osaka and 22.3% in Kumamoto (p < 0.01, among the four groups). (3) Of those who defecated every day, in terms of time of day, the percentage was high between waking and after breakfast, with the highest percentage in Kumamoto (72.8%) and the lowest in Sapporo (61.2%) (p < 0.05, among the four groups). For subjects that responded that they should have a bowel movement every day, the highest percentage was in Osaka (91.0%) and the lowest was in Sapporo (83.9%) (p < 0.05, among the four groups).
STUDY OBJECTIVE: Daily duration of oxygen administration is an important factor in the effectiveness of long-term oxygen therapy (LTOT) for hypoxic chronic pulmonary disease. We have assessed the daily use of oxygen therapy in 930 patients with COPD and examined factors associated with the effective use of this treatment. METHODS: Objective daily duration of oxygen use over a 3-month period was prospectively measured using the counter clock of the oxygen concentrators or by weighing the liquid oxygen container at each delivery. A questionnaire was filled in by an independent investigator asking about home situation, lifestyle, and whether oxygen therapy was used during all domestic and outside activities. In addition, prescribing physicians were asked about the duration and modalities of oxygen prescribed in each case. RESULTS: The patients had been receiving LTOT for 36 +/- 24 months and had hypoxemia (PaO2 = 56 +/- 9 mm Hg), hypercapnia (PaCO2 = 47 +/- 8 mm Hg), and severe airflow obstruction (FEV1/VC = 42 +/- 14%). The mean duration of oxygen treatment prescribed was 16 +/- 3 h/d. The mean duration of oxygen therapy achieved was 14.5 +/- 5 h, but only 45% of the patients achieved oxygen therapy for 15 h or more per day. Patients with effective use of LTOT, ie receiving oxygen therapy for at least 15 h/d, were significantly more hypoxic (PaO2 = 54 +/- 9 vs 57 +/- 9 mm Hg; p < 0.001), more hypercapnic (PaCO2 = 48 +/- 8 vs 46 +/- 7 mm Hg; p < 0.005), and also more obstructed (FEV1/VC = 39.5 +/- 13 vs 45 +/- 14%; p < 0.001) than the rest of the patients under treatment. Other factors associated with effective use of oxygen were (1) initial prescription for 15 h or more per day, (2) supplementary education on oxygen therapy by a nurse or physiotherapist, (3) cessation of smoking, (4) use of oxygen in all domestic situations (toilet, meals, leisure, etc.), and (5) absence of side effects from oxygen treatment. CONCLUSIONS: Attention by the prescribing physicians to such factors could optimize oxygen prescription and constitute goals for education of patients.
Many studies have shown a relationship between obstructive sleep apnea (OSA) and accidents, but to our knowledge, none have investigated prospectively the effects of treatment with nasal continuous positive airway pressure (CPAP). CPAP was proposed to 973 patients, of whom 893 patients actually underwent CPAP. These patients were consecutively invited to enter a prospective follow-up study including a questionnaire before treatment and after 6 and 12 months of treatment; 547 patients completed the study (153 left the study, and only partial data were available for 193). The baseline questionnaire included questions concerning accidents in the previous 12 months, asking whether patients had had an accident and, if so, whether they felt that the accident(s) were related to sleepiness, and whether the patients felt that they had had near-miss accidents due to sleepiness. The questionnaires at 6 and 12 months included the same questions referring to the previous 6 months; the accidents reported on each follow-up questionnaire were cumulated and compared with the accidents during the 1-year period before treatment. The number of patients having an accident decreased with treatment for real accidents (from 60 to 36; p<0.01), as well as for near-miss accidents (from 151 to 32; p<0.01). The average number of accidents per patient also decreased, for real accidents (from 1.6+/-1.3 to 1.1+/-0.3; p<0.01) and for near-miss accidents (from 4.5+/-6.5 to 1.8+/-1.4; p<0.01). The cost, in terms of days in hospital related to accidents, decreased from 885 to 84 days. With caution due to the absence of a control group, it is suggested that treatment with CPAP decreases the number of accidents occurring in OSA patients. This result may have important implications in the evaluation of the cost/benefit ratio when treating OSA patients.

The German cockroach, Blattella germanica (L.), and the closely related species B. bisignata (Brunner) belong to the germanica species group. They are similar in appearance, life history, reproductive cycle, and courtship behavior. The most significant difference is habitat preferences: B. germanica is a household species and lives in crowded conditions, whereas the feral B. bisignata lives outdoors in a solitary manner. Nevertheless, B. bisignata has recently been found in households. A comparison between the two species has shown that B. germanica displays gregarious behavior and produces an aggregation pheromone, whereas both characters are absent in B. bisignata. Mate preference experiments have revealed that B. germanica females accepted only conspecific males, whereas B. bisignata females mated with males from both species, provided that long distance calling was bypassed. In addition, the high reproductive potential of B. germanica outcompeted the other species: when 10 pairs of B. germanica and of B. bisignata were kept together in crowded conditions during 3 mo, B. bisignata was driven into extinction. It is concluded that the chances of B. bisignata becoming a new household species are remote.
Conflicting interpretations questioning whether brief intervals of real time are perceived as passing more or less rapidly with increasing age have been reported. Reasons for these inconsistent results lie in semantic confusions, in variations in methodology, and in the effects of other mediating variables. The present study examined relationships between age, value of time over both the short and the long term, perceived time to death, self-perceived activity levels, and estimates of brief time intervals in a sample of older institutionalized males. The production method was used to obtain the estimates of time passage. Time intervals were increasingly underestimated with advancing age, indicating that time units are shorter with increasing age. While a number of other variables were interrelated, the only other factor consistently related to time estimates was the short-term value of time. Implications for future research and for interventions in institutional settings are discussed.

The relationship between tumour incidence rate and habitation patterns was investigated in 3289 patients registered between 1959 and 1968 as cutaneous malignant melanoma in the Swedish Cancer Registry. A linear correlation analysis between latitude and melanoma incidence rate in various regions showed a coefficient, r, was -0.74, which implies a decreasing incidence with increasing latitude. This result supports the hypothesis that ultraviolet irradiation is the predominant cause of melanoma. However, considerable deviations from the regression line were seen in some regions. Moreover, a comparison between town and country indicated melanoma incidence increased with population density, an increase not explained by overdiagnosis and thus not in agreement with the working hypothesis of UV-irradiation as the predominant cause of melanoma. Increase of foreign travel, as estimated by passport issue, may explain the overrepresentation of melanoma in some regions namely city and county of Stockholm, city of Malmö and Göteborg county. As foreign travel in Sweden generally means sunshine trips, this town versus country paradox may also be explained on the basis of increased irradiation, as may the anomalies observed in some counties. A regression analysis of the epidemiological index for UV-irradiation and melanoma incidence, adjusted for foreign travel, demonstrated a close agreement with the results of the latitude gradient analysis.
