Determining provider choice for the treatment of mental disorder: the role of health and mental health status.

This article specifies and estimates a model of provider choice for mental health services. Three types of providers are identified: specialty mental health providers, general medical providers, and informal providers. Specific attention is paid to the role of health and mental health status in determining provider choice. The model is estimated using a multinomial logit approach applied to a sample of 2,800 respondents to the Baltimore Epidemiological Catchment Area Survey. The results are largely consistent with the previous work of Wells et al. (1982), suggesting that health and mental health status play an important role in the decision to seek care but have little effect on the type of provider chosen. The results also reveal that 22 percent of individuals obtaining mental health care did so through the informal care sector. One exemplary benefit design simulation is performed using the estimation results.

The domestic servant as family planning innovator: an Indian case study.

This paper reports on the relatively low fertility of female domestic servants in India. domestic servant have fewer numbers of ever-born and living children than women working in other occupations and women who are unemployed. This low fertility, which appears to be volitional, may have its roots in the incompatibility between the servant's reproductive and the productive roles, as well as in the changing values generated by continued exposure to a wealthier lifestyle. It is suggested that family planning messages aimed at and elicited by more "elite" classes may have a greater impact on fertility behavior than message from prompters with a socioeconomic background similar to the clients', which tend to concentrate on the harsher day-to-day realities of poverty.

[Urbanization in tropical countries].

Rural populations are still the most numerous in tropical countries. But we can witness an unprecedent process of urbanization. However the dynamics of the phenomenon differs greatly in different countries and in different towns. As a matter of fact, the greatest overcrowded metropolis attract the greatest part of the migrants from rural areas; the attractive factors are multifarious and universal. This not easily controllable trend seems to be irreversible. The migrant farmers will generally find a job within the informed economic sector. The zones of spontaneous and precarious settlement are often their first environmental living conditions. Some of these unhealthy dwelling areas are subject to development plans; some of them being extremely well designed.

Forecasting physician workforce requirements in Minnesota.

The existing oversupply, specialty imbalance, and geographic maldistribution of clinicians are inconsistent with the goals of health system reform in Minnesota. Health professional workforce projections can help the private and public sectors make informed decisions to assure that the future supply of health professionals reasonably approximates requirements. Appropriate methodologies for projecting national health professional requirements, based on existing HMO staffing patterns, should be applied to Minnesota. In the absence of workforce projections, a variety of assumptions about changes in Minnesota's health care system that will affect workforce supply and requirements can, nonetheless, be made. Limitations on the state's ability to influence the health professional workforce within its borders are noted.

Valuing the unseen emotional labour of nursing.

This paper argues that comforting, listening, being reassuring and showing sympathy are important nursing skills that are vital to a patients' well-being and, as such, should be both valued and remunerated. The author also argues that the invisibility of these skills has been compounded by the NHS and Community Care Act 1990. This has meant that long-term care is being transferred into the hands of the private and voluntary sectors and informal carers. As an alternative to residential care, many social services departments now use the home care services of not-for-profit care agencies who employ 'ordinary' people to enable those needing long-term care to remain in their own home. Such workers are often paid in a way that does not remunerate the emotional element of their labour.

Vibration exposure and prevention in Japan.

Working conditions of vibration exposure have generally improved, but many difficult problems must be solved such as (1) hygienic improvements in a variety of vibrating tools; (2) improving working conditions, for example, by limiting the time of operation in spite of economic difficulties such as those faced by those who work on a piece rate basis; (3) gathering more complete information about the risk population because of the large number of self-employed in informal employment sectors; and (4) finding work places after rehabilitation for patients, particularly in mountainous rural areas or in small scale industries. Historical observation of vibration and preventive measures in Japanese national forests was presented on the basis of the results of a retrospective cohort study in Kyushu, Japan. Prevalence rate of VWF remarkably changed from 58.4% in the groups that began to operate chain saws in 1960 to only a few cases in the groups who started the operation after 1971. When we compare the relationships between the results of long term cohort study and the consequences of preventive measures of vibration syndrome, the most important factor is the decrease of vibration exposure (improvement in chain saws plus the time restriction system). The comprehensive prevention system used in Japanese national forests consists of the following: (1) Health care system; (2) Work regulation system; (3) System for improving mechanized tools; (4) Warming system to protect against cold conditions; and (5) Education and training system.

Health seeking behaviour and self-treatment for common childhood symptoms in rural Guatemala.

This study was conducted in the Guatemalan highland department of Sacatepequez, in a sample of 146 rural women insured by the Social Security system. It examined their health care behaviour when their children presented common childhood symptoms such as diarrhoea, fever, cough and worms. The mothers generally sought help and treatment advice from an older woman in the family, and did so more often for diarrhoea (82%) and fever (64%) than for cough (43%) or worms (28%). Obtaining advice in a pharmacy or from a drug seller ranked second (range: 8%-38%, depending on the symptom), before the procurement of professional help at a medical service (range: 8%-23%). Traditional healers were hardly consulted (range: 0%-3%). In the case of self-treatment the women predominantly relied on Western drugs: around 80% in diarrhoea and fever, and above 50% in cough. Herbs and traditional external remedies were little used, except in cough (27% herbs) and worms (58% external remedies). None of the mothers reported ORS as home treatment for diarrhoea. Problems of geographical or financial accessibility could not explain the low utilisation of the Western health care system. The acceptability of public services, however, was poor. Largely because the Social Security clinic did not prescribe the "potent" modern drugs mothers preferred for the treatment of childhood symptoms--at least, not for uncomplicated illness episodes. Women hence turned to the--partially informal--private sector, which unabashedly responds to their demands. Clear away the discrepancy between the "rational" needs perceived by the official health sector and the demands of the population is one of the bigger challenges to health care planning in transitional communities such as the one studied.

[Unequal ventilation: phenomenon of trapped helium].

The results of preliminary research designed to prepare a method for using mouth measurements to identify alveolar air trapping phenomena which cannot be detected by normal techniques are reported. With a present rapid thermoconductivity CO2/He analyser, it was shown that even after the completion of a standard He wash-out process, it is possible to reveal the persistence in the expiratory air of bronchiolostenotic patients of residual traces of inert gases alongside CO2 originating from normally ventilating sectors. The information obtained from the mouths of patients corresponds with the results of measurements taken inside the lung. Finally it is shown that with adequate therapy the trapping phenomenon may be reversed.
