HIV-AIDS in Nepal: the coming crisis.

The number of officially reported cases of HIV infection and of AIDS in Nepal remains low in comparison with numbers in many other Asian countries. But Nepal's open border with India (where HIV infection rates are rapidly rising) and the high level of physical mobility within Nepal and abroad, associated with widespread labor migration and encouraged by the recent development of road transport, means that there is a real danger of a rapid spread of HIV within Nepal. The major means of infection is through heterosexual encounters involving male clients and female sex workers, but other sections of the population are also at risk from infection. Media attention has focused on female sex workers, particularly those who have worked in India, but the issue is far broader than this. Social and economic factors forcing or encouraging young men and women to seek employment away from home underlie the widespread growth of ¿the sex industry¿ and the ¿trafficking¿ of girls and young women. The state's capacity to respond effectively is limited, in part through lack of resources; international agencies are supporting local nongovernmental organizations in a variety of activities designed to ¿educate¿ and to ¿support¿ those at risk; but in the last analysis, it is the local communities from which young men and women migrate and to which they return that are obliged to find ways of coping. Sometimes these communities are supportive of the victims, sometimes not. The spread of HIV-AIDS is not just a ¿health¿ issue, but an issue of economic and social development, of gender relations and of human rights.

Urbanization patterns: European versus less developed countries.

This paper develops a model in which the interaction between transport costs, increasing returns to scale, and labor migration across sectors and regions creates a tendency for urban agglomeration. Demand from rural areas favors urban dispersion. European urbanization took place mainly in the 19th century, with higher costs of spatial interaction, weaker economies of scale, and a less-elastic supply of labor to the urban sector than in less developed countries (LDCs) today. These factors could help explain why primate cities dominate in LDCs, whereas a comparatively small share of urban population lives in Europe's largest cities.

Spinal cord stimulation electrode design: a prospective, randomized, controlled trial comparing percutaneous with laminectomy electrodes: part II-clinical outcomes.

OBJECTIVE: Spinal cord stimulation, in use for more than 30 years, has evolved into an easily implemented technique involving percutaneous or laminectomy electrode placement. In a randomized comparison of four-contact percutaneous and four-contact insulated laminectomy electrodes placed at the same level in the dorsal, epidural midline, quantitative measures of stimulator performance revealed significant technical advantages for the laminectomy electrodes. Our prospective, randomized, controlled trial compares clinical results in these same patients. METHODS: Impartial third parties followed our series of 24 patients with failed back surgery syndrome to gather clinical outcome data. We defined "success" as at least 50% sustained relief of pain and patient satisfaction with the result of treatment. RESULTS: At a mean follow-up of 1.9 years, 10 of 12 patients receiving the laminectomy electrode and 5 of 12 patients receiving the percutaneous electrode reported a successful outcome (P < 0.05). Follow-up at a mean of 2.9 years showed that this result was maintained in 5 of 12 patients with the laminectomy electrode and 3 of 12 with the percutaneous electrode (not statistically significant). Many patients reported improvements in most activities of daily living, and loss of function was rare. In addition, 9 patients with laminectomy electrodes and 4 with percutaneous electrodes reducted or eliminated analgesic intake (P < 0.05), and 2 returned to work. No electrode migration was observed. CONCLUSION: Laminectomy electrode placement, although more invasive than percutaneous placement, yields significantly better clinical results in patients with failed back surgery syndrome at mean 1.9 years follow-up. In our small sample, however, the statistical significance of this advantage disappeared at mean 2.9 years follow-up.

Effects of dexamethasone, levamisole, and dexamethasone-levamisole combination on neutrophil function in female goats.

Effects of dexamethasone, levamisole, or combined dexamethasone-levamisole administration on polymorphonuclear neutrophil (PMN) function in healthy, adult female goats were studied. Goats were assigned to treated (n = 6) and control (n = 6) groups. In experiment 1, treated goats were given levamisole (6 mg/kg of body weight, IM). In experiment 2, treated goats were given 0.1 mg of dexamethasone/kg, IV, for 3 consecutive days, 1 mg of dexamethasone/kg, IV, for 6 consecutive days, and 6 mg of levamisole/kg, IM, with a 4th injection of 1 mg of dexamethasone/kg. All injections were administered 12 hours before blood collection. The PMN were evaluated for random migration and chemotaxis under agarose, ingestion of Staphylococcus aureus, cytochrome C reduction, iodination, and antibody-dependent cell-mediated cytotoxicity. Levamisole alone did not alter the function of caprine PMN. Both doses of dexamethasone caused increased random migration and decreased cytochrome C reduction and iodination. Dexamethasone resulted in no changes in chemotaxis, S aureus ingestion, and antibody-dependent cell-mediated cytotoxicity. Random migration and cytochrome C reduction returned toward base line in cells from dexamethasone and levamisole-treated goats. Although iodination activity in cells from dexamethasone-treated goats remained significantly (P less than 0.05) lower than those of controls after levamisole administration, a rebound toward base-line activity occurred.

Embryonic migration relative to maternal recognition of pregnancy in sheep.

Twenty-five crossbred ewes were utilized to examine the timing of embryonic migration relative to maternal recognition of pregnancy. These ewes also were utilized to examine whether ovine embryos synthesized estradiol-17 beta in association with embryonic elongation and intrauterine migration. Embryos were flushed on d 11 through 15 from hemiovariectomized ewes. Recovery of embryos from the uterine horn contralateral to the remaining ovary indicated that migration had occurred. Ewes subsequently were returned with rams to determine their interestrous interval. Recovered embryos were classified morphologically, their length determined and individually incubated. Changes in estradiol within the medium were determined after a 6-h incubation. Embryo migration began on d 14 (P less than .05); on consecutive days from 11 through 15, 0, 0, 0, 60 and 100% of ewes examined, respectively, had an embryo in the contralateral horn. Extended estrous cycles (greater than 20 d) were observed in 0, 0, 40, 80 and 100% of ewes examined (P less than .05) following removal of embryos on d 11 through 15 of the cycle. Ovine embryos were longer (P less than .05) on d 14 (4.8 +/- 1.1 cm length, mean +/- SE) compared with d 13 (.2 +/- .1 cm) and increased further (P less than .05) on d 15 (7.8 +/- 1.1 cm). Incidence of intrauterine migration was correlated with embryonic length (r = .83; P less than .01) and estradiol synthesis (r = .77; P less than .01).(ABSTRACT TRUNCATED AT 250 WORDS)

Canine intestinal myoelectric activity after open versus laparoscopically assisted right hemicolectomy.

BACKGROUND: It is a common belief that a laparoscopic procedure results in a shorter duration of postoperative ileus compared with the equivalent open procedure. This study was undertaken to determine whether laparoscopically assisted right hemicolectomy in the dog results in a shorter duration of ileus compared with open right hemicolectomy. METHODS: Eight bipolar serosal electrodes (4 on the small bowel, 4 on the left colon) were implanted in each dog (n = 10). Three weeks after electrode implantation baseline recording was made for 5 days; then 5 dogs underwent laparoscopically assisted right hemicolectomy and 5 underwent open right hemicolectomy. Myoelectric activity was recorded continuously for 72 hours postoperatively. Tracings were analyzed for the time of reappearance, duration, migration velocity, and cycle length of phase 2, phase 3, and the migrating colonic complex. The criteria used for the resolution of postoperative ileus were the return of phase 2, phase 3, and the migrating colonic complex. RESULTS: All dogs had temporary loss of organized myoelectric activity postoperatively. The mean reappearance time (minutes +/- standard deviation) for phase 3 was 857 +/- 574 versus 761 +/- 600; the phase 2 reappearance time was 1,845 +/- 610 versus 1,590 +/- 668; and the migrating colonic complex reappearance time was 534 +/- 365 versus 572 +/- 552, open versus laparoscopically assisted right hemicolectomy, respectively. The times were not different (Wilcoxon rank sum test, P > 0.05). The time required for phase 3, phase 2, and the migrating colonic complex to attain preoperative configuration also was not different between the open and laparoscopically assisted group. CONCLUSION: Myoelectric resolution of postoperative ileus did not occur earlier in the dog undergoing laparoscopically assisted right hemicolectomy compared to the dog undergoing open right hemicolectomy. This data does not support the hypothesis that a laparoscopically assisted colectomy results in a shorter duration of postoperative ileus than the equivalent open procedure.

The effect of levamisole on postoperative immunosuppression.

Cell mediated immunity was studied before and after surgery in patients with colorectal cancer. Thymus-derived (T) lymphocytes were enumerated and their non-specific activity was measured by assessing phytohaemagglutinin (PHA)-induced blastogenesis. Specific T lymphocyte activity was measured by leucocyte migration inhibition in response to purified protein derivative of tuberculin (PPD) and autochthonous tumour extract. One group of patients received levamisole postoperatively and the other group consisted of controls. There was a more rapid return to preoperative levels of leucocyte migration inhibition to both antigens by the levamisole group. However, levamisole administration had only a marginal effect on the postoperative depression of cell counts and did not restore depressed lymphoblast transformation.

Structural and functional roles of cytoskeletal proteins during repair of native guinea pig intestinal epithelium.

The cytoskeletal events that assist restitution of the native intestinal epithelium are poorly understood. To enhance our understanding of repair mechanisms in the native intestinal epithelium we assessed the functional role of actin and the temporal and spatial alterations in actin and villin that occur in native enterocytes migrating in response to injury. Using a well-characterized in vitro Ussing chamber model of native intestine epithelial restitution, the actin inhibitor cytochalasin D (CD) was applied to determine the functional importance of actin to restitution as assessed by sensitive electrophysiological means and structural techniques. Additionally we used phalloidin and indirect immunohistochemistry to localize and semiquantitate F-actin and villin in migrating cells during restitution. We report new data that shows that when cytoskeletal changes were impaired with CD, the epithelial monolayer was re-established in fewer than 20% of CD-treated villi, cells bordering the epithelial defect did not assume the characteristic phenotype associated with migrating cells, and transepithelial resistance did not return to pre-injury levels. F-actin and villin were present at the leading edge of the migrating cells, basolateral F-actin was decreased, and cytoplasmic villin was increased as determined by phalloidin and immunohistochemical methods. We conclude that in vitro repair of the native intestinal epithelium is functionally and structurally dependent on major changes in the cytoskeleton of cells involved in re-establishing the epithelial monolayer over a complex extracellular matrix.

Place-to-place migration in Israel: estimates of a logistic model.

"A multinomial logit model focusing on economic and other locational factors is formulated and applied to data on place-to-place migration in Israel. Results indicate the effects of expected industrial wage differentials, in accordance with the hypothesis of Harris and Todaro (1970), and of disparities in the structure of industrial employment, suggesting that perceived risk as well as expected return enter into the decision to migrate, as Stark and Levhari (1982) have argued. Other effects include those associated with regional differentials in amenities and agglomeration associated with urbanization, population mobility by age group, center-periphery migration trends, border security hazards, and the like. Implications of the analysis for the Israeli policy of population dispersion are discussed."

Redox regulation of neuronal migration in a Down Syndrome model.

Down Syndrome (DS), one of the major genetic causes of mental retardation, is characterized by disrupted corticogenesis produced, in part, by an abnormal layering of neurons in cortical laminas II and III. Because defects in the normal migration of neurons during corticogenesis can result in delayed cortical radial expansion and abnormalities in cortical layering, we have examined neuronal migration in murine trisomy 16 (Ts16), a mouse model for DS. Using an in vitro assay for chemotaxis, our data demonstrate that the number of acutely dissociated Ts16 cortical neurons migrating in response to glutamate or N-methyl-D-aspartate (NMDA), known chemotactic factors, was decreased compared to normal littermates, suggesting a defect in NMDA receptor- (NMDAR-) mediated events. Ts16 neurons did not lack NMDAR since expression of mRNA and protein for NMDAR subunits was observed in Ts16 cells. However, the number of cells that generated an observable current in response to NMDA was decreased compared to normal littermates. Similar to DS, Ts16 CNS demonstrated an inherent oxidative stress likely caused by the triplication of genes such as SOD1. To determine if the abnormal redox state was a factor in the failure of NMDAR-mediated migration in Ts16, we treated Ts16 neurons with either n-acetyl cysteine (NAC) or dithiothrietol (DTT), known antioxidants. The reduction in NMDAR-mediated migration observed in Ts16 neurons was returned to normal littermate values by NAC or DTT. Our data indicate that oxidative stress may play a key role in the abnormal glutamate-mediated responses during cortical development in the Ts16 mouse and may have an impact on neuronal migration at critical stages.

A case of Aptos thread migration and partial expulsion.

BACKGROUND: In our practice, the use of dented polypropylene suture has become a very simple, conservative, and effective procedure for lifting flabby ptosed facial tissue, improving it without surgery. As with some cosmetic procedures, we have seen complications with this technique. OBJECTIVE: To present a case report of migration and partial expulsion of Aptos thread (TOTAL Charm, Moscow, Russia). METHODS: After successful fixation of facial tissue with 10 Aptos threads (5 on each side, 3 in the upper and central cheek, and 2 in the mandibular area), the patient returned 28 days later owing to migration and partial expulsion of one of the Aptos threads. RESULTS: The expulsed thread was easily removed, and the patient was treated with antibiotic therapy without further complications. CONCLUSION: After this experience, we can conclude that Aptos threads are an additional tool in our arsenal for the treatment of facial aging. We must be especially cautious and aware that our patients can present with previously unknown complications secondary to newly described procedures, as with this recent technique.

[Migration from Southern Italy to Argentina: Calabrians and Sicilians (1880-1930)].

"The study analyzes Italian emigration to Argentina from Sicily and Calabria between 1880-1930, compared with out-migration flows from Piedmont. The concepts of cultural patrimony and of migratory strategy are used to measure the different potentials and job opportunities in the Argentinean labor market as well as in the Italian context for those returning home. Considering the high proportion of returnees, a positive or negative correlation between region of origin and of destination can be proposed. Southern Italians indicate a more permanent settlement. The authors conclude that the Argentinean society in its Italian component is the result of Southern rather than Northern influences." (SUMMARY IN ENG AND FRE)

The voluntary repatriation process of Chilean exiles.

"The purpose of this paper is to analyse the main characteristics of the reintegration process of Chilean exiles returning to their country: the voluntary repatriation process of those who left Chile in 1973 when a military coup overthrew the democratically elected government, and who began to return to Chile in 1978, especially after 1990 with the re-establishment of democracy. Data have been obtained from secondary sources (publications, working papers, institutional documents), official publications provided by governmental agencies, and informal interviews with professionals assisting the returned population. The research was undertaken in situ in Santiago, Chile, during May and June 1992." (SUMMARY IN FRE AND SPA)
