Congenital ossifying fibroma (osteofibrous dysplasia) of the tibia--a case report.

Ossifying fibromas of the long bones of the leg are benign lesions occurring in the pediatric age group identical in histological appearance to the similarly named tumor of the jaw in adults. Most frequently presentation occurs after minor trauma with symptoms of a swelling of the tibia or fibula which may be painful. Pathological fracture or limp are also occasional presentations. Congenital cases are extremely rare. We describe an otherwise normal male neonate who presented at birth with a bowed right lower leg. The limb was 1 cm shorter than the other side, with tibia vara and a firm mass situated anteriorly. X-ray showed a mixed lytic and sclerotic lesion in the proximal metaphysis of the tibia. Biopsy showed collagenous stroma containing spindle cells and irregular trabeculae of woven bone rimmed by plump osteoblasts. As the appearances were typical of an ossifying fibroma (osteofibrous dysplasia) no surgical treatment was given. The patient was well with no growth of the tumor and with radiological evidence of healing at 1 year follow up. This case is presented to draw attention to the clinicopathological features of this unusual lesion which must be considered in the differential diagnosis of congenital lesions of the tibia.

Desmoplastic fibroma of bone: a report of 18 cases.

Desmoplastic fibroma is a rare benign primary bone tumor, histologically identical to the extra-abdominal desmoid tumor of soft tissues. The incidence in major series of bone tumors is 0.1%-0.3%. In the present study 18 cases from the files of the Netherlands Committee on Bone Tumors are reported, with an emphasis on the radiographic features. Eight female and ten male subjects were affected, with a fairly even age distribution from the second to the seventh decade of life. The most frequent sites of involvement were the femur and the pelvis. Radiographically the lesions nearly always demonstrated benign features with respect to pattern of destruction, margins, and reactive bone formation; however, soft tissue extension was sometimes present. Although desmoplastic fibroma usually presents with pain or swelling, in 5 of the 18 cases the existence of the lesion was an accidental finding, and in 2 cases the patient had long-standing, vague complaints of pain. Though overall benign radiographic features may support the final diagnosis, the main and often troublesome histological differential diagnosis is of a low-grade fibrosoma. En bloc resection is the treatment of choice in view of the high incidence of recurrence after curettage.

Renal medullary fibroma: a case report and review.

A case of renal medullary fibroma in a woman is presented. A CT scan, which was performed for examination of cholelithiasis, incidentally revealed a mass in the left renal pelvis, and retrograde pyelography showed a filling defect in the same site. Left nephrectomy was performed. Histological examination revealed a renal medullary fibroma. The diagnostic and therapeutic dilemmas associated with this condition are discussed.

Fibroma of the cecum: report of a case.

A woman suffering from recurrent attacks of abdominal pain was found at operation to have a fibroma of the cecum. The clinical, operative and pathologic findings are described. The medical literature was reviewed and the lesion was found to be uncommon. The importance of specific histologic stains to distinguish the true fibroma from other spindle-cell tumors is stressed.

Desmoplastic fibroma-like tumor of maxillofacial region associated with tuberous sclerosis.

Desmoplastic fibroma is a rare primary tumor of bone that histologically and biologically mimics the extra-abdominal desmoid tumor of soft tissue. It usually presents in patients during the first three decades of life and often involves the mandible or long bones of the skeleton. Its clinical behavior is characterized by a locally aggressive, infiltrating, and destructing course, often with invasion of surrounding tissues but without metastasis. We present herein the clinicopathological features of a desmoplastic fibroma-like tumor involving the left maxillofacial region in a 14-year-old Hispanic boy with tuberous sclerosis.

[Sclerotic fibroma].

The clinical and histological diagnosis of sclerotic fibroma is important because of its potential association with Cowden's syndrome. Despite its distinctive histological appearance the lesion is often mis-diagnosed. We therefore present five of our own cases (2F, 3M) in which the tumor was located on the head (n = 2), arm (n = 2) and leg (n = 1), respectively. Clinically, the lesions were white to flesh-colored firm nodules ranging in size from 0.5 to 1.2 cm. None of our patients revealed any clinical evidence of Cowden's disease. Simple surgical excision seems to be curative. Histologically, they were well-circumscribed but not encapsulated dermal nodules composed of stori-form-arranged sclerosing collagen bundles and vimentin-positive fibroblastlike cells interspersed in three cases by a number of alpha-smooth-muscle actin-positive myofibroblasts. Approximately 50% of cells (dermal dendritic cells (DD)) also reacted for factor XIIIa evenly scattered throughout the lesion in contrast to the very few (< 5%) CD34+ DD found predominantly at the lower border, thus possibly reflecting the distribution of these cells in normal skin. Sclerotic fibroma expands the spectrum of fibrous lesions that may express alpha-smooth-muscle actin.

MR imaging of calcifying aponeurotic fibroma of the thigh.

Calcifying aponeurotic fibroma is a rare, benign soft-tissue proliferation that occurs in the distal extremities in children. Because this lesion has a tendency to recur after surgical resection, MR imaging to determine the extent of the tumor for surgical planning is optimal. We report the MR findings in a 4-year-old boy with a calcifying aponeurotic fibroma of the thigh.

Desmoplastic fibroma of the mandible in an infant.

We report the CT and MR imaging findings of a case of a desmoplastic fibroma of the mandible in a 6-month-old girl who presented with a rapidly enlarging facial mass and who represents one of the youngest reported children with this rare tumor. Although imaging is non-specific and these lesions are best distinguished histologically, the desmoplastic fibroma should be considered when there is an occurrence of any bubbly or cystic expansile mandibular lesion presenting in childhood.

Intra-articular fibroma of tendon sheath involving the scapholunate and radiocarpal joints.

Fibroma of tendon sheath is an uncommon benign soft tissue tumor with a predilection for the hand. A unique case involving the scapholunate and radiocarpal joints with bone erosion is reported. The patient presented with a transilluminating mass over the volar radial wrist associated with neuropathy in the superficial radial nerve distribution. Magnetic resonance imaging demonstrated a heterogeneous and lobulated mass with nonspecific signal characteristics closely associated with the scapholunate interval and the volar wrist soft tissues. Histologically it represented a fibroma of tendon sheath. The tumor is discussed and the relevant literature is reviewed.

Collagenous fibroma of the arm: a report of two cases.

A recently proposed addition to fibrous tumors in soft tissue was first described as desmoplastic fibroblastoma and later renamed collagenous fibroma. This tumor is clinically and morphologically distinct and benign. However, only a few series have been reported, and the clinicopathologic features are not widely recognized. We present two cases of collagenous fibroma of the arm. Both patients presented with an enlarging, well-circumscribed and mobile soft tissue mass. Magnetic resonance imaging showed areas of low signal intensity on both T1- and T2-weighted sequences. Needle aspiration cytology revealed nondiagnostic samples because of the low cellularity of the tumors. Each of the resected tumors was composed of low-cellular spindle- to stellate-shaped cells in a fibrous matrix with clear margination. After the marginal excisions, no recurrences were observed. Clinicians should be aware of this entity to prevent overtreatment, because imaging findings and cytologic features are similar to those of desmoid tumor.

Desmoplastic fibroma of bone: MRI features.

Desmoplastic fibroma of bone is a very rare benign tumor, which may be locally aggressive. In contrast to the well-documented radiological appearance, the literature on MR imaging features of this tumor is scarce. The MR imaging characteristics in our case are compared to those previously reported. Although there is a considerable overlap in the MR imaging features with other bone tumors, an interesting MR feature of desmoplastic fibroma is the presence of low to intermediate signal intensity foci on T2-weighted images, which radiographically does not correspond to calcifications. This feature may help narrow the differential diagnosis.

Ovarian fibroma: findings by contrast-enhanced MRI.

Ovarian fibromas are solid neoplasms that are difficult to differentiate radiologically from uterine leiomyomas. In this report, we describe the contrast-enhanced magnetic resonance imaging features of a 13-cm-diameter solid pelvic mass that allowed us to make the prospective diagnosis of ovarian fibroma.

Desmoplastic fibroma in the thoracic spine: an unusual localization of a rare primary bone tumor.

A case of an intraspinal growing desmoplastic fibroma of the thoracic spine (T9-T11) is reported. Desmoplastic fibroma is a rare tumor of connective tissue that shows a locally infiltrative and destructive growth. An affection of the thoracic spine is an extremely rare manifestation. Preoperative CT documents the extent of osseous destruction and tumor associated cortical erosion. In central tumor areas an inhomogeneous, intermediate to low signal is demonstrated by MRI using T1- and T2-weighted spin-echo and turbo-spin-echo sequences. Contrast-enhanced MRI shows marked enhancement in peripheral areas depicting the extraosseous and intramedullary extension.

Cross-sectional imaging patterns of desmoplastic fibroma.

The aim of this study was to work out the cross-sectional imaging characteristics of desmoplastic fibroma (DF). In 3 patients with histologically proven DF, the imaging characteristics obtained with cross-sectional techniques were reviewed retrospectively. Radiographs and CT scans were available in all patients, and plain and contrast-enhanced MR examinations in 2 patients. Compared with conventional radiographs, CT allowed more accurate assessment of the extent of bone destruction including cortical breakthrough and articular invasion. Intramedullary tumor growth and soft tissue extension was best detected with MRI. Apart from heterogeneity on MR images, DF displayed nonspecific low signal intensity on unenhanced T1-weighted images and an intermediate to high signal intensity including areas of low intensity on T2-weighted images. Desmoplastic fibroma showed a distinct, inhomogeneous gadolinium enhancement. Although cross-sectional imaging features of DF are nonspecific, some MR characteristics, such as inhomogeneous contrast enhancement and the presence of low-intensity regions on T2-weighted images, are helpful in determining the differential diagnosis. Cross-sectional imaging of DF is useful for local staging of the tumor because it provides valuable information about the extent of bone destruction as well as medullary and extraosseous spread.

Omental fibroma: CT and US findings.

The US, CT, and X-ray findings in a patient with omental fibroma of the lesser omentum are described. Ultrasound showed a solid mass with cystic areas in the central region. At CT the lesion showed peripheral enhancement and central hypodensity. On X-ray studies with barium, there was border distortion in the lesser curvature of the stomach. The mass was resected surgically. A pathologic diagnosis of fibroma was confirmed.

Pleomorphic fibroma of the eyelid.

BACKGROUND: This report describes the pathology of an unusual variant of fibroma of the eyelid. The soft ovoid tumour was excised from the centre of the left lower lid in an 84-year-old patient had been growing slowly for several years. METHODS: The tumour was studied by conventional histology, immunohistochemistry and electron microscopy. RESULTS: The tumor was formed by fibroblasts of spindle and multinucleate type, and the collagenous stroma contained mast cells and blood vessels which exhibited minor inflammatory changes. With the immunohistochemical methods the only positive marker was for vimentin. CONCLUSION: On morphological grounds it was possible to distinguish this extremely rare tumour from other mesenchymal tumours. The presence of distinctive multinucleate giant cells makes the term pleomorphic fibroma an appropriate name for this type of lesion.

Benign nonepithelial fibroma in the pancreas.

This is, to our knowledge, the first report of a fibroma originating in the pancreas in which preoperative imaging findings or explicit descriptions of histopathologic observations are clearly described. A 64-year-old man was referred to our hospital for further evaluation of a pancreatic mass. Clinical examination and laboratory analysis revealed no abnormal findings, except for mild elevation of serum trypsin (550 ng/ml). Abdominal ultrasonogram, computed tomographic scan, magnetic resonance imaging, and angiography demonstrated a well-demarcated circular mass in the tail of the pancreas. A laparotomy was performed on April 26, 1995. The tumor was observed to be tan-colored and smooth, and was localized in the front of the pancreas tail, suggesting that it had arisen from the pancreatic capsule. The patient underwent enucleation of the tumor. The resected tumor presented macroscopically as a fibrous nodule measuring 5.5 x 6.0 cm. The pathological diagnosis was fibroma. Immunohistochemically, spindle cells were positive for vimentin and negative for alpha-smooth muscle actin, desmin, S-100, and neuron-specific enolase.

[Cellular fibroma of the ovary: description of a case].

Cellular fibromas of the ovary are rare neoplasms belonging to the group of sex-cord stromal tumours. They have been described to show from 1 to 3 mitotic figures per 10 high power fields (HPF) and they generally behave in a benign fashion. Herein we describe the clinicopathological features of a case of ovarian cellular fibroma. The patient, a 22-year-old woman, presented with acute abdominal pain. Laparotomy revealed a large ovarian mass. Histologically the lesion was composed of spindle cells showing slight or moderate pleomorphism and 3 mitoses per 10 HPF. The spindle cells were immunoreactive for vimentin, smooth muscle actin and inhibin alpha-subunit. The differential diagnoses that we considered included the mitotically active leiomyoma because of the strong positivity for smooth muscle actin, but positive immunoreaction with anti alpha-inhibin antibody helped in confirming a sex-cord stromal tumour. Electron microscopy did not show any evidence of smooth muscle differentiation.
