[New approaches to the combined therapy of gonorrheal infection and accelerated methods for predicting its efficacy].

The authors suggest a combined method for the treatment of new torpid and chronic gonorrheal urethritis, involving preliminary studies of the agent's cellular respiration activity, and offer the technique for predicting the efficacy of such treatment. If gonococcal cellular respiration activity is low, aerobic processes in these agents are stimulated with daily subcutaneous injections of plasmol (a biostimulant), starting from 1 ml and building up the dose by 1 ml daily, and local irrigations of the urethra with oxygen cocktails via specially designed irrigators (Authors' Certificate No. 827071 USSR); such stimulation is carried out for 4 days prior to specific therapy. This treatment modality is maintained by an Authors Certificate No. 1107867 USSR. To check up the efficacy of the antibiotic chosen for therapy, a rapid method has been developed, consisting in measurement of the patient's blood serum antibacterial activity carried out in special chambers for cell microcultivation.

[Studies on the therapeutic efficacy of a HeNe laser].

Numerous manufactures recommend He-Ne-Lasers for a wide range of applications in medicine and dentistry. An athermal biostimulating mechanism of action is postulated to result in rapid reduction of pain and accelerated wound healing. The efficacy of a commercially available He-Ne-Laser in the treatment of alveolitis, solitary aphthae, ulcers caused by dentures, herpes simplex labialis, marginal periodontopathy and pericoronitis was investigated in 90 cases. Except for a placebo effect no reproducible therapeutic effects could be seen. Nevertheless, there is a high degree of acceptance for laser application by most of patients.

Low-energy laser treatment and exercise for chronic low back pain: double-blind controlled trial.

Twenty patients with chronic low back pain were enrolled in a randomized double-blind trial to test the efficacy of low-energy laser biostimulation combined with exercise. Ten patients received low-energy gallium-arsenide laser treatment, and ten received placebo laser treatment. Both groups were also placed on an active exercise program. Visual analogue and disability pain scores were assessed pretreatment and one month posttreatment and showed significant (p less than .02) improvements in both groups, but no relative advantage was found for either group. Objective parameters using computerized triaxial measurements of range of motion, isometric torque, and isodynamic velocity were also performed before and after treatment. There were significant improvements in objective parameters in both the laser and placebo groups, but no relative advantage accrued to either group. Under the conditions of this study, low-energy laser stimulation plus exercise did not provide a significant advantage over exercise alone.

[The effect of laser therapy on the mechanisms for generating healing in long-term nonhealing stomach ulcers].

A study was made of the effect of copper laser therapy on the content of PGE and PGF2 alpha and on the adenylate cyclase system (cAMP, cGMP and adenylate cyclase content) in patients with gastric ulcer. Seventy patients with indolent (from 3 months to 2 years) gastric ulcers were examined. The patients were assigned into 2 groups: group I received drug therapy combined with the influence of laser on copper vapours on the ulcerous surface (a single radiation dose 10 to 15 J). As compared to group I, the patients of group II manifested a considerable rise of the content of cAMP and prostaglandins, as well as adenylate cyclase activation in the gastric mucosa. Nonspecific biostimulating action of laser radiation exercised via the influence on the dysregenerative processes in the epitheliocytes of long nonhealing ulcer edges is under discussion.

[Improvement of primary and secondary prevention of occupational cochlear neuritis].

The methods of primary and secondary prophylaxis of cochlear neuritis in workers exposed to a high noise level are described. It is emphasized that hearing impairment can be prevented in a more efficient manner at the early stages: noise effect on the hearing organ and signs of the effect or mild symptoms of cochlear neuritis. A complete scheme of therapy is presented which includes vasodilators, vitamins, biostimulants, sedatives, physiotherapy. It is stressed that methods of primary and secondary prophylaxis applied in due time can prevent appearance and development of hearing impairment and potential invalidism of workers working in a high noise environment.

[The pathogenetic treatment of age-related macular dystrophies].

The paper analyses results after medicamentous treatment of 181 patients with senile macular dystrophies, using methods of pharmacologic correction of sympathetic-adrenal (phosphaden, pyridoxine, ethymisole, ascorbic acid), callicreine-kinin (andecaline, paramidine, cinnarisine) activities and methods of traditional therapy consisting of vasodilative, antisclerotic preparations, vitamins and biostimulants. The comparative analysis has shown that the above preparations normalize KA metabolism and processes of kininogenesis. Stabilization of the dystrophic process is achieved in 82.6% of cases as compared with 71.4% of patients treated by traditional methods; the duration of the therapeutic effect after one course of treatment increases by 3-4 months; the incidence of appearance of transudative-hemorrhagic changes in the central segments of the eye fundus in the presence of atrophic dystrophies of the retina decreases by three times.

[Histophysiology of tissue basophils of the cerebral dura mater after laser irradiation].

By means of the histochemical and morphometric methods the reaction of the tissue basophils of the brain dura mater has been studied to the one-time and varying-duration (0.5 sec.-3 hr.) irradiation by a helium-neon laser of the wave length 632.8 nm, power density 0.76 mvt/sm2. It has been found, that the laser irradiation had a biostimulating effect upon the tissue basophils; the first peak of activity is in the case of a 3-second continuous irradiation; the second--from 15 min. to 1 hr. In symmetrical parts of the right (irradiated) and left (nonirradiated) regions of the dura very similar changes of the functional activity of the tissue basophils activity were seen.

Lasers in medicine--some current issues.

Although lasers have been applied to medical and surgical problems for many years, it is only recently that a significant momentum has been created. Lasers are now a routine part of many medical and surgical procedures. This article surveys a selection of current technical issues in this rapidly changing field, including applications in angioplasty, gastroenterology, dentistry, gynaecology, ophthalmology, lithotripsy, biostimulation and safety. As far as possible, only work published in the last three years is covered.

[Laser applications in dermatology].

Argon laser, dye-, carbon dioxide- and Neodymium YAG laser are the most widely used in dermatological practice. The primary usage for argon- and dye lasers are vascular lesions. 60 to 70% of adult patients with port-wine nevus are successfully treated. Vascular lesions give the better results, when treated by a dye laser. Adverse side effects are minimal. After argon laser treatment adverse side effects like hyper- and hypopigmentation (-17%), atrophy and scars (4 to 5%) are observed. Carbon dioxide laser can be used to treat benign and malignant tumors of the skin. Tattoos can be removed by argon laser. Carbon dioxide- or neodymium YAG laser. The cosmetic result is not optimal for all the three laser systems. Neodymium YAG lasers can be used for the treatment of benign and malignant skin tumors in difficult areas and older patients. But the results are not better than with other procedures. Controversial results have been published concerning a phenomenon known as "biostimulation" caused by helium-neon laser. Rational indication for this laser treatment cannot be recommended in the moment.

[Predicting the effectiveness of antibiotic therapy in the treatment of gonorrhea].

A complex method is suggested to prognosticate the efficiency of treating gonorrheal urethritis in men with allowance for a preliminary determination of the cell respiration activity in agents. In the case of low indices of these processes, it is possible to stimulate aerobic processes in gonococci if applying for 4 days before the specific treatment daily subcutaneous injections of plasmol (a biostimulator) by the ascending scheme and local irrigation of the urethra by oxygen cocktails through irrigators of special design. The developed method has the author certificate (A. c. 1107867 USSR). Application of the method shortens significantly the terms of treatment, the number of the infection recurrences being considerably decreased. To accelerate the determination of the sensitivity to antibiotics the procedure of genococcus microcultivation in special chambers is applied which permits observing the growth and reproduction of microorganisms in the phase-contrast microscope.

[Acupuncture as a bioenergetic reequilibration means in the treatment of chronic mixed marginal parodontopathies].

The authors discuss the use of acupuncture in the treatment of chronic mixed marginal parodontopathies as an eventual means of bioenergetic reequilibration. The various studies carried out for some time by the Department of Odontology and Parodontology of the Bucharest Faculty of Stomatology have demonstrated the lateration of electrical bioresistance in certain acupuncture points in the cephalic extremity at the level of the gingival mucosa in pathologic parodontal states. This suggested the possible applicability in chronic mixed marginal parodontopathies of acupuncture instead of the known means of biostimulation.

[Biosynthesis of vitamins B by the fungus Pleurotus ostreatus in a submerged culture].

The intra- and extracellular contents of vitamins were studied in the course of submerged cultivation of the higher basidial mushroom Pleurotus ostreatus (Jacq.: Fr.) Kummer st. IMBF-1300 on liquid nutrient media. This strain was found to be autotrophic in respect of thiamin (vitamin B1), riboflavin (vitamin B2), niacin (vitamin B5), pyridoxine (vitamin B6) and biotin (vitamin B7), but it failed to synthesize cyanocobalamin (vitamin B12). The composition and pH of the culture medium, containing such complex biostimulating supplements as maize extract and concentrated potato sap noticeably influence the contents of vitamins B1, B5 and B7 in the mycelium, and to a less degree they change the level of the intracellular biosynthesis of vitamins B2 and B6. Higher excretion of vitamins B5, B7 and especially B6 was observed on the semisynthetic media during the postexponential growth. Under experimental conditions vitamins B1 and B2 were accumulated only in the cells. The dry mycelium of P. ostreatus obtained by submerged cultivation on liquid media is a valuable source of B vitamins and, especially, of niacin. Thus the oyster mushroom and other edible mushrooms can be put at one of the top places among food-stuffs by the content of niacin.

[Nature of the stimulating substances found in the vital activity products of yeastlike fungi].

The cultural broth of Candida tropicalis was shown to contain an active compound which stimulated the synthesis of levorin, a polyene antibiotic Succinic acid was found to stimulate the antibiotic synthesis. The stimulating effect of the active compound increased in proportion to the content of succinic acid in it and became maximal at the same concentration of succinic acid as in a pure preparation. However, succinic acid was not an only compound responsible for the elevated synthesis of the antibiotic since the biostimulating effect was higher than that of pure succinic acid.
