[Effect of dexamethasone on water-salt metabolism and renal function in healthy humans].

Dexamethazone at a single dose of 2 mg caused in the next 24 hours retention in the organism of a healthy man of a liquid and salts against the background of an abrupt fall of excretion by the kidneys of 17-oxycorticosteroids. Most distinctly this effect manifested itself after an aqueous test (20 ml/kg). A functional test with dexamethazone may be used clinically in assessing the condition of the hormonal link of the water-electrolyte metabolism regulation and renal function.

[Degree and frequency of activation of different adrenal cortex functions in diabetes mellitus patients].

The indices of three adrenal cortex functions were investigated in 517 patients with a moderately severe and severe forms of diabetes mellitus of various duration, outside of the ketoacidosis and hypoglucemia states. Both during decompensation and at the period of unstable compensation (subcompensation) of the disease 3--60% of the patients displayed increased indices of glucocorticoid and mineralocorticoid functions, and 20--38%--of the androgenic ones. Along with different frequency of detection, there was revealed a distinct non-uniformity of different indices elevation. A prevalence of the mineralocorticoid function activation was noted. As suggested, at the mentioned periods of diabetes mellitus functional activity of the adrenal cortex reflected various phases of the oscillatory adaptation process to the frequently repeated stress actions (metabolic disturbances). Possible causes of prevalent aldosterone excretion elevation are discussed.

Is the rhythm amplitude related to the ability to phase-shift circadian rhythms of shift-workers?

The magnitude of the circadian acrophase adjustment deltaphi to a phase shift of socio-ecologic synchronizers (as in shift-work) varies from subject to subject. According to J. Aschoff, the ability to adjust rapidly to a phase-shift could be associated with having a small amplitude for certain circadian rhythms. To test this hypothesis, the correlation coefficient (r) between the mean circadian rhythm amplitude A and the acrophase shift deltaphi (measured after the first night-shift) were calculated from estimates of chronobiological time series analyses (Halberg's single cosinor method). Data were obtained from two groups of selected shift-workers (20 and 5 subjects, respectively). A negative correlation between A and deltaphi (the smaller the amplitude, the greater the delpaphi) was observed in the circadian rhythms of the following variables: oral temperature (r = .63;P less than .01), peak expiratory flow (r = . 53: P less than . 01), and urinary 17-OHCS (r = . 60; P less than . 01), but not for other variables such as : grip strength, urinary K+ and Na+. The small amplitude of certain circadian rhythms could be considered as an index of an individual's ability to phase-shift easily. However, chronobiological characteristics, other than the small circadian rhythm ampliture, remain to be identified, for both a better detection of one's ability to do shift-work and a better knowledge for practical applications.

[Clinico-neurohumoral interrelationships in neuroses].

The paper is related to studies of the sympathico-adrenal activity, hypophysisadrenal and entero-chromaphine systems in patients with hysterical and phobic neuroses. They detected certain regularities in the initial activities of these systems as well as of their reactivity depending upon the character of the functional symptom, psychological personality traits and cerebral dysfunctions.

[Changes in hemodynamic and endocrine-metabolic parameters in the early post-resuscitation period].

Changes in the system hemodynamics, renal and hind limb blood flow, as well as some endocrine-metabolic parameters were studied in 24 dogs which survived 17-minute circulatory arrest. As revealed, relative compensation and normalization of several functions and metabolic parameters in the course of 1 hour after reanimation were followed by a new wave of disturbances developing unsimultaneously, irregularly and gradually in the whole organism and some peripheral tissues. Thus, disturbances of the peripheral circulation and of the central hemodynamics served as one of the basic pathological symptoms of postreanimation disease.

Human circadian rhythms: a multioscillatory system.

After a brief review on temporal order within the human organism and implications for chronopharmacology, the paper discusses evidence indicating that the human circadian system consists of a multiplicity of oscillators. Subjects who live in isolation without time cues show free-running circadian rhythms deviating from 24 hours. Often, the activity rhythm (wakefulness and sleep) and other rhythmic variables (e.g., temperature) have the same circadian period of about 25 hours (referred to as the state of internal synchronization) but on occasions the activity period may become substantially longer (e.g., 33 hours) while the other rhythms continue with a period of about 25 hours. Such a state is termed internal desynchronization. There are also cases where the activity rhythm can reach extreme values of about 50 hours; to such a circa-bi-dian rhythm other variables are again synchronized, but in a 2:1 ratio. Internal desynchronization can occur both by shortening and by lengthening the activity rhythm. In these two cases the periods of other rhythms also change slightly in a direction opposite to that of the activity rhythm, indicating a loss of coupling between two classes of basic oscillators that both influence the two groups of overt rhythms but by different extents.
