Computed tomographic anatomy of the buttock.

Computed tomography is an ideal method for imaging the buttock. Thorough knowledge of the anatomic structure of the buttock is necessary to accurately evaluate the many pathologic processes that originate from or extend into the buttock. A detailed review of buttock anatomy is presented here, with particular emphasis on cross-sectional anatomy as seen by computed tomography.

Gunshot wounds to the buttocks: predicting the need for operation.

BACKGROUND: Gunshot wounds to the buttocks may cause significant intra-abdominal injuries. Policies of aggressive abdominal exploration or extensive diagnostic testing have been suggested to avoid delays in treatment and consequent morbidity. Our group has recently suggested that clinical examination is a safe and reliable tool for triaging patients with anterior and posterior abdominal gunshot wounds. OBJECTIVE: This study was undertaken to test the hypothesis that patients with gunshot wounds to the buttocks can be managed selectively on the basis of clinical findings. SETTING: A large academic Level I trauma center was the setting for this study. PATIENTS AND METHODS: Fifty-nine consecutive patients, suffering from gunshot wounds to the buttocks with potential retroperitoneal trajectories, were managed during a 12-month period in our center. RESULTS: Based on clinical findings, 19 (32.2 percent) patients were operated on, with significant intra-abdominal injuries in 17 (28.8 percent). The remaining 40 (67.8 percent) patients were successfully observed. There were no missed injuries or delays in diagnosis. Sensitivity and specificity of clinical examination for identifying significant intra-abdominal injury was 100 percent and 95.3 percent, respectively. CONCLUSION: Clinical examination is a safe method for selecting patients with gunshot wounds to the buttocks for nonoperative treatment.

Autoprosthesis buttock augmentation during lower body lift.

With the increasing popularity of bariatric surgery, patients with multiple body contour deformities have become more common in plastic surgery practice. Most of the deformities involving the abdomen, thighs, and buttocks can be effectively corrected with belt lipectomy and lower body lift. A common problem with this procedure is postoperative loss of gluteal projection and resulting flattened buttock contour, which is directly proportional to the extent of lower body lift achieved. The use of local myocutaneous flaps to provide coverage for the lumbosacral defects is a common plastic surgery procedure. The authors have used these techniques to create an autologous buttock implant for additional projection during a lower body lift. A local myocutaneous flap originating within the regularly excised supragluteal tissue is rotated caudally to function as an autologous buttock implant. This flap has reliable circulation, can be custom designed for each patient, requires minimal additional operating time, and allows the creation of more than one flap if necessary. This article describes the results of this procedure used for 20 consecutive women. There were no major complications, and the most common minor complications included delayed wound healing and local hardness in the area, suggesting fat necrosis, which resolved without intervention in a few months. High patient satisfaction combined with a low complication rate suggests that this reliable, versatile technique nicely complements the lower body lift procedure.

Late intracapsular seroma in subfascial buttock augmentation: a case report.

Late intracapsular seroma and hematoma have been described frequently as a complication of breast augmentation surgery. However, there are few reports of late intracapsular seroma in buttock augmentation. Textured and polyurethane implants have been used for subfascial enlargement of the buttocks. In this article, the authors report a large late intracapsular bilateral seroma caused by a retrofascial polyurethane-covered implant used for buttock augmentation: 1,200 ml on one side and 300 ml on the other. This patient's intracapsular seroma was of sudden onset 2 years and 6 months after surgery, and apparently had no relation to any trauma, use of medication, or physical exercise. Local tissues, including the gluteal fascia, were distended such that the implant was replaced inside the muscle and there was no relapse of the seroma. Late intracapsular hematomas or seromas from implants for buttock and breast augmentation may have the same etiology, and textured implants seem to be involved. All the reports on the matter are associated with textured implants. Any late seroma or hematoma caused by a smooth implant is related to an additional factor that may be considered as the etiologic agent. Further studies aimed at a large-scale assessment of the cause and actual incidence of late seromas and hematomas from textured implants are required.

Model experiments to study the stress distributions in a seated buttock.

Mechanical stress states that develop in the buttock during sitting may exceed tissue tolerance and lead to decubitus ulcer formation in susceptible patients, such as those with spinal cord injury. The danger of this complication can be reduced by using suitable cushions to minimize stress magnitudes and gradients within soft tissues. In this investigation, a two-dimensional physical model of the buttock-cushion system was developed to aid in cushion design. The model consists of PVC gel simulating flesh, cast around a wooden core simulating the ischium bone. A grid etched on the gel permits measurement of strains via photographs of the undeformed and deformed model buttock supported by various cushion materials. The displacement field is analyzed, using a finite strain theory and a strain energy function, to obtain the "tissue stresses'. In this manner, the performances of five clinically used cushion materials were compared with respect to the high stress regimes developed in the model buttock.

Near-infrared spectroscopy and transcutaneous oxygen pressure during exercise to detect arterial ischemia at the buttock level: comparison with arteriography.

BACKGROUND: Noninvasive tests are required to detect (in both male and female subjects and side by side) arteries toward the hypogastric circulation that are likely to present significant lesions as a cause of buttock claudication. METHODS: We compared the accuracy of near-infrared spectroscopy (NIRS) and transcutaneous oxygen pressure (TCP o 2 ) on both buttocks during walking tests to detect lesions on the arteries toward the hypogastric circulation. NIRS was considered abnormal if recovery time to pre-exercise values was greater than 240 seconds for tissue oxygen saturation (absent data being coded 0), and TCP o 2 was coded abnormal if the minimal value of buttock changes minus chest changes was lower than -15 mm Hg. The study was conducted in a university hospital; there were 30 ambulatory patients with stage 2 claudication of the Fontaine classification. RESULTS: Angiography showed 36 abnormal (stenosis > 75%) and 24 normal arterial axes toward the buttocks circulation. NIRS and TCP o 2 provided respectively 55% (range, 41.6% to 67.9%) and 82% (range, 69.6% to 90.5%) accuracy (95% confidence interval) to predict the presence of arteriographically proven lesions; P < .05. CONCLUSIONS: Using available cut-off points proposed in the literature, NIRS showed a lower diagnostic accuracy than TCP o 2 for the prediction of lesions on the arterial tree to the hypogastric circulation. NIRS is a recent technique as compared with TCP o 2 , and its diagnostic accuracy might improve in the future. Currently, one should carefully weigh the advantages and limits of NIRS and TCP o 2 when a choice is to be made between them to monitor exercise-induced changes resulting from lower limb arterial disease at the proximal level.

Construction of a female urethra using the vaginal wall and a buttock flap: experience with 40 cases.

METHODS: From 1975 to 1996 a urethra was constructed using tubularized anterior vaginal wall covered with a buttock flap in 40 female patients in whom urinary incontinence was associated with a short or absent urethra. The underlying pathology included bilateral single ectopic ureters, cloacal malformation, urogenital sinus malformation, previous failed surgery of the urethra, severe trauma, myelodysplasia, female hypospadias, ectopic ureterocele with destruction of the urethra, congenital epispadias, bladder exstrophy, and previous hysterectomy and vaginectomy for clear cell carcinoma caused by intrauterine exposure to diethylstilbestrol. Other reconstructive procedures in these patients included bladder neck narrowing from above in 33 patients, ureteral reimplantation in 35, and bladder augmentation in 21. In the prone position, a tube of vaginal wall was used to create a urethra when it was absent or lengthen the urethra if it was too short. The neourethra was extended up to the base of the clitoris, using a multilayered closure of soft tissue over it distally with introital muscle and adjacent mucosa. The proximal neourethra was covered with a buttock flap. RESULTS: All patients were originally wet. Thirty-four are now dry. Four patients have minimal stress incontinence. Two patients have urinary diversions. The majority (n=25) void normally. Fourteen patients self-catheterize to empty their bladders. One patient wears an external appliance on the abdominal wall for collection of urine. Complications included: creating stress incontinence in one patient who had been previously dry by extracting a stone from the bladder, necrosis of a buttock flap from applying a tight perineal dressing, fistula requiring surgical closure in three patients, and slippage of the buttock flap in one patient. CONCLUSION: This operation is a useful method to correct diverse congenital and acquired pathology that causes incontinence in girls.

Buttock augmentation: case studies of fat injection monitored by magnetic resonance imaging.

This article examines the injection of megavolumes of autologous fat cells as a means of buttock augmentation in 162 patients over a 7-year period. The author documents the use of magnetic resonance imaging in six patients to visualize the intramuscular location, integration, and duration of the injected fat. With the patient under epidural or general anesthesia, fat cells were harvested with a 5-mm blunt cannula and then stored in an empty sterile intravenous bag or bottle trap. Decantation was the only process used to separate the fat cells from the saline and serosanguineous components. Up to 1260 cc of fat cells were been injected into each buttock, the largest amount of fat grafting ever reported. Clinical assessment estimated a 20 percent loss of augmentation effect during the first 4 months. Patients were generally pleased with the final shape and volume of the buttock contour. In follow-up evaluation, magnetic resonance imaging supported the clinical indicators that the injection of large quantities of fat cells appears to be a safe and effective method for buttock enhancement. This process has inherent advantages; nevertheless, further research is required to clarify our understanding of the predictability and longevity of this technique.

Estimation of the functional role of arterial pathways to the buttock circulation during treadmill walking in patients with claudication.

The aim of the study was to estimate the functional contribution of the arterial inflow pathways to the pelvic circulation during walking in patients with stage 2 lower extremity arterial disease. Transcutaneous oxygen pressure (Ptc(O(2))) changes during exercise can be used to estimate the severity of regional blood flow impairment while walking. Seventy patients with stable lower limb claudication were studied using a multivariate linear regression model. The relationship between exercise-induced buttock Ptc(O(2)) changes, the ipsilateral calf Ptc(O(2)) changes, and the arterial diameters of the pelvic arteriographic pathways were analyzed. The ipsilateral hypogastric and lumbar pathway, as well as the ipsilateral calf Ptc(O(2)) changes, were the only variables significantly related to buttock Ptc(O(2)) changes (r = 0.47; P < 0.001). Their normalized respective contribution to the regressive model was 39%, 19%, and 18%. None of the contralateral hypogastric, mesenteric, and sacral pathways or pathways stemming from the external iliac artery showed significant correlation to buttock Ptc(O(2)) changes. The ipsilateral hypogastric and ipsilateral lumbar pathways are the major pathways responsible for the functional buttock blood flow supply during walking. The role of contralateral hypogastric, inferior mesenteric, and median sacral pathways and arteries distal to the internal iliac trunk is negligible in the normal or compensatory blood flow supply. Distal Ptc(O(2)) decrease at exercise aggravates proximal Ptc(O(2)) decrease, possibly through the occurrence of a "steal phenomenon" of distal over proximal circulation during walking.

Reliability of fetal buttock blood sampling in assessing the acid-base balance of the breech fetus.

This prospective investigation was undertaken to compare fetal buttock capillary blood pH and umbilical artery and vein pH in fetuses presenting by the breech. The study group comprised ten term patients with singleton fetuses in the frank breech presentation who met established criteria for vaginal breech delivery. All patients had a normal labor pattern. Eight fetuses had a normal fetal heart rate tracing and two had severe variable decelerations during the second stage of labor. We obtained capillary blood for pH measurement from the fetal buttock within 15 minutes of delivery. Immediately after delivery, we collected arterial and venous blood samples from the umbilical cord for measurement of pH. In all fetuses, there was a highly significant correlation between fetal buttock capillary blood pH and umbilical artery pH (r = 0.96; P less than .001) and umbilical vein pH (r = 0.88; P less than .001). We conclude that measurement of pH in capillary blood obtained from the fetal buttock provides accurate assessment of the acid-base status of the breech fetus.

Exposure of buttock burn wounds to stool in scald-abused infants and children: stool-staining of eschar and burn wound sepsis.

Between July 1, 1987 and June 30, 1990, 30 consecutive deliberately scalded children with buttock involvement were prospectively studied. Mean age was 22.5 months. Mean burn size was 18.1 per cent TBSA (total body surface area). Thirty per cent (n = 9) had diarrhea complicate their wound or autograft care. For those requiring surgery for their buttock/perineal burns, various combinations of preoperative mechanical bowel prep, oral antibiotics, postoperative occlusive intrarectal catheter, nothing-by-mouth, and rigid postoperative positioning did not protect buttock wounds and autografts from stool. Four patients had stool staining of the superficial burn wound exudate, none of whom developed burn wound sepsis or died. Four (13.3%) patients with a mean burn size of 32.3 per cent TBSA, diarrhea, and burns involving the buttock, perineum, and external genitalia died of burn wound sepsis, three of whom had deep stool staining of their burn wound and Gram-positive bacteremia. Buttock burn wounds should be examined carefully and frequently for the presence of deep stool staining, an ominous predictor of burn wound sepsis and death. Such wounds, if present, should be emergently excised.

Circulating buttock cells in non-Hodgkin's lymphoma.

Leukemic B cells with a characteristically sharp nuclear cleft seemingly dividing the nucleus into two or more parts have been entitled "buttock cells" and are subject of this study. These cells were found in leukemic non-Hodgkin's lymphomas (NHL) and usually have been related to follicular center cell lymphomas. However, buttock cells also closely resemble cells present in intermediate lymphocytic lymphoma (ILL) and mantle zone cells of reactive lymphoid tissues. Ultrastructurally, it became apparent that the separate nuclear lobes of buttock cells were connected by chromatin bridges. Immunophenotypically, circulating buttock cells had a variable phenotype, which may indicate either a follicle center or mantle zone origin. The use of CD5 and FMC7 monoclonal antibodies might be of discriminative help. These leukemic NHL have to be differentiated from classical chronic lymphocytic leukemia (CLL) with help of cytomorphology and immunophenotyping, since the former usually have a worse prognosis and generally will require a different treatment.

Hip and buttock implants to enhance the feminine contour for patients with HIV.

The antiretroviral therapy for patients with human immunodeficiency virus (HIV) causes lipodystrophy, or a change in the distribution of body. Treatment for the facial changes is well addressed and covered in the recent literature, but female patients also report changes in their buttocks and lower limbs. There is no treatment for the lower limb deformity, but plastic surgeons can do something for the buttock. The authors propose a classification for the deformities of these patients and a new solution to improve the contour of this area and to reduce the social impact of deformity on women with HIV. This consists of placing two silicone implants, in the buttock and on the hip, to give a rounder appearance. The authors think that hip implants may be indicated also for gender reassignment surgery and for women with masculine features.

Beautiful buttocks: characteristics and surgical techniques.

The authors describe four characteristics that determine attractive buttocks in addition to the balanced anatomical dimensions of the maximal point of gluteal projection. Based on this information, defects of the gluteal region can be classified into five types. The most appropriate size and shape of gluteal implants and the proper surgical technique for gluteal augmentation can be chosen by first identifying the buttock type ofa patient. The proposed method of buttock classification helps achieve a gluteal contour that closely matches the standard of beauty.

Buttocks lifting: how and when to use medial, lateral, lower, and upper lifting techniques.

New buttocks-lifting techniques are evolving rapidly, with some so recent that very few surgeons have performed them. However, a few new principles may be drawn from the various techniques in an attempt to produce better results and minimize scars. The indications for use of a medial, lateral, lower, or upper buttocks lift are presented in this article. There is no technique for all seasons and a surgeon who intends to perform a buttocks lift must evaluate the four techniques and the indications for each. An intramuscular implant helps to improve results even further and should be used when appropriate.

Meconium pseudocyst presenting as a buttock mass.

A newborn 2.4-kg baby girl with a prenatal diagnosis of sacrococcygeal teratoma by ultrasound had a soft cystic mass extending from the infracoccygeal region into the right buttock. The cyst contained meconium leading to a large retroperitoneal cavity. The cyst was drained through an extended buttock incision, and healed without a site of bowel perforation ever identified. Meconium pseudocyst should be included in the differential diagnosis of sacrococcygeal and buttock tumors in the newborn.

Weighted speculum buttock burn during gynecologic surgery.

Genital tract and perineal electrical burns during resectoscopic surgery are occasionally encountered. However, perineal or buttock burns from other causes have not been reported.A 44-year-old overweight woman with a history of menometrorrhagia underwent endometrial ablation with a thermal balloon device uneventfully under general anesthesia. Upon removal of the weighted speculum it was noted that the buttocks had sustained two oval burns under the speculum ball. Experiments demonstrated that the weighted speculum cools off differentially, with the ball temperature remaining at higher than 45C in room air for at least 30 minutes after autoclaving.The use of the weighted speculum within 30 minutes of autoclaving may result in buttock burns. The speculum cools to lower than 40C within 1 minute when rinsed or bathed with at least 1 L of saline solution.

[Actinomycosis of the buttock].

INTRODUCTION: Cutaneous actinomycosis of the buttocks is a rare granulomatous bacterial infection that usually starts in the perianal area. We present an exceptional case in the form of a pseudo-tumor. CASE REPORT: A 69 year-old woman, in general good health, developed an indurate mass on the supra-external quadrant of the right buttock. The tumor was centered by an ulcerated nodule with a diameter of around 10 centimeters. Imaging showed invasion of the soft tissue of the skin in the internal psoas muscle, the adipose tonality of which was compatible with a liposarcoma. The skin biopsy revealed characteristic bacterial grain in the center of a cholesterol granuloma. Subsequent culture in aerobic milieu identified Actinomyces gerencseriae. Cure was obtained following complete exeresis of the fibrous tissue and 8 months of antibiotic amoxicillin-clavulanic acid therapy. DISCUSSION: Other than the most unusual clinical aspect, the originality of this case of actinomycosis of the buttocks is based on its potential appendix origin, 4 years after acute appendicitis, with slow posterior fistulation. Other cases of actinomycosis of appendix origin have been reported and its delayed onset following the intervention has been documented. The pseudo-sarcomatous aspect was responsible for diagnostic wandering. The histological image and, subsequently, the results of the bacteriological culture confirmed the diagnosis.
