231 patients over 80 years old were operated at the Clinique de chirurgie digestive of Geneva University Hospital between 1981 and 1990 for a malignant tumor of the gastrointestinal tract. The following parameters were studied: intra- and postoperative morbidity and mortality, duration and quality of survival. Surgery proved valid in this age group given its low mortality (15%). Postoperative morbidity (44%) did not prolong hospital stay, since 90% of all patients were discharged within one month of surgery. Following surgery, quality of life was excellent: 75% of patients returned home, and 80% were better off than pre-operatively. Such operations provide better results in term of mortality and survival when performed on an elective basis and curatively.
OBJECTIVE: To determine the frequency of malignant neoplasia of the digestive tract in patients seen at the IMSS concentration hospital in Culiacán, Sinaloa, México. METHODS: From 32,097 histopathologic studies performed at No. 1 Regional General Hospital Pathology Department in Culiacán, Sinaloa, 406 were identified as digestive tract malignant neoplasia (DTMN); patient's age and gender were analyzed as well as involvement site and histological type. Benign and metastasic neoplasias were excluded. RESULTS: DTMN prevalence: 1.26%; more affected: 60-69 years; male predominance (66%), p < 0.0001. As a whole, 47.2% of the studies reported stomach cancer; colorectal cancer: 17.5%; gall bladder and bile duct cancer; 8.4%; liver cancer: 8.1%; the remainder were located in other sites. DTMN rate by histologic type were similar to other concentration hospitals of the country. CONCLUSIONS: DTMN has an outstanding magnitude in IMSS Sinaloa patients if other hospital registrations are considered. There are possible regional factors that may contribute to this behavior.
Sixty two patients with a gastrointestinal carcinoma were evaluated pre-operatively by ultrasonography, CT scan and laparoscopy to seek liver metastases and/or peritoneal carcinomatosis. Performance levels of laparoscopy, ultrasonography and CT scan were comparable regarding the diagnosis of liver metastases. Laparoscopy was markedly better than CT scan and ultrasonography in the diagnosis of peritoneal carcinomatosis. Thus the investigation of choice for the detection of liver metastases is a single imaging technique. In contrast, if excision surgery is envisaged, all gastrointestinal carcinomas should be assessed by ultrasonography and/or CT scan and laparoscopy to definitely eliminate peritoneal carcinomatosis or small superficial liver metastases, in such a way as to avoid unnecessary laparotomy.

This study was designed to evaluate the most effective administration method of NCS for advanced carcinoma of the stomach, mainly nonresectable and/or recurrent cases which were collected by our cooperative study group. Nine hundred seventy six cases were available for clinical evaluation. Rate of the efficacy of NCS alone and combined with 5-FU was higher (P less than 0.1) in the cases administrated intravenously by drip infusion than in those by one shot injection method, and adverse effects were fewer in the former than in the latter cases. The rate of efficacy of NCS was higher in cases treated with 4,000 mu/day alone intermittently 2-3 times for a week than in those with 2,000 mu/day alone every day. The rate of clinical effect was higher in the cases treated with 4,000 mu of NCS combined with 50 mg of 5-FU intermittently than in those with 2,000 mu of NCS combined with 250 mg of 5-FU every day, especially 20.6% of the former cases given more than 50,000 mu of NCS were interpreted as clinically effective by Karnofsky's criteria of I-A over.
The present study was designed to evaluate the efficacy of chemotherapy by NCS for advanced and/or recurrent carcinoma of the pancreas, mainly nonresectable cases in which exploratory laparotomy or construction of biliary fistula were performed. Three hundred fifty seven cases were available for the study. Of the 357 cases, 116 were treated with NCS alone 5-FU. In the former, the efficacy rate was higher in cases with 2,000 mu/body of NCS every day than in those with 4,000 mu/body of NCS intermittently 2-3 times for a week. 19.6% of the cases treated with more than 40,000 mu of NCS alone in total were interpreted as effective clinically by Karnofsky's criteria of I-A over. In the latter, the efficacy rate was higher in cases with 2,000 mu/body of NCS combined with 250 mg of 5-FU every day than in those with 4,000 mu of NCS combined with 500 mg of 5-FU intermittently, 22.5% of the cases given more than 40,000 mu of NCS in total were interpreted as effective clinically by Karnofsky's criteria of I-A over. The major adverse effects of NCS such as anorexia, vomiting and nausea were observed in 20% of total cases respectively, leucopenia in 10% and fever in 15% of them.
Biological methods for cancer therapies are very important. A small and efficient target carrier is the key component for anti-cancer drugs. In our laboratory, the antibody IgY was extracted from egg yolk of a SPF hen. The SPF hen was immunized with an antigene of P110 protein which was purified from human stomach cancer MGC-803 cells. Results indicated that the antibody IgY can specifically recognize gastrointestinal system cancers. It may become an important carrier for antitumorigenic drugs.

Scutellaria barbata (S. barbata), a traditional Chinese herbal medicine native to southern China, is widely used as an anti-inflammatory and a diuretic in China. Several studies have indicated that extracts of S. barbata have growth inhibitory effects on a number of human cancers. Treatment of lung cancer, digestive system cancers, hepatoma, breast cancer, and chorioepithelioma by S. barbata extracts was reported. However, the mechanism underlying the antitumor activity was unclear. In this study, we studied the growth inhibitory effect of S. barbata and determined its mechanism of antitumor activity using human lung cancer cell line A549. Our results showed that ethanol extracts of S. barbata greatly inhibited A549 cell growth, with IC50 of 0.21 mg/ml. The major mechanisms of inhibition included cell apoptosis and cytotoxic effects. cDNA microarray analysis showed that 16 genes, involved in DNA damage, cell cycle control, nucleic acid binding and protein phosphorylation, underwent more than 5-fold change. These data indicated that these processes are involved in S. barbata-mediated killing of cancer cells. A surprising finding is that CD209, related to dendritic cell (DC) function, was dramatically downregulated by 102-fold. Further functional studies are needed to assess the role of the array-identified genes in S. barbata mediated anticancer activity.
A retrospective cohort mortality study of 10,763 Amoco Corporation oil refinery workers employed between 1970 and 1980 showed low overall mortality; the standardized mortality ratio for all causes of death was 73 for white males and 68 for black males. White male mortality was examined by several exposure classifications devised for group cohort members by potential for exposure to refinery processes and exposure to two components of petroleum (light aromatic hydrocarbons and heavy oils). Statistically significant elevations were found in various exposure groups for skin cancers, digestive system cancers, and benign neoplasms. Of these, skin cancer mortality appeared to increase with increasing exposure.
