Objective and subjective outcomes in surgery for chronic sinusitis.

Endoscopic and radiological findings in patients with chronic sinusitis do not always correlate with symptoms. Studies suggest that postoperative endoscopic examination of the sinonasal cavity provides prognostic information regarding the potential for future episodes of sinusitis and the need for revision surgery. It is recommended that findings on nasal endoscopy be included in future outcomes studies on sinusitis.

Nasal specific technique as part of a chiropractic approach to chronic sinusitis and sinus headaches.

OBJECTIVE: To demonstrate the use of nasal specific technique in conjunction with other chiropractic interventions in managing chronic head pain. CLINIC FEATURES: A 41-yr-old woman was treated for chronic sinusitis and sinus headaches. She had suffered weight loss and pain over a 2-month period. INTERVENTION AND OUTCOME: Chiropractic manipulation and soft tissue manipulation administered 2-6 times per month for approximately 1 yr had minimal long-term effect on the patient's head pain. When additional interventions (nasal specific technique and light force cranial adjusting) were added to the treatment regimen, significant relief of symptoms was achieved after the nasal specific technique was performed. The duration of the relief increased with successive therapeutic sessions, with minimally persistent symptoms after 2 months of therapy. CONCLUSION: The nasal specific technique, when used in conjunction with other therapies, may be useful in treating chronic sinus inflammation and pain. Further investigation is needed to identify the usefulness of the nasal specific technique as an independent intervention, the use of the technique in other types of patients and presentations, and the mechanism of therapeutic benefit.

One-year low-dose erythromycin treatment of persistent chronic sinusitis after sinus surgery: clinical outcome and effects on mucociliary parameters and nasal nitric oxide.

OBJECTIVE: In 17 patients with chronic sinusitis persistent after sinus surgery, long-term, low-dose erythromycin therapy was tested. The aim of the investigation was to study the clinical outcome and effects on nasal nitric oxide (NO), ciliary beat frequency (CBF), and mucociliary transport (saccharine transit time). STUDY DESIGN AND SETTING: We conducted a prospective open study at a tertiary teaching hospital. Symptoms were assessed using visual analog scales. NO was measured using a chemiluminescence analyzer, and mucociliary transport was measured with the saccharine crystal technique. CBF was measured in nasal brush samples using a phase contrast microscope. All patients were treated with erythromycin succinate 250 mg 2x daily or clarithromycin 250 mg 1x daily and were assessed after 3 months. In cases where there was no response, treatment was abandoned. The remaining patients (responders) were reassessed after 12 months of treatment. RESULTS: Of 17 patients, 12 responded to treatment. The 12-month follow-up showed an improvement in saccharine transit time (P < 0.05) but no significant change in CBF. There was a trend toward an increase in NO (P = 0.12). Endoscopic nasal examination scoring improved significantly (P < 0.01). In the visual analog scale scoring, the most pronounced improvements were seen in nasal congestion, sticky secretion, and runny nose at 3 and 12 months (P < 0.01). Improvements were also seen in headache (P < 0.05). CONCLUSION: The present study suggests that long-term, low-dose treatment with erythromycin is effective in persistent chronic sinusitis that does not respond to sinus surgery or systemic steroid/antibiotic treatment. SIGNIFICANCE: Long-term, low-dose erythromycin therapy seems to be a promising alternative when more conventional therapy fails. However, placebo-controlled studies are needed to validate the potential of this treatment.

[Rehabilitation therapy after nasal endoscopic surgery for nasal polyp and chronic sinusitis patients].

OBJECTIVE: To analyze the possible factors affecting the results of nasal endoscopic surgery in nasal polyp and chronic sinusitis patients and to discuss the ways to improve the results of operation. METHOD: Retrospective analysis was done in 226 patients followed up more than 3 year. The analysis was focused on patients' occupation, personal habits, extent of persisting on rehabilitation therapy after operation and clinical recovery status. RESULT: The patients who work in the environment rich of irritating substance and dust, the patients having unhealthy habits (e. g: spicy food and alcohol addicting) and the patients who did not take or insist on rehabilitation therapy after operation (e. g: local and systemic medication, nasal lavage and cleaning up of surgical cavities under nasal endoscope) had higher frequency of relapse and quicker recurrence. Rehabilitation therapy was the most important factor affecting the results of operation. CONCLUSION: Insisting on systemic rehabilitation therapy after operation is a occupation protection and kicking the unhealthy personal habits can prevent and reduce the recurrence of nasal polyp and chronic sinusitis after nasal endoscopic surgery.

Bacteriologic features of chronic sinusitis in children.

Aspiration of chronically inflamed sinuses was aseptically performed in 40 children. The median age was 11 years (range, 6 to 16 years). The sinuses were the maxillary (15 cases), ethmoid (13), and frontal (seven). Pansinusitis was present in five patients. All aspirates were cultured for aerobes and anaerobes and yielded bacterial growth in 37 patients. A total of 121 isolates (97 anaerobic and 24 aerobic) were recovered, accounting for 2.7 anaerobes and 0.6 aerobes per specimen. Anaerobic organisms were recovered from all 37 culture-positive specimens, and in 14 cases (38%) they were mixed with aerobic organisms. The predominant anaerobic organisms were Bacteroides species (36), anaerobic Gram-positive cocci (28), and Fusobacterium species (13). The predominant aerobic isolates were alpha-hemolytic streptococci (seven), Staphylococcus aureus (seven), and Haemophilus species (four). These findings indicate the major role of anaerobic organisms in the polymicrobial cause of long-term sinusitis in children.

Nebulization treatment with saline compared to bromhexine in treating chronic sinusitis in asthmatic children.

Twenty children aged 3 to 14 years with a history of bronchial asthma complicated by chronic sinusitis were studied in a double-blind study. Patients received, at random, over a period of 2 weeks, either 2 ml saline or 2 ml bromhexine (2 mg/ml) t.i.d. by means of a home nebulizer. A significant decline of clinical symptoms during both nebulization treatments as compared to the pretreatment symptom score was observed (mean score of 1.5 +/- 0.7 and 0.5 +/- 0.8, respectively, P less than 0.01). Both types of nebulization were equally efficient in reducing the symptom score. Radiological abnormalities were significantly more reversed after saline nebulization as compared with bromhexine (P less than 0.05), although both treatments showed radiological improvement (P less than 0.01). The present study indicates that nebulization with saline may have some beneficial effect on chronic sinusitis in asthmatic children. Bromhexine was not superior to saline for this purpose.

Fungal rhinosinusitis in patients with chronic sinusal disease.

AIM: Fungal rhinosinusitis in patients with chronic sinusal disease study. In the past decades, there has been an increase in fungal infections, and fungal rhinosinusitis (FRS) has been diagnosed more frequently. Knowing the fungal flora, its prevalence and symptomatic presentation in patients with chronic rhinosinusitis (CRS) will allow a better understanding of this disease, permitting a correct diagnosis, and treatment and formulating its prognosis. STUDY DESIGN: clinical retrospective with transversal cohort. MATERIAL AND METHOD: 62 patients diagnosed with FRS were selected among 890 cases of CRS undergoing endoscopic surgery. We assessed clinical history otolaryngologic examination with nasal videoendoscopy, CT scan, microbiologic and histopatologic tests. RESULTS: The prevalence of FRS was 6.7% and the fungal type most frequently found was the gender Aspergillus. Fungal ball was found in more than half the cases, and allergic fungal rhinosinusitis (AFRS) in more than one third of the patients. Evolution after endoscopic sinus surgery was more favorable in patients with fungus ball, with a minor number of recurrences. CONCLUSIONS: The symptomatic evolution following endoscopic surgery was more favorable in patients with fungal ball, who require a lower number of re-interventions.

[The status of autonomic nervous system in children with chronic sinusitis].

Such parameters of autonomic nervous system as initial vegetative tonicity, vegetative reactivity, vegetative support of activity were for the first time assessed in children with chronic sinusitis. The vegetative dysfunction was found in purulent and purulent-polypous sinusitis. The above vegetative unbalance dictates the necessity of addition of various vegetotropic modalities to combined treatment of chronic sinusitis.

Myeloperoxidase and interleukin-8 levels in chronic sinusitis.

We have recently phenotyped inflammation in non-infectious allergic and non-allergic chronic maxillary sinusitis using sinus biopsies and lavage fluids. In this first paper, we have concentrated our work on the eosinophil, T cell, mast cell and macrophage infiltrates. However, many unresolved questions remain and particularly the role of neutrophils needed to be addressed. In the present study, we focused on the neutrophilic inflammation: myeloperoxidase (MPO) and interleukin-8 (IL-8) were measured by immunoassays and neutrophils were enumerated by conventional staining in the sinus lavage fluids of 16 patients with chronic sinusitis and six control subjects. Both MPO and IL-8 levels were significantly higher in patients than in controls (P < 0.01 and 0.005, respectively). There was a significant correlation between MPO levels and neutrophil numbers, and between MPO and IL-8 levels in the sinus lavage fluid (P < 0.0001, Spearman rank correlation). The presence of high levels of IL-8 in the lavage fluids of patients suffering from chronic sinusitis, levels which correlate with those of MPO, suggests that this cytokine may activate neutrophils in this chronic disease.

Scanning electron microscopy findings of human respiratory cilia in chronic sinusitis and in recurrent respiratory infections.

Acute and chronic infections cause morphological changes in the respiratory mucosa. The ultrastructure of human respiratory mucosa was studied by scanning electron microscopy from the maxillary sinuses of 28 patients, with chronic sinusitis, from middle turbinates of 60 patients, with recurrent respiratory infections, and from healthy sphenoidal sinuses of 31 patients. A loss of ciliated cells and an increasing number of nonciliated columnar cells with microvilli were seen in 62 per cent of the maxillary sinus mucosa. Ciliary disorientation was seen in 81 per cent of the chronically infected sinus mucosa and eight per cent in the healthy sphenoidal sinuses. Also metaplasia and extrusion of epithelial cells were prominent in chronic infections. Compound cilia were seen in 52 per cent of the samples from patients with chronic sinusitis and in 31 per cent of the healthy sphenoidal sinuses. Short cilia were often seen in infected mucosa indicating ciliogenesis.

Bacteriological findings and antimicrobial susceptibility in chronic sinusitis with nasal polyp.

CONCLUSIONS: We recommend amoxacillin/clavulanate, cephalosporins and macrolides rather than penicillin as the first-line drug in chronic sinusitis with nasal polyps. In cases where there is no improvement of symptoms, cultures should be taken from the middle meatus, followed by appropriate selection of second-line antibiotics according to the sensitivity test results. OBJECTIVE: To investigate the causative bacteria and the antimicrobial susceptibility in patients with chronic sinusitis and nasal polyps in Korea. MATERIALS AND METHODS: The bacteriology and antimicrobial susceptibility of maxillary sinus aspirates from 81 patients were evaluated. RESULTS: Aerobes were isolated from 58.0% of the cultures from the middle meatus and from 48.1% of those from the maxillary sinus. Staphylococcus aureus, Haemophilus influenzae, and Streptococcus pneumoniae were the most prevalent aerobic pathogens. Anaerobes were isolated from 8.6% of the cultures from the middle meatus and from 18.5% of the cultures from the maxillary sinus. The predominant anaerobic organisms were Prevotella and Peptostreptococcus in adults but none of them were cultured in children. A high rate of concordance of the middle meatus and maxillary sinus was noted. Monomicrobial infection was most commonly observed. Ampicillin-resistant H. influenzae isolates were cultured in 46% of the cases. Penicillin resistance rates were 93% for Staph. aureus; 25% of Strep. pneumoniae were intermediate and 25% were resistant.

Comparison between low-dose and standard-dose multidetector CT in patients with suspected chronic sinusitis.

OBJECTIVE: This study was designed to compare low- and standard-dose multidetector CT (MDCT) findings in patients with suspected chronic sinusitis. SUBJECTS AND METHODS. Fifty patients underwent MDCT at 10 and 150 effective mAs. The low-dose MDCT protocol delivered a radiation dose of 0.047 mSv in men and 0.051 mSv in women, whereas the standard-dose MDCT protocol delivered a radiation dose of 0.70 mSv in men and 0.76 mSv in women. Scans of the right and left sides of sinonasal cavities were reviewed by three radiologists, with each physician reviewing a scan twice over an interval of more than 2 weeks. The reviewers were asked to evaluate the scans for eight mucosal and two bone abnormalities. We calculated the number of discrepancies in observed abnormalities between pairs of reviewers, among all three reviewers, and between findings on scans acquired with the two radiation doses. RESULTS: The mean number of discrepancies in observed abnormalities on scans acquired with different radiation doses ranged from 0 to 5.2. Discrepancies between pairs of reviewers ranged from 1.0 to 12.8 for low-dose scans and from 1.0 to 13.0 for standard-dose scans. Discrepancies among all reviewers ranged from 1.0 to 10.3 for low-dose scans and from 1.0 to 8.7 for standard-dose scans. In analyzing cases of significant discrepancies in observations, we found greater variation between pairs of reviewers and among all three reviewers than between findings obtained with different dose levels. CONCLUSION: Dose reduction played a far less important role in discrepancies of detected abnormalities than did the human element of reviewer observation. Given this finding and the fact that low-dose MDCT delivers a radiation dose that is no higher than that delivered by a four-view radiographic examination, low-dose MDCT should be considered the imaging method of choice in patients with suspected chronic sinusitis.

[Chronic sinusitis in patients infected by HIV: therapeutic strategies].

Chronic sinus pathology is a frequently encountered disease in HIV infected patients. The responsible bacterial agents and management are yet to be settled. The authors report the results of a retrospective study led in the unit of Head and Neck Surgery of the University Hospital of Nice. 25 patients where HIV holders and had a sinus pathology which had lasted more than 6 weeks, despite one or several antimicrobial drug administrations. Clinical and CT scan data are detailed as well as pathology results of the samples harvested during sinus surgery (bacteria free 32%, Pseudomonas aeruginosa 32%, Streptococcus pneumoniae 16%, Staphylococcus aureus 16%, Hemophilus influenzae 16%, anaerobic agents 16% and Toxoplasma gondii 4%). All patients underwent surgery (antral puncture with maxillary sinus drainage 17, functional endoscopic sinus surgery 8). Results where gathered at 4 months with 76% of relapse (100% in patients with less than 200/mm3 CD4 cells). In conclusion empiric antimicrobial drug therapy will be expected to be also effective on Pseudomonas aeruginosa, the surgical management being most often deceiving and leading to relapse especially when considering the frequency of bilateral morbidity and the level of CD4 cell below 200/mm3.

Is septal deviation a risk factor for chronic sinusitis? Review of literature.

INTRODUCTION: The purpose of this work is to examine the role of septal deviation in adults in the pathogenesis of chronic sinusitis. This evaluation would allow a better understanding of the contributing factors to this pathology and would improve results in their management. MATERIAL AND METHODS: This review was based on a Medline research over English and French literature published between 1980 and May 2001. Twenty-five papers were scrutinized. RESULTS: Few investigators examined the role of the deviated septum on sinus disease. Methodologies and results are often contradictory. Furthermore, these studies are mainly based on radiological imaging. In most studies, prevalence of septal deviation looks the same when having compared among patients with radiological rhinosinusitis and among general population. An increased incidence and severity of sinus disease correlated to an increasing angle of septal deviation in the ostiomeatal complex area is reported by Calhoun (7), Youssem (4), Matschke (18) et Elahi (20). One study (1) demonstrates a significant association between the shape of the nasal septum and the location of the sinus disease: septal crest is associated with homolateral sinusitis, "watch-glass shaped diformity" with controlateral sinusitis. Stammberger (21, 25) suggests a pathophysiological role of the septum through a mechanical obstruction on the ostiomeatal complex; Danese (1) and Blaugrund (19) by an alteration of the ciliary activity secondary to a modified air flow. Bachert (24) demonstrates a connection between septal deviation and antral ventilation. CONCLUSION: This literature review cannot establish a definite role to the nasal septum neither as the pathogenesis of chronic sinusitis nor as a contributing factor. No relationship between septal surgery combined with sinus surgery, and the postoperative prognosis nor on the subjective comfort of patients can be demonstrated. Subsequently, it appears that performing septoplasty only aims at relieving nasal obstruction complaint or at improving surgical access to the ethmoïd sinus.

Adenovirus and respiratory syncytial virus in chronic sinusitis using polymerase chain reaction.

The aim of this study is to investigate the role of adenovirus and respiratory syncytial virus (RSV) in chronic sinusitis using the polymerase chain reaction (PCR) to assay for the viruses. PCR has proved to be more sensitive and specific than viral cultures and immunoassays in the detection of viruses. Adenovirus and RSV are among the most common viruses to cause upper respiratory tract infections. Sinus mucosa biopsies from 20 patients undergoing endoscopic sinus surgery were sterilely collected. Four specimens (20%) tested positive for RSV by PCR and none tested positive for adenovirus. Only one specimen tested positive for RSV and one for adenovirus by viral culture and immunofluorescence. Bacterial cultures tested positive in 40% of the 20 specimens. PCR can be used to detect RSV in patients with chronic sinusitis and PCR is more sensitive than viral culture and immunofluorescence techniques on sinus polyps and mucosa.

[Natural humoral and cellular factors of nonspecific defense in the treatment of chronic sinusitis in children].

Sparing treatment of chronic purulent sinusitis in children is an urgent problem of rhinology. Usage of preparations of donor (parental, of the same group) autoleukomass and blood plasm which possess all the components of humoral and cellular immunity proved highly effective against chronic purulent sinusitis in children. The drawback of this method is the necessity of regular lavage of the sinuses after insertion of the leukomass and heparinized plasm. The use of donor serum (parental, of the same group) also produced a good effect in purulent sinusitis. The serum advantage over the leukomass or plasm is that it can be left in the sinuses until the next procedure.

Chronic sinusitis refractory to standard management in patients with humoral immunodeficiencies.

Chronic refractory sinusitis is a common feature in patients with primary immunodeficiencies. The efficacy of standard therapeutic strategies is questionable. In an open trial we evaluated the efficacy of azithromycin, N-acetylcysteine and topical intranasal beclomethasone (100 microg twice daily for 6 weeks) in 16 patients with primary immunodeficiencies (median age 13.5 years, range 5-32 years). All patients suffered from chronic sinusitis despite regular immunoglobulin replacement therapy every 3 weeks. Magnetic resonance imaging (MRI) scans were performed before and after 6 weeks of treatment to evaluate morphological changes in the paranasal sinuses. Nasal swabs and washings were taken for microbial analysis and measurement of inflammatory mediators (IL-8, tumour necrosis factor-alpha (TNF-alpha), eosinophilic cationic protein (ECP)) before and post therapy. Inflammatory mediators in nasal secretions were significantly elevated in patients: IL-8 median 2436 pg/ml (range 441-5435 pg/ml), TNF-alpha 37.3 pg/ml (3.75-524 pg/ml) and ECP 33 ng/ml (1.5-250 ng/ml) versus age-matched healthy controls: IL-8 median 212 pg/ml (99-825 pg/ml), TNF-alpha 3.77 pg/ml (2.8-10.2 pg/ml) and ECP 1.5 ng/ml (1.5-14.8 ng/ml) (P < 0.0001). Inflammation of the maxillary sinuses was confirmed by MRI scans in all patients, additionally infection of the ethmoidal and frontal sinuses was recorded in five patients. Bacterial growth appeared in 11 out of 16 cultures. In spite of therapy, no improvement in sinal inflammation visualized by MRI was achieved. Moreover, no significant decrease in pathogens and levels of inflammatory mediators could be detected (IL-8 1141 pg/ml, 426-4556 pg/ml; TNF-alpha 13.9 pg/ml, 4.1-291.6 pg/ml; ECP 32.3 ng/ml, 3.7-58.4 ng/ml). Our results demonstrate that conventional management of sinusitis is of little benefit in patients with chronic refractory sinusitis with an underlying immunodeficiency. More studies are needed to test antibiotic regimens, probably combined with surgical drainage and anti-inflammatory agents.

Neurological consequences of scuba diving with chronic sinusitis.

Sinus barotrauma from scuba diving is relatively common, usually self-limiting, and often the result of transient nasal pathology. We describe serious neurological sequelae occurring in two scuba divers who had chronic sinusitis We suggest guidelines for evaluating and treating divers who have chronic sinusitis. Divers with nasal or sinus pathology should be aware of the potentially serious consequences associated with scuba diving even after endoscopic sinus surgery to correct this condition.
