[Reflex therapy of vertebrogenic radicular pain syndromes].

The authors analyse results of the treatment of 120 patients with vertebrogenic radicular pain syndromes by reflexotherapy, electropuncture, electroacupuncture and acupuncture, applied both alone and combined under conditions of a neurological department. Examination of the patients included a comprehensive analysis of neurological symptomatology and its variations in the time course of the treatment exercised according to a specially elaborated schedule of case history. Approaches to the choice of the most effective methods of reflexotherapy, depending on the extent of injury, intensity of the pain syndrome and the state of the vegetative nervous system are offered and the efficacy of the methods of reflexotherapy is clinically compared.

[Effect of electromagnetic EUHF-radiation of non-thermal intensity on the function of the respiratory and cardiovascular system and some parameters of the cell immunity in respiratory pathology].

The electromagnetic radiation of mm-range and 3 mW/cm2-flow density has been studied for its effect on the function of the respiratory-hemodynamic system in some parameters of the cell immunity and on the sensory sphere of patients with chronic obstructive diseases of the lungs. The therapeutic exposure was performed by the method of cascade EUHF-reflexotherapy. It is shown that such a radiation promotes stabilization of the chronic broncho-obstructive process. The exposure features revealed allow recommending the cascade EUHF-reflexotherapy for a routine use in the complex rehabilitation of patients with chronic obstructive diseases of the lungs.

[The importance of the metamere in the interpretation of the therapeutic results obtained with acupuncture. Elaboration of the dermatoneuromeric theory and critical comment].

The Authors after personal experimental studies and bibliographic researches, propose, through the formulation of the dermatoneuromeric theory, an explanatory model for the understanding of the reflexotherapeutic results in the visceral pathology treatment. The fact that different authors propose many different points for the functional balance of the internal organs and consequent diseases, that these points are located in many cases on the same dermatomes and that these are connected with the starting neuromes of autonomic fibres (Ortho and Parasympathetic) to the organ to be cured, indicates that Acupuncture is a spinal metameric reflexotherapy with scanty modulation of the supraxial centres, in opposition to Acupuncture analgesia. The Alarm (Mo) and Concurring (Yu) points of each channel are the most distant and nearest to the spinal cord and their stimulation establishes the upper and lower limit of the part of cord that we want to stimulate. Anesthetic blocks outside the "firing tract" do not influence the output, while the selective blocks can limit or abolish the Acupuncture effect. Ancient and modern therapeutic methods utilize these reflexes, giving an unique interpretation to alla reflexotherapies.

[Therapy of articular and periarticular pain with local anesthetics (neural therapy of Huneke). Long and short term results].

OBJECTIVE: Assessment of the outcomes on the articular and periarticular pain of the knee, by a reflexotherapy with local anesthetics, denominated also neuraltherapy. EXPERIMENTAL DESIGN: Retrospective observational study with 3 years' follow-up. ENVIRONMENT: An Ambulatory Center for Pain Management. PATIENTS: All the patients (N = 115) with knee pain treated with neuraltherapy in the period 1982-31 August 1996 (retired patients 2, mean age 68 +/- 15.2 years in the remaining). INTERVENTIONS: The employed reflexotherapy, denominated also neuraltherapy by Huneke, uses a local anesthetic (prevalently lidocaine) in 0.5-1% concentration and small quantity (0.5-1 ml for point) on tender/trigger and/or acupuncture points, as well as in anatomical structures like articulations, nerves, arteries or veins. The number of sessions has been of 7.03 +/- 3.3. OUTCOME MEASURES: Outcomes are evaluated in terms of percentage subjective improvement of the pain at the end of the therapy and at 1-3-6 months, 1-2-3 years, and then divided in 5 groups (no improvement-fair-good-very good-excellent). RESULTS: At the end of the therapy the improven cases were the 91.2%, the 83.7% at 1 month, the 64.6% at 3 months, the 41.3% at 6 months, the 22.5% at 1 year, the 12.1% at 2 years and the 7.9% at 3 years. CONCLUSIONS: These outcomes suggest a good effectiveness of this treatment in the painful diseases of the knee, but further randomised double-blind trials are necessary.

Function of myocardial contraction and relaxation in essential hypertension in dynamics of acupuncture therapy.

Forty-nine patients with initial stages of essential hypertension (EH) were investigated using phase systole analysis and echocardiography during acupuncture (reflexotherapy) treatment. It was established that even at the initial stages of EH increasing of myocardial mass and the disturbances of contractile and diastolic activity of myocardium take place. As a result of acupuncture a stable hypotensive effect, improvement or normalization of contractile function and diastolic values, a decrease of energy loss, reversal of myocardial hypertrophy were achieved. It is the decrease of sympathetic influences on the circulation system that plays an important part in the onset of these positive changes.

[Acupuncture in daily practice. Headache].

The aetiopathogenesis of headache is explained in a general review of this subject. Attention is directed the various forms of reflexotherapy that can be cmployed in its treatment. Acupuncture is examined in some detail, from both the general and the symptomatic standpoint. Stress is laid on the different picture presented by headache in different patients.

[Use of electroacupuncture in the treatment of vibration diseases].

Vibration is known to adversely affect the nervous, cardiovascular and muscular systems, which feature is most liable to lead to decrement of performance. Employed in former times in dealing with the above affections was a complex medicamentous mode of treatment involving the use of vasoactive agents, sympatho- and cholinolytics, neuroplegic agents, physiotherapeutic and sanatorium-and-health resort methods, and other therapeutic modalities. There has been studied efficiency of use of electroacupuncture versus a complex medicamentous therapy in patients presenting with vibration disease. Methods are described for management of patients with acupuncture procedures. With reflexotherapy techniques, electroacupuncture in particular, it takes less time for the clinical-and-physiological indices to return to normal and for the patients to recover their strength after the illness. Electroacupuncture can be recommended for treating vibration disease in an outpatient, in-patient, and sanatorium-and-health resort setting. The above method permits the treatment time to be reduced by 20%, which fact has a major economic effect.

[Complex treatment of elderly patients with pain reflectory manifestations of lumbosacral osteochondrosis and peripheral circulation disorder].

Involutional age makes difficult the treatment of pain syndromes of vertebra origin. Concomitant diseases, age stress syndrome, hormone disbalance constrain the management of the disease. Combination of laser stimulation, reflexotherapy, psychodiagnostics and psychocorrection enhances the efficiency of the treatment of pain syndromes in patients with lumbo-sacral radiculitis and peripheral circulation disorder.

[Use of the method of pulsed thermopuncture in the treatment of patients with chronic alcoholism during therapeutic remission].

The paper is concerned with pulse thermopuncture, a new method of reflexotherapy. Its modes and parameters, indications for clinical use are based. The method is especially indicated in the treatment of patients with vegetovascular and neurotic disorders. It provided a beneficial effect in the treatment of chronic alcoholics in a state of therapeutic remission in order to correct the above-indicated disorders and prevent the disease relapses.

[Autonomic disbalance in patients of different age with stomach ulcer and ways of its correction].

The study of peculiarity a tone of vegetative nervous system at patients of different age with ulcer defeats of a stomach, types of temperament, a level of neurotics, expressiveness introversion and extraversion. WE USED: Modified Eysenk Personality Questionnaire, EGG, standard vegetative test CITO (SVT CITO). OUR RESULTS: Rise of neurotic level in patients with stomach ulcer, indications of vegetative distones with characteristic apportionment of activity of VNS. Reflexotherapy, carry on counting variation of tone VNS expedite scaring of ulcer defeat.

[Use of stimulation techniques in pain treatment].

Stimulation techniques (SB) include manipulation, acupuncture, acupressure, physiotherapy, transcutaneous electrical nerve stimulation, reflexotherapy, laser treatment and epidural stimulation technique. The purpose of this paper is to investigate the scientific evidence for these techniques. The Cochrane Library and Medline were searched for all techniques from 2000 to date. Only randomised controlled studies written in English were included. Search words were used, such as; acupuncture and neck pain, shoulder pain, etc. In total 587 papers were identified for the following diseases; headache, neck pain, shoulder pain, elbow pain, low back pain and knee pain. 415 papers were excluded, and the remaining 172 papers, a total of 20,431 patients, are the basis for this study. The effect of acupuncture and epidural stimulation technique is scientifically well-supported. For the remaining techniques, the scientific evidence is dubious.

[The effectiveness of the combined treatment of enuresis].

The effect of therapeutic measures was assessed for 229 enuresis children aged 3-14 (160 boys and 69 girls). Neurotic form of the disease was in 71, primary neurosis-like in 121, secondary neurosis-like in 37 patients. All the children underwent combined treatment involving prolonged reflexotherapy with 25% silver nitrate solution associated with differentiated drug, dieto- and psychotherapy. The efficacy of the treatment was found to be higher in neurotic enuresis (cure in 85.9%) and reduced in primary neurosis-like one (in 71.1%). Age-adjusted comparison showed higher effect for age 3-6 years (92.2%), lower for 7-11 years (61.9%) suggesting the necessity of early treatment onset.

[Analysis of function in shoulder-arm pain].

Aiming at a particularized functional analysis 70 patients with shoulder-hand syndrome were diagnosed; aspects of reflexotherapy (manual and neural therapy) were taken into consideration on this occasion inclusive a comment on the psychical condition. This article refers to several peculiarities of the interaction therapeutist-patient. The importance of radiological aspects is discussed and cybernetic model-conceptions of biological systems are also mentioned. A comprehensive diagnostic differentiation inclusive search for the source of disturbances and a psychological judgement are recommended.

[Treatment of patients with ureteral calculi].

Various methods of treatment were introduced to improve the results of management of patients with stones in the ureters and early restoration of their working capacity. Emergency ureterolithotomy (197 patients) reduced the incidence of postoperative complications by 2.4 times and the duration of in-patient treatment by 1.8 times. Hermetic sealing of the ureteral suture with glue in ureterolithotomy (43 patients) led to airtightness of the suture in 93.0% of cases. Needle reflexotherapy relieved an attack of renal colics in 94% of patients. After general multicomponent vibration (71 patients) and sonic stimulation (216 patients) the stones were discharged in 80.3 and 93.1% of patients, respectively. The application of organizational measures (creation of a city center of emergency urological service) and introduction of new methods for treatment of ureteral stones reduced the terms of rehabilitation of patients by 1.8 times, the economical effect in this case came to 392,3 rubles per each patient.

About the neurobiological foundations of the De-Qi--stimulus-response relation.

In acupunctural reflexotherapy, it is usually stated that no appreciable therapeutic effect is obtained under a certain stimulation level, which is determined by the appearance of a particular sensation known as De-Qi. Given that afference control actions of the central nervous system do not work in this way, but rather adapt, with some peculiarities, to a classical schedule of proportionality between stimulus and response, our aim is to evaluate the extent to which acupunctural stimuli of increasing intensity can also increasingly modify neurophysiological parameters, by focusing on quantification of metameric levels, by an F-wave study and by means of somatosensorial evoked potential in the telencephalus. Twenty-one healthy volunteers were studied; values corresponding to the parameters indicated in a basal situation were taken and both a non-acupunctural point and the classical 4IG point were punctured simply, until the De-Qi sensation was obtained, following which, electrical stimulation was applied. Clear progressiveness was observed in the modifications of both the F-value and somesthesic potential values with increasing stimuli. It is true that the main inflexion occurs where obtaining De-Qi, but when overstimulation is applied from that time, greater variations take place, which is particularly patent in the case of somesthesic potential latency. Furthermore, the puncture of an extrachannel point shows that the mere puncturing of the skin does not produce any of the aforementioned effects. Despite the classical postulates, it is observed that, the greater the intensity of the acupunctural stimulus, the greater the modifications in the neurophysiological parameters studied. Control of somesthesic afference by acupuncture has already been demonstrated and quantified; now, however, the direct relation between the intensity of this effect and that of the stimulus used to produce it, which is quite far removed from the classical energetic conceptions, is also observed. The effect of acupunctural stimulation is closely related to the intensity of the stimulus applied and the De-Qi sensation is not required to obtain neurofunctional modifications and stimulations that go beyond it producing greater effects.

[The rehabilitative treatment of victims with multiple and combined trauma].

Traditional methods of rehabilitation period treatment are not always possible with poly-trauma patients due to the presence of several fractures in association with internal injuries, bed-rest regime, hypokinesia of long duration and other factors. The author offers a complex approach to treatment with the provision for individual state of the patient. A consistent and gradual application of individual rehabilitation programmes together with physiotherapy, physical exercises, acupuncture, reflexotherapy and psychotherapy often helped to obtain good results.

[Treatment of local hyperkinesis].

Insufficient effectiveness of medicamentous therapy of local hyperkineses necessitated quest for other treatment options. In view of pathogenetic features of this pathological process we have chosen reflexotherapy to be instituted in our patients: auriculopuncture, electro-corporal- and acupuncture which have been shown to be expedient and effective in treating 65 patients with local hyperkinesis (blepharospasm, facial paraspasm, writer's spasm, spasmodic torticollis). Positive findings from clinical and paraclinical methods of investigation corroborate the results of the treatments administered.

[The control of somesthesia caused by neuroreflex stimulation: quantification of stimulus response as opposed to energy].

INTRODUCTION: In the reflexotherapy of classical acupuncture it is usually claimed that below a certain level of stimulation, which is determined by the appearance of a peculiar sensation known as De-Qi, no appreciable therapeutic effect is obtained. The control exerted by the afferences over the central nervous system, however, do not work in this way, but instead adjust themselves, albeit with certain peculiarities, to a classical pattern of proportionality between stimulus and response. Our aim is, therefore, to evaluate the extent to which increasingly intense acupuncture stimuli can modify neurophysiological parameters in a way that also grows. We will focus on the quantification on a metametic level, through the study of the F-wave, and in the telencephalon, through somatosensory evoked potentials. PATIENTS AND METHODS: A total number of 21 healthy volunteers were studied and we collected the values corresponding to the parameters indicated in the basal situation, when needling an extra-acupunctural point and following needling of the classical point IG4 until the sensation of De-Qi was felt, and electrostimulation was added after detecting the abovementioned sensation. RESULTS: We observed a clear progression in the modifications of both the F values and the somesthetic potential, with growing stimuli. Basic inflection does indeed occur after obtaining De-Qi, but on overstimulating from then onwards greater variations are produced. This is especially obvious in the case of the latency of the somesthetic potential. Needling a point that is not on a meridian guarantees that merely piercing the skin does not exert any of the effects mentioned above. DISCUSSION: In spite of the classical postulates, it can be observed that the more intense the acupuncture stimulus is, the greater the modifications to the neurophysiological parameters being studied are. The control of the somesthetic afference by acupuncture is something that has already been demonstrated and quantified. What is now seen, in addition, is the direct relation between the intensity of that effect and that of the stimulus used to produce it, which is quite different from the classical conceptions of energy. CONCLUSIONS: The effect of acupuncture stimulation is closely related to the intensity of the stimulus applied; the sensation of De-Qi is not necessary to obtain neurofunctional modifications and stimulations that go beyond it cause greater effects.
