Leeches to hirulogs and other thrombin-directed antithrombotics.

Leeches have been used for various medicinal purposes since before written history. Bloodletting and leeching declined with the advent of modern medicine. Nevertheless, the European medicinal leech has made a comeback in reattachment and plastic surgery. The antithrombotic substance of this leech is the small protein, hirudin, which has recently been cloned and advanced as an antithrombotic. From speculating how hirudin interacted with thrombin and before knowledge of the crystallographic structures of hirudin-thrombin complexes, the bridge-binding double-ligand concept was born and led to the highly specific thrombin inhibitors of the hirulog class. Like heparin, hirulogs and recombinant hirudins are not orally active but should fill needs where heparin and its derivatives have shortcomings. On the other hand, they most likely will be supplanted by small-molecule thrombin inhibitors when sufficiently specific and nontoxic ones are found. Other approaches to antithrombotic therapy include modulating cellular functions of thrombin. Because thrombin has central bioregulatory functions in thrombosis and hemostasis, as well as wound healing, it is an attractive target for antithrombotic intervention.

[Individualized hemodilution in acute brain infarct using a 20% albumin solution and physiological saline solution].

OBJECTIVE: To determine the effect of normovolaemic haemodilution in patients after a cerebrovascular accident. DESIGN: Prospective, randomized clinical trial. SETTING: St Lucas Hospital, Amsterdam. METHOD: Normovolaemic haemodilution was achieved by means of bloodletting and administration of a 20% solution of albumin plus crystalline infusion fluids under haemodynamic and rheological monitoring during the acute phase of the cerebral infarction. All patients were subjected to general intensive care and monitoring with a pulmonary artery catheter. This custom-tailored fluid therapy was guided by a pulmonary wedge pressure of 12 mm Hg (SD 3) and a haematocrit (Ht) of 0.32 l/l (SD 0.02). The control group only received individually dosed rehydration with crystalline infusion fluids. Endpoints of the study after 3 months were mortality and dependence/independence concerning everyday functioning. RESULTS: The results in the total haemodilution group and the control group did not differ significantly. However, in the subgroup with normal Ht (< 0.45 l/l; n = 201) there was a significant reduction (p < 0.05) of the mortality after 3 months (27% and 16%, respectively) and an increase of independence at home (35% and 48%, respectively) due to a reduction of the viscosity by means of haemodilution with albumin (a specific viscosity effect in the normovolaemic group). In the control group with raised Ht (dehydration; Ht > or = 0.45 l/l; n = 50) there was a significant decrease (p < 0.005) of the mortality after 3 months (27% and 8%, respectively) and an increase of independence at home (35% and 59%, respectively) compared with the control group with normal Ht without signs of dehydration (Ht < 0.045 l/l; n = 102), due to rehydration exclusively with crystalline infusion fluids (a specific rehydration effect in the dehydrated group). CONCLUSION: In cerebrovascular accident patients haemodilution should be adjusted individually; in normovolaemic patients haemodilution should be carried out with an albumin solution; the higher the Ht, the more rehydration with crystalline infusion fluids is to be carried out.

[The pewter bleeding bowls].

In the late seventeenth century, then along the eighteen and nineteenth centuries the amount of the bloodlettings was measured by means of three pewter bleeding bowls that held three ounces of blood, about 300 millilitres. In the middle of the nineteenth century new and large bleeding bowls with metric graduations were manufactured only by the Parisian potters.

Blood in ancient Jewish culture.

The article analyzes the Jewish attitude towards blood, conceived both as the vehicle of life, and as a polluting product of feminine bodies. The author analyzes numerous Biblical sources concerning the 'unapproachable' blood of menstruation, the role of blood in the generation of the fetus, the blood as source of illness, the practice of bloodletting, and finally the idea that male menstruation exists as a peculiarity of the Jews.

[Replantation of amputated ear with microtechnique].

OBJECTIVE: To report replantation of amputated ear with microtechnique in 5 cases. METHODS: From 2002 to 2005, 5 totally amputated ears were replanted and 4 obediently, 1 retrogressively. In every case, 1-2 arteries and 1-3 veins were anastomosed. Seven vessels were detective and 1-6 cm vessels were transplanted from forearm or dorsal side of hand. Amputated ears obtained blood supply again in about 6-10 hours after injury and exploration was enforced for venous crisis in 2 cases, and bloodletting in 1 case. RESULTS: All 5 cases survived. Auricles possess perfect shape, no pigmentation, slight atrophy and perfect sensation. CONCLUSIONS: Amputated ear should be replanted as long as no obvious contusion occurred, and the keys to prevent vascular crisis are cutting off unhealthy vessels, grafting superficial veins for bridging, and the high quality of vascular anastomosis.

[Production of a fibrinogen concentrate from small amounts of human autologous blood plasma and its short-term characteristics].

Simple procedure was developed for production of fibrinogen concentrate from small amounts of human autogenous blood using precipitation with polyethylene glycol and ammonium sulfate. Maximal yield of fibrinogen was obtained using polyethylene glycol 2,000 and 6,000 daltons at concentrations 7% and 4.5%, respectively. The fibrinogen preparations included fibronectin, inhibitors of proteinases, factor XIII of blood coagulation, plasminogen. Sigma-Aminocapronic acid and contrical should be added during bloodletting as well as into blood plasma before isolation of fibrinogen to avoid possible fibrinogen proteolysis.

[Effects of volume and rate of blood loss on indicators of auto-hemodilution].

The results of synchronous monitoring of blood impedance versus body impedance in dog experiments showed that the speed of physiological hemodilution related to blood loss up to 30 ml/kg is poorly dependent on the rate of blood loss (0.4-1.0 ml/kg/min) and averages 0.186 +/- 0.02 ml/min/kg. Termination of blood loss is followed by marked reduction in the rate of auto-hemodilution. In bloodletting the studies revealed a linear dependence between the volume of blood loss up to 30 ml/kg and tissue fluid volume entering the blood stream. In blood loss intestinal absorption of fluid does not block the fluid introduction from the interstice. When associated, both these flows increase volume of fluid filling vascular bed and promote stable recovery of the baseline circulating blood volume.

[Characteristics of oxygen balance and acid-base equilibrium of the blood in patients with hereditary erythrocytosis].

Blood oxygen balance and acid-base equilibrium were studied in 22 Chuvash patients with hereditary erythrocytosis. Despite the lack of the anomalous types of hemoglobin in the given type of erythrocytosis a number of the patients showed secondary tissue hypoxia associated with hyperoxia of the peripheral venous blood. It is shown that tissues are capable of extracting oxygen from the blood. It is assumed that the development of tissue hypoxia in the patients is determined by a fall in the rate of oxygen diffussion from the capillaries to the tissues or by blood shunting via the arteriolo-venular anastomoses because of microcirculatory disorders. The lowering of hematocrit to the level close to normal with the aid of bloodlettings makes oxygen balance return to normal and reduces tissue hypoxia.

[Assessment of the erythropoietin-producing function of the kidney and liver under controlled perfusion].

The balance of erythropoietin production by the dog kidney and liver was studied during controlled normoxic perfusion. The hormone production was stimulated by acute posthemorrhagic anemia (bloodletting of 25% total blood volume) combined with subcutaneous injection of cobaltous chloride (250 microM/kg body weight). The increase in erythropoietin level was revealed in posthypoxic animal perfusate after 6 hours of perfusion. The amount of hepatic erythropoietin was shown to be 2.5 times higher than that excreted by kidneys.

[Functional morphology of the hypothalamo-neurohypophyseal system and juxtaglomerular apparatus in acute vascular insufficiency].

The functional morphology of the hypothalamic neurohypophyseal system (HNHS) and juxtaglomerular apparatus (JGA) were studied in 50 male cate. A rising secretory activity in HNHS and JGA, accompanied by accumulation of hormone-containing granules, was seen in acute vascular insufficiency caused both by the injection of a ganglion-blocking agent and by bloodletting. This factor is qualified by the authors as a specific sign of hypotonic stress, consisting in incomplete use of HNHS and JGA adaptive capacity in acute pressor hormone deficit.

Effect of missile velocity on the pathophysiology of injuries.

The investigations were carried out on sheep weighing 55 kg on the average. Under general anaesthesia both hind legs were shot through from a distance of 15 m separately with missiles of high or medium initial velocity. For producing haemorrhagic shock in some animals rapid bloodletting was done with removal of 45% of the circulating blood volume. At definite time intervals the serum levels of adrenaline, noradrenaline, glucose, free fatty acids and insulin were determined in the arterial blood. The heart rate, pressure in the aorta and central venous pressure were measured. Following shots with high velocity missiles the observations were as follows: 1) signs of shock appeared earlier and were more dramatic, 2) the extent of exit wound was on the average threefold as large, 3) haemodynamic disturbances developed earlier, increased more sharply and were much greater, 4) the rise of catecholamines level was much more violent reaching many times higher values and persisting longer, 5) changes or glucose, insulin, free fatty acids and insulin were much more pronounced, developed earlier, and regressed after a longer time than after injuries caused by missiles of medium initial velocity.

[Mechanism of changing the cellular composition of the bone marrow during experimental toxic diphtheria].

Hemopoietic shifts in the bone marrow under experimental diphtheria intoxication have been studied. The number of immature elements of the erythroid series was shown to be lowered in the presence of an increase in the granulocyte pool. Under these conditions the erythroid sprout did not respond to bloodletting and the number of reticulocytes in the peripheral blood significantly dropped. However, there was an increase in the inclusion of 59Fe in red cells. The adrenomimetic ephedrine raised the content of reticulocytes in blood and 59Fe inclusion in red cells in the experimental animals and stimulated the reproduction of the erythroid series cells.

[Animal and viral mRNA translation in a mRNA-dependent cell-free system and rabbit reticulocytes].

A system of cell-free synthesis from rabbit reticulocytes is described. A method of repeated bloodlettings for induction of reticulocytosis was used. The lysate of washed reticulocytes was treated with micrococcus nuclease enabling effective inhibition of synthesis in endogenous templates. The translating activity is manifested most completely in the presence of the following compounds: ATP, GTP, ATP-regenerating system, SH-compounds, polyamines, tRNA. This system must be optimized in K+ and Mg2+ concentrations for different templates. The system is tolerant to high concentrations of rRNA which allows the utilization of total RNA preparations in the experiments. This system may be satisfactorily used for translation of mRNA of different origins.

This article is based on the review of more than eighty references involving clinical treatment, analgesia, auriculoacupoint for diagnosis and its mechanism. In the first part, selection of acupoints, methods of manipulation, therapeutic effects and related experience in 56 kinds of respiratory, circulatory, digestive, urological, gynaecological, obstetrical, pediatric, otolaryngologic and ophthalmologic, dermatologic diseases and diseases of the nervous systems, as well as treatment of acute abdominal diseases, reduction of body weight, abstinence from smoking and drinking, etc. are discussed. A variety of stimulation methods, such as auriculoacupuncture with filform needles, needle embedding therapy, point injection, bloodletting, laser irradiation, aurioular plaster and pressing therapy, etc are also introduced. In the second part, experiences of surgical operation and endoscopy under auriculoacupuncture anesthesia are described. The third part deals with diagnosis using auriculoacupoints. Also described in this part is the clinical application of auricular acupoints in the diagnosis of cancers, coronary diseases, cholelithiasis, hepatitis, tertian malaria, etc. The correlation between the morphology and function of auricular acupoints, i.e. the relationship between auricular acupoints and visceral function, and main development in clinical therapy, diagnosis and mechanism of auriculoacupuncture are introduced in the fourth part, including the results of investigations of reactions of auriculoacupoints during disorders of the body and viscerae, reactions of viscerae during stimulation of auriculoacupoints and their transmission routes. Based on the analysis of the history and current status of research on auriculoacupuncture, the author emphasizes the necessity of attaching importance at the same time to prophylaxis and treatment of serious diseases and standardization of nomenclature. Suggestions in this connexion are also made.

Neocytoapheresis in the treatment of polycythemia vera.

In absence of thrombocytosis, periodic bloodlettings represent the elective therapy of polycythemia vera. In the present study we tested if neocytoapheresis, a method able to remove large quantities of younger, and then bigger red cells could represent an alternative therapeutic choice in these patients. We found that the employment of neocytoapheresis produced a remarkable delay in the time of procedures with a mean interval of 100 +/- 55 days. The mean value of hematocrit before neocytopheresis is resulted statistically different in comparison with the hematocrit after the procedure (p = 0.0001). The reticulocyte count is resulted higher in apheresis product in comparison with the blood control measured before procedure (p = 0.0001). In the same way, the mean corpuscular volume in the collection bags was higher than the volume measured before neocytoapheresis (p = 0.0095). We did not find any variation about the mean values of white blood cells and platelets before and after neocytoapheresis in the patients examined. These preliminary data seem to underline a better withdrawal of big size cells by this technique suggesting the efficacy of neocytoapheresis in the treatment of polycythemia vera.

[Organization of mobile structures in the blood stream--basis for the function of perfluorocarbon "artificial blood"].

Research into the synthetic blood substitute of the Perftoran emulsion was shown that particle dynamical chains were formed by the turbulization of a particle motility in a blood flow. This chain display an activity of the oxygen carrier channel into the tissue of vessels from the erythrocyte which remained after a bloodletting. Moreover the review presented other results of safety and efficacy of perfluorocarbon blood substitutes and theirs prognosis of future progress.
