[Headgears in orthodontics: mechanical principles and clinical application].

Understanding of the mechanics of displacement of molars during headgear treatment makes an efficient clinical application possible, while undesirable molar displacements can be prevented or compensated. Of paramount importance is the relationship of the line of force to the center of resistance of the tooth to be moved. This relationship is examined in three dimensions. Subsequently the applicability of mechanical principles in clinical use of headgears is discussed.

Clinical applications and technical aspects of parathyroid hormone measurements.

A review of human parathyroid hormone measurements is presented. Although parathyroid hormone was among the first hormones to be measured with the radioimmunoassay technique in 1963, there are even now still many pitfalls in both the technical aspects and clinical applicability of this assay. The main reasons for these difficulties are presented: firstly the complex intra- and extraglandular metabolism of parathyroid hormone results in the heterogeneity of the hormone in serum; secondly, the scarceness of pure human parathyroid hormone delays the development of a complete homologous human assay. The existing assays however have, despite their limitations, enormously contributed to our knowledge of parathyroid hormone secretion in both the normal state and in disease. The clinical applicability of these assays is reviewed and future technical improvements are suggested.

A vacuolar ATPase inhibitor, FR167356, prevents bone resorption in ovariectomized rats with high potency and specificity: potential for clinical application.

UNLABELLED: FR167356, a novel inhibitor of vacuolar ATPase, has high potency against osteoclast V-ATPase and low potency against lysosomal V-ATPase. FR167356 is the first compound of this nature to be tested. It has the potential to be useful for clinical application. INTRODUCTION: It has been suggested that the key issue regarding the therapeutic usefulness of V-ATPase inhibitors is their selectivity. MATERIALS AND METHODS: In in vitro and in vivo studies, we compared FR167356 with other vacuolar ATPase (V-ATPase) inhibitors, bafilomycin A1 and SB242784. H+ transport by various membrane vesicles was assayed by measuring uptake of acridine orange. Inhibitory activity against in vitro bone resorption was examined by measuring the Ca2+ release from cultured calvariae. In vivo, hypercalcemia was induced by retinoic acid in thyroparathyroidectomized-ovariectomized rats, and the effect on serum Ca2+ level was assessed. Ovariectomized rats were treated with FR167356 or SB242784. One week after surgery, free deoxypyridinoline levels in 24-h urine samples, which were collected from 6 h after administration of FR167356, were measured by ELISA. After 4 weeks of treatment, plasma biochemical parameters were analyzed. BMD of the distal femur metaphysis was measured with pQCT. Histomorphometric analysis of the proximal tibias was performed. Blood gases of rats treated with FR167356 were measured with a blood gas analyzer for estimating the effect of FR167356 on in vivo function of renal V-ATPase. RESULTS: FR167356, which is distinctly different from other V-ATPase inhibitors, has a high potency against osteoclast V-ATPase and low potency against lysosomal V-ATPase. Similarly, FR167356 inhibited bone resorption in vitro when stimulated by PTH, IL-1, and IL-6. FR167356 reduced retinoic acid-induced hypercalcemia in thyroparathyroidectomized-ovariectomized rats in a dose-dependent manner. Moreover, FR167356 was shown to restore BMD of ovariectomized rats caused by the inhibition of bone resorption. Ovariectomized rats treated with FR167356 did not show adverse symptoms, whereas SB242784 caused a decrease in body weight gain and significant changes in two plasma biochemical parameters. Interestingly, FR167356 treatment did not affect blood acid-base balance; however, FR167356 inhibited renal V-ATPase with a similar potency as for osteoclast V-ATPase inhibition. CONCLUSION: Comparison of FR167356 with SB242784 implies that the characteristics of FR167356 may be more appropriate for clinical application as a V-ATPase inhibitor.

GnRH agonists-antagonists--clinical applications.

We know that there is a wide range of clinical applications for GnRH analogues in the field of benign gynaecological disorders. As we understand more of the physiology and mechanism of these GnRH agonists, and there appear more varied and perhaps efficient delivery systems, and antagonists become available, it may be possible to develop a graded approach in suppression of the hypothalamic pituitary axis. The true potential of these agents is then yet to be fully realized. There can be no doubt they are going to influence practice dramatically over the next decade. The potency of these agents, perhaps specifically their effects of oestrogen deficiency and calcium bone metabolism, suggests that limitation of duration of use and timing of recurrent administration to individuals is likely to be necessary.

[The transverse coloplasty pouch. Review of experimental data and clinical application].

The transverse coloplasty pouch. Review of experimental data and clinical application. The transverse coloplasty pouch is a novel technique for sphincter preserving colonic reconstruction after total mesorectal excision. The method was developed in an animal model and resulted in favorable early functional results. Our results in patients with rectal cancer demonstrate that the technique is technically easy and safe. The transverse coloplasty pouch provides a neorectal reservoir of adequate volume and a favorable early functional outcome. It leads to a reduction in stool frequency compared with a straight anastomosis and the outcome is comparable to other colon reservoirs.

Flow cytometry: basic concepts and clinical applications in immunodiagnostics.

OBJECTIVE: To review current and possible future clinical applications of flow cytometry in immunodiagnostic procedures. DATA SOURCES: Recent research and review articles and textbooks on laboratory diagnosis and flow cytometry. STUDY SELECTION: Not applicable. DATA EXTRACTION: Performed by the author. DATA SYNTHESIS: Flow cytometry is the measurement of cellular and fluorescent properties of stained cells in a fluid stream as they move past a set of fixed detectors. The instruments are capable of measuring these properties on thousands of cells in but a few seconds. Many specimen types can be analyzed on the flow cytometer. CONCLUSION: Flow cytometry is especially useful in monitoring immunodeficiencies, leukemias, and other malignancies, but it has other uses, as well. Improvements in instruments, reagents, and methods will cause an increase in the number of applications for flow cytometry. More laboratories will be able to purchase the equipment as it becomes less expensive. Flow cytometry provides the clinician valuable information in the diagnosis and treatment of disease.

Emerging clinical applications of electrical stimulation: opportunities for restoration of function.

Emerging clinical application of electrical stimulation in three systems is reviewed. In the bladder, stimulation of sacral posterior roots reduces reflex incontinence and significantly improves bladder capacity. With the combination of anterior and posterior root stimulation, bladder control can be achieved without the need for rhizotomy. Preliminary research demonstrates that bladder contractions may also be generated by stimulation of the urethral sensory branch of the pudendal nerve, even after acute spinal cord transection, while inhibition of the bladder and control of urge incontinence can be achieved by stimulation of the whole pudendal nerve. Spinal cord stimulation can modulate the activity of the intrinsic cardiac nervous system involved in the regulation of regional cardiac function and significantly reduce the pain associated with angina pectoris. Finally in the area of upper airway disorders, functional electrical stimulation has great potential for increasing life support as well as for quality of life in chronic ailments, particularly obstructive sleep apnea and dysphagia.

[Evaluation of clinical application of intravascular ultrasound image].

OBJECTIVE: To prospectively evaluate the clinical applications of intravascular ultrasound (IVUS). METHODS: Four hundred and thirty seven patients with coronary artery disease (CAD) or suspect CAD were imaged with CAD and IVUS, including (1) assessment IVUS in 135 cases with normal coronary angiography (CAG) (2) comparison IVUS with CAG pre and after angioplasty in 52 patients with CAD (3) assessment of detecting calcification lesions with CAG and IVUS in 150 patients (4) predicting the cardiac events after stent implantation guided by IVUS (n = 47) and CAG (N = 53). RESULTS: IVUS may detect coronary atherosclerosis lesions with normal by CAG. CAG overestimated effect of angioplasty. IVUS was more accurate to determine effect and complications of PTCA compared with CAG. IVUS interpreted the mechanism of vessel dilatation in PTCA. The cardiac events after stent implantation were significantly reduced after IVUS examination as compared with CAG. CONCLUSIONS: IVUS represents a radically different approach to vascular anatomy. Unlike angiography, which displays the coronary artery as a silhouette of the contrast-filled lumen, IVUS generates a cross-sectional tomographic image of the lumen and vessel wall. IVUS plays an important role as a guidance in cardiac intervention.

[Evaluating the clinical applications of CTVL in laryngeal carcinomas].

OBJECTIVE: To evaluate the clinical applications of computer tomography virtual laryngoscopy (CTVL) in laryngeal carcinoma. METHOD: 34 cases of laryngeal carcinomas and 5 cases of normal larynx were scanned by spiral CT and analyzed by computers. The virtual endoscopy findings of all patients were compared with those of fiber optical laryngoscopy and surgical findings. All the carcinomas were pathologically proved. RESULT: The visualization of normal anatomical structure in larynx on CTVL is the same as that of fiber optic laryngoscopy. Tumor local extension showed in CT virtual endoscopy is superior to that of fiber optic laryngoscopy. Images of early laryngeal carcinomas showed in CT virtual endoscopy is not so good as those of fiber optic laryngoscopy. CONCLUSION: CT virtual laryngoscopy is a good complement to fiber optic laryngoscopy.

[Clinical application of portable spirometry in asthma].

OBJECTIVE: To investigate the clinical applications of portable spirometry in asthma. METHODS: Twenty patients with asthma were recruited from Peking Union Medical College Hospital. Flow-volume loop, simultaneous asthma symptoms, and mood were monitored three times a day for consecutive 14 days. RESULTS: In patients with a normal daytime spirometry, marked decline of forced expiratory volume in one second (FEV1) and peak expiratory flow (PEF) were observed at night and/or in the early morning. A within subject correlation analysis between FEV1, PEF, and asthma symptoms showed that the correlation between symptoms and airway obstruction was found only in seven out of twenty patients (35%). Four patients (20%) reported many symptoms with nearly normal portable spirometry. Accordingly, their symptoms were not correlated with FEV1 and PEF. This group of patients was defined as over-perceivers. On the contrary, another two patients (10%) did not report any symptoms while obvious airways obstruction was recorded by a portable spirometry. These patients were defined as under-perceivers. CONCLUSIONS: Dynamic monitoring of flow-volume loop with a portable spirometry is more accurate than routine lung function test in assessment of asthma severity. In addition, combined with simultaneous monitoring of symptoms, it would be of particularly helpful in identifying two specific types of asthma patients, e.g. over-perceivers and under-perceivers.

[Clinical application of sural neurocutaneous island flaps].

OBJECTIVE: To explore the clinical application characteristics of sural neurocutaneous island flaps. METHODS: Sural neurocutaneous island flaps were used to repair the skin defect accompanied bone and tendon exposure in the lower leg, around the ankle and foot in 21 cases, including 4 cases to repair the foreside of the foot back . Direct flap was used in 5 cases and reverse flap in 16 cases. Meanwhile the coverage and formation of sural nerve were surveyed together with the starting point of peroneal perforator. RESULTS: All the 21 sural flaps were survived, including sural nerve (18 cases) anastomose 12 cases, single trunk 4 cases, double trunk 2 cases. The anastomose site of medial sural cutaneous nerve and the communicating branch of lateral sural cutaneous nerve was at the point of 11 - 14 cm above the ankle in 12 cases. The lower was the anastomose site, the shorter was the sural nerve. The site is 4 - 7 cm above the ankle in 15 out of 18 sural nerve perforator branch cases, and the other 3 cases is 10, 11, 11.5 cm above the ankle respectively. CONCLUSIONS: Sural neurocutaneous island flaps are easy to separate. Major arteries are not injured. It is the ideal flap to repair the skin defect accompanied by bone and tendon exposure in lower leg, around ankle and foot. The nerve must be anastomosed when repairing the heel.
