[Quality assurance in plastic surgery--initial experiences with "breast reduction" tracer diagnosis].

Quality assurance has to be an integral part of medical work as it is required by actual health laws. "Tracers" have been used in different parts of medicine as a reliable method. In 1997 we started to establish an internal program for a quality assurance with five different tracers (breast augmentation, breast reduction, oncological and reconstructive breast surgery, liposuction). The outcome is prospectively documented using an operation-specific documentation form. Every three months, the results are discussed in a surgical audit. The overall complication rate after mamma reduction (n = 81) was reduced about 40% to 21% (mean reduction weight 1.7 kg). Therefore, these measurements could promptly improve the quality of treatment of our patients.

[Complicated antiphospholid antibody syndrome].

HISTORY AND CLINICAL FINDINGS: A 51-year-old female patient suffered from recurrent ischemic strokes and venous thromboembolism although treated with ASS and phenprocoumon, which mainly occurred after diagnosis and treatment of an invasive-ductal mamma carcinoma. The severely ill patient presented with right-sided hemiparesis and dysarthria, a swollen leg, a painful necrotic-ulcerative lesion at the left-lateral ankle and a systolic heart murmur. INVESTIGATIONS: Laboratory data revealed a haemoglobin of 8,4 g/dl, a leucocyte count of 3,1 x 10 (9)/l and a platelet count of 87 x 10 (9)/l. C-reactive protein, ESR and plasma fibrinogen were markedly increased. Levels of antinuclear, IgG-anticardiolipin and anti-doublestranded-DNA antibodies were excessively elevated. A test for lupus anticoagulant was strongly positive. Sonographic examinations showed deep vein thrombosis and significant mitral valve regurgitation. Suspected pulmonary embolism was demonstrated by CT scan. We diagnosed systemic lupus erythematosus with secondary antiphospholipid antibody syndrome (APAS). TREATMENT AND COURSE: Phenprocoumon was stopped and full-dose anticoagulation with low-molecular-weight heparin initiated. Additional immunosuppressive therapy consisted of cyclophosphamide, azathioprin and prednisone resulting in prevention of further thromboembolism and significant improvement of clinical symptoms. The observed severe interference of LA with the prothrombin time after cessation of phenprocoumon (INR 3,6; FII activity 81 %) suggested that the effect of oral anticoagulation had been overestimated in the past. CONCLUSION: APAS may present as an acute and life-threatening disorder. In this case interdisciplinary co-operation and a highly individualised treatment strategy are mandatory.

[Percutaneous radiofrequency ablation of liver tumors using the LeVeen 4 cm array probe].

BACKGROUND: The purpose of this study was to evaluate safety and efficacy of a radiofrequency ablation system in clinical practice. METHODS: In 35 patients (age 63,9 +/- 12,6 years, range 22 - 83) a total of 65 liver tumors were percutaneously treated using a 200 watt radiofrequency generator and a LeVeen 4 cm array probe (RF3000, Boston Scientific). The interventions were performed under CT guidance in local anaesthesia and sedation. Adapted to the tumor size, the LeVeen Probe was repositioned during the procedure with an additional safety margin of 1 cm. Primary tumors were colorectal in 22, and mamma tumors (n = 4), zystic pancreas tumors (n = 2), gastric cancer (n = 1), zystadenocarcinoma of the liver (n = 1), lung cancer (n = 1), gastrointestinal stroma tumor (n = 1), duodenal carcinoma (n = 1), cholangiocellular carcinoma (n = 1) and hepatocellular carcinoma (n = 1). Post interventional control and follow-up was performed with multislice-CT (collimation 2.5 mm, unenhanced and contrast enhanced, arterial and portal filling) at 4 weeks, and every three months. RESULTS: One to 4 metastases were treated per patient during one or up to 4 procedure sessions. Mean lesion size was 2,3 +/- 1,2 cm (range 0,2 to 7,0). The corresponding size of the necrosis achieved was 4,6 +/- 1,4 cm (range 2,0 - 8,2). Primary technical success with complete tumor ablation was reached in 60 of 65 lesions. In 4 cases two treatment sessions were necessary in order to achieve the intended results. In one case the procedure was aborted because of a close relationship between lesion and right colon. 63 tumors were treated in sedation and local anesthesia. General anesthesia was necessary in two cases, in one who refused intervention in sedation, and in another case with insufficient analgetic effect. Morbidity was 9.2 %: Bleeding complications (n = 3, one arterial bleeding from the ablation tract, two intrahepatic bleedings with extrahepatic hematoma) were confirmed by selective angiography of the hepatic artery and were treated with coil embolisation of the respective segmental arteries. One case with subcapsular tumor ablation suffered from a large subcapsular hematoma requiring a blood transfusion. In one case with a subphrenic location of the metastasis, the needle electrode had passed the costophrenic recessus and resulted in an hematothorax. This patient was treated by pleural drainage for two days. One patient suffered from fever up to 39 degrees C and inflammation of the biliary tract and received a cholecystectomy 22 days post interventionally. There was no peri interventional mortality. Mean follow-up is 5,6 +/- 3,3 months (range 0 to 13). 21 of 35 patients showed no evidence of tumor recurrence. One case is scheduled for a second treatment session for complete tumor ablation. 13 of 35 patients suffered from tumor recurrence, either local recurrences and/or new metastases. At the sites of prior RF-ablation 9 local recurrences were detected in 7 patients, two cases with isolated local recurrences and 5 cases with local recurrences and new metastases. 6 Patients showed no evidence for local recurrences but new metastases. In these 11 cases a total of 37 metastases were found at new locations. In three patients tumor recurrence was treated by means of a second RF-ablation. The remaining 10 patients received chemotherapy. CONCLUSION: RF-ablation can be performed in local anaesthesia and sedation with low peri interventional morbidity and mortality. Using the LeVeen probe and a 200 watt generator, appropriate necroses can be achieved. CT follow-up is required every three months because of the tumor recurrence rate and reinterventions may be required.

[Prognosis, surgical therapy and progression in cervical and upper-thoracic tumor osteolysis].

INTRODUCTION: This study analyzes the early and mid-term results of our modular rod-screw implant system for the posterior instrumentation of the occipito-cervical, cervical and cervico-thoracic spine (neon occipito-cervical system, Ulrich, Germany) in patients with tumor osteolysis. The prognosis of the patients was evaluated using the Tokuhashi score. METHODS: The cervical and upper thoracic spines of 14 patients (7 males, 7 females, mean age 61 years, range 40-77 years) with osteolysis due to plasmocytoma (n = 2), bronchial (n = 3), mamma (n = 4), thyroid (n = 2), esophageal (n = 1) and pancreatic (n = 1) carcinomas as well as melanoma (n = 1) were instrumentated between June 2001 and April 2004. RESULTS: A stable fixation without loosening or failure of the fixator system was achieved in all cases. No impairment of the neurological status was observed. In our cohort different prognosis scores failed to make a reliable estimate of the expected survival at the time of surgery. CONCLUSION: Posterior instrumentation of the cervical spine including the occipito-cervical and the cervico-thoracic regions with a modular angle-stable rod-screw implant system (neon) offers good stabilization and allows simultaneous decompression. Since tumor masses are predominantly located in the anterior portion of the spine, blood loss can be controlled well. In this patient collective appears difficult to estimate the time of survival by a scoring system.

Phenolic esters from the rhizomes of Cimicifuga racemosa do not cause proliferation effects in MCF-7 cells.

Five phenylpropanoid esters, caffeoylglycolic acid, 2-caffeoylpiscidic acid (cimicifugic acid D), 3,4-dihydroxyphenacyl caffeate (petasiphenone), 3,4-dihydroxyphenyl-2-oxopropyl isoferulate (cimiciphenol) and 3,4-dihydroxyphenacyl isoferulate (cimiciphenone) were isolated from a commercially available extract of the rhizomes of Cimicifuga racemosa (L.) Nutt. (syn. Actaea racemosa L.) for the first time; the known cimicifugic acids A, B, E, F, fukinolic acid, fukiic acid and caffeic acid were also obtained. Cimiciphenone and caffeoylglycolic acid are new natural products. The structures were elucidated by means of spectroscopic data (ESI-MS, 1H-, 13C-NMR, COSY, HMQC, HMBC and NOE experiments). Ferulic acid and isoferulic acid were detected by HPLC analysis in comparison to standards. The estrogenic activity of the isolated compounds was tested in an estrogen-dependent MCF-7 mamma carcinoma cell line; 17beta-estradiol (10(-11) M) and the phytoestrogen coumestrol (10(-7) - 10(-5) M) were used as references. The results suggest that, in contrast to an earlier report, the phenolic esters do not exert a proliferative (estrogenic) effect in this test system.

[Breast cancer: analysis of treatment quality in Germany 2004: retrospective survey by Arbeitsgemeinschaft Gynäkologische Onkologie (AGO)].

The Organgruppe Mamma der Arbeitsgemeinschaft Gynaekologische Onkologie (AGO) conducted a nationwide 3-phase analysis of the care structure and standard of therapy given to patients with breast cancer from 2002 (4th quarter) to 2004 (4th quarter). The analysis includes 1069 patients from 257 institutions; 34% with early breast cancer and 66% with metastatic disease. No reliable data on the pattern of care of these patients have been published to date in Germany. The extent to which national and international therapy recommendations were implemented in routine clinical practice was unclear. Evaluation of the data shows that treatment based on the guidelines is now being implemented very reliably in certain sectors. This is of particular relevance to the pattern of adjuvant treatment in early breast cancer. At that time 68 % of the patients received an anthracyclin based chemotherapy and in addition 18% received an anthracycline and taxane based chemotherapy. Participation in clinical trials peaked with 30% in the neoadjuvant setting. The problem with metastatic breast cancer is the complexity of the interdisciplinary treatment involved. The present analysis conducted by the AGO was the first attempt to analyse the treatment given to metastatic patients and to systematise the approach to treatment. The fundamental problem remains, irrespective of the stage of the tumour, namely that too few patients are treated in randomised clinical studies.

[Comparison of the diagnostic value of the individual examination steps (triple diagnosis) in breast cysts].

Basic examination (palpation and mammography) as well as triple diagnosis (sonography, aspiration cytology, pneumocystography) are employed in the diagnosis and therapy of mamma cysts. Among 133 aspirated cysts we found by sonography 6 complicated cysts; 4 of these were cyst wall carcinomas. We found one further complicated cyst by aspiration cytology with a PAP IV; therefore, this sonographically uncomplicated cyst was extirpated as well resulting in cystic mastopathy without any signs of malignancy. Sonography and pneumocystography agreed perfectly in the diagnosis of cysts. Additionally mammography revealed 3 occult carcinomas located beside uncomplicated cysts. Hence, in no case should mammography be omitted. Sonography is a reliable instrument in judging cysts; therefore, aspiration cytology or pneumocystography may be left out in special cases.

[The value of MR-mammography at 1.5 tesla in the differential diagnosis of non-puerperal mastitis and inflammatory breast carcinoma].

PURPOSE: The distinction between mastitis and inflammatory breast carcinoma is an important one. Current methods of evaluation including mammography, ultrasound and clinical examination do not enable this distinction. Dynamic magnetic resonance mammography (MRM) is a study with potential in this regard. MATERIAL AND METHODS: 12 patients, in whom clinical examination, mammography and ultrasound could not distinguish between both diseases, were reviewed retrospectively by means of MRM using a 1.5 T Siemens Magnetom SP and a circular mamma coil. We used dynamic 3-D gradient echo sequences with a duration of one minute. RESULTS: At present MRM cannot definitely distinguish between mastitis and inflammatory carcinoma, 80% of the inflammatory carcinomas were found to enhance more than 100% in the first minute, compared to 43% for mastitis. No other differences were seen. CONCLUSION: MRM proved useful in the follow-up of treated mastitis to demonstrate the success of antibiotic treatment of mastitis and to diagnose a histologically unconfirmed inflammatory carcinoma by means of a different follow-up.

[Gynecologic interventions in the 10th decade of life].

We report on 28 surgical procedures performed between 1978 and 1992 performed on patients aged 90 years or older. 18 of them had a malignant tumour (11 mamma, 2 corpus, 3 vulva and 2 ovarian cancer), one had an ovarian cystoma. Three patients suffered from pelvic relaxation, four had postmenopausal bleeding, one vulvar dystrophy and one patient suffered from pyometra. Four laparotomies, three vaginal hysterectomies with colporrhaphy, four minor operations of the vulva, six curettages and eleven mammary operations were performed. During the surgical procedures, none of the patients died, whereas two patients died some time after the operation: so-called hospital mortality. The survival of the other patients was comparable to that of their age group. Two patients died of gynaecological diseases. After the surgical procedure, 21 patients reattained their preoperative level of health. 23 were satisfied with the result of the operation because of improvement of their complaints. If surgery in the aged allows the patients to lead a comfortable life, the operational risks can be justified.

[A rare clinical manifestation of a combination tumor of cystosarcoma phylloides malignum and an intraductal cancer].

A combination of cystosarcoma phylloides and ductal carcinoma in the breast of a 47-year old woman is reported. Only few cases of this simultaneous tumour formation is described in the literature. Therapy, prognosis and follow-up are discussed. Current standard therapy is ablatio mammae, modified according to Patey, with axillary lymphnode dissection. Decision in respect of chemotherapy depends on the results of staging. Follow-up control of the patients for early detection and therapy of metastasis is recommended. The reported case underlines, that histological examination of every breast node is indicated, even if there is no sign of malignancy.

[Multiple cancers in gynecologic oncology].

An analysis of 97 patients with multiple primary gynaecological neoplasms (90 double-, 6 triple- and 1 quadruple carcinomas) was done at the Gynaecological Department of the University of Innsbruck. Two thirds of the tumour combinations were located in the female genital tract including the breast. Concerning the early detection of simultaneous or subsequent malignancies of the cervix or corpus uteri, one should pay attention to the mamma being involved in the treatment or aftercare of such cancers. In the same way the inner female genital tract has to be observed in breast cancer patients. Regular colposcopic and cytological examinations of the cervical portio as well as a systematic prevention of endometrial cancer (pistolet in local anaesthesia) is highly recommended. The prognosis of the "highly malignant" tumour does not worsen as a result of a second malignant cancer.

[In vitro NMR spectroscopy of healthy, pathologically changed and carcinomatous breast tissue samples correlated with histological findings].

Of 57 patients with clinically suspected mamma carcinoma 121 samples were analysed by in vitro 1H-NMR spectroscopy at 300 MHz and correlated with histological data. Within the relevant spectral region between 2.7 and 4.1 ppm strong signals were observed from fat, (phosphoryl-) choline, (phospho-) creatine, and carnitine. Furthermore, with high regularity, 8 weak, partly overlapping signals were resolved and attributed to glucose, glycine, threonine, serine, inositol, and sucrose. Their intensities were determined by an iterative fitness program. From the intensity ratios, different kinds of tissue could be distinguished based on spectroscopic criteria. Healthy or mastopathically modified tissue could be discriminated from more than 50% carcinoma affected tissue with a specificity of better than 99.5%. This fact is explained by the lower content of fatty acids in the malignant tissue. Differences between spectra of healthy of mastopathically affected tissue were only small.

[Soft roentgen therapy in cutaneous metastases of the eyelids].

HISTORY: Within 6 months a 88-year-old woman developed cutaneous metastases with infiltration of the eye-lid so that an opening of the eye-lids became impossible. The primary tumor histologically resembled a mamma carcinoma. THERAPY: Within a few days the cutaneous metastases of the eye-lids decreased by using x-ray therapy with the Dermopan II with a single dosage of 5 Gy and a total dosage of 35 Gy on the eye-lids. Even 12 months after palliative therapy the opening of the eye-lids was possible without any problems. CONCLUSIONS: By using the x-ray therapy immediate and gentle help could be given to the terminally ill patients so that eye-lid surgery could be avoided.

[Platin-containing cytostatic combinations in the therapy of advanced gynecologic carcinomas].

95 patients with advanced and recurred gynecological carcinomas (57 ovarian carcinoma, 22 cervical carcinoma) were treated by a platinum (CDDP) based polychemotherapy platinum, adriamycine, cyclophosphamide (PAC); platinum, adriamycine, bleomycine (PAB), 22 out of 48 patients with low differentiated epithelial carcinoma of the ovary stage III and IV achieved a remission (13 CR; 9 PR). Today 6 of the CR-patients survive 23 months without recurrence; 7 had a medium survival of 17 months. Medium survival of PR-patients was 15 months. 6 NED-patients survive 22 months without recurrence and 2 NED-patients died after 19,5 months. The overall response rate (CR, PR, NED) was 63%. A retrospective comparison with 78 patients (low differentiated ovarian carcinoma stage III and IV) treated by a cyclophosphamide monotherapy shows an elevated 2-year survival rate of the PAC treated patients. The other patients suffered from carcinoma of the mamma, the tubes, the corpus uteri, the vagina, and the vulva. The therapeutic effect of CDDP based polychemotherapy cannot be judged because of the small number of cases; but the side effects at these patients were registered thoroughly. Side effects of totally 514 therapeutic cycles (460 mg CDDP per patient, 78 mg CDDP per cycle) are presented.

[Breast feeding habits before and after the introduction of a partial rooming-in system].

From January 1981 to May 1982 a prospective study was conducted investigating the breast-feeding patterns following the introduction of a partial rooming-in-system in 1978. The present data were compared to a previous survey from the period before 1977. The incidence of breast-feeding increased over that time from 67% to 86,6%. Accordingly the rate of non-breast-feeding mothers decreased from 33% to 13,4%. In 10% of these mothers lactation-inhibition was carried out with drugs and in 3,4% a severe hypogalactia was found. The lactation-inhibition was carried out in 80% for maternal and in 20% for neonatal reasons. Concerning the maternal causes one-third of the mothers did not want to breast-feed; other predominant causes were severe hypogalactia, adoption of the baby, maternal diseases, breast-feeding problems following preceding deliveries and single cases of mastitis and state after mamma-surgery. Among the neonatal reasons prematurity was predominating, followed by antenatal fetal death and perinatal death, malformations and diseases of the infant. Our results suggest, that the introduction of a partial rooming-in led to an increase of the incidence of breast-feeding at our department. Certainly, however, also the current trend back towards breast-feeding has contributed to these results. Nevertheless, we believe that much could still be improved by more adequate medical advice, which should start during pregnancy already. A reasonable promotion of breast-feeding should be the concern of all obstetricians today.

[Course of illness in malignant primary tumors with skeletal metastases].

About 70,000 malignant primary tumors with skeleton metastases are regarded with respect to tumor age, survival time with metastases respectively with primary tumor and life expectancy. Some cases with mamma carcinoma showed the vertebral type of bone metastasizing whereas the other primary tumors showed the typical type of spreading metastases over the lung. Patients with skeleton metastases are younger than the others. They have a longer survival time than the other patients without bone metastases but they die earlier. In those cases in which the primary tumor was diagnosed because of bone metastases without any earlier tumor treatment the survival time with metastases was longer than in other patients. Metastases with low grade histological differentiation were more often to be seen in patients who got their primary tumor in younger age. These metastases arose in a relatively late stage of the disease.

[The lactating breast in the magnetic resonance tomogram. Spin-echo and fat-water images].

MR criteria of a lactating mamma (15 days post partum) are imaged for the first time via spin echo technique and with fat-water images. Claear characterisation of milk in the glandular parenchyma as well as of fibrosed areals and postoperative scars was achieved. Significant additional information to the spin echo images was supplied by the fat-water images. Possible clinical uses are discussed.

[Possibilities for the use of manual and automated scanning procedures in ultrasonic mammography].

The limitations and advantages of the different ultrasonic breast-scanner (manual and automated) were described, likewise experiences with an automated waterpath scanner (Octoson). For breast cancer screening the automated scanner seems to be an acceptable concept, because it manifests the breast with an excellent anatomic view. In addition the mamma is in a stable position and the examination can be properly delegated. The manual examination can be used as an additional examination. Ultrasound scan can be correlated with the manual examination and a cyst can be punctured under ultrasonic control. Essential technological improvements are to be expected by both ultrasonic systems.
