Use of a breathalyzer in a local bar: A community education project.

In completing a faculty-assigned service-learning project, a group of nursing students sought to educate their peers and the public about the effects of alcohol consumption. Breathalyzer readings were obtained from 150 bar patrons and correlated with the number of drinks consumed and number of hours spent drinking. The results were used to educate bar patrons about the difference between how intoxicated they felt and their actual blood alcohol levels. The students also distributed information about substance abuse assistance and safe transportation home. This screening technique served as an application of secondary prevention principles for the nursing students. The project was considered innovative in that no similar study was found in the literature. Selected Healthy People 2010 goals were also addressed in this project.

Occupational therapy entry-level program applicants: a survey of northwest schools.

Recruitment of occupational therapy students requires both expansion and broadening of ethnic, cultural, age, sex, and geographic distribution. Information used to guide recruitment activities can be limited, quickly outdated, and regionally nonspecific. Applicants to all entry-level programs in the Northwest were surveyed to assess trends that could influence recruitment practices. One hundred thirty-five (82.8%) of the 163 applicants surveyed responded to a questionnaire that probed for sources of exposure to occupational therapy, career goals, and educational preferences. Applicants indicated having an initial interest in allied health or education fields, yet only 36% began college with occupational therapy education in mind. Volunteer or work experience in specific practice settings was identified by approximately 80% of the applicants as an influence in seeking an education in occupational therapy. Career influences, goals, and educational program preferences did not differ among applicants based on age, residential background, grades, or previous degree. An absence of applicants of ethnic minority allows limited application of findings to these targeted groups.

Medical records in litigation: the Dalkon Shield story.

The Dalkon Shield was manufactured by A.H. Robins Inc. in the early and mid seventies, before it was withdrawn from sale because of the influx of lawsuits against the manufacturers. The case has become the largest tort case in history, with approximately 200,000 claimants worldwide and will not be wound up for years to come. Slater and Gordon is an Australian firm of solicitors with offices in three states. They have the largest Dalkon Shield practice in the world and represent almost 3,000 claimants. One of their most difficult tasks in preparing the cases is the gathering of medical evidence to substantiate claimants' assertions. This entails collecting relevant medical records from across the country and around the world going back almost twenty years for almost 2,000 women. The project has magnified the importance of accurate and complete documentation, kept intact and made accessible. The influence of medical record administrators is highlighted.

Referral rates to a regional adolescent psychiatric service: a comparison of visited and unvisited general practitioners.

A survey was undertaken through personal visits to general practitioners to determine the effectiveness of distributing information about a Regional Adolescent Psychiatric Service for emotionally and conduct disordered adolescents. The referral rates to the Unit over the four years before, and over the four years after the period of the visits were recorded both for those GPs who were visited and those who were not. The findings show an immediate significant increase in referral rates among the visited general practitioners, which was still evident after four years. This finding supports the hypothesis that a personal approach is more likely to be an effective means of disseminating information than posted printed materials. This paper also contains a description of other valuable, but unquantifiable results.

Complying with the Patient Self-Determination Act: legal, ethical, and practical challenges for hospitals.

On 1 December 1991, the Patient Self-Determination Act became law. This Act requires certain health care providers to distribute information to patients concerning advance directives (i.e., living wills and health care power of attorney). The information must include a statement of an individual's rights under the law of the state where the care is being provided. In fact, the provider must actually promulgate written policies and procedures regarding compliance with the Act. The entire law is only a few paragraphs of legislation, and yet its legal, ethical, and practical effect for the health care provider is broad. This article is designed to discuss why the Act was promulgated, what its provisions are, how to comply with it, and what can occur if a health care provider fails to comply.

[The realization of the concept of Harm Reduction in Russia].

By the present moment cases of HIV infection among injecting narcotic users (INU) have been registered in more than 100 countries. To prevent HIV infection, the program "Harm Reduction" has been developed in Britain; the program states that the prevention of HIV infection must be considered more important than the prevention of the use of narcotic drugs, as this infection is a growing danger for both drug users and public health in general. Breaking drug dependence must not be the only aim of services working with drug addicts, because this will exclude persons decisively disposed to the mode of life including the prolonged use of drugs from the sphere of their activity. Active INU having no contacts with organizations which must give them treatment and assistance find themselves in dangerous situations more often than INU maintaining contacts with such organizations. Penetration into such hidden group and its education must be the primary task. Propaganda plays a decisive role in this process, as the only way to penetrate into such group is to develop work in its territory, so that drug addicts, supplied with the necessary means could change their behavior in the desired direction. In the Russian Federation work on the project "Harm Reduction" has been carried out in 50 regions. This work has contributed to conducting teaching seminars, working out teaching programs, as well as to augmenting the interest among specialists of different professions to the problem of decreasing the spread of HIV infection. The importance of information distributed by the narcological service and the probability that very responsible persons take correct decisions on the basis of their understanding the situation have increased. The rating of public organization has risen.

[The present status of andrology in the Czech Republic].

Andrology is defined as a branch of science and medicine dealing with the male reproductive organs in animals and in men and with the diseases of these organs. Included in this definition are all related specialized branches of science (Definition of the International Society of Andrology). This definition is very wide for the perspectives of andrology it is not optimal. In Czech Republic the Andrological Section of the Czech Urological Society was founded in March 1998. This section aims to disseminate knowledge on andrology by coordination and organization of congresses, conferences and courses in andrology, to encourage the development of basic and clinical research in andrology, to facilitate the collaboration by distribution information and to encourage sponsoring of publication related to the field of andrology.

Internet resources for dentistry: computer, Internet, reference, and sites for enhancing personal productivity of the dental professional.

At the onset of the new millennium the Internet has become the new standard means of distributing information. In the last two to three years there has been an explosion of e-commerce with hundreds of new web sites being created every minute. For most corporate entities, a web site is as essential as the phone book listing used to be. Twenty years ago technologist directed how computer-based systems were utilized. Now it is the end users of personal computers that have gained expertise and drive the functionality of software applications. The computer, initially invented for mathematical functions, has transitioned from this role to an integrated communications device that provides the portal to the digital world. The Web needs to be used by healthcare professionals, not only for professional activities, but also for instant access to information and services "just when they need it." This will facilitate the longitudinal use of information as society continues to gain better information access skills. With the demand for current "just in time" information and the standards established by Internet protocols, reference sources of information may be maintained in dynamic fashion. News services have been available through the Internet for several years, but now reference materials such as online journals and digital textbooks have become available and have the potential to change the traditional publishing industry. The pace of change should make us consider Will Rogers' advice, "It isn't good enough to be moving in the right direction. If you are not moving fast enough, you can still get run over!" The intent of this article is to complement previous articles on Internet Resources published in this journal, by presenting information about web sites that present information on computer and Internet technologies, reference materials, news information, and information that lets us improve personal productivity.
