Observation of curative effects of huatuojiaji in the treatment of 300 cases of apoplexy.

Huatuojiaji are extra-channel acupoints which are first described in A Handbook of Prescriptions for Emergencies, and the clinical application of which has been expanded a great deal in modern times. In the end of 1970s, the successful application of Huatuojiaji points in acupuncture anaesthesia brought the research on Huatuojiaji points to its climax; and in recent years, great progress has been made in the study of clinical application, improvement of curative effects and functional mechanism of Huatuojiaji points. According to the theories of acupuncture and moxibustion that puncturing Huatuojiaji can dredge channels, promote flow of qi, regulate yin and yang, and activate the Du Channel and the Urinary Bladder Channel, 300 cases of apoplexy were thus treated with good curative effects. There are altogether 56 points, 28 on each side of the spine, 0.5 cun lateral to the spinous process. The prescription for apoplexy includes C1-7, T1-9 and L2-4. Of the 300 cases of apoplexy, 100 were cured, 107 markedly effective, 87 improved, and 6 ineffective, the total effective rate being 98%.

[Clinical observation on treatment of hyperinsulinemia and hyperandrogenism anovulatory patient with replenishing kidney-yin drugs].

In order to investigate the effect of Chinese herbal medicine for replenishing Kidney-Yin in treating hyperinsulinemia and hyperandrogenism anovulatory syndrome, 35 patients were treated with replenishing Kidney-Yin drugs for 3 months, basic body temperature, ultrasonic examination and blood levels of sex hormones were taken for monitoring the ovulation, and changes of serum insulin, blood sugar as well as oral glucose tolerence test were observed before and after treatment. Thirty-five patients showed high serum insulin and testosterone levels but normal dehydroepiandrosterone (DHEA) level. Twenty-nine percent of their luteinizing hormone/folliclestimulating hormone (LH/FSH) ratio were in normal range. Twenty four cases completed the regular treatment and 20 of them showed ovulation in the 43 menstrual cycles (59.7%). Seven of 17 (41.2%) infertile cases become pregnant within 6 months. After 3 months of treatment, blood sugar and insulin level significantly decreased but the latter was still slightly higher than normal level. Serum testosterone level decreased significantly and reached normal. Results suggested that replenishing Kidney-Yin Drugs could provide a good microcircumstance for ovarian follicular growth, and resulted in ovulation and pregnancy. The mechanism is remained to be further explored.

[Effects of herbs for replenishing yin to tonify kidney on testicular ultrastructural changes in rats with glucocorticoid-induced kidney deficiency].

OBJECTIVE: To investigate the effects of herbs for replenishing yin to tonify kidney on testicular ultrastructural changes in rats with glucocorticoid-induced kidney deficiency. METHODS: Thirty rats were randomized into three groups: normal control group, untreated group and Erdi Biejia Decoction (EDBJD)-treated group. Kidney deficiency rat model was induced by glucocorticoid. Testicular structure and ultrastructure were observed under the light microscope and the transmission electron microscope. RESULTS: The light microscope revealed the decreased thickness of seminiferous epithelium layer, reduced number of spermatogenic cells, interstitial tissue edema, and a qualitative reduction in the number of interstitial cells and hematoxylin-eosin stained cytoplasm in the untreated group. The transmission electron microscope revealed loose structure of seminiferous epithelium, degenerative mitochondria, and increased amounts of lysosome, presence of numerous cytoplasmic vacuolation and chromatin in interstitial cell cytoplasm in the untreated group. The aggregates of heterochromatin around the nuclear membrane and the shrinkage of nucleus were also observed. Compared to the untreated group, these pathological changes were inconspicuous in EDBJD-treated group. CONCLUSION: EDBJD as replenishing yin to tonify kidney herbs can suppress glucocorticoid induced pathological changes in the testes of rats.

[Clinical features of Wei Yuheng].

Wei Yuheng held that the endogenous miscellaneous diseases are mostly caused by liver diseases, highly stressed the treating method of nourishing yin to generate liver. He claimed that liver deficiency is mostly caused by congenital factors, sexual strain or improper treatment. He criticized that the improper treatment by elder generations mainly was caused by their ignorance of tonifying the liver based on the saying of "no tonifying treatment for the liver". Wei Yuheng's medical theories and clinical experiences are scattered in his book Xu Mingyi Lei'an (Supplement to Classified Case Records of Celebrated Physicians). Wang Mengying collected part of his thoughts and experiences and wrote the book Liuzhou Yihua (Liuzhou Medical Talks). Although Wang Mengying's book could deduce part of the thoughts of Wei Yuheng, it didn't disclose his theories completely. It is known to all that the compound recipe Yiguan Jian is a representative formula of Wei Yuheng to tonify liver yin. Actually, it can be concluded from the analysis of Xu Mingyi Lei'an that he used Fructus Lycii (Gouqi) as a main herb to tonify liver yin in the method of nourishing yin to generate liver. Based on the idea that there are accept, restriction, generation and transformation relationship among the lung, kidney and liver yin, so he also always used Radix Glehniae (Beishashen), Radix Ophiopogonis (Maidong), Radix Rehmanniae Recens (Shengdihuang), and Radix Rehmanniae Preparata (Shudihuang) in his compound recipes. If the yin blood is insufficient, Semen Ziziphi Spinosae (Suanzaoren) should be added. In order to prepare a out way for phlegm-heat caused dy liver yin deficiency, a lubricative, Semen Trichosanthis (Gualouren) was always used and raw Semen Coicis Recens (Shengyiyiren) too. Sichuan Rhizoma Coptidis (Chuanhuanglian) can be used to clear fire-heat, and Fructus Meliae Toosendan (Chuanlianzi) for pain caused by fire-heat.

[Clotting factor X deficiency resulted from an T 58-->G mutation within exon 1 of human factor X gene].

OBJECTIVE: To identify the genetic defect of a patient with clotting factor X deficiency (QGZ). METHODS: PCR and DNA sequencing were used to screen mutation in factor X gene. PCR primers were designed covering all the coding regions for exon and flanking intron sequences. RESULT: A single nucleotide substitution T 58G in exon 1, which caused a missense mutation Ser(AGT) 11 Arg(AGG) in signal peptide, was identified by DNA sequencing. CONCLUSION: This nucleotide substitution might be the molecular etiology of factor X deficiency.

[Relation of cytochemistry changes in the peripheral blood ANAE in chronic glomerulonephritis and differentiation-syndromes].

The ANAE reaction of the peripheral blood from 24 patients of chronic glomerulonephritis (grouped according to the identification of TCM) and 30 normal individuals were observed by cytochemistry and microspectrophotometry. It was found that the percentages of positive lymphocytes of the deficiency of Kidney-Yin was 47.8 +/- 7.6%, among which the percentages of lymphocytes of spot granular pattern and scattered granular pattern were 42.4 +/- 6.3% and 5.4 +/- 2.2% respectively, the ANAE relative quality of monocytes of the deficiency of Kidney-Yin was 51.26 +/- 2.59. They were 39.7 +/- 7.4%, 31.2 +/- 5.8%, 8.5 +/- 2.7% and 41.84 +/- 2.66 respectively in the deficiency of Kidney-Yang. The results of normal group were 65.5 +/- 6.8%, 53.6 +/- 6.7%, 11.9 +/- 3.4% and 54.78 +/- 3.34 in their given order. The differences in three groups each other were marked significantly. The authors have found the difference between the patients of chronic glomerulonephritis and the normal individuals was significant statistically.

[Preliminary (correction of Priliminary) study on effects of "planning treatment according to diagnosis" on physiological changes during simulated weightlessness].

OBJECTIVE: To observe effects of the planning treatment according to diagnosis on body syndromes caused by simulated weightlessness. METHOD: Ten subjects underwent HDT -6 degrees for 14 d were randomly divided into the traditional Chinese Medicine group (ME) and control group (CON). Differentiation of syndromes were made and parameters related to the differentiation syndromes, including plasma cortisone, blood viscosity, red cell deformation, excretion rate of urine xylose, and amount of urine were surveyed simultaneously. RESULT: Both differentiation of syndromes and the physiological parameters in group ME could be maintained at the pre-bed rest levels or changed only slightly. CONCLUSION: The Chinese herb compound had an adjusting effect on deficiency of kidney-Yin, blood stasis, insufficiency of spleen-Qi and changes of related physiological parameters caused by 14 d bed rest simulated weightlessness, among which the effect on deficiency of kidney-yin and blood stasis were more distinct.

Traditional Chinese medicine and treatment of neonatal jaundice.

OBJECTIVE: Treatment with herbs may increase the risk of neonatal jaundice (NNJ). It is logical to look into the current practice in some hospitals in China where herbs are being used in the treatment of NNJ. It is also the purpose of this study to find out the chemical constituents and actions of the herbs, and the rationale of the treatment. METHODS: Twenty reports, from 1973 to 1989, from different parts of China, come in a published book and the paediatric journals written in the Chinese language. The Zhong Yao Da Zi Dian, an encyclopedia of Chinese materia medica, and other books on the pharmacology and applications of Chinese materia medica were also referred to in the study. FINDINGS: Yin-chen (oriental wormwood or Artemisia) was the most commonly used herbs for NNJ (95%). Others were Da-huang (rhubarb or Rheum officinale), Huang-qin (skullcap root or Scutellaria), Gan-cao (licorice or glycyrrhiza) and Huang-lian (goldthread rhizome or Copts chinesis). Huang-lian, which contains the alkaloid berberine, was used in 4 centers (20%). Berberine can cause severe acute hemolysis in babies with G6PD deficiency. Currently, Yin-chen comes as a decoction Artemisia composita and an intravenous preparation. These preparations have potential central nervous system and cardiovascular toxicities. CONCLUSIONS: Chinese herbs have many pharmacological substances and therefore multiple actions. In recent years, Chinese herbs are used in conjunction with "Western" drugs, rendering the study of the effects of herbs on NNJ extremely difficult. The efficacy and safety of phototherapy for NNJ have been firmly established, thus diminishing the need for drug treatment. What is the present day role, therefore, of herbal medicine for NNJ? Is there a place for further research of these herbal medicines?

[Exploring the substantial basis of the features yinxu huowang in girls with precocious puberty].

Differential diagnosis to 75 girls with true precocious puberty was made using method of TCM. It was observed that each girl had Yinxu Huowang (intense internal heat due to deficiency of vital essence) to a certain extent, these features were very remarkable in most of them. To explore the substantial basis of the features, the functions of hypothalamic-pituitary-ovarian axis were determined. The results showed that the levels of serum FSH, LH, E2 in the girls rise significantly than those in normal girls of the same age, and LHRH stimulating test displays characteristic changes of excessive activity. Treated with TCM of Ziyin Xiehuo, (nourish vital essence reducing intense internal heat) and remitted, the levels of serum FSH, LH, E2 descend significantly, the volume of uterus and ovary reduce markedly, secondary sexual characteristics retrogress evidently, while the features of intense internal heat due to deficiency of vital essence mitigate obviously. According to the above mentioned, the authors consider that the hypothalamic-pituitary-ovarian axis starting beforehand and being hyperfunction are likely to be the substantial basis of the features--that deficiency of Shen Yin, (vital essence) accompanied with intense Fire of the Vital Gate--of the girls with true precocious puberty.

Therapeutic effects of the blood-activating and stasis-reducing method in 80 cases of coronary heart disease.

The pathogenesis and treatment of coronary heart disease (CHD) may vary, whereas the author believes that the disease is due to blood stasis on a fundamental basis of deficiency, and the principle of treatment is activation of blood. Since stasis may lead to deficiency and vice versa, CHD cannot be cured without removal of stasis, and stasis cannot be completely removed without proper correction of deficiency. The basic method for the treatment of CHD is therefore activation of blood for reduction of stasis. At the same time, according to the conditions of individual patients, the methods of replenishing Qi, warming Yang, nourishing Yin, reducing phlegm and subduing Yang should be applied simultaneously to resolve the problem of deficiency. The author treated 80 cases of CHD by the above rationale and found the results satisfactory, i.e., 28 cases (35%) showed marked symptomatic relief, 46 (57.5%) showed symptomatic improvement, while 6 cases (7.5%) failed. The total effective rate was 92.5%. ECG examination indicated 9 cases markedly effective, 27 improved, while 44 cases failed, for a total effective rate of 45%. Blood pressure and blood lipids also showed significant drops (P < 0.001).

[Observation on clinical therapeutic effect of "mother-son" reinforcing-reducing method on stroke at restoration stage].

OBJECTIVE: To probe into the role of "mother-son" reinforcing-reducing method of five Shu points in clinical treatment of stroke at restoration stage. METHODS: Sixty-six cases of stroke were randomly divided into a treatment group (n = 34) and a control group (n = 32). The treatment group were treated by acupuncture at acupoints of The Hand-Yangming and The Foot-Yangming Channels, and "mother-son" reinforcing-reducing method of the five Shu for reinforcing deficiency and reducing excess according to deficiency and excess of channels differentiated by Cunkou Renying diagnosis; the control group were treated only by acupuncture at acupoints of The Hand-Yangming and Foot-Yangming Channels with uniform reinforcing-reducing method. Blood flow velocity in Cunkou and Renying Vessels before and after treatment were determined with transcranial Doppler ultrasono-blood flow meter and their clinical therapeutic effects were compared. RESULTS: There was a very significant difference before and after treatment in blood flow velocity in Cunkou and Renying vessels in the treatment group; the clinically basic recovery and markedly improving rate after treatment in the treatment group was significantly superior to that in the control group (P < 0.01). CONCLUSION: The acupuncture therapy of "mother-son" reinforcing-reducing method of five Shu points based on differentiation of yin, yang and deficiency and excess of channels by Cunkou and Renying Vessel methods can increase clinical therapeutic effect on stroke at restoration stage.

In vivo anti-osteoporotic activity of isotaxiresinol, a lignan from wood of Taxus yunnanensis.

Isotaxiresinol, the main lignan isolated from the water extract of wood of Taxus yunnanensis, was investigated for its effect on bone loss, on serum biochemical markers for bone remodeling and on uterine tissue, using ovariectomized (OVX) rats as the model of postmenopausal osteoporosis. After oral administration of isotaxiresinol (50 and 100mg/kg/d) for 6 weeks, bone mineral content (BMC) and bone mineral density (BMD) in total and cortical bones were increased as compared to those of OVX control rats, and decreases of three bone strength indexes induced by OVX surgery were prevented. Serum biochemical markers for bone remodeling revealed that isotaxiresinol slightly increased bone formation and significantly inhibited bone resorption without side effect on uterine tissue. These results suggest that isotaxiresinol may be useful for treatment of postmenopausal osteoporosis, especially for prevention of bone fracture induced by estrogen deficiency.

Immune responses in mice deficient in Ly-GDI, a lymphoid-specific regulator of Rho GTPases.

Ly-GDI (lymphoid-specific guanosine diphosphate (GDP) dissociation inhibitor), also called D4-GDI, is preferentially expressed in hematopoietic tissues including bone marrow, thymus, spleen and lymph nodes. It binds to the small guanosine triphosphate (GTP)-binding protein Rho and inhibits GDP dissociation from Rho proteins. To explore the function of Ly-GDI in lymphocytes, we have generated Ly-GDI-deficient mice by gene targeting. These mice showed no striking abnormalities of lymphoid development or thymocyte selection. The mice also exhibited, for the most part, normal immune responses including lymphocyte proliferation, IL-2 production, cytotoxic T lymphocyte activity, antibody production, antigen processing and presentation, immune cell aggregation and migration, and protection against an intracellular protozoan. However, Ly-GDI-deficient mice exhibited deregulated T and B cell interactions after in vitro cultivation of mixed lymphocyte populations in concanavalin A (Con A) leading to overexpansion of B lymphocytes. Further studies revealed that Ly-GDI deficiency decreased IL-2 withdrawal apoptosis of lymph node cells while dexamethasone- and T cell receptor-induced apoptosis remained intact. These data implicate the regulation of the Rho GTPase by Ly-GDI in lymphocyte survival and responsiveness, but suggest that these functions may be partially complemented by other Rho regulatory proteins when the Ly-GDI protein is deficient.

[15 years' observation and related research on diagnosis and treatment according to traditional Chinese medicine of elevated serum lipids in 54 cases of coronary disease].

The increasing rate of cholesterol (CH) of 54 cases with coronary heart disease was 33.33%. According to CH level, the authors divided them into two groups: the normal one (I) and the high one (II). By fifteen-year observation, CH of Group I kept normal, only with its triglyceride (TG) a little higher than ordinary level. But the CH and TG of Group II, which had been treated with anticholesteremic drugs, was obviously higher than Group I (P less than 0.05). In connection with identification according to TCM, both CH and TG of deficiency of Yang, and deficiency of both Yin and Yang were higher, especially TG (P less than 0.01). TG of the type of blood stasis was higher (P less than 0.01) except anticholesteremic drugs for Group II. The two groups were treated with the same combined treatment of TCM and WM. The long term effect of fifteen years in Group I was more obvious than in Group II. This showed that the treatment of high serum lipid of CHD was more difficult.

[Zhongxu-qizhi type of chronic gastritis and Campylobacter pyloridis infection].

According to the syndrome differentiation of TCM, one hundred cases of chronic gastritis were recognized as Zhongxu-Qizhi type (Group I, 57 cases) and the other types (Group II, 43 cases), the latter further divided into 36 cases of disharmony of Liver and Stomach type and 7 cases of deficiency of Stomach Yin type. The pathohistological investigation and urease test showed that the campylobacter pyloridis (CP) infected rate in the Group I (92.9%) was very significantly higher than that in Group II (58.1%, P less than 0.01); severe degree and deep location of CP infection occurred more significantly in Zhongxu-Qizhi type (52.8% and 73.6% respectively). Between the two groups, active chronic gastritis and severe invasion of polymorphonuclear cells were significantly different (P less than 0.05). All these findings suggest that there were some relationships between CP infection and syndrome differentiation of TCM.
