Temporal lobe epilepsy caused by dermoid cyst.

A 15-year-old boy presented with a dermoid cyst in the left temporal lobe manifesting as complex partial seizures. Magnetic resonance imaging demonstrated a tumor with mixed signal intensity in the left anterior temporal subdural area, but no evidence of rupture. Intraoperatively, the tumor was located mainly in the deep sylvian fissure, adjacent to the amygdala, and had compressed the hippocampus. Intraoperative electrocorticography (ECoG) showed sporadic interictal spikes in both the adjacent areas of the tumor and over the anterior segment of the hippocampus. Total removal of the tumor and gliotic area of the surrounding tissue including the amygdala was performed. The hippocampal epileptic region was treated by transection of the pyramidal layer to preserve verbal memory function. Histological examination showed the dermoid tumor was closely attached to the brain parenchyma. The complex partial seizures ceased completely after surgery. Intraoperative recording of ECoG from the hippocampus and other limbic structures was very important to determine the epileptogenic area even if the tumor did not directly invade the hippocampus.

Subgaleal dermoid cyst of the anterior fontanelle: diagnosis with sonography.

We report the plain film and sonographic findings in six children (2 1/2 months to 5 years old) with a subgaleal dermoid cyst at the anterior fontanelle. Plain films of the skull made in all patients showed a soft-tissue mass superficial to the anterior fontanelle with a normal subcutaneous fat layer above it. In three children, the margins of the anterior fontanelle were beveled to match the contour of the mass. Sonography in the five children with open fontanelles showed the mass to have a well-defined sonolucent interior with a clear wall separating it from the superior sagittal sinus and other intracranial contents. The combination of clinical evaluation, plain films, and sonography appears to be sufficient to make the diagnosis of subgaleal dermoid cyst of the anterior fontanelle.

Rhabdomyomatous mesenchymal hamartoma associated with nasofrontal meningocele and dermoid cyst.

A 1-year-6-month-old girl presented with a subcutaneous tumor of the forehead, which had developed since birth. The preoperative examinations showed nasofrontal bone defect with meningocele and subcutaneous tumor with cyst. The patient underwent excision of the tumor and reconstruction of the bone defect. Histologically, the skin tumor exhibited disordered arrangement of striated muscle fibers among normal dermal components, and the cyst was lined by cornified epithelium with a few hair adnexa and contained lamellated keratin. These findings were consistent with rhabdomyomatous mesenchymal hamartoma (RMH) and dermoid cyst. This is an interesting case of RMH co-existing with nasofrontal meningocele and dermoid cyst in the same area. We suggest embryologic errors as a possible etiology, which is incomplete dysjunction of the neural ectoderm from the cutaneous ectoderm. Failure of insertion of mesoderm between the ectoderms caused the bone defect and the disordered proliferation and differentiation of mesoderm-derived tissue, leading to formation of hamartoma.

Excision of ovarian dermoid cyst by laparoscopy and by laparotomy.

OBJECTIVES: Our purpose was to evaluate the efficacy of laparoscopic ovarian dermoid cystectomy and to compare the operative course, postoperative course, and complications between the laparoscopy and the laparotomy techniques. STUDY DESIGN: The safety and efficacy of laparoscopic ovarian dermoid cystectomy were evaluated in 40 women. Twenty-nine of 40 patients underwent laparoscopic excision of a solitary dermoid cyst without any additional procedure. The operative course, the postoperative course, and complications among these 29 women were compared with those of 26 other women who underwent a similar procedure by laparotomy. RESULTS: Spillage of the cyst's content did not lead to any complication. The operating time in the laparoscopy group was 73.5 +/- 4.7 minutes and in the laparotomy group it was 41.4 +/- 2.9 minutes. The duration of hospitalization was significantly shorter in the laparoscopy group (0.7 +/- 0.2 days) than in the laparotomy group (3.8 +/- 0.1 days). CONCLUSIONS: Although ovarian dermoid cystectomy by laparoscopy is associated with a longer operating time than by laparotomy, the duration of hospitalization is shorter and recovery is faster. Spillage of the contents of the dermoid cyst does not lead to any complication; perhaps this is due to the liberal irrigation of the peritoneal cavity.

Malignant melanoma arising in mature ovarian cystic teratoma (dermoid cyst). Report of two cases and literature analysis.

Malignant melanoma may originate from melanocytes in ovarian cystic teratomas (dermoid cysts). Two cases are reported here and an additional 17 cases are found in a literature review covering 1903-1995. The review also found cases of benign and malignant melanotic ovarian lesion unassociated with dermoid cyst, including four melanomas, three cases of benign nevus, five cases of benign melanosis, and four cases of benign and malignant retinal anlage tumors. The extremely rare primary ovarian melanoma may be differentiated from the more common melanoma metastatic to the ovaries by its unilaterality, the presence of junctional change, and a detailed history and physical examination to exclude another primary site. Overall, 50% of patients with Stage I dermoid-associated melanoma were alive at 2 years compared with 89% of patients with Stage I dermoid-associated squamous carcinoma at 5 years. In the last 10 years, however, with the application o aggressive chemotherapy, survival has improved.

The beneficial role of transvaginal ultrasound in the preoperative characterisation of dermoid cysts of the ovary.

The advent of transvaginal sonography has improved the ability of radiologists to visualise both ovaries and has facilitated the preoperative characterisation of many of these lesions. We describe the beneficial role of transvaginal sonography in the accurate preoperative characterisation of dermoid cysts of the ovary in two patients and describe the characteristic features of ovarian dermoid cysts.

Dermal sinus with dermoid cyst in the upper cervical spine: case note.

We describe a 5 year-old girl who had a skin dimple of the back of her upper neck. MRI showed a dermal sinus tract in the upper cervical spine, associated with an intramedullary dermoid cyst at C 2-3, and spina bifida. A laminectomy was performed, the dermoid cyst and the sinus tract were completely removed. This congenital complex is very rare.

A dermoid cyst of the paravaginal space.

We describes a patient with a dermoid cyst in the paravaginal space. The cyst recurred after repeated drainage procedures and became infected. The cyst was ultimately excised.

[A case of fistula between the urinary bladder and an ovarian dermoid cyst].

A 20-year-old woman was referred to our hospital for further evaluation on persistent pyuria. Physical examination showed a tender solid suprapubic mass. Computed tomography showed a right ovarian multilocular cystic tumor just above the urinary bladder. Cystoscopy showed pus leakage from an orifice in a hemisphere protrusion of the bladder wall. A small catheter could be inserted into the orifice. It revealed a fistula 4 cm in length between the right ovarian tumor and the vesical cavity. The tumor and the adjacent thickened bladder wall with an abscess and fistula were resected en bloc. Also a small left ovarian cyst was enucleated. Histopathological examination showed bilateral ovarian dermoid cysts, abscess formation, and marked inflammatory change around the fistula in the vesical wall. The fistula was thought to be the consequence of infection of the ovarian cyst.

Congenital nasal dermoid cysts and fistulas.

Eleven patients who underwent surgical excision of nasal dermoid cysts or fistulas are reported. The patients are discussed according to the aetiology, sex distribution, location on the nose, and possible inheritance. Our surgical results are satisfactory with only one case which required reoperation. We found that late diagnosed cysts had a higher frequency of deep involvement and therefore required more extensive surgery.

Primary amenorrhea associated with hirsutism, acanthosis nigricans, dermoid cysts of the ovaries and a new type of insulin resistance.

We describe a 15 1/2 year old presenting with primary amenorrhea, hirsutism, acanthosis nigricans and insulin resistance. Ovarian vein catheterization studies revealed bilateral excess plasma testosterone and androstenedione secretion, and at surgery multiple dermoid cysts of both ovaries were found in association with polycystic ovaries. The suggestion that the dermoid cysts may be causative in the evolution of the polycystic ovarian disease has been made. The mechanism of the insulin resistance appears to be at the post receptor level. The acanthosis nigricans diminished following surgery with normalization of the plasma androgens.

Ovarian dermoid cyst associated with tuberculosis, cystadenoma and torsion.

An 87-year-old woman who had had a lower abdominal mass for 32 years underwent a laparotomy. Surgery revealed a 21-cm left ovarian dermoid cyst with torsion. Pathologic examination showed a hemorrhagic infarct due to torsion, mucinous cystadenoma and tuberculosis within the tumor. Tuberculosis was also found in the endometrium, both ovaries, oviducts and omentum. The patient's recovery was uneventful after surgery and medical treatment. The clinical manifestations of tuberculosis can vary widely. Tuberculosis of an ovarian dermoid cyst may be silent for a long period of time unless other complications occur.

Dermoid cysts of the anterior fontanelle.

A review of the medical records at The Hospital for Sick Children in Toronto, Ontario, from 1967 through 1980 showed that, of the 94 cases of dermoid cyst of the skull treated at this institution by the neurosurgical staff, 25 were located over the anterior fontanelle. This is the largest single group of cases reported to date. The children were 2 months to 13 years of age, but two-thirds were treated before their 1st birthday. Females outnumbered males 2:1, and all of the patients presented with a nontender swelling over the anterior fontanelle, which had been present from birth. The lesions were treated by a variety of surgical approaches, all of which were curative. None of the lesions has recurred. The most interesting finding was that all but 2 of the children were white, in contrast to all previously published series. This confirms our belief that the racial incidence of dermoid cysts of the anterior fontanelle is a reflection of the general population. A review of previously published cases and the pathology and embryology of the lesion is included.

Dermoid cyst in the middle ear.

A case of a moderate essentially low tone unilateral conductive hearing loss without a previous history of secretory otitis media in a six-year-old female patient is described. The condition was found to be due to a dermoid cyst in the tympanic cavity, which was confirmed by pathological examination of the surgically removed specimen. This is the sixth documented case of dermoid cyst in the tympanic cavity.

[Benign cystic teratoma (dermoid cyst) of the ovary perforated into the urinary bladder].

In a 25-year-old female patient with right-sided adnex tumour and inveterate infection on the urinary tract a benign cystic teratoma (dermoid cyst) perforated into the lumen of the urinary bladder could diagnosed cystoscopically. As therapeutic method of choice the tumour with the adjacent part of the wall of the urinary bladder was operatively removed. The histological investigation confirmed the right ovary as starting point of the tumour. Pathogenetic and etiologic points of view of the perforation of dermoid cysts of the small pelvis into adjacent hollow organs are discussed in short. When a perforation into the lumen of the urinary bladder is present, among others the clinical symptoms of the flux of hair and fatty paste with the urine belonging to this give good diagnostic references.

Dermoid cyst at the suprasternal notch: an adult case.

We present a case of a 39-year-old Japanese man with a two-year history of a mass in his suprasternal notch. Histopathologically the diagnosis was confirmed as a dermoid cyst; the cyst wall was almost normal with an epidermal structure consisting of sebaceous and apocrine glands and on the inside there were keratin materials and hair shafts. A dermoid cyst at the suprasternal notch is rare, and in an adult is the first as far as we know.

An exception to the rule: ectopic ACTH production from functional neuroendocrine tissue in an ovarian dermoid cyst.

Ectopic ACTH production accounts for 15% of Cushing's syndrome presentations and is characterized by the presence of an excess of ACTH precursors. However in the case presented here ectopic ACTH production was from functional pituitary tissue within an ovarian dermoid cyst. Endocrine investigations showed that the problem behaved more like pituitary-dependent Cushing's disease and this is discussed. Furthermore, this case is one of familial dermoid cysts, another unusual phenomenon.
