Immobilization with external fixation bridging the knee joint in extension is frequently used after sustaining a war injury to the region of the knee joint with femoral and tibial bone fractures. Immobilization of the knee with plaster splints is performed in the same position. This usually prolonged treatment results in extension contractures of the knee joint. From June 1991 until March 1994, 54 patients with extension contractures of the knee caused by war wounds were treated at the Department of Orthopedics, Zagreb University Hospital Center, in Zagreb, Croatia. Results of surgical treatment of 44 patients are presented. The operative procedure consisted of extensive intra- and extra-articular adhesiolysis of the knee. The control group included 30 patients with knee contractures caused by injuries sustained in car crashes or secondary to previous operative procedures. The mean duration of immobilization was 5 months and 6 days. The mean preoperative knee motion amplitude ranged from 5 degrees in extension to 38 degrees in flexion. The mean postoperative knee motion was 98 degrees (range, 2 to 110 degrees). Treatment results did not depend on either duration of preoperative immobilization of the knee or previous infection in the region of injury. Previous knee joint bridging with an external fixator had no impact on the results of extension contracture treatment. Adequate intra- and extra-articular adhesiolysis with appropriate long-term postoperative rehabilitation is essential for the success of the operative treatment for knee joint contracture.
OBJECTIVE: The study was to verify the existence of the nutritional artery accompanying the sural nerve and to design a distally based island skin flap nourished by this artery for repairing skin defects on the remote part of the lower limb. METHODS: Anatomical study on 15 cadaver lower limbs and clinical applications in 7 cases were performed. RESULTS: The sural nerve nutritional artery is plexiform, forming anastomoses with the vascular network in the superficial and deep fascia. All 7 skin flaps depending on this arterial supply survived in clinical practice. CONCLUSION: This flap has high success rate. The operation is less traumatic and easy to perform. For the repair of the lower limb, it is a good option.
In-line skating is an increasingly popular recreational activity for both children and adults. As with any high-speed sport, traumatic injury is an associated hazard. It was the purpose of this study to focus on the injuries in children associated specifically with in-line skating with an emphasis on education and prevention. During a 9-year period, 331 injuries were identified, with 105 families completing an in-depth epidemiologic questionnaire. Sixty-one percent of the victims were boys, with an overall average age of 12 years. The upper extremity was most commonly injured (64%); the head and neck area was injured in 16% and the lower extremity in 20%. Fractures represented 38% of all injuries, with inexperience appearing to be the most common cause. An analysis of preventive strategies for childhood in-line skating was undertaken using Haddon's 10 basic preventive principles. Educational programs regarding instruction in in-line skating and the value of protective equipment need to be part of the school curriculum.
Most dance injuries are of the overuse variety. Their gradual onset, coupled with the intense competition for professional positions, often results in injuries being ignored and thus seen late in their course. While treatment of the specific injury is of paramount concern, maintenance of the extremes of flexibility, strength, and conditioning necessary to dance professionally is an equally important treatment element.
OBJECTIVE: To introduce the applied anatomy, operative method and clinical application of the anterolateral skin flap and fascial flap of the lower leg. METHODS: Anatomic dissection was performed on 14 adult cadavers' legs and one amputated lower limb. The origin, course and distribution of cutaneous branches of the superficial peroneal vessels were traced. Four types of anterolateral flaps were designed in the lower leg including the island skin flap, the island fascial flap, the rectangular flap, and the cross-leg flap. Since 1988, twenty-six cases of leg defects(21 patients), chronic osteomyelitis of the tibia(3 cases) and defects on the back of the opposite heel and ankle(2 cases) were treated with the operative methods. RESULTS: All the flaps survived. Primary healing occurred in 23 and secondary in 3 cases. Twenty cases were followed-up for 4 months to 5 years. The flaps were growing well, the tibia fracture healed 3 months after the treatment and the chronic osteomyelitis of tibia had no recurrence. CONCLUSION: This flap is a useful method for repairing various defects in the leg and adjacent regions. It is simple, safe and reliable in manipulation and has minimal influence to the donor site.

OBJECTIVE: To provide an anatomical basis for the clinical applications of the medial fascinocutaneous flap of calf and to verify its clinical value. METHODS: In 20 lower limb specimens of adult human cadavers, the number, course, size, position and distribution of septocutaneous branches of the posterior tibial vessels are marked by means of red latex and black ink irrigations through femur artery and septocutaneous branches of the posterior tibial artery respectively. RESULTS: The posterior tibial artery gives off several septocutaneous branches at the upper, middle and lower one-third of the leg respectively. Each septocutaneous artery has one or sometimes two concomitant veins. Based on this result, anterograde or reverse pedicled fasciocutaneous flap can be performed for the purpose of repairing soft tissue defects of leg and foot. The flap was clinically applied to treat leg and foot soft tissue defects in 12 cases with satisfactory results. CONCLUSION: The flap is easy to dissect, the posterior tibial artery can be preserved with high successful rate. Therefore, it offers an useful alternative in the repairing and reconstruction of nonextensive soft tissue defects in the leg and foot.

In the paper, a 20-year experience with practice of the sport and ballet-trauma department at Central Institute of Traumatology and Orthopedy, named after N. N. Priorov, and 7-year activities of the medical-ballet dispensary at Bolshoi Theatre of the USSR on diagnosis, treatment and prophylaxis of occupational traumas in ballet-dancers are summarized. Some basic principles of diagnosis, treatment and prophylaxis in occupational injuries in ballet-dancers are described. As a result of the employed prophylactic measures the incidence of trauma has fallen thrice as low.

We report the case of a subcutaneous hyalohyphomycosis of a 24-year-old man, a rural worker with an ulcerative lesion in the right leg of approximately one year duration. It was caused by traumatic implantation of a yerba mate branch. The diagnosis was made by direct microscopic examination with 20% potassium hydroxide (KOH) and revealed several septate hyaline hyphae. It was confirmed by culture of several samples where Fusarium solani was isolated. The patient received local and systemic antifungal therapy. Therapeutic response could not be ascertained at any point in the disease.
To evaluate the efficacy of the new diclofenac-N-(2-hydroxyethyl)-pyrrolidine gel formulated with lecithin (DHEP lecithin) compared with diclofenac-N-(2-hydroxyethyl)-pyrrolidine gel (DHEP gel) without lecithin in mild-to-moderate posttraumatic injuries (grade 1 ankle, knee and muscle injuries), a multicenter, double-blind, controlled study was carried out. A total of 100 patients were enrolled and randomly assigned to either DHEP lecithin (n = 52) or DHEP gel (n = 48) treatment. All patients concluded the treatment period except for five, who did not turn up to their respective investigational sites for the follow-up visits. According to an intention-to-treat approach, they were all included in the statistical analysis. As for the efficacy and safety analysis, the primary variable was "pain on movement" as measured by a Huskisson visual analog scale. During the first 3 days of treatment each group recorded a significant within-group decrease, but patients treated with DHEP lecithin showed a decrease in absolute value that was statistically greater than that obtained with DHEP gel (p = 0.025). At the end of the treatment period (day 10) the difference between groups was still statistically significant (p = 0.036). The statistical analysis of the secondary efficacy variables showed significant results in favor of DHEP lecithin treatment. These were superimposable on the results found for the primary variable. The global efficacy and tolerability judgments, reported either by patient or by physician, showed no statistical difference between treatment groups. Due to the presence of lecithin in the new gel formulation, DHEP lecithin showed a faster and significantly more marked therapeutic effect compared with that of DHEP gel.
