A retrospective examination of secondary sporozoite- and trophozoite-induced infections with Plasmodium falciparum: development of parasitologic and clinical immunity following secondary infection.

A retrospective study was made of clinical records to determine parasitemia and episodes of fever of 59 patients reinfected with Plasmodium falciparum for treatment of neurosyphilis, which was considered standard medical care at the time. Records were collected at the National Institutes of Health laboratories in Columbia, South Carolina and Milledgeville, Georgia during the period 1940 to 1963. Nineteen patients were infected via the bites of Anopheles albimanus, An. quadrimaculatus, or An. freeborni mosquitoes; the median prepatent period was 11.5 days. It was evident that clinical immunity, as measured by the frequency of fever, particularly high intensity fever (> or = 104 degrees F), was increased following reinfection. The parasitologic immunity, as measured by the frequency of asexual parasite counts and gametocyte counts, was also evident. In general, in secondary infections with homologous and/or heterologous strains of P. falciparum, fever episodes > or = 101 degrees F and > or = 104 degrees F were reduced in number, parasitemia was reduced, and gametocyte production was reduced. However, despite long courses of parasitemia during their primary infections, most patients developed fever and, in some cases, high-density parasitemia and gametocytemia following reinfection. The intensity of the secondary response did not appear to be associated with the length of the previous course of parasitemia. In addition, current infection with heterologous strain parasites did not prevent the development of fever or higher density parasite counts following imposition of the new strain of parasite.

A retrospective examination of sporozoite- and trophozoite-induced infections with Plasmodium falciparum in patients previously infected with heterologous species of Plasmodium: effect on development of parasitologic and clinical immunity.

A retrospective examination was made to determine parasitemia and episodes of fever in 97 patients, previously infected with Plasmodium malariae, P. ovale, and/or P. vivax, who were reinfected with P. falciparum for treatment of neurosyphilis, the standard treatment at the time. Data were collected at the National Institutes of Health laboratories in Columbia, South Carolina and Milledgeville, Georgia during the period 1940 to 1963. Results were compared with observations recorded for patients following primary infection with P. falciparum. The mean daily percentage of patients with fever > or = 101 degrees F during the first 20 days of primary infection with P. falciparum was 42.4; the percentage with fever > or = 104 degrees F was 19.9%. Those previously infected with P. ovale, P. vivax, and P. malariae had mean daily percentages of fever > or = 101 degrees F and > or = 104 degrees F of 39.1% and 14.8%, 39.1% and 19.4%, and 28.4%, and 11.3%, respectively. Previous infection with P. ovale or P. vivax had little, if any, effect on subsequent clinical malaria due to P. falciparum, whereas infection with P. malariae resulted in reduced frequencies of fever. A similar comparison was made for parasite counts > or = 1,000/microl and > 10,000/microl. The percentages for 268 patients during the first 20 days of primary infection with P. falciparum parasite counts > or = 1,000/microl and > or = 10,000/microl were 58.2% and 29.9%, respectively. Those previously infected with P. ovale, P. vivax, and P. malariae had mean daily percentages of parasitemia > or = 1,000/microl and > or = 10,000/microl of 58.0% and 24.3%, 57.3% and 31.1%, and 45.9% and 19.0%, respectively. Previous infection with P. malariae resulted in a reduction in the frequency of high-density parasitemia (> or = 10,000/microl) as well as an asexual parasite count > or = 1,000/microl. These results suggest that P. falciparum and P. malariae share common antigens that are able to induce parasitologic and clinical protection when infection with P. falciparum follows that with P. malariae. The results did not suggest that protection to P. falciparum is provided by previous infection with P. ovale or P. vivax.

Origin of an insular population of the wood mouse based on parasitological evidence.

Parasitological data were used to test the hypothesis that the wood mouse (Apodemus sylvaticus) population of Fair Isle, Shetland, originated from the British Isles rather than Scandinavia, as is usually argued. This study was based on the assumption that the mice were likely to share most of their parasite fauna with conspecifics from their ancestral home. The ecto- and endo-parasites of wood mice on Fair Isle between 18 June to 10 July 1987 and 17 to 26 August 1991 were identified and compared with those reported from conspecifics in the two putative source areas. All eight species of metazoan parasites that infected A. sylvaticus on Fair Isle were common to mice of the British Isles, whereas just one parasite on Fair Isle, a mite, had been recorded from Scandinavia. This lends support to the hypothesis that the mice originated from Britain rather than Scandinavia.

[Liver fluke infestation in New World camelids. Parasitology, pathology, clinical findings and therapy].

In Llamas and Alpacas infestation with Fasciola hepatica or Dicrocoelium dendriticum can cause liver damage, sometimes even with lethal outcome. Once infected South American Camelids (SACs) react more sensitively to these parasites than other domestic ruminants. We report here on the pathology, parasitology, clinics and therapy of this disease. Concerning Dicrocoelium dendriticum we describe own clinical results and therapeutic outcome in addition to the pathological investigation. According to anatomic corrosion casts, the bile ducts of SACs show more similarity with the equine bile system than with the bile system of domestic ruminants.

Parasitological response of Plasmodium falciparum infection to chloroquine treatment in malaria patients in Port Moresby.

A 7-day in vivo test system was applied to assess the parasitological response to chloroquine treatment in patients with falciparum malaria in the Central Province and National Capital District of Papua New Guinea. 30 patients were investigated but only 23 took a full course of chloroquine and were completely followed up. Of the 23 patients, 13 (57%) were negative for malaria parasites on day 2, 4 (17%) had significantly reduced parasitaemia by day 2 and cleared parasites by day 7, and 1 (4%) showed a partial response (R2). In 5 (22%) of the patients resistance at the R3 level was observed. The indication from this study is that chloroquine should continue to be the first-line drug for the treatment of uncomplicated falciparum malaria. However, judicious use of chloroquine in uncomplicated falciparum malaria is required to halt the spread of chloroquine-resistant strains of Plasmodium falciparum.

[Parasitologic survey of schistosomiasis due to Schistosoma mansoni in Katana, Democratic Republic of Congo].

It is now widely recognized that development of irrigation projects in endemic areas for schistosomiasis invariably leads to a recrudescence of the disease by increasing the habitat of the intermediate snail host of Schistosoma mansoni. This fatality was again demonstrated by experience in the Katana region of the Democratic Republic of Congo where development of 43 fresh water reservoirs for raising Tilapia nilotica led to multiplication of Biomphalaria pfeifferi. A parasitological study was conducted in three new villages around these basins and in neighboring villages. Stool examinations were performed in a total of 787 people. Infestation rates were 8.1 p. 100 and 4 p. 100 respectively. Infestation exceeded 25 p. 100 in children between the ages of 10 and 14 years. These findings underline the need for preventive measures.

Socioeconomic status, clinical features, laboratory and parasitological findings of hepatic amebiasis patients--a hospital based prospective study in Bangladesh.

Socioeconomic status, clinical, laboratory and parasitological features of 31 hospitalized amebic liver abscess (ALA) and 8 amebic hepatitis (AH) patients were studied. Thirty-seven (94.9%) of the total 39 cases were from low socioeconomic class and 2 (5.1%) were from middle class (p<0.001). Sixteen (51.6%) ALA and 5 (62.5%) AH patients were admitted with duration of disease for 3 weeks or more. Twenty-one (67.7%) ALA and 3 (37.5%) AH cases gave no previous history of diarrhea or dysentery. Epigastric pain was the predominant symptoms in 71% patients compared to high fever (19.4%), nausea and vomiting (9.7%). Neutrophilic leukocytosis was found in 9 (29.0%) ALA and 2 (25%) AH cases. Raised alkaline phosphatase was the predominant abnormal liver function test found elevated in 22 (71.0%) ALA and 5 (62.5%) AH cases. Three (7.7%) of the 5 (12.8%) microscopy positive stool samples yielded growth of Entamoeba histolytica in culture. The right lobe was involved in 28 (90.3%) ALA cases; 29 (93.5%) patients had single abscess. Bacterial super infection was observed in 1 (12.5%) abscess, reactive changes in right lung was observed in 6 (19.4%) ALA and 1 (12.5%) AH cases. Ascaris lumbricoides was the predominant associated intestinal parasite.

Clinical and parasitological appraisal of patients with imported malaria.

The study group comprised 118 patients suspected of malaria who underwent an examination following their return to Poland from the tropical countries. Evaluation of the admitted patients was based on epidemiological, clinical and parasitological criteria. The obtained data allowed to confirm acute malaria in 20 patients (16.9%) and malaria in the past, in 32 patients (27.1%). In the remaining 65 patients (55.1%) we excluded malaria. Malaria caused by P. vivax was confirmed in 5 patients, by P. falciparum in 9 patients. A mixed invasion was diagnosed in 6 patients including: P. falciparum and P. vivax in 3 cases; P. vivax, P. ovale and P. malariae in 2 cases; P. falciparum, P. vivax and P. ovale in 1 patient. We noted no relationship between the severity of the clinical course and IFA results. It was found an atypical clinical course of malaria and diagnostic difficulties in patients who used chemosuppression for malaria prophylaxis.

Rural bancroftian filariasis in north-western Cameroon: parasitological and clinical studies.

A cross-sectional epidemiological, parasitological and clinical survey of bancroftian filarieasis was conducted in the rural north-western Republic of Cameroon. Of the 1020 respondents (622 males and 398 females) randomly drawn from 4 ethnically homogeneous communities, 148 were microfilaraemic giving an overall prevalence of 14.5%. Association between the ages of individuals and the prevalence of microfilaria (mf) in peripheral circulation was statistically significant (chi 2(6) = 12.59, P < 0.05). More males (7.8%) than females (6.7%) of all ages exhibited mf and was significant statistically (chi 2 = 0.98, P < 0.01). Community-specific prevalence of mf ranged from 11.9% in Bambui to 17.0% in Bambeli. At the community level the overall geometric mean intensity (GMI) of infection was 192 mf per 10 ml of blood (range 165 mf p ml to 248 mf p ml). Overt clinical manifestations of chyluria, elephantiasis and hydrocele were detected among the mf-positive respondents. Thirty one cases (20.9%) involving 15 (18.6%) males and 16 (23.15%) females exhibited chyluria which was absent in children aged 1 < or = 10 years but appeared to be associated with advancing age. Elephantiasis especially of lower extremities and breast was observed in 8 (10.0%) males and 15 (22.0%) females. Highest cases of elephantiasis were recorded for female adults aged 41-60 years (34.6%) of total cases. Hydrocele affected 20.0% of mf-positive males ages > or = 11 years with cases increasing with advancing age. A positive association seems to exist between hydrocele prevalence and the prevalence of microfilaraemia. A total of 1440 female mosquitoes belonging to 3 species were dissected for the presence of microfilariae. Anopheles gambiae complex s.l. was the dominant species and the only one found to be naturally infected with mf. The apparently low mf. intensity in the population, pattern of microfilaraemia, distribution of clinical syndromes and vector species are discussed.

Paragonimiasis in Yongjia County, Zhejiang Province, China: clinical, parasitological and karyotypic studies on Paragonimus westermani.

Paragonimiasis in Yongjia County, Zhejiang Province, China, has been of such public health concern that a specialized Institute for Paragonimiasis Control has been established there. The study reported here involved both parasitological research on Paragonimus westermani in the endemic areas and a clinical analysis of 94 cases of paragonimiasis. Eggs were found in sputum, feces and brain tissue in 45 cases. Twenty patients, 19 of whom where children under 10 years of age, showed hepatic injury. Treatment with praziquantel or bithionol had satisfactory results in all cases. Dimensions of the parasite eggs, metacercariae from freshwater crabs and adult flukes suggested that the form of P. westermani in Yongjia is the 'small type': a suggestion supported by karyotypic studies on gonad cells from adult flukes collected from experimentally infected animals as all were found to be diploid. In conclusion, the 'small type' or diploid form of P. westermani is the cause of paragonimiasis pulmonary symptoms in Yongjia.

Parasitological and immunological changes in murine hepatic schistosomiasis before and after praziquantel treatment.

Hepatic fibrosis was assessed by estimating hydroxyproline content in liver tissues at the course of the disease and after PZQ treatment. Parasitological investigations included egg count in faeces and tissues (liver and intestine) and oogram pattern. The results showed that treatment of infected mice with PZQ, modulated the course of schistosomiasis as evaluated by highly significant decline in hepatic hydroxyproline content as well as egg count in stool and tissues. In addition, the oogram pattern showed that PZQ had a lethal effect on mature eggs in tissues. PZQ treatment of S. mansoni infected mice, is effective in reducing the severity of the disease and in attenuating hepatic fibrosis, particularly when the treatment starts early with a suitable dose.

[Clinical and parasitological diagnosis of human dirofilariasis].

The paper reports two new cases of human dirofiliriasis (its causative agent is Dirofiliria repens) with a rare clinical diagnosis of funiculitis of the right scrotal spermatic cord in a 12-year-old boy from the Tambov Region and of erysipelatous inflammation of the outer side of the right leg in a 68-year-old woman who resides in Moscow and lives in her country house in the Shakhovsky District, Moscow Region, in summer. Both cases of invasion are regarded as local. The bodies of the D. repens females detected measured 110 x 0.476-0.48 and 135 x 0.56-0.60 mm, respectively, and the females were identified as impubertal and pubertal, but infecund. In the former case, dirofiliriasis was an etiological factor of spermatic cord funiculitis. In the latter case, the location of Dirofiliria provoked prolonged (7-month) erysipelatous inflammation. Emphasis is placed on the implication of dirofiliriasis in human pathology. To have a more intimate knowledge of parasitic etiology will help physicians in making a differential diagnosis of dirofiliriasis and in elaborating a correct treatment policy after a clinical and parasitological diagnosis is established.

Real-time quantitative PCR in parasitology.

Standard techniques for counting parasites are often time-consuming, difficult and inaccurate, and occasionally unpleasant. Real-time quantitative polymerase chain reaction has recently been applied to parasitology, specifically Plasmodium, Toxoplasma, Leishmania and Neospora. These techniques are truly quantitative, give results over a range of 6-7 orders of magnitude, are quick to perform and require no manipulations post-amplification. They can be used to count genome numbers and to study levels of gene expression. The advantages and limitations of existing thermocyclers and applicable detection systems are discussed here, and promising new developments are highlighted.

[Parasitological survey, with special reference to schistosomiasis mansoni, conducted in the region of Jacupiranga (Ribeira valley, State of São Paulo). Survey of the location and distribution of planorbidian snails in the State of São Paulo. VII].

The results of a parasitological survey, emphasizing particularly schistosomiasis mansoni, are presented. The area under study was the district of Cajati, situated in the "município" of Jacupiranga (State of São Paulo, Brazil) near schistosomiasis-endemic areas in Vale do Ribeira. Though the presence of parasitized people by Schistosoma mansoni has been reported and focuses of Biomphalaria tenagophila, the species responsible for the spreading of the parasites through great areas of the state, has been localized, it has not been reported any autochthonous case nor the presence of schistosoma-infected planorbidian snails. As a contribution to the planorbidical chart of the State, it is reported as original the discovery of the species Drepanotrema cimex and D. depressissimum in that area. The detection of antitoxoplasmic antibodies in a sample of the studied population showed a significant prevalence rate corresponding to 76,4%.

[Preliminary results in the clinical, epidemiological and parasitological study of Bancrofti filariasis in Madagascar].

In 1958, a large study on the distribution of Bancroft filariasis was set up in Madagascar. In order to update these data, the authors have studied in the same areas the parasitological and clinical prevalence of Bancroft filariasis. Here are the preliminary results of this study, concerning 1862 people, aged 10 years and more. The mean prevalence rate of microfilaria-carriers was of 22.9%, and the average parasitic density was of 3.6 parasits/20 microliters of blood. The prevalence rate of chronic clinical symptoms was of 14.7% for men (elephantiasis, hydroceles, chyluria) and of 2% for women (elephantiasis). These were usually mild symptoms, which very little impact on way of life.

Parasitological aspects of Capillaria philippinensis recovered from Egyptian patients.

Seven new cases of intestinal capillariasis were detected in Kasr El-Aini University Hospitals, Cairo, during the period from July 1997 to June 1998. A parasitological study concerning the morphological features of Capillaria philippinensis recovered from the stool of Egyptian cases was carried out. Eggs were separated, cultured and used to infect Oreochromis nilotica niloticus, a famous popularly consumed type of fresh water fish in Egypt in a trial for identification of the fish intermediate host for this parasite, but infection had not developed in the intestine of this species of fish.
