This paper presents the case of a 35-year-old man who consulted the department of venereology because of healing problems with some wounds caused by burning his skin perianally with cigarettes as part of a sexual satisfaction ritual. Knowledge of such lesions may be useful to physicians and social workers. Sadomasochism and 'offers' in the intimate-massage clinics in Copenhagen are surveyed.

The authors offered conjoint treatment of sexual dysfunction to an unselected population in a military hospital for one year. Of the 10 couples who requested treatment, 7 were treated by techniques outlined by Masters and Johnson. The couples who completed the treatment exercises improved, and neurotic interactions, presumably resulting from the symptoms, often disappeared. The couples who avoided the exercises had interactional patterns dominated by projection and blame, sadomasochism, and depressive features; typically their marital difficulties were aggravated by the treatment.
The author describes one specific facet of the psychological trauma attendant upon physical violence - the implications of the victim's forced silence about the event. This prohibition of communication, enforced by some implicit or explicit threat, constitutes a secondary trauma of enormous import and has been ignored in the literature. Exploration of this concept can sensitize therapists and point the way toward empathetic and effective intervention.

The authors collected therapists' ratings of 10 masochistic personality traits in 59 patients who were in psychotherapy. The traits were found to be common and to have good internal consistency. These data were presented to the Advisory Committee on Personality Disorders of the Work Group to Revise DSM-III to facilitate discussion of the possible inclusion of masochistic personality in the revised version of DSM-III.

One of the major controversies during the development of DSM-III-R was the possible inclusion of self-defeating personality disorder. The authors review the clinical literature that serves as the conceptual basis for this diagnosis. The development of the diagnostic criteria is described, including the rationale for specific criteria and the objections raised by opponents of the category. The authors describe how some of these objections led to changes in the diagnostic criteria in an attempt to distinguish self-defeating personality disorder from normal reactions to victimization and abuse.
