Regulation of selected biological processes in neoplastic cell lines by halogen derivatives of 5-hydroxy-2-hydroxymethyl-4-pyranone.

The present study was undertaken to investigate the effects of eight halogen derivatives of 5-hydroxy-2-hydroxymethyl-4-pyranone on a) neoplastic cell growth, b) DNA, RNA, and protein synthesis, c) cytoplasmic phosphorylation, and d) cAMP accumulation in culture medium. Two neoplastic cell lines (the murine leukemia L1210 and the rat pituitary GH4C1) were used in the present study. The inhibitory effects of the halogen derivatives on GH4C1 cell division when compared to L1210 cells were found to be almost equal. Two halogen derivatives (2-chloromethyl-5-hydroxy-4-pyranone and 6-bromo-5-hydroxy-2-hydroxymethyl-4-pyranone) were found to inhibit DNA, RNA and protein synthesis. Moreover, the electrophoretic profile of low molecular mass cytoplasmic proteins was markedly influenced by certain halogen derivatives of 5-hydroxy-hydroxymethyl-4-pyranone in L1210 cells.

Contrast-enhanced MR imaging with fat suppression in adult-onset septic spondylodiscitis.

Spinal infections typically involve vertebrae as well as discs, and for this reason they are called septic spondylodiscitis. Magnetic resonance imaging is the most sensitive imaging method for the evaluation of this group of spinal diseases. The use of contrast-enhanced T1-weighted sequences with fat suppression, if correctly applied, may increase information provided by MRI. Firstly, this technique allows the primary vertebral focus, which often precedes disc involvement, to be identified at a very early stage. When the disease spreads, T1-weighted fat-suppressed gadolinium dimeglumine (Gd-DTPA) enhanced images provide macroscopic details of the primary vertebral focus, disc involvement patterns, and pathways of infection diffusion. All this information, when correlated with laboratory tests, may be useful in identifying the infectious agents (tuberculous vs piogenic forms), thus enabling a suitable therapy to be started. This technique is also useful in the assessment of the real extension of the disease, providing a clear depiction of paravertebral space involvement and of psoas muscle abscesses. Dangerous complications, such as meningitis, myelitis, and epidural abscesses, may be more promptly diagnosed and fully evaluated with fat-suppressed post-contrast T1-weighted images. Finally, this imaging technique may help to differentiate infectious processes from degenerative disorders, extradural neoplastic processes, and rheumatic diseases.

[Function of sensorimotor equilibrium function in elderly patients on the basis of vestibular evoked myogenic potentials (m.p.p.w.)].

Observed elderly had general weakness of motor and mental function. The slower reactions to environment stimulus in elderly above 60 years result first of all from weakness of sensory receptors as visible, hearing, smell and taste. In this age group disturbing of balance system function demonstrated vertigo, dizziness, darkness of visible, disequilibrium was more encountered. Up to now there is not specified that "presbyequilibrium" (presbyvertigo, presbyastasis) as physiology ageing process causes this condition or destructive processes, infections, vascular insufficiency and neoplastic processes in inner ear and acoustic tracts and centres. Vestibular symptoms often are related to age by the doctors so for that reason basic and sometimes dangerous disease may be missed. Proposition of this paper is estimation of reflex function of balance system in connection with ageing process. To this end VEMPs were used and latencies and amplitudes of ipsilateral and contralateral VEMP recordings were calculated in young and elderly groups. Lower reflex efficiency in elderly disclosed longer latencies and lower aptitudes of p14 n21 of VEMPs than young people but without significant differences.

[Tumor markers in the diagnosis of tumors of the liver and biliary tract].

Tumour markers are a valuable contribution to the diagnosis and monitoring of tumours processes in the liver. They are used for early diagnosis as well as for monitoring of treatment. In particular monitoring of the dynamic of changes is important. A dominating place is still held by alpha fetoprotein. The importance of CEA is in differentiation of metastatic processes. In the diagnosis of neoplastic processes in the biliary pathways antigen CA 19-9 is most important. An important finding is also than non-specific increases of tumours markers occur in some chronic processes of the liver and biliary pathways. Despite the large number of tumour markers they are useful only in conjunction with other clinical and imaging methods.

[Oncoproteins and anti-oncoproteins in blood serum as biomarkers of early health effects induced by occupational and environmental carcinogens].

Over a whole life-span a genetic machinery of human cells is exposed to carcinogenic effect of various chemical, physical and biological factors leading to cellular changes in the genome like point mutation and translocation or amplification of genes. Together with growing an ageing of the human organism, the number and heterogeneity of mutated cells increase. Thus, the presence or absence of the neoplastic process may depend on the length of life, efficiency of the immunological system, cumulation of adverse affects of environmental factors or inherited susceptibility to a disease. There is no doubt that neoplasms which develop due to inheritance of different genetic defects or due to direct environmental effect progress in line with similar mutations, resulting finally in the genome destabilisation and disturbances in the balance between proliferation and differentiation processes. The knowledge as to how far environmental carcinogens affect cellular genome is still limited. Nevertheless, data collected to date help to understand the mechanisms by which environmental carcinogens may initiate the process of transforming normal cells into neoplastic ones. It is most likely that activation of some oncoproteins and deactivation of some suppressive genes and related qualitative and quantitative changes in oncoproteins and anti-oncoproteins participating in the growth control and cellular division, are the essential mechanisms which are involved in this process. The discovery that activation of oncoproteins and related emergence of oncoprotein in cells, increased in the number of changed, can be monitored in easily available biological fluids, such as blood or urine, has become of great importance for occupational medicine and environmental health. That has provided new diagnostic measures for evaluating carcinogenic effects of the working and living environments. The fact, that the emergence of oncoproteins in biological fluids may precede by many months or even years clinical manifestations of neoplastic disease should greatly contribute to undertaking more effective preventive measures.

Aggressive fibromatosis (desmoid tumor) is a monoclonal disorder.

Aggressive fibromatosis (also called deep fibromatosis or desmoid tumor) is a proliferation of cytologically benign-appearing fibrocytes, often resulting in significant functional loss. The nature of the lesion is controversial: some evidence suggests that it is a reactive process, whereas other evidence supports a neoplastic etiology. The pattern of X chromosome inactivation, using a technique based on polymerase chain reaction (PCR) amplification of a hypervariable CAG repeat region flanking Hhal restriction sites of the human androgen receptor gene, was determined in four cases in which cryopreserved tumor and adjacent normal tissue were available. All four tumors demonstrated a monoclonal pattern, while the adjacent normal tissues demonstrated a polyclonal pattern. This demonstrates that aggressive fibromatosis is proliferation of cells derived from a single clone with a growth advantage, and thus is likely a neoplastic process.

[Analytical morphodynamics. Practical applications].

The analytical morphodynamic method, based on the study of the fate of the biogic units of an organ at the level of well or poorly developed lesions, has been applied to the analysis of chronic obliterating arteriopathies of the extremities, of benign tumors of the mammary parenchyma, and of malignant hepatomas. In morbid states as different as a reactive process and a benign or malignant neoplastic process, it allowed to reconstritute the histogenesis of complex tissue changes and led to interesting prospects, in the field of general biology and pathogenesis.

Clinicopathology of non-neoplastic and neoplastic lymph node swelling.

Lymph node swelling, which can be problematic when attempting differential diagnosis between non-neoplastic or neoplastic types, can be grouped into three categories. The first is lymph node swelling due to heterotopia, hamartoma or hyperplasia, the second is due to non-neoplastic lymphoid cell and/or histiocyte proliferation with the appearance of neoplastic proliferation and the third is due to true neoplastic proliferation, but apparently simulating a non-neoplastic condition. The present paper describes some of the clinicopathological features of representative diseases or cases in each category, e.g. Castleman's disease, necrotizing lymphadenitis, "IBL"-like T-cell lymphoma, and lymphoma showing an unusual course initially simulating a non-neoplastic process and then gradually disclosing its neoplastic nature in the late stage. These cases emphasize the importance of close collaboration between the clinician and pathologist together with immunohistochemical studies of lymph node pathology.

Biliary adenofibroma. A heretofore unrecognized benign biliary tumor of the liver.

Benign biliary tumors of the liver are uncommon. In this report, we describe a distinctive biliary tumor of 7-cm diameter occurring in the right lobe of the liver of a 74-year-old Chinese woman. The lesion, characterized by a complex tubulocystic nonmucin secreting biliary epithelial and an abundant fibroblastic stromal components, is distinct from other well-recognized biliary lesions. A number of unusual features are focally present, namely, intraluminal bile concretions, apocrine-like epithelial change, acute inflammation, and granuloma. The tumor shows a striking resemblance to Meyenburg's complex (MC), but the large size of the lesion and the absence of any typical MC in the background liver are exceptional for the latter. Its expansile growth, possession of mitoses, and foci of epithelial tufting and cellular atypia favor a neoplastic process. Previous reported cases of adenomatous neoplastic transformation of MC are dissimilar. We therefore conclude that this is a hitherto unrecognized biliary tumor that may be yet another neoplastic form of MC and propose the designation biliary adenofibroma. The course appears benign, but malignant epithelial transformation may supervene if the lesion is left untreated.
