Peyronie disease treated with ultrasound and hydrocortisone.

Thirty patients with Peyronie disease were treated with ultrasound using hydrocortisone ointment as the conducting vehicle. Twenty-five patients completed at least one course of treatment. Of those 25, 19 had at least some benefit with evidence that multiple courses of treatment were more beneficial than a single course. We conclude that ultrasound with hydrocortisone is beneficial in relieving pain, relieving deviation, and reducing the size of the fibrous plaque. It has the advantages of being noninvasive, repeatable, and without any side effects.

HLA-A and B antigens in patients with Peyronie disease.

Using microdroplet lymphocyte cytotoxicity test, as a method of HLA typing, 9 patients with Peyronie disease were studied from a viewpoint of relationship between this disease and HLA-A and B antigens. The frequencies of each antigen in the patients were compared with those of healthy controls. No significant difference of distribution in HLA antigens was observed between the patients and the control group.

Experience with Jonas malleable penile prosthesis.

Experience with the use of the Jonas penile prosthesis in 69 patients is presented, and the implantation of this prosthesis through a penoscrotal incision is described. Use of this prosthesis generally allows the penis to hang in a dependent position or to project outward from the body when coitus is desired. A malleable prosthesis such as the Jonas also has an additional advantage when it is used for treatment of impotence due to Peyronie disease, since its implantation allows the penis to be straightened without performing a simultaneous corporoplasty.

New surgical treatment for Peyronie disease.

Five patients with Peyronie disease were treated successfully with a surgical technique derived from Nesbit's technique for the treatment of congenital curvature of the penis. Excision of ellipses of tunica albuginea from the convex side of the penis as described by Nesbit was not done, but instead the tunica was reeved with nonresorbable sutures to achieve straightening. The surgical technique combined with prior radiotherapy was successfully applied to patients with severe forms of Peyronie disease. More experience is necessary to establish the value of the proposed treatment concept.

Combined Modality Therapy↗

Use of Dacron patch graft in Peyronie disease.

A Dacron patch graft for surgical correction of Peyronie disease was used in 4 patients. Results were satisfactory as far as the correction of the penile deformity and resolution of the pain during erection were concerned. No change was found in the patients' potency except in 1 patient whose potency has improved postoperatively. A penile prosthesis was inserted subsequently in 1 patient with decreased potency preoperatively. We conclude that this is a useful and simple surgical treatment for Peyronie disease.

Ultrasound imaging in Peyronie disease.

Fifty-three patients with Peyronie disease were examined sonographically. The use of a water path facilitates good localization and measurement of the fibrotic plaques in the area of the tunica albuginea. Calcifications can be clearly detected. Penile sonography is painless, free of risk, and easy to perform and yields precise morphologic pictures. It is ideal for documentation and assessment of the anatomic alterations in Peyronie disease as well as for control of its course.

Utility of Rigiscan and papaverine in diagnosis of erectile impotence.

The Rigiscan machine was used to measure nocturnal penile tumescence and rigidity in 41 men referred for evaluation of erectile impotence. In 6 patients, the Rigiscan was compared with the Tumistore and Snap-Gauge bands. Two patients achieved significant tumescence as measured by Tumistore, and 3 had significant tumescence as indicated by the breaking of two or three of the Snap-Gauge bands. None of these men achieved sufficient rigidity for intercourse as measured by the Rigiscan. Fourteen patients with organic impotence received intracorporeal injections of papaverine and were then monitored by Rigiscan. Six of the 14 were noted to have a Peyronie plaque. Of the remaining 8, 6 of these men achieved lasting erections sufficient for intercourse. The other 2 had erections that lasted ten minutes and were considered to have venous leak impotence. All patients with Peyronie disease had tumescence after papaverine injection, but in 5 the penis became rigid only at the base. Complications of papaverine injection were uncommon and minor. The Rigiscan and papaverine are useful in the diagnosis and management of erectile impotency.

Experience with lyophilized human dura for treatment of Peyronie disease.

Peyronie disease was treated by excision of the plaque and patch grafting with lyophilized human dura in 17 patients. Results were satisfactory as far as correction of penile curvature was concerned, but the incidence of impotence and glans hypoesthesia was disappointing. Less complex operations such as the Nesbit procedure may be a better choice of treatment for the surgical management of Peyronie disease.

Pathophysiology of erectile dysfunction.

During the past decade, our knowledge of the hemodynamics, functional anatomy, neurophysiology, and neuropharmacology of erectile function has evolved substantially. The change of smooth muscle tone has emerged as a key factor in erection and detumescence. However, future studies are needed to elucidate the cellular and molecular basis of erectile physiology. With insight into normal physiology we will understand the pathologic process and be able to treat it.

Evaluation of erectile dysfunction by dynamic infusion cavernosometry and cavernosography (DICC). Multi-institutional study.

In a cooperative, private practice, multi-institutional impotence study, dynamic infusion cavernosometry and cavernosography (DICC) was performed on 743 patients to make an accurate diagnosis and/or identify candidates for penile revascularization. Maximum equilibrium intracorporeal pressure achieved following papaverine and phentolamine injection (Phase I) averaged 29.42 +/- 0.76 mm Hg, approximately one third of normal erection pressure. Corporeal pressure fall from 150 mm Hg over thirty seconds (cavernosometry) averaged 82.38 +/- 1.33 mm Hg (Phase II). The gradient between systolic and cavernosal artery pressure averaged 42.84 +/- 1.12 mm Hg on the right and 43.33 +/- 1.13 mm Hg on the left (Phase III). Cavernosography at 90 mm Hg erection pressure was performed in Phase IV. Of the 124 patients from one center who were reviewed in greater detail, pure cavernosal artery insufficiency (CAI) was found in 25 (20.2%), corporeal veno-occlusive dysfunction (CVOD) in 26 (21.0%), and 73 patients (58.9%) demonstrated combined CAI and CVOD. Diabetics (n = 69) achieved lower equilibrium intracorporeal pressures than nondiabetics, had similar CVOD, and worse CAI. Smokers (n = 365) and patients with Peyronie's disease (n = 32) had erectile dysfunction similar to those without these conditions. Patients impotent after trauma (n = 124) were younger, achieved higher intracorporeal pressures, and showed better corporeal veno-occlusive function than those without trauma. Complications of DICC were minimal and infrequent. After DICC, 169 patients underwent internal pudendal arteriography, 105 had arterial bypass surgery with or without penile venous ligation procedures, and 45 had venous surgery alone. Dynamic infusion cavernosometry and cavernosography is a useful erectile function study to evaluate impotence and can be performed easily in a private practice setting.

Vein grafting of tunical incisions combined with contralateral plication in the treatment of penile curvature.

OBJECTIVE: To evaluate whether multiple incisions of Peyronie's plaque with placement of vein grafts to cover the tunical defects coupled with contralateral tunical plication is effective in straightening the penis while allowing preservation of normal erectile function. METHODS: Nine patients with Peyronie's disease and 2 patients with congenital curvature of the penis were surgically treated with a vein patch graft technique to correct their penile curvature. All patients underwent relaxing transverse incisions of their plaque with placement of a vein graft from the deep dorsal vein of the penis and/or the saphenous vein. Transverse relaxing incisions about 1 to 2 cm long were made on the tunica albuginea where a curvature was identified by an artificial erection. A corresponding size of the harvested vein was sewn into the defect created by the relaxing incisions. If there was evidence of a residual curvature after the vein grafts were sewn in, a plication of the contralateral surface of the tunica albuginea was performed. RESULTS: Of 10 patients who were potent preoperatively, 9 retained their potency post-operatively. Complete straightening of the erect penis occurred in 9 of 11 patients. Penile shortening occurred in three men. None of the patients permanently lost sensation in the shaft or glans of the penis. Two patients have anesthesia on part of the skin of the penile shaft. In all patients, the grafts were unable to straighten the penis 100 percent, thereby requiring at least one plication suture in the contralateral corpus. CONCLUSIONS: The use of vein grafts to cover multiple incisions of the tunica albuginea combined with contralateral corporeal plication is an easy alternative and an effective way to treat penile curvature while attempting to preserve erectile function.

Multiple clonal chromosome abnormalities in Peyronie's disease.

Numerical and structural chromosome aberrations were found in cell plaque metaphases from 9 of 14 patients with Peyronie's disease. In two cases there was evidence of clonal evolution for some of the chromosomal aberrations observed. The Y chromosome was the most frequently involved in numerical changes. Four of 9 cases with abnormal karyotype showed more than one abnormal, cytogenetically unrelated clone. Our findings suggest the possibility of a multiclonal origin for this benign tumor, and confirm the presence of chromosome instability in this cell growth disorder.

The effects of self-focused attention, performance demand, and dispositional sexual self-consciousness on sexual arousal of sexually functional and dysfunctional men.

Sexually functional (N=26) and sexually dysfunctional heterosexual men with psychogenic erectile disorder (N=23) viewed two sexually explicit videos. Performance demand was manipulated through verbal instruction that a substantial genital response was to be expected from the videos. Self-focused attention was manipulated by introducing a camera pointed at the participant. Dispositional self-consciousness was assessed by questionnaire. Performance demand was found to independently inhibit the genital response. No main effect of self-focus was found. Self-focus inhibited genital response in men scoring high on general and sexual self-consciousness traits, whereas it enhanced penile tumescence in low self-conscious men. Inhibition effects were found in both volunteers and patients. No interaction effects of performance demand and self-focus were found. Subjective sexual arousal in sexually functional men was highest in the self-focus condition. In sexually dysfunctional men, subjective sexual response proved dependent on locus of attention as well as presentation order.

Surgical treatment of Peyronie's disease by the intracavernosal plaque excision method: a new surgical technique.

OBJECTIVE: A new surgical method of treating Peyronie's disease consisting in intracavernosal excision of the plaque is presented. MATERIAL AND METHODS: The operation was performed on 16 men aged from 34 to 65 years (mean 50.2 years). The angle of penile curvature was 30 degrees to 60 degrees. Thirteen (81.25%) had impaired intercourse because of penile deformity and in 3 (18.75%) patients it was prevented by marked penile curvature. The mean quality of life score (QoL) was evaluated as 4.8. The operation consists in incising the corpus cavernosum parallel to the plaque and through this incision removing the plaque from the inside without incising or replacing the underlaying tunica albuginea. RESULTS: Follow-up examinations made after 3, 6 and 12 months revealed normal, painless erection in all the patients. In 2 (12.5%) intercourse was impaired only by persistent penile curvature of over 20 degrees. Mean QoL score -1.1. CONCLUSIONS: In our opinion intracavernosal plaque excision is a simple method, easier to perform and less invasive than the operative methods applied to date. It eliminates the pain, does not result in a shortening of the penis, loss of sensation or erectile dysfunction. It ensures a considerable improvement in the QoL of the patients treated.
