[Successful therapeutic use of intraglobin-F in idiopathic immuno-thrombopenia, refractory to corticosteroid and immunosuppressive therapy as well as to splenectomy].

The authors present the case of a 21-year old woman patient who was admitted to hospital because of typical symptoms of chronic idiopathic thrombopenic purpura. Prednisolone therapy alone and subsequently combined with Immuran as well as later applied large dose of Methylprednisolone failed to be effective. Owing to the ineffective therapies and increasing hemorrhagic diathesis splenectomy was performed under perioperative Venagamma protection without success. Intraglobin-F therapy brought perfect recovery. The authors review the expectable results of immunoglobulin therapy, the strategy of ITP therapy changed as a consequence of the new therapeutic possibility as well as the indications of immunoglobulin treatment.

Activation of the cytokine network by muroctasin as a remedy for leukopenia and thrombopenia.

N2-[(N-Acetylmuramoyl)-L-alanyl-D-isoglutaminyl]-N6-stearoyl-L-lysine (MDP-Lys(L18), muroctasin), a derivative of muramyl dipeptide (MDP), is known to promote recovery of peripheral leukocyte and platelet counts by inducing cytokines, especially colony stimulating factor (CSF) in myelosuppression secondary to anticancer therapy. This paper reviews the results of a double-blind comparative study on lung cancer patients, administered either 200 micrograms or 20 micrograms of muroctasin for 6 days post-chemotherapy. It demonstrates the efficacy of muroctasin, at 200 micrograms doses levels only, in promoting early recovery of leukocyte and platelet counts and hence the potential for earlier initiation of chemotherapy cycles. The most common side effect encountered during the study was transient fever.

[Platelet aggregation tests in 26 cases of heparin-induced thrombopenia. Methodological, diagnostic problems and therapeutic aspects].

A diagnosis of heparin induced thrombocytopenia (HIT) in the 26 patients was based on: 1. normal platelet count prior to heparin administration; 2. its fall to less than 100 Giga/l (m = 46 +/- 23) at time of first sample collection for test to detect a platelet aggregation factor (PAF); 3. restoration of normal values after discontinuation of heparin treatment during which the thrombocytopenia had appeared. The result of the first PAF test in these 26 patients was positive in 22 cases, negative in 2; twice the plasma provoked platelet control aggregation without in vitro addition of heparin. The origin, dose and mode of administration of the heparin did not appear determinant in the production of the thrombocytopenia: 16 of the patients were later treated with a low molecular weight heparin (LMWH). An in vitro "compatibility" test was able to be performed 8 times previously and was negative in 7 cases in the 16 patients. Samples were collected during LMWH treatment and were negative in 13 cases, and this in agreement with the increased platelet count after 7 +/- 3 days and the clinical improvement. In 3 patients the test was positive: in one case the count did not return to normal, in the second case this did occur, but slowly (21 days); the PAF test in the last patient was positive prior to LMWH treatment and remained so while the count became normal. Concordance exists therefore between negativity of the test practiced with LMWH and the increase in count when this heparin is administered; inversely, the positivity of the test does not exclude normalization of the platelet count.

[Thrombogenic thrombopenia related to heparin. Clinical, biological and therapeutic results. Apropos of 32 cases].

Heparin induced thrombocytopenia is characterized by often dramatic evolution of thrombotic arterial and venous complications. These occurred or are worsened in curative or preventive heparin therapy and the treatment remains delicate. The authors report 32 observations of thrombocytopenia complicated with thromboembolic events; seven deaths are to mention. The tests of platelet aggregation with standard heparin and platelets poor patient's plasma confirm the diagnosis in 28 cases of 30 very early studied and in 2 cases around the 6th day only after the stop of standard heparin. The choice of anticoagulant therapy is carried out the negative tests of platelet aggregation with low molecular weight heparin (L.M.W.H.) (CY 216, CY 222 Choay, PK 10109 Pharmuka). In L.M.W. heparin therapy, clinical and biological improvement is obtained in 26 cases of 30 treated cases. In three cases, the rapid climbing of platelet countings is not present although negative tests of platelet aggregation with selected L.M.W.H. In one case, after a initial climbing of platelet countings, the thrombocytopenia recurs rapidly with L.M.W. heparin after a operation. The immediate or secondary passage with K-antivitamins, platelet antiaggregant stabilized medium and long term's evolution.
