[A quality control of interpretation of rolled radiophotogramme used for mass screening of lung cancer in general habitants--cases difficult-to-diagnose and cases difficult-to-detect].

A mass screening for lung cancer was performed on 1,906,660 general habitants of Chiba Prefecture, using rolled radiophotogrammes (RP), a traditional mass screening method used for tuberculosis. Of those examined, 270 lung cancer cases were found. In order to evaluate the quality of RP interpretation, cases difficult-to-diagnose and cases difficult-to-detect were picked out from the detected lung cancer cases. Difficult-to-diagnose cases were defined as cases in which pulmonary tumor was not suspected in the screening. Among these cases, tuberculosis was the disease most frequently suspected. Thus, we reexamined the RP which were suspected as being tuberculosis or pulmonary tumor. There was no difference between the two diseases in terms of the location of pathological shadows. Isolated small nodular shadows and infiltrating shadows were seen as an indication of tuberculosis, while large nodular shadows were thought to indicate pulmonary tumor. In addition, by retrospectively examining the RP of the cases judged as normal in the screening of the previous year, tumor shadows could be found in 54.2% of the cases. These cases were considered to be difficult-to-detect cases of lung cancer. The tumor shadows of many cases were present in the hilar region in these re-examined X-ray films. It was difficult to recognize these shadows in the rolled X-ray films because small nodular shadows or infiltrating shadows overlapped with the opacities of normal structures. To overcome these problems of difficult-to-diagnose cases and difficult-to-detect cases, the following observations are important. (1) Care should be taken in order to obtain photogrammes with excellent quality.(ABSTRACT TRUNCATED AT 250 WORDS)

[Measurement of serum PSA by DELFIA PSA kit and its application for mass screening. The Gunma Urological Oncology Study Group].

The significance of prostate specific antigen (PSA) measured by DELFIA PSA kit in the 1,177 serum samples examined by mass screening for prostate cancer was evaluated. All subjects were examined by questionnaire, digital rectal examination (DRE) and prostatic acid phosphatase (PAP) and the subjects in whom prostate cancer was suspected from abnormal DRE and/or elevated PAP were recommended to receive the secondary screening for the presence of prostate cancer. All serum specimens used for this study were kept in our serum bank. The cut-off value was investigated between non-cancer subjects (diagnosed as normal, voiding disturbance or BPH) and prostate cancer patients. When the cut-off value was 2.89 ng/ml, the sensitivity, specificity and accuracy as prostate marker was 80.6%, 90.0% and 89.4%, respectively. Therefore, the cut-off value was determined as 3.0 ng/ml. The significance of PSA was retrospectively evaluated compared to PAP in the subjects examined by our mass screening. Twenty eight of the 39 palpable prostate cancers which could not be detected from the PAP level were detected from the PSA level, namely the sensitivity of the detection using the PSA level is more excellent than that using PAP. However, the false negative rate obtained using PSA was high (30.3%) in the patients with stage B disease. The relationship among serum PSA concentration, prostate size estimated by DRE and age was investigated. PSA was increased with age and prostate size. This estimation should be confirmed by using an ultrasonography because the prostate size obtained by DRE is inaccurate as compared with that obtained by ultrasonography.

Progression of prostatic cancer in relation to age in patients detected by mass screening program.

With the aim of revealing the natural history of prostatic cancer, the distribution of age was examined in relation to disease progression in patients detected by mass screening programs. Between 1975 and 1996, such programs detected a total number of 132 patients with prostatic cancer. The mean ages of patients with Stage B, Stage C and Stage D disease were 71.2, 73.8 and 75.3 years, respectively. This might suggest that the difference of 3 years in mean age between Stage B and Stage C disease reflects the time interval of the progression of prostatic cancer from the early (Stage B) to the advanced (Stage C) stage. These results coincided well with the natural history of prostatic cancer, which we proposed previously based upon the "size of ranking" analysis of latent prostatic cancer and doubling time obtained from ultrasonic measurement of prostatic volume. An epidemiological study of prostatic cancer detected by mass screening would offer data of value for the elucidation of the natural history of that disease.

[Mass screening for gastric cancer performed in Costa Rica].

We performed mass screening for gastric cancer by means of X-ray in Costa Rica from 1996 through 1999. Screening was performed on 10,064 subjects and 69 gastric cancers were detected (screening group). During the same period 172 gastric cancer patients were referred to us (non-screening group). Results of screening in Japan (Japanese group) were quoted from the annual report of the Japanese Society of Gastroenterological Mass Survey. This study is a comparison of these 3 groups. The detection rate was 0.68% in the screening group, 0.11% in the Japanese group. The operability was 92.7%, 76.1%, 97.0%, the resectability 96.8%, 83.2%, 98.6%, the rate of early gastric cancer 64.5%, 30.3%, 65.9%, and the rate of curability A 79.0%, 38.5%. 82.6% in the screening group, non-screening group and Japanese group respectively. The results in the screening group were exactly equal to those in the Japanese group. These results show that the same results can be obtained in Costa Rica as in Japan, if screening is performed with the same diagnostic level and skill as in Japan.

[Characteristics and progress of treatment for gastric cancers detected by mass screening].

We devised an X-ray examination to detect early gastric cancer, especially small gastric cancer, during the period between 1996 and 1997. As a result, the rate of gastric cancer detection by mass screening was 0.50%; and the rate of early gastric cancer detection was 79%. 1. Characteristics of gastric cancer detected by mass screening: Characteristics of gastric cancer were depressive-type undifferentiated cancer less than 4.0 cm diameter in work-place screening and depressive-type differentiated cancer less than 4.0 cm in area screening; 2. gastric cancer less than 2.0 cm in diameter was 75 (68%) of the 110 cases (total detected gastric cancer). 2) We have to consider that gastric cancer differs by sex and age. 3) EMR accounted for 39 (35%) of the 110 cases in treatment. This method resulted in a higher gastric cancer detection rate and served to maintain QOL.

alpha1-Fetoprotein measurement in blood spotted on paper: discriminating test for hereditary tyrosinemia in neonatal mass screening.

We describe an electroimmunodiffusion technique for measuring alpha1-fetoprotein in blood spotted on chromatography paper. The system is being used as a complementary test in a neonatal mass-screening program for detection of inborn metabolic diseases in the Province of Quebec. In a series of 102 cases of neonatal hypertyrosinemia, the test has proven to be highly discriminative for hereditary tyrosinemia. It has permitted early detection of eight cases of this disease, including two that would have been missed by the previously used screening procedure, tyrosine measurement only. The test not only virtually eliminates the risk of misdiagnosis or missed diagnosis, but also permits earlier diagnosis of hereditary tyrosinemia and considerably reduces the follow-up work required for newborns with transitory tyrosinemia. The AFP test is simple, fast, practical, and inexpensive. Combined with tyrosine determination, it constitutes an optimal device for mass screening of hereditary tyrosinemia.

[Mass screening for prostate cancer is the best approach to early diagnosis and treatment of prostate cancer].

To achieve the goal of early detection, diagnosis and treatment of prostate cancer in China, mass screening for prostate cancer using serum prostate specific antigen (PSA) has been performed in men over 50 years. We compared this result with the clinical cases. It proved that only through the mass screening can we really find the early stage prostate cancer and be given the chance of curing the cancer. We also investigated the current treatment situation and problems and figured out the further direction on the prostate cancer research.

[Comprehensive radionuclide lung imaging in preventive mass screening of industrial workers].

The results of a combined roentgenoradionuclide investigation of the lungs were studied in 45 persons at risk of developing chronic nonspecific pulmonary diseases, diagnosed in a mass screening of 3000 workers at an industrial enterprise. The results of fluorographic, roentgenoscopic and roentgenographic investigations of the lungs, roentgenopneumopolygraphy and radionuclide investigation of lung perfusion (pulmoscintigraphy with 99mTc-labeled albumin macroaggregates) were compared. During this combined investigation the signs of nonspecific pulmonary diseases were diagnosed in 40% of the patients, changes of the pulmonary blood flow--in 71%, changes of ventilation--in all the examinees. A conclusion was made of the necessity of additional combined investigation of patients belonging to risk groups, identified during mass screenings.

[A comparative analysis of interval breast cancer with breast cancer detected by mass screening or in outpatient clinics].

To investigate the characteristics and the prognosis of interval breast cancer, 21 interval cases were reviewed and compared with a total of 87 patients with breast cancer detected by mass screening and 266 found in outpatient clinics on the basis of the clinicopathological features. The postoperative cumulative 7-year disease-free rates were 61.1%, 81.4% and 69.0% in the interval cases, mass screening cases and outpatient clinic cases, respectively. The percentage of cases showing frequent mitoses in cancer cells was statistically higher for interval cancer than for the other breast cancers. The average tumor size of the interval cases calculated from the speculated tumor doubling times, was 1.51 cm. It is surmised that interval breast cancer is characterized by a marked proliferation of tumor cells and has a poorer prognosis than the other breast cancers. This might be due to the marked proliferation of interval breast cancer rather than under-counting because there were no malignant findings at the time of the last screening.

Low serum cholesterol as a risk factor for hemorrhagic stroke in men: a community-based mass screening in Okinawa, Japan.

The relation between the level of total serum cholesterol and stroke is controversial. The relation between serum total cholesterol and subtypes of stroke was examined in the participants of a community-based mass screening program in Okinawa, Japan. A total of 38,053 subjects, whose serum level of cholesterol had been determined during a mass screening carried out in 1983, were examined to see whether they had experienced stroke during a 3-year period from 1988 to 1991. Of them, 315 subjects aged 33-93 years (174 men, 141 women) had had a stroke during that period. The types of stroke were cerebral infarction in 164, cerebral hemorrhage in 111, subarachnoid hemorrhage in 19, and others in 21. In men, the odds ratio of cerebral hemorrhage was 0.71 (95% confidence interval, 0.55-0.95), and the odds ratio of cerebral hemorrhage associated with serum level of cholesterol < or =167 mg/dl, 168-191 mg/dl, 192-217mg/dl, and > or =218mg/dl were 1.00 (reference), 0.70 (0.38-1.30), 0.77 (0.55-1.08), 0.73 (0.56-0.96), respectively. Lower serum cholesterol was an independent predictor of cerebral hemorrhage in men.

A twenty-year period in mass screening for uterine cervical cancer in Fukushima Prefecture.

To explore the change in age distribution of patients with uterine cervical cancer and its precursors, we analyzed 2,168,923 women in a series of mass screening for uterine cervical cancer in Fukushima Prefecture. The first examination rate was not increased over a 20-year period. During the screening period, severe dysplasia was found in 693 women, carcinoma in situ in 672 women, microinvasive carcinoma in 517 women, and frank invasive carcinoma in 421 women. The numbers of patients with frank invasive carcinoma was decreased gradually in every age group, which might reflect the effectiveness of our mass screening project. Although the detection rates of severe dysplasia, carcinoma in situ and microinvasive carcinoma have not changed in patients groups over 40 years old, the detection rates were increased in patients under the age of 39. These results may reflect a recent tendency for cervical cancer to increase in young women. It is important, therefore, to increase the screening rate for young women to prevent the progression with advanced cervical cancer.

Effectiveness of mass screening for breast cancer in Japan.

BACKGROUND: Breast cancer screening has been conducted in Japan mainly by physical examination, the standard method for breast cancer screening according to the Law of Health Services for the Elderly. The purpose of this study was to evaluate the effectiveness of mass screening for breast cancer in Japan. METHODS: We calculated the average coverage-rates for breast cancer screening per year from 1986 to 1995 for women aged 30-69 years for all of the 3255 municipalities in Japan, selecting "high coverage-rate" municipalities with average coverage-rates of 20%, 30%, 40% or more. Two municipalities were selected as "controls" for each high coverage-rate municipality, and were matched for population, National Health Insurance rate, and the age-adjusted death rate from cancer of the female breast in the period 1986-90. We compared the change in the age-adjusted death rate from 1986-90 to 1991-95 of the high coverage-rate municipalities and the comparable controls. RESULTS: The percent reduction in the age-adjusted death rate from cancer of the breast in the high coverage-rate municipalities was statistically significantly greater than those in the controls. CONCLUSIONS: The results suggest that mass screening for female breast cancer, mainly by physical examination, contributed to the reduction of mortality from breast cancer.

[Prostatic specific antigen (PA) in mass screening for prostate cancer].

The usefulness of prostatic specific antigen (PA) was compared with that of prostatic acid phosphatase (PAP). PA was determined in the serum of 2,183 patient examined by the mass screening for prostate cancer from 1987 to 1990. The serum samples of these patients were obtained from our serum bank. PA was measured by the E test "TOSOH" II (PA). The relationship of PA and PAP to prostate size estimated by digital rectal examination (DRE) and ultrasound tomography (US), and age was investigated. PA and PAP correlated with aging and prostate size estimated by DRE. However PA was more apparently related with these things. The correlation between PA and prostatic size estimated by US was relatively high (r = 0.53), but the correlation between PAP and prostate size estimated by US was low (r = 0.20). When the upper limit of normal range was set at 6.0 ng/ml, the sensitivity, specificity and efficiency was 64%, 97% and 62%, respectively. PA was more sensitive than PAP and could be more useful since none of the patients with prostate cancer was PAP positive and PA negative. We conclude that PA should be a reliable tumor marker in our mass screening system.

Mass screening of prostate cancer in a Chinese population: the relationship between pathological features of prostate cancer and serum prostate specific antigen.

AIM: To investigate the pathological features of the prostate biopsy through mass screening for prostate cancer in a Chinese cohort and their association with serum prostate specific antigen (PSA). METHODS: A total of 12027 Chinese men in Changchun were screened for prostate cancer by means of the serum total prostate specific antigen tPSA test (by Elisa assay). Transrectal ultrasound-guided systematic six-sextant biopsies were performed on those whose serum tPSA value was > 4.0 ng/mL and those who had obstructive symptoms (despite their tPSA value) and were subject to subsequent pathological analysis with the aid of the statistic software SPSS 10.0 (SPSS. Inc., Chicago. USA). RESULTS: Of the 12027 cases, 158 (including 137 patients whose serum tPSA values were 4.0 ng/mL and 21 patients [serum tPSA < 4.0 ng/mL] who had obstructive symptoms) undertook prostate biopsy. Of the 158 biopsies, 41 cases of prostatic carcinoma were found (25.9 %, 41/158). The moderately differentiated carcinoma and poorly differentiated carcinoma accounted for 61% and 34%, respectively. A significant linear positive correlation between the serum tPSA and the Gleason scores in the 41 cases of prostatic carcinoma (r = 0.312, P < 0.01) was established. A significant linear positive correlation between the serum tPSA value of the 41 prostatic carcinoma and the positive counts of carcinoma in sextant biopsies was established (r = 0.406, P < 0.01), indicating a significant linear relationship between serum tPSA and the size of tumor. CONCLUSION: This study was the first to conduct mass screening for prostate cancer by testing for serum tPSA values and the first to investigate the pathological features of prostate cancer in a cohort of Chinese men. Our results reveal that the moderately differentiated carcinoma is the most common type of prostate cancer. This study also has shown that the serum tPSA value in prostate cancer is associated with the Gleason score and the size of tumor.

Bayesian classification of myocardial excitation abnormality using magnetocardiogram maps for mass screening.

We propose a novel classification method based on the Bayes rule to utilize the magnetocardiogram (MCG) in noninvasive mass screening. The cardiac excitation is directly tracked by maps of the MCG field generated by myocardial excitation current through the excited wave front. To adopt the characteristics of the excited wave fronts as a parameter for the Bayes theorem, we developed a parameterization procedure that consists of a two-dimensional wavelet approximation and a cluster analysis of magnetic field maps. With the parameter determined by this procedure, the probability of a subject to belong to a disease group or to the normal group is estimated by the Bayes theorem. The subject is classified into the group of the highest probability. We applied the proposed method to ST-T period of MCG data of 6 old myocardial infarction (OMI) patients and 15 normal controls. The method showed sensitivity of 83%; specificity, 100%; positive predictive value, 100%; and negative predictive value, 94% in the classification of OMI patients and normal controls. The processing time is less than 5 seconds per one subject. It suggests a possible application of the proposed method in mass screening of abnormal MCG patterns.

High cholesterol levels: is mass screening the best option?

Individuals with a high cholesterol level (greater than 6.5 mmol/L) are at a relatively high risk of developing coronary heart disease in the future and an important part of any programme to prevent coronary heart disease is the identification and treatment of these individuals. Mass screening can identify those who are at an increased risk but the likely benefits and costs of such a programme have not been assessed adequately. Before such a method is promoted, we need to know: how well it detects high-risk individuals (yield); what reductions in the risk of coronary heart disease can be expected in those who are screened as "positive"; what adverse effects screening may have on those who are screened as "negative"; and what the logistics and costs (both to those who are screened and to the community as a whole) are. Until we know this information the urge to promote mass screening for cholesterol levels should be restrained, and only those who are at high risk of coronary heart disease should be selected for the determination of their cholesterol levels.

Bladder origin neuroblastoma detected by mass screening.

Pelvic neuroblastoma is relatively rare and most are a presacral mass. We present a case of asymptomatic neuroblastoma arising from the dome of the bladder wall, detected by a mass screening program. Only 1 case of neuroblastoma originating from the bladder has been previously reported. Both the reported case and our present case arose from the dome of the bladder wall. The differential diagnosis for bladder tumor in children is discussed; although uncommon, neuroblastoma should be considered a probable choice, especially for tumors arising from the dome of the bladder wall.
