Ovarian strumal carcinoid with markedly high serum levels of tumor markers.

We report a 54-year-old woman with ovarian strumal carcinoid in association with dermoid cyst and mucinous cystadenoma in the same ovary and who had markedly high serum levels of CEA (202 ng/ml), CA125 (710 U/ml), and CA19-9 (11,500 U/ml). These tumor markers were not found in the thyroid tissue or carcinoid by immunohistochemical methods, but their serum levels decreased to below the cutoff levels after surgery. In our case, the change of serum levels of these tumor markers may be useful for the follow-up after surgery.

Carcinoid Tumor↗

Carcinoid tumors of the ovary.

Carcinoid tumors of the ovary are uncommon, but 150 cases of primary ovarian carcinoids and 40 cases of carcinoid tumors metastatic to the ovary have been reported. Primary carcinoid tumors of the ovary are divided into insular, trabecular, strumal and mucinous types. The insular type is most common, followed by the strumal type. The majority of primary ovarian carcinoids occur in association with mature cystic teratoma, but a considerable number present in pure form. Only the insular type is associated with the carcinoid syndrome. The age incidence of patients with ovarian carcinoids shows a wide range but most patients are postmenopausal. Primary carcinoids of the ovary are invariably unilateral. They form a solid nodule within a cystic teratoma, or when pure a solid yellow-grey mass, and vary from microscopical to large tumors measuring in excess of 20 cm in the longest diameter. The metastatic carcinoids are nearly always bilateral and scattered tumor deposits are present throughout both ovaries. Primary ovarian carcinoids metastasize only occasionally, and should be treated as ovarian tumors of low malignant potential.

Laparoscopic removal of ingested pin penetrating the gastric wall in an immunosuppressed patient.

Foreign body ingestion is a well-recognized and relative common problem. Most foreign bodies pass spontaneously and uneventfully through the digestive tract. In some cases, however, the ingestion of foreign bodies is associated with a high risk of complications because of their size or shape or the hosts medical status. We report a case of successful laparoscopic removal of an accidentally ingested pin that was penetrating the anterior gastric wall in a immunosuppressed patient. After removal of the pin, the opening of the gastric wall was closed with an extracorporeal hand-suturing technique. The patient recovered uneventfully and was discharged in good health on the 5th day after the procedure. Laparoscopy should be considered the approach of choice for the removal of ingested foreign bodies when surgery is indicated.

Thyroid disease in pregnancy.

It is frequently difficult to establish a diagnosis of hyperthyroidism in association with pregnancy. Signs and symptoms suggestive of hyperthyroidism may occur in normal pregnancy. Moreover, tests to rule out hyperthyroidism under these conditions are not readily available. Mild hyperthyroidism is not a hazard to an otherwise normal pregnancy and does not require therapy on the basis of this presumption. Hyperthyroidism of a clinically significant degree is safely treatable by medical means without hazard to the fetus. Hyperthyroidism is an uncommon cause for the failure of pregnancy to proceed to term. Treatment with thyroid based on a presumption of this diagnosis is justified, but such patients should be studied carefully after delivery to establish the true state of thyroid function for the future. Other conditions of thyroid dysfunction, including thyroiditis, thyroid carcinoma, nontoxic goiter, and ovarian struma rarely interfere with an otherwise normal pregnancy.

A strumal carcinoid primary in the ovary.

A primary ovarian carcinoid tumour of insular strumal type in a 78-yr-old patient is reported. Strumal carcinoid of the ovary is a rare tumor of germ cell origin characterized by an intimate mixture of thyroid tissue and carcinoid. Correct diagnosis depends on a pathologic examination with the use of special stains. Because of its benign character, the treatment is primarily a surgical one and consists of a simple oophorectomy or salpingo-oophorectomy.

Calcitonin secreting struma-carcinoid tumor of the ovary.

The ultrastructure of strumal carcinoid tumors at times may reveal a far more complex structure than can be ascertained from light microscopy. It may at times be indistinguishable from medullary carcinoma of the thyroid gland. Both tumors appear to be capable of producing 5-hydroxyindolacetic acid and calcitonin. The common shared biologic and ultrastructural features suggest a common origin, from neuroectodermal cells.

Ovarian strumal carcinoid with severe constipation: immunohistochemical and mRNA analyses of peptide YY.

Functioning ovarian carcinoid tumors are well known to cause carcinoid syndrome. Recently, strumal and trabecular ovarian carcinoid tumors are reported to cause severe constipation possibly because of tumor-producing peptide YY (PYY). We studied a case of primary ovarian strumal carcinoid who had had severe constipation until the tumor was removed by surgical operation. Immunohistochemically, many tumor cells were strongly positive for PYY. By Northern blot and reverse transcription polymerase chain reaction analyses, PYY mRNA was expressed in a complete form as detected in normal human colon mucosa. From these findings, an ovarian strumal carcinoid is strongly suggested to express complete PYY mRNA and therefore complete PYY protein that results in severe constipation.

Peritoneal strumosis.

A rare case of peritoneal strumosis is described. A 49-year-old woman was operated on because of a uterine leiomyoma. Laparotomy disclosed a left sided ovarian mass and multiple implants in the omentum. Microscopic examination revealed a benign ovarian struma with omental spread. There were no signs of a thyroid neoplasm. 32 months later and after three courses of chemotherapy, the patient was free of the disease. Although the nature of this lesion is equivocal, one cannot exclude maturation of a highly differentiated follicular cancer which could have arisen in the ovarian struma.

Mucinous adenocarcinoma and strumal carcinoid tumor arising in one mature cystic teratoma of the ovary with synchronous cervical cancer.

Malignant transformation of mature cystic teratoma is an uncommon complication. While any of the constituent tissues of a teratoma has the potential to undergo malignant transformation, squamous cell carcinoma is the most commonly associated cancer. We present an unusual case of a postmenopausal woman with synchronous mucinous adenocarcinoma and strumal carcinoid tumor from one of two ovarian mature cystic teratomas (one in each ovary) with synchronous cervical cancer. We suggest that malignant transformation of mature cystic teratoma and synchronous cervical cancer be treated by hysterectomy, chemotherapy, and radiotherapy.

A study of human calcitonin in an ovarian carcinoid and ovarian cancers.

High concentration of human calcitonin (hCT) was found in an ovarian carcinoid by radioimmunoassay. The hCT value was not affected by the presence of protease inhibitors. To confirm the presence of hCT in an ovarian carcinoid, hCT was isolated by the Baghdiantz method. The molecular weight of the ovarian hCT was determined using Sephadex G-75 gel filtration. Though the molecular weight of the ovarian hCT was variable, 90% corresponded to that of the authentic hCT. The carcinoid cells were examined by immunoperoxidase techniques. Those composed of strumal and trabecular structure were all argyrophilic, but hCT was only found in strumal structure. Significant concentrations of hCT were also found in ovarian cancers.

[Struma carcinoid tumor within a dermoid cyst as an incidental finding in cesarean section].

A 25-year-old primigravida with breech presentation was admitted near term. During Caesarean section, a dermoid cyst of the right ovary was detected. The dermoid cyst was enucleated, preserving a plum-sized ovary. Histology revealed a strumal carcinoid within the excised dermoid cyst. From the histological point of view it was classified as a tumour of low malignancy. The clinical study provided no evidence of any metastases. On the 14th postoperative day the patient could be discharged from hospital subject to close aftercare control.
