[The IDF evacuation from southern Lebanon in 2000: impact on the delivery of health care to displaced Lebanese population].

BACKGROUND: The State of Israel controlled a protective security zone in southern Lebanon from 1982 until May 2000, when the Israel Defense Forces (I.D.F.) evacuated this area within two days. Officers and soldiers of the South Lebanese Army, a local militia, which had formerly cooperated with the I.D.F., chose to come to Israel with their families. The provision for the health needs of this displaced population of over 5,861 persons was coordinated by the Northern Region Health Office of the Ministry of Health. AIM: To describe the deployment and provision of health care for this displaced population and to present the lessons learned. METHODS: Data was collected from interviews with the personnel most actively involved in the decisions and execution of the enterprise, as well as from relevant documents. RESULTS: The main health needs of the displaced South Lebanese were related to anxiety, outbreak of chickenpox, dental problems and provision of medical equipment for the disabled. CONCLUSION: The experience accrued by the Northern Region Health Office over the years in provision of care to displaced populations at short notice was implemented. Cooperation within the health sector and between the health services and the military and defense forces was invaluable and essential to deal successfully with this unusual situation.

The clinical picture of acute combat stress reaction among Israeli soldiers in the 1982 Lebanon War.

This study examined 100 randomly selected medical charts of Israeli psychiatric casualties of the 1982 Lebanon War in order to determine the clinical picture of combat stress reaction as recorded by clinicians on the battlefield. We found nine categories of symptoms. The most prevalent clinical picture (48% of cases) was polymorphic, followed by anxiety (13%) and labile states (11%). Only 41% of cases were mono-symptomatic and stable. Examination of the polymorphic and labile clinical pictures revealed that the most prominent symptomatology was anxiety and depression. In the labile states, the most widespread beginning symptom was anxiety and the most prevalent end state was found to be depression. These findings are discussed in light of reports from other wars.

Wandering intravascular missiles: report of five cases from the Lebanon war.

From 1980 to 1986, during the Lebanon war, five patients with missile embolization were seen at the American University of Beirut Medical Center. Three had entry in the heart or thoracic aorta with peripheral embolization, and two had entry in the internal carotid artery and inferior vena cava with embolization to the middle cerebral artery and heart, respectively. Embolization was suspected when, in the absence of an exit, routine x-ray films showed the missile in a distant location. Angiography and echocardiography confirmed the diagnosis. Peripheral arterial emboli were extracted while cerebral and venous emboli were kept, as they caused transient symptoms and remained silent.

Analysis of 285 cardiac penetrating injuries in the Lebanon war.

This study evaluates factors influencing survival in 285 battle cardiac injuries, treated in Lebanon from 1969 to 1982. Survival factors included age (mean 18 years), transportation lag (mean distance 2 miles), wounding agents (shrapnel, gunshot), hospital logistics, and early surgical treatment. The overall survival was 73% (208 patients) compared to 60% from World War II and 67% from recent civilian report. Treatment logistics included a specialized centre with ambulance radio communication. The state of shock at arrival influenced survival: 146 of 188 patients with mild shock survived (78%) with 61% (53 patients) survival in the profound shock group, treated similarly. The site of cardiac injury influenced outcome. Survival was best in the 9 patients with coronary vessel wounds (100%), atrial wounds 80% (56 survivors) but dropped to 46% (17 survivors) in left ventricular injury reflecting pump failure, and 51% (19 survivors) in multiple cardiac wounds. The causes of cardiac mortality and survivor follow-up were evaluated. The study indicates that despite predetermined factors overall survival is significantly improved by early transportation, precise logistics, and urgent surgery.

The molecular basis of beta-thalassemia in Lebanon: application to prenatal diagnosis.

A study of the molecular lesions of beta-thalassemia in Lebanon revealed the presence of eight different mutations in 25 patients with Cooley's anemia. The IVS1 position 110 mutation predominated with a frequency of 62% and was almost invariably associated with Mediterranean chromosome haplotype I. Five other mutations commonly found in the Mediterranean area occurred with frequencies of 2% to 8%. In addition a G----C substitution in IVS1 position 5 (a lesion previously found in Chinese and Asian Indians) was demonstrated in a patient with Mediterranean haplotype IX. A new mutation at codon 29 was found in two other patients with haplotype II. The characterization of these beta-thalassemia mutations should allow the implementation of a prenatal diagnosis program in that country.

Lessons relearned--when denial becomes impossible: therapeutic response to combat stress reaction during the Yom Kippur War (1973), the Lebanon War (1982) and the Intifada.

This article presents the attitudes toward CSR during and after the 1973 Yom Kippur War, the 1982 Lebanon War and the Intifada. It analyzes the transition from denial to recognition which evolved following the Yom Kippur War. Several explanations are suggested and discussed, including structural, political and psychoanalytic perspectives.
