RI lymphography by submucous injection in the pars vaginalis of the cervix uteri.

We have improved conventional RI lymphography, and investigated its usefulness as a screening method for lymph node metastasis. We injected 99mTc-sulfur colloid directly under the mucous membrane of the pars vaginalis of the cervix uteri of patients with cervical cancer, and took images every 5 minutes for 2 hours. At the same time, we analyzed the data stored in a computer, and attempted to observe the movement of radioisotopes taken into the lymph nodes. The subjects were 100 patients (200 study units) with cervical cancer, on whom histological examinations were performed. Of these, 41 study units were seen to be clouded by invasive cancer in the lymph nodes or parametrium. In the group in which both lymph nodes adjacent to the uterine cervix and the common iliac lymph nodes were delineated, lymph node metastasis was ruled out in 96.2%. Also, in some of the cases in which metastasis was seen, even though the lymph nodes were delineated, there was some delay in taking up the isotope. Therefore, it seems that as the main method, a diagnosis of "true negative" in the case of no metastasis is more useful than a "true positive" for the existence of metastasis.

Lymphography and its prognostic value in childhood leukemia.

Thirteen boys and five girls with leukemia were investigated with lymphography. Eleven of these children were followed-up with postlymphography films. Children with small atrophic lymph nodes seemed to have worse prognosis than children with normal appearance of the nodes. No prognostic meaning of contrast medium persistence could be evaluated.

[Lymphography, computed tomography and ultrasonic examination in determining the involvement of retroperitoneal and abdominal lymph nodes in Hodgkin's disease].

Altogether 45 patients with Hodgkin's disease were examined with the help of lymphography (LG), computerized tomography (CT) and ultrasonography (USG). The LG, CT and USG results obtained in 20 patients were compared with the histological findings. The retroperitoneal lymph nodes contrasted in LG, when involved in a pathological process, were detected with approximately the same accuracy in CT and USG. The abdominal lymph nodes undiscernible on lymphograms, were visualized in CT and USG, the sensitivity of USG was higher than that of CT. An optimum scheme for examination of patients with Hodgkin's disease with USG as the chief method was proposed. CT and LG are conducted depending on USG results and a clinical picture of disease.

[Lymphography of malignant tumors of organs of the small pelvis].

Basing on the results of lymphographic investigation of 1070 patients some regularities of lymphogenous metastatic spreading of uterine cervix and body cancer, prostatic, bladder and rectal cancer were determined. With a local spread of a malignant tumor the frequency and volume of metastasizing increased. More frequently metastases were located in the external and common iliac lymph nodes. Involvement of the pelvis and lumbar region on both sides was noted irrespective of the site of a primary tumor and direction of its growth. Great importance was attached to the results of lymphography for the choice and planning of therapeutic measures, indications for its use were defined.

[The role of lymphography in the choice and planning of treatment in patients with malignant tumors of the lesser pelvis].

Lymphographic studies were carried out in 1070 patients to identify patterns of lymphogenic dissemination of cancer of the cervix and corpus uteri, prostate, urinary, bladder and rectum. A direct correlation between the frequency and degree of metastatic spread and local extension of tumor process was established. The most frequent sites for metastasis were the external and common iliac lymph nodes. Whatever the site of primary tumor or the predominant direction of its growth, these lymph nodes on both sides of the pelvis and lumbar area were involved. The significance of lymphographic results for selecting and planning treatment modalities as well as indications for lymphography were identified.

[Multicollector lymphography and phlebography in lower limb edema following radiation and combination therapy].

A multimodality x-ray study of the lymphatic and venous vessels was performed in 50 patients with secondary edema of the lower limbs. A complete block of the limb superficial lymphatic collectors was observed in 44% of the patients, a partial block in 26%. In 80% of the cases the level of partial or complete occlusion of the lymphatic vessels was noted in the ilioinguinal region and in 20% on the femur and leg. Distrubed permeability of the deep vessels of the femur and ilioinguinal region in the form of a complete or partial block was noted in 46% of the cases. In disturbed peripheral lymphodynamics different compensatory adaptation mechanisms like the functioning of the collateral tracts, lymphovenous anastomoses, the overflow of a radiographic contrast agent from the superficial vessels into the deep ones were observed in 60% of the patients. Indications for multicollector lymphography were defined. In secondary edema of the lower limbs, combined injuries of the lymphatic and venous systems were marked in 62%, an isolated disorder of the outflow of the peripheral lymph through the collector vessels was marked in 38%.

[Thoracic and retroperitoneal cystic lymphangiomyomatosis. Value of lymphography].

Standard radiological examinations in a woman of 40 years of age with a pneumothorax and chylothorax revealed the presence of a posterior mediastinal mass compressing the oesophagus, and an interstitial syndrome. Retroperitoneal and mediastinal cystic lymphangiomas were found on lymphography. These findings are in accordance with the lymphangiomyomatosis described by Cornog and Enterline in 1966. The similarity between this disease and tuberculous sclerosis is underlined in present-day publications.

The technique and applications of lymphography.

The technique of radiographic examination by lymphography of normal lymphatic structures regional to the hind limbs is described in the rabbit, dog and rhesus monkey. Radiographic appearances of normal lymph nodes and possible applications to veterinary science are indicated.Normal lymph node microstructure is illustrated using in vivo intralymphatic injection of a brightly colored radiopaque silicone rubber. Angiogenesis in active germinal follicles during the period of an immune response is displayed. Further applications of the casting technique are suggested.

[The diagnostic value of lymphography in invasive carcinoma of the cervix stage 1B (author's transl)].

In invasive carcinoma of the cervix stage 1B our investigations showed 19% lymph node metastases (n = 47) which were verified by operation and microscopic examination. In all of these patients no lympographic evidence of metastases was observed. The value of lymphography in stage 1B invasive carcinoma of the cervix is limited. Since we found 19% false negative results and 0% false positive results, the operation of invasive carcinoma stage 1B of the cervix should therefore include a lympadenectomy which is not dependent on the results of lympography.

Visceral radioisotopic lymphography of the testicle.

Scintigraphic studies made in 75 patients, respectively on 130 testicles with the aid of the organ radio-isotopic lymphography enabled to determine more exactly the location and the variety of the testicular lymph centers. It should be pointed out that in 58.5% of the patients the lymph centers are situated on the left of the middle line and only in 6.1% on the right. In some cases iliac lymph nodes were demonstrable. Attempts were made to use this method in investigation of the patients with malignant tumours of the testicles.

Variations in the canine thoracic duct system and the effects of surgical occlusion demonstrated by rapid aqueous lymphography, using an intestinal lymphatic trunk.

An alternative method for lymphography was developed in an attempt to improve the outlining of the thoracic duct system. Lymphograms obtained on 20 normal dogs demonstrated a large extent of anatomic variation. A complete obstruction of the thoracic duct was created in six dogs through transpleural resection and ligation. The lymphatic system responded by opening lymphaticovenous anastomoses proximal to the site of obstruction.

[Value of CT and lymphography in the staging of lymphomas (author's transl)].

The contribution of CT is evaluated in association with conventional diagnostic investigations in the staging of lymphomas. The data obtained by means of CT are an integration of those obtained by means of lymphography. On the other hand CT offers elements which cannot be obtained by others methods, especially concerning the retroperitoneal region. This diminishes the average of patients who must undergo laparoscopy and laparotomy. Moreover CT allows to follow the evolution of the disease and to control the results of the treatment.

Thyroid lymphography.

We have presented the lymphographic findings in 13 patients with thyroid enlargement due to various disorders and discussed the diagnostic accuracy of this procedure. It is possible to diagnose uncommon thyroid diseases such as Hashimoto's disease and retrosternal goiter by thyroid lymphography. We recommend more regular use of this useful diagnostic aid.

Lymphography in the assessment of mycosis fungoides.

Extracutaneous manifestations of mycosis fungoides imply a bad prognosis and are a major cause of death. Benign dermatopathic lymphadenopathy is associated with mycosis fungoides and often precedes lymphomatous infiltration. In this study, 10 patients in the early stages of mycosis fungoides underwent clinical and lymphographical examinations. In one the lymphoma was already present in lymph nodes. Six had signs of dermatopathic adenopathy which was verified by lymph node biopsy in 5. In one of these the disease later progressed to a malignant lymphoma. The frequent occurrence of lymph node involvement justifies the use of lymphography collaterally with staging laparotomy to determine the presence of pathologic retroperitoneal lymph nodes.

[Bipedal lymphography in mediastino-pulmonary sarcoidosis. Report concerning 9 cases].

The relevant literature is briefly reviewed and lymphographic data obtained from 9 patients admitted to the S. L. Gonzaga Hospital are examined. In all cases, the diagnosis of sarcoidosis had been given prior to lymphography and confirmed histologically. This small series helps to show the extent of lymph node lesions in sarcoidosis and the more commonly observed adenographic features.

[Results obtained from staining lymphography, using carbon with lipoidal suspension, in cases of female genital carcinoma (author's transl)].

Reported in this paper is the use of a carbon-lipoidal suspension for presurgical staining lymphography which was applied to carcinomata colli uteri and corporis uteri. The suspension could not be prepared until permission had been obtained from the National Drug Expert Committee. Better colour contrast representation of the femoral and iliacal lymph nodes during the operation was thus achieved, and, consequently, surgical radicality was substantively improved. The approach proved less expensive and tedious than previous methods used for colour representation of lymph nodes in the minor pelvis. The dye used did not cause any complications. The advantages of the method are discussed. General application seems to be commendable, following further testing.
