Physiological aspects of the aging cardiovascular system.

Most putative studies on the physiology of the aging cardiovascular system in humans have been cross-sectional. The results are therefore bedeviled by selective mortality and probably influenced by subclinical disease processes. Even accepting these problems, critical reading of the published literature shows that there is very little substantive evidence for the "well-known" age-related changes in cardiovascular physiology such as baroreflex impairment, sympathetic hyperactivation, adrenoceptor dysfunction, or neurohumoral abnormalities.

The influence of fixatives and other variations in tissue processing on nuclear morphometric features.

The influence on the area and numerical density of nuclei was investigated in 5-mm-thick slices of guinea pig liver for different fixatives and variations in tissue processing: delay in fixation, air drying, degree of acidity of 10% formalin (= 4% formaldehyde), Bouin and mercury-formalin fixatives, acetone and ethanol dehydration and understretching and overstretching of the paraffin-embedded sections. Air drying (either forced or as a result of delayed fixation), the type of fixative and the degree of acidity affected the nuclear area. Regarding the latter, nuclear area was approximately 25% lower for pH less than or equal to 3 as compared with pH greater than 5. In comparison with the standard tissue processing used, the nuclear density was higher after all of the variations studied (air drying, acetone dehydration and fixation). These findings indicate that nuclear area, in contrast to other tissue components, is relatively insensitive to variations in tissue processing. However, it is essential to regularly measure the pH of the fixative: deviations from pH = 7 should be carefully avoided in order to keep nuclear area variations as a result of tissue processing within acceptable limits.

Legal abortion: the impending obsolescence of the trimester framework.

Women who wish to terminate a pregnancy, and physicians willing to perform abortions, are subject to increasing harassment from groups which challenge the constitutional abortion right upheld by the Supreme Court in Roe v. Wade. Their vulnerability, in fact, parallels the vulnerability of the abortion right. This Article analyzes the inherent weakness and impending obsolescence of the trimester framework established in Roe. Present medical evidence of maternal health risks and fetal viability demonstrates that the trimester framework is inconsistent with current medical knowledge, and will likely be rendered obsolete by developments in medical technology. The Article suggests that adoption of an alternative constitutional basis for legal abortion is necessary to preserve the abortion right, and explores the utility of two arguments grounded in the equal protection doctrine. Finally, it discusses means of preserving legal abortion within the confines of the trimester framework established in Roe v. Wade.

The making of an adolescent clinic.

This paper addresses the nationwide need for comprehensive health care services directed toward adolescents, outlining a multidisciplinary team program designed to provide such services. The national problem of teenage pregnancy is discussed, and the multidisciplinary team's success with family planning counseling is described. The authors strongly encourage the development of more school- or community-based clinics based on a comprehensive health care team model.

Marine fish as source of protein supplement in meat.

For the past 2 decades, a great deal of research has been done in fish technology, particularly in the area of mechanically deboned minced fish. Minced fish is the edible muscle flesh of fish that has been mechanically separated from the bones and skin. Ideally, the product is prepared from a high quality fish and resembles hamburger meat. In its final form, minced fish is used either as an ingredient or as an extender in seafood or in food products that require further processing. On the basis of technological advancements, the National Marine Fisheries Service of the U.S. Department of Commerce and the National Fisheries Institute jointly petitioned the U.S. Department of Agriculture in 1980 to add minced fish at a level of 15% in the meat formulation of frankfurters. This paper explores certain aspects of processing, production, acceptance, and hazard assessment of minced fish ingredients as possible protein supplements in meat and poultry food products relative to this request.

Client relations in South Asia: programmatic and societal determinants.

Client relations constitute a neglected area of research in family planning. Findings from studies in northern India and Bangladesh reveal considerable variation in both the quantity and quality of contacts in programs that function under roughly comparable socioeconomic conditions. Client relations are determined by a complex set of forces in which both programmatic factors and conditions pertaining to the societal environment play a key role. Worker-client exchanges have a net, incremental effect on contraceptive use.

[Examination and patient management in x-ray diagnosis using a computerized hospital communication system].

Concept, realization and our 2-year experiences in the routine use of a computerized data-processing communications system supporting the central radiology department in a 1,625-bed, municipal hospital are discussed. Via the hospital ward-linked terminal network, routine X-ray examination is so ordered that individual medical case information is provided, which certifies qualified clinical and diagnostic procedure along with good patient preparation and transportation. Complete information and documentation are also ensured. It was shown that good motivation among involved radiologists and their close working-association with other departments are mandatory from the start to maintain exact time schedules and satisfactory completion of ordered X-ray procedures. Subsequently, the assistence of computerized data processing is accepted for routine use, as time goes by, insofar as it is appropriate by used.

[Study on remodelling of bone grafts--an application of nuclear medicine (author's transl)].

The remodelling of bone grafts depends to a large extent upon the type of graft and the condition of the recipient site. We applied 99mTc-phosphate scintigraphy in a follow-up study on cases treated by bone grafting, and quantitative analysis of the scintigram by computer to make clear the difference of remodelling time or the process of acceptance between a cancellous and a cortical bone grafting or due to various graft and conditions. The result revealed that the grafted bones which were smoothly adapted for subsequent growths or functions could restore normal accumulation ratios by 36 months after the grafting. When the cancellous bone was grafted to grafting beds with good conditions such as osteotomy site in the cases with coxarthrosis deformans or congenital dislocation of the hip, it could attain the quickest recovery of the normal accumulation ratio. The next quickest recovery of accumulation ratio was attained by grafting the cancellous bone to the grafting beds with poor conditions such as osteomyelitis, pseudoarthrosis, and bone tumor. The third best accumulation ratio could be attained when the cortical bone was grafted to grafting beds with good conditions, while the slowest recovery to normal accumulation ratio was noted when the cortical bone was grafted to grafting beds with poor conditions.

Medical, legal and ethical issues in critical care.

This report is intended to familiarize the reader with the current legislation concerning state definitions of death, the pertinent judicial decisions concerning termination of life support and the current status of medical/ethical committees. However, it is of paramount importance to all health care personnel to become involved in the resolution of these problems. Current information regarding legal clarity to these issues leaves much to be desired. Unless we wish major decisions that will affect us all to be decided by others, it is strongly suggested that we become directly involved in this process and accept the challenge and the opportunity to influence major legal, ethical, and medical decisions.

Prediction of primate splice junction gene sequences with a cooperative knowledge acquisition system.

We propose a cooperative conceptual modelling environment in which two agents interact: the machine and the human expert. The former is able to extract knowledge from data using a symbolic-numeric machine learning system, and the latter is able to control the learning process by accepting and validating the machine results, or by criticizing those results or the explanation that the system produces on them. The improvement of the conceptual modelling relies on the cooperation between the two agents. Results obtained with our method on prediction of primate splice junctions sites in genetic sequences are far better than those reported in the literature with other symbolic machine learning systems, and are as better as those obtained with some artificial neural networks methods reported at present. But in opposite to neural networks which lack of argumentation, our system provides the user a plausible explanation of its prediction.

[Systemic family therapy with stroke patients].

Systemic family therapy is an useful method of support in the rehabilitation of patients with strokes. The process of accepting the new conditions can be fostered within the network of relationships. This acceptance enables the patient and his family to develop a new meaningful future. The people involved have to consider new possibilities of organizing their relationships within their ideas and values according to their changed psychic needs.

Maternal care receptivity and its relation to perinatal and neonatal mortality. A rural study.

A longitudinal study was conducted on 212 pregnant women from May 1987 to April 1988. Maternal Care Receptivity (MCR) "an innovative approach" was adopted for the assessment of maternal care services provided to pregnant mothers at their door steps. During follow-up, scores were allotted to each of the services rendered and antenatal status of pregnant women. Depending on the score--MCR was classified as high (11 to 8), moderate (7 to 4) or poor (3 to 0). Perinatal and neonatal deaths were recorded and an inverse relationship between MCR and perinatal and mortalities was observed (z = 5.46, p < 0.0001). Significantly, no perinatal or neonatal deaths occurred in women with high MCR. One of the most important cause of high PNMR and neonatal mortality rate in developing countries is poor MCR, i.e., under utilization of even the existing maternal health services. The main reasons for this under utilization appear to be poverty, illiteracy, ignorance and lack of faith in modern medicine.

[Cell and gene transfers for therapeutic use into the central nervous system].

The current conception of progressive nerve cell loss associated with neurodegenerative diseases is no longer compatible with the completely irreversible process regularly accepted for so many years. This optimistic observation is based on the demonstration that the central nervous system in the adult is a very plastic entity. Data on neuroplasticity suggest that neurons could be replaced by integrating cells into adult nervous circuits and thus counteract neuron death although the initial cause of the disease would remain unknown. Intracerebral neuron transplantations, used as substitution material, and the transfer of genes encoding neuroprotective factors are among the latest therapeutic approaches which, it is hoped, will open a new era in neurology where the neurologist's main activity will no longer be simply diagnosis.
