[Correlation between the number of sessions and therapeutical effect in patients suffering from low back pain treated with acupuncture: a randomized controlled blind study].

AIM: The aim of this study is to verify the therapeutical effect of acupuncture reflexotherapy in low back pain treatment and to test if the number of sessions could influence the results at the end of therapy. METHODS: Thirty-one patients, suffering from low back pain, were randomly divided into 2 groups: the first group (16 patients, 5 males and 11 females, mean age 57.17 +/- 13.06 years) received 5 weekly somatic acupuncture sessions; the second group (15 patients, 4 males and 11 females, mean age 49.36 +/- 11.98 years) underwent 10 weekly somatic acupuncture sessions. The acupoints used were the same in both groups. Pain was monitored by a daily self rating chart. Pain was recorded using a card filled in by the patient, every day. At the end of therapy, a remaining pain scored between 0% and 50% of original pain was considered a good result; unsatisfactory result was a pain between 51% and 80%; poor result a score of 81% or more of original pain. RESULTS: In the first group, 11 patients (68.75%) obtained a good result, 1 patient (6.25%) an unsatisfactory result and 4 patients (25%) a poor result. The remaining pain was 65.5% of the original pain (unsatisfactory result). In the second second group, 13 patients (86.66%) obtained a good result and 2 patients (13.33%) a poor result. The remaining pain was 43.9% of the original pain (good result). CONCLUSION: Ten sessions of acupuncture seem to gain a better therapeutical effect than 5 in the treatment of chronic low back pain.

[The role of activation of lipid peroxidation of the tissues in response of the body to nociceptive stimuli].

Taking into consideration that activation of free radical processes is an indispensable consequence of any extreme action on man and animals and that lipid peroxidation (LPO) metabolites may play a role of original "primary mediators of stress", an attempt was made to define LPO participation in body response to nociceptive stimuli. Investigations carried out in patients belonging to different nosological groups, differing in the intensity of pain, established the presence of a direct relationship between the potency of painful action and LPO activation. LPO activation was also observed in experiments on animals exposed to painful tourniquetting of the limbs. The data obtained allowed a conclusion about possible participation of free radical lipid peroxidation processes in the formation of nociception. This may be confirmed by the circumstance that reflexotherapy determined remarkable LPO activation together with removal of the painful syndrome.

[Possibilities provided by chronopuncture in treatment of arterial hypertension in the young].

The aim of the study was to evaluate the effectiveness of anti-hypertensive therapy with application of chronopuncture as a non-drug method of arterial pressure correction in patients with essential hypertension (EH). An offered and introduced utility model was applied. Its application leads to achievement of maximal anti-hypertensive effect of reflexotherapy, which shortens the treatment and increases the effectiveness of acupuncture in treatment of EH.

[Pyoderma vegetans as an early sign of HIV infection].

Skin and mucosal lesions are described in a 40-year-old man who suffered from HIV infection in the stage of secondary diseases. The diagnosis was supported by demonstration of antibodies to HIV in the immune blotting test. The clinical manifestations were reduced to steady fever, generalized lymphadenopathy, pyoderma vegetans and chancriform pyoderma, candidiasis of the oral cavity, and seborrheic dermatitis, HIV infection was diagnosed 4 years after the appearance of pyoderma which initially was amenable by external antibacterial agents and then became resistant to them. Eruptions of pyoderma vegetans simulating wide condylomas were located in inguinal folds, on the internal surface of the thighs and scrotum; the chancriform focus was located in the internal preputial layer. The treatment with azothymidine combined with antibacterial and antimycotic agents led to the improvement of the patient's general status and to the reduction of local lesions. However, on recommendations of an unknown witch doctor who started treating him with the aid of reflexotherapy, phytotherapy and bioenergetic actions the patient stopped receiving the treatment prescribed and did not report for control examinations. After 5 months he died of brain edema.

[Encephaloangioscintigraphy in the diagnosis of posttraumatic cerebral venous circulatory disorders].

Encephaloangioscintigraphy performed in 30 patients with a closed craniocerebral trauma has shown simultaneous inhibition of the venous cerebral blood flow both in the vascular regions of the cerebral hemispheres and in the lateral sinuses indicating the common pathogenesis of disorders of the venous circulation in these parts of the circulatory bed. The most informative characteristic feature of the venous outflow is a period of RP half-life, determined on a radiocirculogram of the brain. The diagnostic efficacy of the above method in the detection of disorders of the venous circulation of the brain in some vascular regions provides an opportunity for a purpose-oriented use of adequate methods of reflexotherapy.

[Evaluation of the effectiveness of the program of primary prevention of arterial hypertension at an industrial plant based on a prospective epidemiological study].

Whether the primary arterial hypertension prevention programme can be applied among employees of a Novosibirsk industrial enterprise within the framework of the second stage of the USSR Cooperative Study of Arterial Hypertension Control, is studied. For this, the authors used sodium-restricted diet concurrent with non-drug therapy (physio-, balneo-, reflexotherapy and acupuncture), and active sanitary education. The 2-year prospective follow-up showed a sufficiently high efficiency of health measures: the incidence of borderline hypertension was reduced by 10.5% at the end of the study, blood pressures could be maintained at the level of the borderline and subnormal values in most patients with persistent hypertension, the patients at risk developed no persistent hypertension, cases. In a 20% blind sample from the matched population, the changes in the frequency of various blood pressures were insignificant during this period.

[Organization of therapy and rehabilitation of patients with sensorineural hearing disorders in the psychotherapeutic neurological center].

In 1979, a psychotherapeutic surdoneurological center consisting of an outpatient department and a 60-bed hospital was opened in the city of Dnepropetrovsk. The responsibility of the center is to render social and rehabilitation aid to patients with hearing loss of sensorineural origin. Rehabilitation procedures include non-drug therapy, psychotherapy, physical therapy, acupuncture and reflexotherapy, surdopedagogical lessons in the alert and hypnotic state, special talks to exclude discomfortable sensations, both of psychological and hearing type, related to the use of hearing aids. These rehabilitation procedures yielded the best results in pedagogically trained children who, at the age of 9 years, went to normal schools or schools for children with hearing loss of I-IV degree.

[Efficacy of ambulatory treatment of patients with primary chronic gastroduodenitis].

The authors studied the efficacy of the treatment of 80 patients with primary chronic gastroduodenitis on an ambulatory basis. Use was made of spasmolytics, cholinolytics, antacids, sedative agents together with reflexotherapy, dietetic management and physiotherapy. As a result of the treatment, positive changes were recorded in the clinical, functional (intragastric pH-metry with the sodium carbonate test), endoscopic, histomorphologic, morphometric and immunologic characteristics. The changes in the characteristics indicated were examined both shortly after the treatment (1 to 1.5 month) and in the long-term period. Duodenal ulcer developed in 4 persons (5%) in the long-term period, which is 5 times more seldom as compared to the reported data.

[Anatomo-topographical characteristics of various acupuncture points in children].

Peculiarities of layer-by-layer topography of tissues within the limits of the needle depth of certain acupuncture points of wide spectrum of action have been studied in children of early age. This investigation on anatomical topographic peculiarities of acupuncture points in children is necessary for ensuring security of needle reflexotherapy. Peculiarities of the following points have been described: Bâihui (VG-20), Waiguan (TR-5), Neiguan (MC-6), Yinlingquan (RP-9), Sanyinjiao (RP-6), Hegu (GI-4), Zusanli (E-36), Shenmen (C-7), Yanglingquan (VB-34).

[Complex stimulation of the locomotor and psycholingual develop- ment of children with perinatal lesions of the central nervous system].

In 121 children with perinatal CNS damage a combined therapy was performed including, besides routine drug treatment, imitation stimulation of age-matched posture-++-tonic attitudes and motor skills, metameric reflexotherapy aimed at the CNS region lesioned, magnetotherapy, electric laser puncture targeted at correction of dysfunctioning brain structures. Treatment efficiency was controlled by the brain "development profile" derived from formalized neurological and neuropsychological investigations, and electroneuromyography. The efficiency of the therapy was considerably decreased by the 3rd semester of life.

[Punctate transcutaneous electrical nerve stimulation in migraine therapy].

Pharmacotherapy of migraine is not satisfying at present, especially in case of frequent attacks of headache. Therefore alternative methods of treatment gain increasing relevance. Based upon many years of experience with specific reflexotherapy and conventional stimulation therapy for treatment of pain we started a clinical study in migraine applying the punctual transcutaneous electrical nerve stimulation (PuTENS) performed with self-made pocket electrostimulators (TRG I-III). The study includes self-treatment of patients at home. After more than 6 months PuTENS was prophylactically effective in 225 out of 282 patients, i.e. in 80% of cases the frequency of attacks and the use of drugs were reduced to at least 50%. In 90 patients treated by the physician the rate of improvement was 86% compared to 76% in 192 patients, which performed self-treatment according to the physician's instruction and under medical supervision. 96 out of 160 patients with migraine could alleviate their acute headache by PuTENS applying the pocket electrostimulator at home. In conclusion, PuTENS is superior to drug therapy and conventioned electrotherapy in the treatment of migraine. Its effectiveness is similar to needle acupuncture. The method can therefore be regarded as a valuable alternative and help for patients with frequent headache.

[Reflexotherapeutic effects on biologically active points of the concha auriculae].

By means of histochemical method, for revealing cholinergic nervous structures, and electron microscopy, innervation of biologically active points (BAP) and that of neutral areas of the rabbit ear skin has been studied, normal and after acu-, electro- and electroacupuncture. The BAP have more intensive vascularization and innervation, their specific feature is presence of well pronounced nervous fasciculi. The latter are formed by 6-10 fibers 1-6 mcm in diameter. The diameter of the fasciculi is within the limits 25-30 mcm up to 40-45 mcm. Under the electron microscopic investigation myelin and amyelin fibers are revealed in the nervous fasciculi. In the area of the epidermal basal layer and in the epidermis itself, single nerve terminals are found; they are considered as the point pain receptors. After acu-, electro- and electroacupuncture, intensity of the nervous fibers staining increases, thus demonstrating an increment of acetylcholine esterase activity. After insertion of acupuncture needles and after electrical irritation, the changes in the nervous and cellular elements in the BAP areas are studied electron microscopically. After the effects mentioned, mast cells situating in the BAP area become, as a rule, degranulated. After repeated electroacupuncture irritation of the BAP, an inflammatory focus appears with peculiarities specific for the given process. The reflexotherapeutic effect is supposed to be transferred via the nervous system. The mechanical irritation of the nerve fasciculi and the accompanying traumatization of the surrounding cellular elements initiate the mechanism of reflexotherapy.

[Ultrastructure of the skin after electroacupuncture].

Electron-microscopic examination of the nervous apparatus of the acupuncture points of the rabbit ear has shown that the motor receptor structure of reflexotherapeutic impacts seems to be bundles of nerve fibers with their microenvironment--blood vessels, mast cells, histiocytes. Acupuncture with electrical stimulation and acupuncture based on direct mechanical impact (trauma, compression) and the resultant local inflammation irritate the nerve conductors and initiate the effects of reflexotherapy.

[Acupuncture in ischemic pain of the extremities].

In this paper is put in evidence the efficacy of reflexotherapy (acupuncture) in the treatment of ischemic pain. The vasodilatation obtained brings back to a normal aerobic glicolisis in the ischemic tissues, especially in the muscles and consequentially a total or partial analgesic effect.

[Unusual use of reflexotherapeutic technics for control of pain in cases of phantom limb. Spinal and supraspinal theory].

Treatment of selected cases of amputees suffering from phantom-limbs pains by means of unusual techniques of reflexotherapy is reported. Nose, hand and foot acupuncture and classical auricolotherapy demonstrate in the patients here reported the beneficial effect of these methods. Nosologic, pathologic, clinic results and the anatomo functional mechanisms through which the therapeutic action of reflexoterapy can be explained are discussed. Spinal and trigeminal-reticulo-spinal pathways (central biasing mechanism) are postulated as inhibitory control system for somatic afferences.

[The effect of neuropeptides on memory: prospects for their clinical application].

Effect of vasopressin on various types of human memory was studied. Vasopressin did not affect the mechanical memory but improved the sense memory, favouring the transfer of the most important for the person information into the long-term memory. This effect was especially distinct if the initial level of memory was low. Vasopressin was also used for fixation of negative conditioned reflexes to alcohol in the course of conditioned reflexotherapy of chronic alcoholism. In presence of vasopressin the conditioned vomiting reflex to alcohol was produced faster and was especially distinct. Catamnestic examination, carried out one year after the treatment, showed the higher ratio of complete remission and a decrease of recurrent negative manifestations in a group of patients treated with vasopressin as compared with controls. Mechanisms of action of regulatory peptides are considered.

Chronic pelvic pain: psychosomatic aspects and clinico therapeutical reflections.

After describing the most common causes of chronic pelvic pain in women the Authors stress the importance of psychosomatic factors to the pathogenesis of chronic pelvic pain. Bearing this in mind it is easier to understand why the results of pharmacologic treatments often fail to come up to doctor's and patient's expectations. These limitations have often been stressed in the literature and several alternatives, like individual or group psychological treatment or, more recently, acupuncture, have been suggested. The Authors' proposal for treatment envisaged a dozen meetings to carry out skin reflexotherapy. Significant psychological elements came to light on those occasions and were subsequently discussed. The Authors' experience, though covering 15 cases only, is extremely encouraging and confirms the value of this reflexotherapeutical and psychosomatic approach which can really help patients who are chiefly alexitimic to overcome their problems.
