Sexual enjoyment and orgasm postpartum: sex differences and perceptual accuracy concerning partners' sexual experience.

The sexual relations of parents at the postpartum stage have been researched relatively often, but still there exist serious research deficits (for example, neglect of male partners, of the later postpartum stages beyond the third month, and neglect of sexual feelings, enjoyment and orgasm). The aim of this study was to gain more knowledge about German couples' sexual enjoyment and orgasm with regard to non-genital tenderness, French kissing, breast stimulation, manual-genital stimulation, cunnilingus, fellatio, vaginal intercourse, anal intercourse and masturbation at seven months postpartum. As part of a larger longitudinal study 60 women and men (30 couples) answered a newly developed Sexual Preferences Questionnaire (SPQ), which assesses sexual activity and enjoyment. Descriptive data about sexual enjoyment, sex/gender differences and perceptual differences between self-report and reports of partners' enjoyment are analyzed and SPQ data are validated with interview and other questionnaire data (PFB-tenderness). Both genders find the same activities most pleasant (tenderness, vaginal intercourse, receiving manual-genital stimulation) and most exciting (intercourse, receiving manual-genital and oral-genital stimulation) and reach orgasm most easily through intercourse, masturbation and receiving manual-genital stimulation. But men generally describe a higher sexual enjoyment and overestimate their partners' enjoyment, especially with regard to female orgasm through intercourse. The results are critically discussed with regard to limitations and strengths of the sample and the method.

Social basis of deviant sexual behaviour: a historical perspective.

Sexual perversions are often a product of civilized life. Most societies permit some degree of deviant sexual behaviour as a relief from stress of routine life. Criteria for labelling such behaviour have changed over a period of time. There have been instances of normalizing deviant sexual behaviour by formation of pressure groups by the deviants e.g. homosexual clubs, particularly in Western countries. Attitude of church on masturbation & homosexuality has fluctuated from forbidding these activities to accepting these as harmless acts. Extra marital sex, premarital sex, homosexuality, fellatio, masturbation have been reported from almost all societies. Swinging and mate-swapping is more prevalent in the West. Social factors associated with deviant sexual behaviour are discussed. Incest lobbies have come up in U. S. Prostitution has also been there in all societies since antiquity. Earlier, prostitutes enjoyed a relatively higher social status. Their degradation started with the dawn of Christianity. In 1960s there was sexual revolution in U. S. with emphasis on free sex. There is evidence of slowing down of sexual revolution with the advent of AIDS. Safe-sex and fidelity are now being emphasized.

Sexual behavior of secondary school students in Bangkok metropolis.

OBJECTIVE: To explore sexual behavior in terms of masturbation, homosexual contact and coitus in secondary school students in Bangkok. MATERIAL AND METHOD: This was a cross-sectional survey research from December, 2002 to January, 2003. 362 students were selected by the multistages simple random sampling technique. A self administered questionnaire was used to collect data on baseline characteristics and sexual behavior of the students. RESULTS: 197 male and 165 female students were enrolled in the present study. The mean age was 16.5 years, and 65.7 percent of the students were exposed to sexually explicit contents via the internet. 58.0 percent of the students used the internet for pornographic viewing. According to the sexual behavior of the students, 79.2 percent of the males and 9.1 percent of the females had experience in masturbation (p < 0.05). The male had a slightly higher in homosexual contact than the females (10.2% vs 9.1%) but coital experience of the males was markedly significance higher than the females (25.4% vs 7.9%) (P < 0.05). CONCLUSION: It was concluded that sexual behavior in the internet era; males had more experience in all these categories than females. Sexual education via the internet should be improved in order to make adolescents understand appropriate sexual behavior and practice safer sex.

[Investigation of acquired immunodeficiency syndrome correlated with high risk sexual behavior and knowledge of male homosexuality in Hangzhou Province, China].

OBJECTIVE: To find out the acquired immunodeficiency syndrome (AIDS) correlated high risk sexual behavior and knowledge in male homosexuals in Hangzhou Province. METHODS: Using an self-designed anonymous questionnaire, 927 male homosexual were investigated anonymously in May 2003 to January 2004. RESULTS: Among the male homosexuals surveyed, the average age of this population was (29.8 +/- 9.9) years, the average number of sexual partners per person in past one year was 7.34 +/- 5.61. Among them, 199 (21.46%) had one sexual partner, 165 (17.79%) had 2 - 5 sexual partners, and 563 (60.73%) had more than 5 sexual partners, and 179 (19.30%) also had opposite-sex sexual partner the average number of opposite-sex sexual partner of this group was 1.16 +/- 0.21. According to sexual behavior, 40.66% of them used anal sexual behavior, 62.99% used oral sex, 74.11% used masturbation and 3.66% used oral-anal touch. Among 179 male homosexuals having opposite sexual partners, the percentage of using vagina sex, anal sex, oral sex and masturbation was respectively 59.77%, 36.87%, 40.22% and 26.25%. Although their understanding level about AIDS was high, the risk rate of sexual behavior among them was also very high. Only 11.11% of male homosexuals used condom when they had anal sex with same sex, most of them rarely used or never used protection measures. When having sex with female, almost none of them used protection. CONCLUSION: The risk rate of sexual behaviors in male homosexuals is very high that some appropriating and effective measures for prevention should be taken.

Differences in gay men's AIDS risk knowledge and behavior patterns in high and low AIDS prevalence cities.

Several studies have found reductions in acquired immunodeficiency syndrome (AIDS) risk practices among gay men in high AIDS-prevalence cities since the start of the AIDS crisis. Much less is known about risk behavior patterns among gay men in smaller cities, where AIDS cases are less common and the prevalence of human immunodeficiency virus infection is relatively lower. In the study, men entering gay bars in three cities, one large and two small, completed anonymous surveys of sexual practices and AIDS risk knowledge. Men in high AIDS-prevalence areas were found to have had a greater number of sexual partners, were more knowledgeable about AIDS, were much more likely to engage in low-risk practices (such as mutual masturbation or body rubbing), and had unprotected anal intercourse less frequently than gay men in smaller cities. The most common sexual activity among gay men in the larger city was mutual masturbation, a low-risk practice. The most common sexual activity among gay men in the smaller cities was unprotected anal intercourse. Increased efforts are needed to educate gay men and to promote risk behavior changes among those living in smaller cities and in communities outside the prominent AIDS epicenters.

Sexual practices and condom usage in a cohort of homosexual men in relation to human immunodeficiency virus status.

Between January 1, and October 31, 1987, 420 homosexual men who participated in a cohort study of infection with the human immunodeficiency virus (HIV) completed a questionnaire that examined their sexual practices during the previous six months. Of the subjects, 205 (48.8%) men were HIV-seropositive and 215 (51.2%) men were HIV-seronegative. Although there was an appreciable level of condom usage in both groups, 13.5% of the HIV-seronegative men had engaged in unprotected receptive anal intercourse and 6.3% of the HIV-seropositive men had engaged in unprotected insertive anal intercourse. Condom breakage was reported on approximately 6% of occasions by a minority of subjects. Among subjects who were in a relationship with a regular male sexual partner, the most commonly reported sexual practices were deep kissing, mutual masturbation and receptive oral intercourse without ejaculation. No HIV-seronegative man engaged in unprotected receptive and/or insertive anal intercourse, receptive oral intercourse with ejaculation or receptive and/or insertive "fisting" with a regular partner who was HIV-seropositive. No HIV-seropositive man engaged in unprotected insertive anal intercourse to ejaculation with an HIV-seronegative partner, although they did so with partners who were HIV-seropositive or of unknown status. On multivariate analyses the subject's antibody status was found to be associated with receptive anal intercourse with a condom (P = 0.007) and mutual masturbation (P = 0.001), with HIV-seronegative men being more likely to practise either; no significant independent effect was associated with the partner's antibody status. These findings provide important information on the types and levels of sexual practices in a group of homosexual men after the recognition of the acquired immunodeficiency syndrome in this country.

Increased reactive oxygen species formation in semen of patients with spinal cord injury.

OBJECTIVES: To evaluate reactive oxygen species production of semen samples and Percoll-washed spermatozoa from men with spinal cord injuries and to determine if there is a relationship between this reactive oxygen species production and sperm motility. PARTICIPANTS: Semen samples from healthy volunteers and infertile patients were collected by masturbation. INTERVENTIONS: Semen samples from men with a spinal cord injury were obtained by electroejaculation or by masturbation after treatment with physostigmine. MAIN OUTCOME MEASUREMENTS: Motility was measured using the CellSoft computer-assisted analysis system (Cryo Resources Ltd., Montgomery, NY). Luminol-amplified chemiluminescence was used to measure reactive oxygen species production. RESULTS: Semen samples and Percoll-washed spermatozoa from men with a spinal cord injury produced reactive oxygen species at much higher frequency and levels than equivalent preparations from infertile men or healthy volunteers. There was an inverse relationship between the percentage of motility and reactive oxygen species production in Percoll-washed spermatozoa from men with a spinal cord injury. CONCLUSION: Semen samples and Percoll-washed spermatozoa from men with spinal cord injury produce high levels of reactive oxygen species that may be related to the low sperm motility and infertility observed in these men.

Sleep orgasm in women.

Some of the recent most challenging contributions to our knowledge of female sexuality were provided by direct observations as early as during the second year of life concerning genital self-stimulation and masturbation in girls. Other investigators have advanced the proposition that girls are capable of vaginal masturbation and possibly of experiencing vaginal sensation and stimulation very early in life. Our own clinical study on sleep orgasm, based mainly on data obtained from the analysis of one woman, leads to formulating the hypothesis that in some instances the nursing situation may provide sensations in the genitals (vagina), not only for the mother, but also for the baby. We assume that such sensations in the genitals of the infant female are not the result of mechanical stimulation of the genital organs--as described during the second year and later--but are the result of a "resonance" phenomenon whereby the infant's genitals, including the vagina, are stimulated from within. The observations of orgasm made by others and by the author should be considered the first tentative steps toward an understanding of the complex nature of female orgasm. Eventually, further studies might lead to distinguishing more clearly a sexual orgasm in a narrower sense from a sucking orgasm, from an anger orgasm, and from a stress orgasm--i.e. an unspecific genital discharge. Sleep orgasm can represent gratification of unacceptable disguised sexual wishes and can therefore occur after intercourse and orgasm experienced when awake. The study of sleep orgasm might be of value in relation to the general problems of female orgasm. I would like to close with a reminder that some of the formulations presented in this paper are quite obviously largely speculative.

Sexual repertoires of heterosexuals: implications for HIV/sexually transmitted disease risk and prevention. The ACSF Group, Analyse des Comportements Sexuels en France.

OBJECTIVE: To provide a quantitative and population-based analysis of sexual repertoires among heterosexuals. DESIGN: The French National Survey of Sexual Behaviour (ACSF), conducted between September 1991 and February 1992 on a representative sample of the population aged 18-69 years. METHODS: Sexual practices of the last heterosexual encounter were investigated among 4261 individuals; we measured the combinations of different practices and their correlations with age, and calculated frequencies for the main repertoires and their correlations with the interpartner relationship. RESULTS: Vaginal penetration and caressing were almost systematic, self-masturbation and anal penetration were rare, while mutual masturbation and orogenital practices had intermediate levels of occurrence. Examination of the correlations revealed (1) a very high correlation between practices which are reciprocal or symmetrical, (2) a strong association between genito-manual and genito-oral practices, (3) an association between anal penetration and fellatio, and (4) no clear correlation between any set of practices and vaginal sex or condom use. A small number of repertoires accounted for the vast majority of encounters. Younger people tended to have a more diversified repertoire. Repertoire types and diversity were strongly correlated to the pattern of interpartner relationship, independently of age. CONCLUSIONS: Reciprocity seems a standard feature of the heterosexual repertoire. The absence of a negative correlation between vaginal or anal penetration and other practices argues against promotion of the latter as substitute for the former. By contrast, the independence between condom use and any specific repertoire argues for its promotion as a universal means of protection.

Clomipramine and sexual function in men with premature ejaculation and controls.

PURPOSE: We determined whether clomipramine taken as needed increases ejaculation latency in men with premature ejaculation and controls. MATERIALS AND METHODS: The study included 8 patients with primary premature ejaculation, 6 with premature ejaculation and erectile dysfunction, and 8 controls. A prospective, double-blind, placebo controlled, crossover design was used that included 2, 3-week periods with clomipramine and placebo. During treatment phases subjects took either 25 mg. clomipramine or placebo as needed, that is 12 to 24 hours before anticipated sexual activity (coitus or masturbation). Subjects also visited the laboratory during these phases for evaluation of sexual response using visual erotic stimulation with and without vibration to the penis. Daily logs of sexual activities were maintained during treatment phases. RESULTS: Clomipramine significantly increased the latency to ejaculation during sexual activity (coitus or masturbation) from approximately 2 to 8 minutes in men with primary premature ejaculation. There were no significant effects in controls and men with premature ejaculation plus erectile dysfunction. Laboratory assessment indicated that men with primary premature ejaculation were better able to control ejaculatory response with clomipramine therapy. In these men clomipramine also resulted in increased satisfaction with sex life and relationship. Clomipramine inhibited nocturnal penile tumescence in all subjects. CONCLUSIONS: Clomipramine (25 mg. as needed) effectively increases ejaculatory latency in men with primary premature ejaculation, while treatment is not effective in those with premature ejaculation and erectile dysfunction.

[The effect of complete thymic extract on motility and progressive motility of spermatozoa in asthenozoospermia].

The thymic gland is involved in immunologic and endocrinologic heomostasis of the body. Neonatal thymectomy of experimental animals leads to gonadal dysgenesis. Both eostrogen and androgen receptors have been identified in thymic epitelial cells, and sex steroids can be modulated by secretion of thymosin. High levels of thymosin alpha-1 in seminal plasma are correlated with sperm count, viability and motility in fertile males as compared to infertile ones. In this study we examined the effects of complete thymic extract on motility and progressive motility of spermatozoa obtained from asthenozoospermic men. In the first part of experiment, we studied the influence of complete thymic extract, as a bioactive medium component, on movement of spermatozoa samples with initial motility lower than 30%, in vitro. The study was carried out on two groups of sperm samples, 40 samples each, obtained by masturbation after a 72-hour-abstinence. After completed liquefaction at room temperature (30-40 min.), we determined the usual parameters of sperm quality. Total motility (M%) and progressive motility (PM%) of spermatozoa were analyzed in 10 microscopic view fields. Each group of sperm samples was prepared by swim-up and swim-down method. Then, we estimated the total motility and progressive motility of "washed" spermatozoa in every sample. In addition, every sample was divided in two equal parts. One part was immediately left in thermostat (control). To the second part we added 0.2 mg of complete thymic extract before incubation (experimental group). Total and progressive motilities were analyzed after 1, 4, 12 and 24 hours of incubation. The t-test was used for statistical analysis of results. The second part of the study was carried out on a group of 10 asthenozoospermic men (volunteers). We examined the influence of complete thymic extract, as intra-muscular injection, in a doze of 150 mg daily, during seven days, on motility and progressive motility of spermatozoa. Sperm samples were obtained by masturbation after four days of abstinence. We compared the parameters before and after the therapy. They were statistically processed by a method of difference. The results showed that the complete thymic extract increased the total and progressive motilities of spermatozoa separated by swim-down method (p < 0.001), in vitro (Table 1-4). The parenteral application of complete thymic extract, statistically significantly increased motility and progressive motility of spermatozoa in asthenozoospermic men (p < 0.05) (Table 5). These findings could be useful in assisted reproductive technology.

[Fracture of the penis: description of a case].

Fracture of the penis is not a frequent event and it consists of a rupture of the tunica albuginea of the corpora cavernosa and sometimes of the urethra; the lesion occurs when the penis is erect because during erection the tunica albuginea is very thin and not flexible and it can be easily damaged by a trauma; most lesions occur during intercourse for impact of the erect penis against the female perineum or the pubic synphisis, but lesions during masturbation, fightings or falling off the bed are also described [1-2-3]. Clinically there are pain, detumescence, hematoma and recurvatum of the penis toward the opposite side of the lesion and hematuria if also the urethra is damaged. Usually diagnosis is quite easy with a good anamnesis, clinical examination and echotomography which can reveal presence of hematoma and hypoechogenic areas along the tunica albuginea or the urethra that appears irregular. Nowadays, fractures of the penis need a surgical approach to obtain good results avoiding consequences such as recurvatum or fibrotic areas [3-4-7-8]. We describe two cases of fracture of the penis occurred in two young men during masturbation and treated surgically with excellent results: twelve and eighteen months respectively after surgery patients are well and can enjoy a normal sexual activity with no problems at all.

Premarital counseling clinic at Chulalongkorn Hospital.

BACKGROUND: The premarital counseling clinic at Chulalongkorn Hospital was established in 1978. The characteristics and sexual background were reviewed. AIM: To study characteristics, sexual background and choices of contraceptions of premarital couples attending the premarital counseling clinic at Chulalongkorn Hospital during the past ten years, (1988-1998). DESIGN: Descriptive study. MATERIAL AND METHOD: Two hundred couples (men (M) and women (W)) attending the premarital clinic at Chulalongkorn Hospital from July 1988 to January 1998 were recruited in the study. The counseling program included medical counseling, sex education and family planning counseling. The information of couples was recorded by the Family Planning Unit staff and the residents. RESULTS: The mean age was 31.5 +/- 4.3 yrs. (M), 28.9 +/- 3.8 yrs. (W). Most of the couples had a bachelor's degree education (61.5%--M; 72%--W). Most of them were employees (64.0%--M; 60.0%--W), had an income of 10,000-20,000 baht per month (39.0%--M; 38.0%--W) were Buddhist 97.0 per cent (M), 93.0 per cent (W). There were only 4 couples who had an abnormal physical examination. 85.5 per cent of the men had had sexual experience before while it was only 23.0 per cent in women. Most of the men had frequent masturbation 3-4 times/month (31.5%) and most of the women had no masturbation (86.0%) at all. Most of the couples (71.5%) chose to have contraception (contraceptive pill 57.5% and condom 20.0%). CONCLUSION: Premarital counseling should be adjusted to the background of the couples. Most couples had a high income and education. Sex counseling is important for couples especially women. The propagation of premarital clinic encourages further study and to outreach the general population of Thailand.

Attitudes toward sex in the aged.

Eighty university students and 40 staff members of a nursing home for the aged rated their reactions to three stories: a neutral (decision-making) story, a coital story, and a masturbatory story. For each story, the age and gender of the main character were varied. Results indicated an absence of negative attitudes toward sex in the aged. However, sex in the aged was less credible than sex in young people. Nursing home staff members had more negative attitudes toward sexual stories than did the students. Gender of rater had little effect. Coitus was rated more favorably for the male character than for the female character, and the reverse was true for masturbation.

Sexual behavior of captive orangutans.

Opposite-sex pairs of orangutans were tested for sexual behavior during the intermenstrual period of the female. The male orangutan was the primary initiator of sexual interactions and initiated copulation forcefully on a daily basis, irrespective of female resistance. However, although single copulations occurred daily, copulations beyond the first occurred most frequently during midcycle. Other evidence of cyclicity in behavior was the midcycle decrease in female avoidance of the male and increased grooming, proceptivity, and masturbation by the female. Comparative analysis suggests that differences in sexual cyclicity among the great ape species are related to interspecies differences in sexual assertiveness of males and females. Sexual activity in the cycle is relatively brief when the female controls mating, more prolonged when the male is in control. This finding in man's closest taxonomic affiliates suggests that similar social factors may also influence the distribution of sexual interactions in the human cycle.

Electroencephalographic laterality changes during human sexual orgasm.

Left and right parietal EEGs were recorded while seven subjects experienced sexual climax through self-stimulation. EEG data were quantified by continuous cumulated measurements of the integrated areas of EEG recording during successive 1-sec epochs. In eight out of 12 experiments, this measure revealed a statistically significant change in laterality. Controlled replications with two of the original subjects obviated the possibility that two potential sources of artifact (hand used for masturbation and gross body movements) accounted for the changes. While previous research has focused on vasomotor and myotonic indices of sexual response, this study demonstrates the significance of brain phenomena in sexual orgasm.

Sexual fantasy and pornography: two cases of girls brought up with pornography.

A normal part of late childhood and preadolescence for boys is exposure to pornography. The visual imagery of the forbidden acts in pornography are readily integrated with sexual fantasy in masturbation and in social sex. Money and Ehrhardt (1972) have attributed the fact that proportionately more women (36%) than men (11%) do not make any use of visual imagery during sexual acts to a greater biological readiness to respond to distal, visual images with arousal among men. While such a biological threshold hypothesis may well be correct, the ubiquity of practice with norm-violative behavior in the excited and approving context of the childhood male peer group could make an important contribution to the later uptake of pornographic imagery into fantasy. A full test of the Money and Ehrhardt hypothesis would be impossible, entailing a study of adolescent fantasy among a group of girls who grew up just like boys until adolescence. Two sisters close in age, now women in their 20s, grew up with pornography ad lib because of the family's business in pornographic publishing. They were interviewed for their sexual histories. One uses visual fantasy and has integrated some elements of the imagery of the pornography for a period into her sexual life; the other has not. Implications for the definition of the two gender curricula--what boys learn from other boys and girls from other girls--are discussed.

Deviance as history: the future of perversion.

An approach to the social construction of concepts of sexual deviance and sexual perversions is considered. Deviance is conceptualized as a problem of control, perversion a problem of desire. These are seen as related to the larger sexual and nonsexual discursive practices of society and given to change as these contextualizing practices change. Changing conceptions regarding masturbation, homosexuality, pedophilia, and sadomasochism are examined.
