Three topics are reviewed: iliopsoas bursitis (IPB), iliotibial band frictional syndrome (ITBFS), and Achilles tendinopathy. Although not frequently diagnosed, IPB may be more prevalent than what is commonly thought. Several excellent review articles are presented. Imaging studies are usually needed for confirmation of the diagnosis, and a report on magnetic resonance imaging is presented. Successful treatment of IPB with home exercise is reviewed. Because an increasing number of people are exercising, there is an increasing need to know the syndromes that can result from overuse, such as ITBFS. Several articles on the use of diagnostic ultrasonography and magnetic resonance imaging to aid in the confirmation of this entity are reviewed; one article explores the pathology of ITBFS in a cadaveric study. In the section of the treatment of ITBFS the authors review one article on exercise and one on surgery. Use of ultrasound to aid in the diagnosis of Achilles tendinopathy is reviewed, as well as risk factors related to developing it. Two articles on surgical treatment are reviewed.
OBJECTIVE: Infection imaging with a 99mTc-labeled chemotactic peptide was evaluated in a rabbit model of Escherichia coli infections in burned tissue. MATERIALS AND METHODS: The peptide was radiolabeled with 99mTc via the hydrazino nicotinamide derivative. Three groups of six animals were studied: (group A) unilateral infected burns; (group B) bilateral burns with unilateral infection; and (group C) uninfected burns. Twenty-four hours after injury, groups A and B were infected, and 8 hours later, all animals were injected with approximately 0.50 mCi of 99mTc-peptide. MEASUREMENTS AND MAIN RESULTS: In groups A and B, excellent images of the infections were obtained at 3 to 4 and 16 to 18 hours after injection of the peptide. At 3 to 4 hours after injection, the target-to-background ratios (T/B) were 3.12 +/- 0.28 for group A and 4.33 +/- 0.61 for group B (p = n.s.). At 16 to 18 hours, the T/B ratios increased significantly (p < 0.01): group A = 8.10 +/- 1.03 and B = 7.70 +/- 1.25. The T/B ratio for group C was only slightly greater than unity. CONCLUSIONS: These results indicate that 99mTc-labeled chemotactic peptides are effective radiopharmaceuticals for the rapid detection of focal sites of infection within thermally injured tissues.

One hundred fifty-one patients with fractures of the pelvis and associated injuries have been reviewed. Patients who have these injuries are likely to have uncontrollable, fatal hemorrhage. Of this group, nine died, 11 had exploratory laparotomies, ten had bladder injuries, and two had rectal damage. Blood gas determinations, arterial catheterization, and pulmonary wedge pressure determinations are valuable in making and early diagnosis of pathologic changes resulting from internal hemorrhage.

A retrospective study of the initial lateral cervical spine radiographs (read as normal) of five multiply traumatized quadriplegic children showed a subtle widening between the spinous processes of C1 and C2. To verify the accuracy of this widening, five distances, two calculations, and four angles were defined and measured in the pathologic group and in a control group of 12 normal children. There was a significant difference (p less than 0.05) between the two groups. A widening of greater than 10 mm between the spinous processes of C1-C2 on a neutral lateral radiograph should alert the physician to an underlying spinal cord injury. The other measurements could then confine the pathology at that level.
