[Massive subretinal fat and lipoid deposits (author's transl)].

Both in exsudative retinitis (Coats disease 4 cases [presumed angioma of the chorioid in 1 case]), and in the exsudative form of senile macular degeneration (1 case), massive subretinal fat and lipoid deposits were observed. These form a yellowish central disc approximately between the upper and lower lateral retinal vessels. The centre of the subretinal depositis, and therefore the highest point of retinal detachment (3 dioptres), appears white.

Characterization of the lipoidal derivatives of pregnenolone prepared by incubation of the steroid with adrenal mitochondria.

Using mass spectrometric, radioisotopic, chromatographic and chemical techniques, five fatty acid esters of 3 beta-hydroxy-5-pregnen-20-one (pregnenolone) have been identified as components of the lipoidal derivatives biosynthesized in vitro with bovine adrenal mitochondria. The five compounds are: pregnenolone arachidonate, pregnenolone linoleate, pregnenolone oleate, pregnenolone palmitate, and pregnenolone stearate. The distribution of the fatty acids among these five esters is different from the previously reported (Cmelik, S.H.W., and Ley, H. (1977) Comp. Biochem. Physiol. 56B, 267-270) fatty acid composition of these organelles.

Lipoid cell tumor of the ovary.

An androgen-secreting tumor of the left ovary was preoperatively localized in a 55-year-old virilized woman by radioimmunoassay of blood androgens. The blood sample was obtained by bilateral catheterization of the ovarian and adrenal veins using a percutaneous retrograde femoral approach. A total abdominal hysterectomy and bilateral salpingo-oophorectomy were performed, and revealed a lipoid cell tumor of the left ovary and right ovarian stromal hyperplasia. Despite early postoperative return of plasma adrogens to the normal range, virilizing signs persisted almost unchanged 1 year after surgery.

[Lipoid pulmonary granuloma in Parkinson's disease. Presentation of clinical case].

The authors describe one case of lipoid granuloma of the lung in a patient with PARKINSON'S disease. The cause of this pulmonary sequel is probably to be found in the alteration of the mechanism of deglutition, typical of PARKINSON'S disease, with aspiration bronchopneumonia and subsequent evolution to granuloma formation. In the case presented (a man of 64) the X-ray picture resembled the irregular mass opacities and bronchial amputations characteristically associated with cancer of the lung. As the actual anatomical extent of the process exceeded the limits indicated by radiological examination, the whole left lung was removed at surgery; histological studies, however, revealed the essentially benign nature of the lesion. The patient was discharged in due course of time as surgically cured. There was no major deterioration of his neurological status either at discharge time or at later rechecks.

[On the importance of new test of HDL-cholesterol in lipoid diagnostics with geriatric patients (author's transl)].

1. By introducing a HDL-cholesteroltest in the multiple screening we have the chance to valuate the diagnostical validity of the test value of triglycerides, total cholesterol and HDL-Cholesterol and of the calculated value of LDL-cholesterol and ratio HDL/LDL-cholesterol. 2. We studied the incidence of lipoid findings of 210 patients by graphical statistics. 3. The test values of triglycerides, total cholesterol and HDL-cholesterol manifest a mixed collection with a graph having different maximums whereby statistically a "normal" population can be differentiated from a "pathological" population. 4. The calculated value LDL-cholesterol is normal distributed, the ratio HDL/LDL-cholesterol is logarithmic normal distributed. The causes therefore are statistic effects.

Steroid and gonadotropin secretion in a patient with a 30-year history of virilization due to lipoid-cell ovarian tumor.

A 36-year-old woman with a 30-year history of undiagnosed virilizing lipoid-cell ovarian tumor is described. The tumor was localized by the NP-59 scan. Data of extensive investigations of steroid and gonadotropin secretion are provided. The preoperative dynamics of serum dehydroepiandrosterone sulfate and its decrease after the removal of the tumor suggested that dehydroepiandrosterone sulfate was being secreted by the ovarian tumor. Ovarian-peripheral venous gradients for plasma delta 5 and delta 4 steroids were correlated with the in vitro measurements of various steroidogenic enzymatic activities in the tumor tissue. The proposed metabolic pathway of the tumor is as follows: Pregnenolone----17-hydroxypregnenolone----dehydroepiandrosterone---- androstenedione----testosterone----estradiol. Low basal gonadotropin levels with no discernible pulsatility and no response to gonadotropin-releasing hormone were observed preoperatively, and a gradual normalization in both parameters was observed during the six months after removal of the tumor.

[Results obtained from staining lymphography, using carbon with lipoidal suspension, in cases of female genital carcinoma (author's transl)].

Reported in this paper is the use of a carbon-lipoidal suspension for presurgical staining lymphography which was applied to carcinomata colli uteri and corporis uteri. The suspension could not be prepared until permission had been obtained from the National Drug Expert Committee. Better colour contrast representation of the femoral and iliacal lymph nodes during the operation was thus achieved, and, consequently, surgical radicality was substantively improved. The approach proved less expensive and tedious than previous methods used for colour representation of lymph nodes in the minor pelvis. The dye used did not cause any complications. The advantages of the method are discussed. General application seems to be commendable, following further testing.

Intestinal absorpiton of ascorbic acid and its passage through the lipoid membrane of guinea pigs.

Diffusion of ascorbic acid takes place through intestinal lipoid membrane of guinea-pigs. In consequence, plasma and leucocyte concentrations of ascorbic acid increase. Intestinal absorpiton of ascorbic acid in scorbutic guinea-pigs is impaired in contrast to that in normal and saturated guinea-pigs as shown by the accumulation of ascorbic acid in the intestinal mucosal cells of guinea-pigs.

[A case of lipoid pneumonia showing a pulmonary nodule mimicking lung cancer].

A 49-year-old man was admitted to our hospital because of hemosputum, left chest pain and an abnormal shadow on chest X-ray. Computed tomography of the chest revealed a tumor-like shadow with a low density area in the left lower lung field. Ga scintigraphy revealed strong Ga uptake in the left lower lung. Fiberoptic bronchoscopy showed obstruction of subsegmental bronchi of it. B9+10. Neither transbronchial biopsy nor sputum cytology showed any malignant cells. Transcutaneous lung biopsy showed infiltration of foamy macrophages in alveolar spaces and lymphocytes and plasma cells in alveolar septa. Since there was a possibility of complicating lung cancer, and because of refractory hemosputum, the patient underwent left lower lobectomy. The cut surface of the resected left lobe showed the features of golden pneumonia with cavitation. The histological features of the resected specimen were those of lipoid pneumonia, and no cancer cells were detected.

[A case of exogenous lipoid pneumonia probably due to domestic insecticide].

A 61-year-old female was admitted to our hospital because of cough, hemosputum and right chest pain. The P-A and L-R chest films revealed focal consolidation in the right upper filed and mass-like shadow in the right S3. Transbronchial lung biopsy specimens showed collections of lipid-laden macrophages. The lipid material of macrophages in broncholaveolar lavage fluid was stained yellow-orange with Sudan IV. The patient had no difficulty in swallowing and had no history of regular use of oily drugs. Fiberoptic bronchoscopy and chest CT showed no obstruction of bronchi. She had used much domestic insecticide with the doors closed before going to bed every summer and autumn for 10 years. We concluded that the domestic insecticide containing mineral oil had resulted in exogenous lipoid pneumonia. Her symptoms and abnormal shadows in chest X-ray film disappeared soon after antimicrobial chemotherapy was started. She is now healthy without recurrence.

Permeation mechanisms through artificial lipoidal membranes and effects of synthetic surfactants on xenobiotic permeability.

Through the use of permeation/lipophilicity correlations, the mechanisms of permeation of selected test compounds across artificial lipoidal membranes of the polysiloxane type, in the absence and in the presence of a nonionic surfactant (Polysorbate 80), are investigated, in order to design "in vitro" conditions and features suitable for reproducing "in vivo" intestinal absorption tests, as well as to validate some conclusions arising from "in situ" rat gut experiments about the effects of the synthetic surfactants on drug and xenobiotic absorption processes. Six 4-alkylanilines showing a perfect homology were used as test compounds. The reported results clearly show that the in situ biophysical absorption (diffusion) models are completely reproduced by in vitro tests, provided that perfect sink conditions are achieved. Further selection of artificial membrane polarity should be necessary, however, in order to exactly equalize in vitro and in situ permeation rates. As far as the synthetic surfactant action on permeability is concerned, our conclusions are similar to those drawn from in situ studies, except that the effect of the surfactant on membrane polarity is much smaller and the micelle-solubilizing effect somewhat larger. The disruption of the aqueous stagnant diffusion layers adjacent to the membranes by the surfactant has been conclusively demonstrated. A clear first-element deviation for aniline, which prevents its inclusion as a term of the tested series, has been observed; this feature should be borne in mind whenever any in vivo/in vitro correlation has to be established.

Epithelial iron line in juvenile corneal arcus lipoides.

Iron lines of the corneal epithelium are well-described phenomena in both normal and pathologic conditions. We found a bilateral corneal epithelial iron line at the inner edge of a juvenile corneal arcus lipoides in an otherwise normal eye of a 38-year-old white man. Possible mechanisms that may have led to the development of this new iron line are discussed.

P450scc deficiency (congenital lipoid adrenal hyperplasia): first reported case in Thailand and literature review.

A male infant presented with hyponatremia, hyperkalemia, generalized skin hyperpigmentation, and female type external genitalia. These clinical findings were compatible with mineralocorticoid, glucocorticoid and androgen insufficiency. Serum cortisol, progesterone and testosterone levels were extremely low after ACTH stimulation test, suggestive of defect in all of the adrenal steroidogenesis. Computed tomography demonstrated enlarged adrenal glands. The diagnosis of P450scc deficiency or lipoid congenital adrenal hyperplasia was based on all these characteristics. Physiologic replacement therapy with hydrocortisone and 9 alpha-fluorocortisol were effective and the patient achieved normal growth. The clinical characteristics, differential diagnoses, and prenatal diagnosis are discussed and reviewed.

[An outbreak of lipoid liver degeneration (LLD) in Atlantic salmon (Salmo salar) in a fish farm and attempts to cure the disease (author's transl)].

A description of a field experiment on the therapeutic effect of 4 different drugs against lipoid liver degeneration is given in a natural outbreak of the disease in a fish farm. The diagnosis was based on pathological and histopathological changes in the liver and characterized on the basis of the degree of ceroid deposited in the liver cells. The size and weight of the fish had no influence on the deposition, but it was shown that LLD caused a higher mortality in Atlantic salmon than in rainbow trout, showing a weekly mortality of 2.5 and 0.3%, respectively. In the field experiments therapeutic effect was obtained with 1) Vitamin E, mixed into the feed in amounts of 90 mg alpha-tocopherol + 450 mg ethoxyquin pr. kg wet feed. 2) A combination of alpha-tocopherol and selene mixed into the diet in amounts of 20 mg tocopheryli acetas and 0.4 mg natrii seleniis pr. kg wet feed. The cause of the disease is not verified, but it seemes that rancid material in the diet was the main etiological factor. Predisposing factors influencing the peroxydation of the feed under practical conditions in Norwegian fish farms have been discussed.

[Lipoid pneumonia].

The authors describe in the article etiology, histopathological features, clinical picture and diagnostics of endogenous and exogenous lipoid pneumonia. The advantages of broncho-alveolar fluid examination and computed tomography in the diagnostics were emphasized.
