A preliminary examination of the pornography experience of sex offenders, paraphiliacs, sexual dysfunction patients, and controls based on Meese Commission recommendations.

The Meese Commission Report claims exposure to pornography leads to sex offenses and states it is important to examine the developmental patterns of offenders, in particular age of first masturbatory experience, role of pornography in that experience, age of first exposure to pornography, age of commission of the deviant behavior, and long-term use of pornography and length of duration of deviancy. This study found the frequency of use of pornography, age of exposure to pornography, age of first masturbation experience, and use of pornography during first masturbation experience, for sex offenders, paraphiliacs, sexual dysfunction patients, and controls were not significantly different.

Effect of contaminated preprocessed semen on fertilization rate and embryo quality in assisted reproductive techniques.

We aimed to identify the sources and prevalence of semen contamination from mastrubation and determine the effect of bacterospermia on fertilization rate and embryo quality in standard in vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI). This was a prospective controlled study, in an IVF unit of a university teaching hospital, of 93 consecutive couples undergoing IVF-embryo transfer cycles. We evaluated handwashing; semen collection and processing; and assisted reproductive technology using semen provided by masturbation. The main outcome measures were presence and type of micro-organisms in the semen samples and embryo culture medium; the effect of hand washing on rate of contamination; and the effect of semen contamination on fertilization rate and embryo quality. The first consecutive 52 men of the 93 couples were not instructed to wash their hands before masturbation, and the remainder were so instructed. Forty-nine semen cultures (94.2%) in the first group were contaminated compared to only 16 (39%) in the second (p < 0.016); 27 of the 65 positive cultures (41.5%) were contaminated by more than one organism. The most common contaminators were bacteria usually found on the skin. All but four embryo medium cultures were negative. There was no significant difference in fertilization rate and embryo quality by culture findings in either the IVF or the ICSI procedures. We found that a high percentage of manually obtained semen for standard IVF or ICSI procedures was contaminated, but this had no effect on fertilization rate and embryo quality.

Changing sexual attitudes and behaviour in China: implications for the spread of HIV and other sexually transmitted diseases.

In Imperial China sexual behaviour was regarded as an indispensable activity to reach harmony with the universe, through the unity of the interaction of two opposing forces: yin and yang. Sexual intercourse was accepted when linked to procreation within a family context, while an individual's sexuality was not considered important. Homosexuality was tolerated although not advocated, while masturbation was denounced. Since the One Child Family and Open Door policies in the 1970s and the economic reforms of the 1980s, attitudes towards sexuality in China have changed. Premarital sex has become widely accepted among young people and people in China are now more tolerant toward extramarital sex. Nowadays young people consider that love should dominate marriage and the quality of an individual's sex life is currently more valued than it used to be. Attitudes towards masturbation have become more tolerant and though homosexuality has been hidden by society for a long time, in recent years it has begun to be considered as a legitimate lifestyle choice. Attitudes towards sex and sexual behaviour have become recognized as an individuals' responsibility as long as no offence occurs against society or the interests of other individuals, resulting in the recognition of diversity of sexual behaviour. As part of the changing attitudes to sex and sexual behaviour, heterosexual transmission is becoming the most important route of HIV transmission in China. This is complicated by the internal migration of an estimated 120 million labourers moving from the countryside to the cities as the result of economic reforms, most of whom are sexually active young men. Unless addressed directly, these factors may add to the estimated 300,000 HIV-infected Chinese, further fuelling an already rapidly spreading epidemic. The ramifications of the Chinese HIV epidemic will not only be felt within China, but also within the surrounding Asian countries thereby fuelling the HIV pandemic.

Sexual behaviors and other risk factors for Candida vulvovaginitis.

Sexual behaviors are associated with many genital infections, but the role of sexual variables as risk factors for Candida vulvovaginitis has not been clearly determined. To assess the association between sexual behaviors and other risk factors with the presence of Candida vulvovaginitis, we performed a case-control study comparing these potential risk factors in women with and without culture-documented Candida vulvovaginitis in two Midwestern community-based medical offices. Participants included 156 women with Candida vulvovaginitis and 92 controls, ages 18-60. Risk factors for Candida vulvovaginitis, including sexual and partnership behaviors, demographic data, past genital infections, exposures, and diet, were investigated using logistic regression. The presence of Candida vulvovaginitis was positively associated with recent cunnilingus (odds ratio [OR] = 2.22 for five times a month compared with no times, 95% confidence interval [CI] 1.36, 3.84), but was less likely in women who masturbated with saliva in the previous month (OR = 0.30 if masturbated five times vs. no times, 95% CI 0.09, 0.99). Other independent risk factors included knowing the sexual partner a shorter period of time (OR = 1.56 for 1 year vs. 5 years, 95% CI 1.16, 2.13) and lower milk ingestion (OR = 3.57 for no servings vs. two servings per day, 95% CI 2.00, 6.67). Increased number of sexual partners, early age at first intercourse, and increased frequency of intercourse are not related to risk.

Sexuality in older adults: behaviours and preferences.

BACKGROUND: while much has been written about adult sexuality, relatively little is available about the sexuality of older people. Available literature often does not discuss specific sexual behaviours and includes predominantly married, better-educated, mostly young old. OBJECTIVE: the purpose of this study was to assess a sample of lower-income older adults, about whom there is limited information, to describe a full range of sexual behaviours and to identify the degree to which they are satisfied with their sexual activities. METHODS: subjects were 179 people (60 and older) who were residents of subsidised independent-living facilities, recruited during a lecture or in public areas in the building. Thirteen of 179 were excluded due to age. Most were white (82%), living alone (83%) and female (63%). RESULTS: overall, the majority reported to have had physical and sexual experiences in the past year such as touching/holding hands (60.5%), embracing/hugging (61.7%) and kissing (57%) daily to at least once a month; mutual stroking, masturbation and intercourse were experienced 'not at all' by 82% or more. For all activities except masturbation, participants wanted to participate in sexual activities more often than they did. The most important barrier to sexual activity was lack of a partner. Self-reported health was related to sexual activities wanted, with age also related to some preferences. CONCLUSIONS: most of the elderly surveyed want to maintain a sexual relationship which includes touching and kissing, and they would like to have more sexual experiences than they have accessible. Further studies are needed.

Pregnancy after intracytoplasmic sperm injection of metaphase II oocytes with spermatozoa from a man with complete retrograde ejaculation.

Retrograde ejaculation is an uncommon cause of infertility, which has been treated successfully with different kinds of artificial reproduction technique, e.g. cervical cap artificial insemination by husband, intra-uterine and intraperitoneal insemination, standard in-vitro fertilization, pronuclear stage transfer and gamete intra-Fallopian transfer. All these techniques require a minimal number and motility of spermatozoa obtained after post-masturbation voiding. In some cases, only very few spermatozoa with very poor or no motility are found in the urine voided immediately after masturbation. In such a case, where no more than 14 spermatozoa were recovered over a 3 h search, intracytoplasmic sperm injection of metaphase II oocytes led to the development and replacement of three fair embryos, resulting in an ongoing twin pregnancy. This technique opens up perspectives for the treatment of men with complete retrograde ejaculation and quasi-azoospermic post-voiding specimens.

Study of sexuality-related characteristics in young adults with schizophrenia treated with novel neuroleptics and in a comparison group of young adults.

This study compared characteristics related to sexual history, sexual activities, sexual functioning, and psychological tendencies associated with sexuality in 45 young adults with schizophrenia treated with novel neuroleptics and in 61 young adults from a comparison group. A smaller proportion of young adults with schizophrenia currently had a sexual partner or had ever engaged in sexual relations. They also had sexual relations and sexual desires less often. Whether affected by schizophrenia or not, a smaller proportion of women had ever masturbated, and a smaller proportion of men currently had a sexual partner. Women masturbated less often, felt less sexual desire, and desired sexual relations less often, compared with men. Proportionally more men with schizophrenia treated with risperidone or olanzapine than men in the comparison group had at least one sexual dysfunction, lacked sexual desire, and reported problems with sexual arousal and ejaculation. Women with schizophrenia were more likely to report problems with sexual arousal and galactorrhea, compared with women in the comparison group. Finally, young adults with schizophrenia were more likely to develop negative psychological tendencies associated with sexuality than were young adults in the comparison group. Sexual problems are highly prevalent among young adults with schizophrenia. Sexuality should occupy the space it deserves within psychosocial rehabilitation programs and the treatment of schizophrenia.

Adolescent sexuality and sexual behavior.

PURPOSE OF REVIEW: Adolescence is a time of self-discovery and physical, as well as cognitive, development. It is within this context that adolescent sexual development and sexual behavior occur. While curiosity and experimentation are normal, sexual behaviors, both coital and non-coital, place adolescents at risk for undesired consequences including sexually transmitted disease acquisition and pregnancy. Trends in adolescent sexual behavior are changing, and health care professionals must be aware of these trends to provide necessary medical care and education to this population. RECENT FINDINGS: While the sexual activity of teenagers garners much attention, attention must also be directed at non-coital activities such as masturbation, mutual masturbation and oral sex, as the riskier of these behaviors appear to be increasing. The trends in sexual activity and contraceptive use are encouraging with a decrease in the proportion of adolescents reporting sexual activity, and an increase in the proportion reporting using contraception. These trends, however, are not shared equally among racial groups with the greatest decline reported in the in lowest risk groups. Sexual minority youth continue to report a higher prevalence of high-risk behaviors, both sexual and non-sexual, as compared to their heterosexual peers. SUMMARY: These findings highlight the multiple roles health care professionals can play in caring for this unique population: firstly as health care providers, offering age appropriate, confidential health care; secondly, as reproductive health care educators providing factual, balanced, and realistic information to both teenagers and the community; and thirdly, as advocates lobbying for greater education and services for this at-risk population.

Changes in sexual function in middle-aged and older men: longitudinal data from the Massachusetts Male Aging Study.

OBJECTIVES: To describe within-individual change in sexual function over a 9-year period and to determine whether the amount of change differs by age group. DESIGN: Cohort study; participants interviewed at baseline (1987-89) and follow-up (1995-97). SETTING: Population-based; communities surrounding Boston, Massachusetts. PARTICIPANTS: One thousand eighty-five men aged 40 to 70 at baseline (born between 1917 and 1947) with complete baseline and follow-up sexual function data. MEASUREMENTS: Within-person change (follow-up minus baseline) in the following sexual function variables: sexual intercourse, erection frequency, sexual desire, ejaculation with masturbation, satisfaction with sex, and difficulty with orgasm. RESULTS: Unadjusted analyses showed significant longitudinal changes over the 9-year period in all domains of sexual function except frequency of ejaculation with masturbation, which showed no change between baseline and follow-up. Adjusted for baseline sexual function, within-person change in all outcomes was strongly related to age, with decline in sexual function becoming more pronounced with increasing age. For example, over the 9-year study period, sexual intercourse or activity frequency decreased by less than once per month, two times per month, and three times per month in men in their 40s, 50s, and 60s, respectively. Number of erections per month declined by 3, 9, and 13 in men in their 40s, 50s, and 60s, respectively. CONCLUSION: This research fills a major gap in the literature by providing age-specific estimates of change in sexual functioning over a 9-year period in a cohort of unselected men.

Sexual behaviour of lesbians and bisexual women.

OBJECTIVES: To provide data about the sexual histories of a large sample of lesbians and bisexual women, to inform those who provide health care or carry out research with women who may be sexually active with other women. DESIGN: Cross sectional survey. SETTING/SUBJECTS: 803 lesbians and bisexual women attending, as new patients, lesbian sexual health clinics, and 415 lesbians and bisexual women from a community sample. MAIN OUTCOME MEASURES: Self reported sexual history and sexual practice with both male and female partners. RESULTS: 98% of the whole sample gave a history of sexual activity with women, 83% within the past year, with a median of one female partner in that year. 85% of the sample reported sexual activity with men; for most (70%) this was 4 or more years ago. First sexual experience tended to be with a man (median 18 years old), with first sexual experience with a woman a few years later (median 21 years). Oral sex, vaginal penetration with fingers, and mutual masturbation were the most commonly reported sexual practices between women. Vaginal penetration with penis or fingers and mutual masturbation were the most commonly reported sexual activities with men. CONCLUSIONS: These data from the largest UK survey of sexual behaviour between women to date demonstrate that lesbians and bisexual women may have varied sexual histories with both male and female partners. A non-judgmental manner and careful sexual history taking without making assumptions should help clinicians to avoid misunderstandings, and to offer appropriate sexual health advice to lesbians and bisexual women.

Effects of sexual arousal on lymphocyte subset circulation and cytokine production in man.

OBJECTIVE: Sexual arousal and orgasm induce an increase in sympathetic activity as well as in catecholamine and prolactin plasma concentrations. However, the effects of sexual arousal and orgasm on immune functions in man are unknown. Thus, this study investigated the effects of masturbation-induced orgasm on lymphocyte circulation and cytokine production in healthy young males. METHODS: In a crossover design, 11 volunteers completed an experimental condition in which they were asked to masturbate until orgasm and to participate in a control condition without sexual activity. Blood was drawn continuously for determination of endocrine parameters. In addition, leukocyte and lymphocyte subsets were analyzed via flow cytometry, and the production of lipopolysaccharide-induced interleukin 6 and tumor necrosis factor alpha was measured before and then 5 and 45 min after the orgasm. RESULTS: The results confirmed transient increases in adrenaline and prolactin plasma concentrations. Sexual arousal and orgasm increased the absolute number of leukocytes, in particular natural killer cells (CD3-CD16+CD56+), in the peripheral blood. In contrast, T cell (CD3+) and B cell (CD3-CD20+) subpopulations as well as the production of interleukin 6 and tumor necrosins factor alpha remained unaffected by sexual activity. CONCLUSION: These findings demonstrate that components of the innate immune system are activated by sexual arousal and orgasm.

Effects of methaqualone on social behavior in monkeys (Macaca mulatta).

In an established social group of rhesus monkeys (macaca mulatta) consisting of 1 adult male, 6 adult females, 1 adolescent male and 2 infants, 3 subjects were selected to study the effects of intramuscular methaqualone (10 mg/kg) on behavioral changes in the treated animals, as well as the rest of the colony. Three drug trials alternating with saline controls given to the dominant male, a mid-ranking female and the adolescent male. 20 h of observations were made prior to treatment to serve as a baseline, using 40 discrete behaviors. For each drug and saline period, 2 h of continuous observation was directed towards each target animal (total 36 h), and 12-min behavioral samplings were carried out on each untreated subject in the colony (36 h). Dependent upon the sex and social rank of the subject, different behavioral changes occurred during drug treatment, which were consistent from trial to trial. New behaviors were observed, such as active grooming, autofellatio and masturbation in both males, while some previous behaviors disappeared or decreased. The mid-ranking female showed an increase in aggressive behavior. A biphasic action of the drug was also observed in all subjects. During the first 80--100 min, the males were mainly passive or engaged in grooming, but after this primary phase, they started either to masturbate or autofellate. After approximately 2 h, the social status and behavior of the animals returned to predrug levels. During the drug treatment, marked changes occurred in the behavior of other colony members, e.g. when the dominant male's vigilance was lowered, the highest ranking female replaced him in rank. In general, affiliative activities were increased under the influence of methaqualone, manifesting themselves mainly in grooming, hudding or sitting together: phenomena somewhat parallel to those described among humans taking the drug in group settings.

The sexual profile of rapists in Singapore.

To our knowledge, this is the first paper that examines the sexual profile of rapists in Singapore. A semi-structured interview based on a questionnaire about sexual habits was conducted on 62 convicted rapists and a control group of 63 prisoners comprising 32 convicted for violent non-sexual offences and 31 for non-violent non-sexual offences. Significantly more rapists masturbated at least once a month and were exposed to pornography within six months before the offence when compared to non-violent controls. Violent controls did not differ significantly from rapists in any aspect of the sexual history. The greater similarity between rapists and violent controls lends support to the concept of rape as a violent rather than sexual offence. A subgroup of 17 'hypersexual' rapists were identified who were either having sex or masturbating very frequently but had still resorted to rape. Compared to the other 46 rapists, the 'hypersexual' rapists were more likely to have fantasised predominantly about rape or bondage and to cite domination, aggression or hostility as reasons for committing rape, suggesting an even stronger element of power and aggression than in their less sexually active counterparts.

Circumcision in Australia: prevalence and effects on sexual health.

The results from a telephone survey in 2001-02 of a probability sample of Australian households including 10,173 men aged 16-59 (response rate 69.4%) are used to assess the prevalence of circumcision across social groups in Australia and examine lifetime history of sexually transmissible infection (STI), sexual difficulties in the last year, sexual practices including masturbation, and sexual attitudes. More than half (59%) of the men were circumcised. Circumcision was less common among younger men (32% aged <20) and more common among the Australian born (69%). After correction for age, circumcision was unrelated to reporting STI, but appeared to protect against penile candidiasis. Circumcision was unrelated to most sexual difficulties, but circumcised men were less likely to report physical pain during intercourse or trouble keeping an erection; reasons for this are unknown. There were no significant differences in practices at last sexual encounter with a female partner or in masturbation alone. Circumcised men had somewhat more liberal sexual attitudes. Neonatal circumcision was routine in Australia until the 1970s. It appears not to be associated with significant protective or harmful sexual health outcomes. This study provides no evidence about the effects on sexual sensitivity.

Lesbian sexuality.

A study was conducted to determine what physical sexual activities lesbians engage in and what attitudes lesbians have toward various aspects of their sexuality. The sample consisted of 286 homosexual women. This article reports results in the following areas: first orgasm, first sexual experience with another women, sexual fantasies, number of sex partners, faking orgasm, sex toys, initiating sex, techniques used to reach orgasm with partners, and the attitudes of respondents toward their own lesbianism, erotica, female genitals, masturbation, emotional involvement with sex partners, monogamy, sex toys, oral sex, tribadism, group sex, and sadomasochism. The study suggests that the sample group has very positive attitudes about their lesbianism, their own bodies, masturbation, and the various ways in which they express sexuality with their partners.

Genetic and environmental influences on the frequency of orgasm in women.

This study reports on genetic and environmental influences on the frequency of orgasm in women during sexual intercourse, during other sexual contact with a partner, and during masturbation. Participants were drawn from the Australian Twin Registry, and recruited from a large, partly longitudinal twin-family study. Three thousand and eighty women responded to the anonymous self-report questionnaire, including 667 complete monozygotic (MZ) pairs and 377 complete dizygotic (DZ) same-sex pairs, 366 women from complete DZ opposite-sex pairs, and 626 women whose co-twins did not participate. Significant twin correlations were found for both MZ and DZ twin pairs for all three items of interest. Age effects were statistically significant for some items. Models incorporating additive genetic, shared and nonshared environmental influences provided the best fit for Items 1 and 3, while a model with additive and nonadditive genetic influences along with nonshared environment fitted the data from Item 2. While an independent pathway model fits the data most par-simoniously, a common pathway model incorporating additive genetic (A), shared environment (C), and unique environment (E) effects cannot be ruled out. Overall, genetic influences account for approximately 31% of the variance of frequency of orgasm during sexual intercourse, 37% of the variance of frequency of orgasm during sexual contact other than during intercourse, and 51% of the variance of frequency of orgasm during masturbation. Following Baker (1996), we speculate that this additive genetic variance might arise from frequency-dependent selection for a variety of female sexual strategies.

Phenylalanine-restricted diet should be life long. A case report on long-term follow-up of an adolescent with untreated phenylketonuria.

The aim of this brief report is to stimulate a debate and to advocate life-long treatment with phenylalanine restricted diet to persons with phenylketonuria. We present a case of an adolescent male who today is a 65 year old with moderate mental retardation. He was previously observed as having severe challenging and self-injurious behaviour, sleeping disorder, hyperactivity, and masturbation. In the last year before reassessment he displayed loss of weight, muscle weakness resulting in wheelchair placement, upper respiratory infections, and fungal skin infections. After reassessment in 1998 the patient was started on treatment with a phenylalanine-restricted diet supplemented with vitamin B12, iron, and folic acid. At follow-up one year later the patient had regained his weight and was walking around freely. The challenging and self-injurious behaviour, and the masturbation, had ceased, and he was participating in social activities which he had previously not been able to participate in. At follow-up four years later he was still at this level of accomplishment. In general, this patient's quality of life had increased. We therefore recommend life-long treatment with phenylalanine restricted diet for persons with previously treated, and also for those with untreated, phenylketonuria.

Sexual activity during pregnancy and after childbirth: results from the Sexual Preferences Questionnaire.

The sexual relations of expectant and young parents have been researched relatively often, but most studies are restricted to female participants, samples from the USA, a short postpartum period and a reductionist view of sexuality as equivalent to intercourse. The aim of this study is to gain more knowledge about sexual activity of German couples with regard to non-genital tenderness, French kissing, breast stimulation, manual-genital stimulation, cunnilingus, fellatio, vaginal intercourse, anal intercourse and masturbation during pregnancy and the first 6 months postpartum. As part of a larger longitudinal study, 60 women and men (30 couples) answered a newly developed Sexual Preferences Questionnaire (SPQ) which assesses sexual activity and enjoyment. The frequency of most heterosexual activities declined during pregnancy, reached almost zero in the first 3 months postpartum, and then began to increase; male masturbation remained relatively constant. Agreement between the two members of each couple was assessed and SPQ data were validated with other questionnaire data and interview data. The results are discussed with regard to limitations of the sample. Future research should broaden the range of questions investigated, the range of research methods applied (questionnaires and interviews) and the range of persons researched (both partners).
