Distance learning in mammographic digital image processing.

The potential of interactive multimedia and Internet technologies is investigated with respect to the implementation of a distance learning system in medical imaging. The system is built according to a client-server architecture, based on the Internet infrastructure, composed of server nodes conceptually modelled as World Wide Web (WWW) sites. Sites are implemented by integration and customization of available components. The system evolves around network-delivered interactive multimedia courses and network-based tutoring, which constitute its main learning features. This potential has been demonstrated by means of an implemented system, validated with digital image processing content, specifically image enhancement. Image enhancement methods are theoretically described and applied on mammograms. Emphasis is given in the interactive presentation of the effects of algorithm parameters on images. The system end-user access depends on available bandwidth, so high speed access can be achieved via LAN or local ISDN connections.

The system is built according to a client-server architecture, based on the Internet infrastructure, composed of server nodes conceptually modelled as WWW sites. Sites are implemented by customization of available components. The environment integrates network-delivered interactive multimedia courses, network-based tutoring, SIG support, information databases of professional interest, as well as course and tutoring management. This capability has been demonstrated by means of an implemented system, validated with digital image processing content, specifically image enhancement. Image enhancement methods are theoretically described and applied to mammograms. Emphasis is given to the interactive presentation of the effects of algorithm parameters on images. The system end-user access depends on available bandwidth, so high-speed access can be achieved via LAN or local ISDN connections. Network based training offers new means of improved access and sharing of learning resources and expertise, as promising supplements in training.

Assessment of Kupffer cells by ferumoxides-enhanced MR imaging is beneficial for diagnosis of hepatocellular carcinoma: comparison of pathological diagnosis and perfusion patterns assessed by CT hepatic arteriography and CT arterioportography.

To investigate the clinical significance of the radiographic assessment of Kupffer cells and hemodynamics in the diagnosis of hepatocellular nodules, both magnetic resonance (MR) imaging enhanced by ferumoxides and CT hepatic arteriography (CTHA)/CT arterioportography (CTAP) were undertaken for 118 patients with 158 primary nodular hepatocellular lesions. The radiographic findings were analyzed in the context of the pathological diagnosis. Among nodules presumed to be pre- or early HCC by CTHA/CTAP, all 13 hyperintense nodules identified by MR imaging (MRI) were found pathologically to be hepatocellular carcinoma (HCC). In contrast, in 14 hypointense nodules, no advanced (moderately or poorly differentiated) HCC was pathologically identified and none of these progressed to advanced HCC during the follow up period (mean: 24 months). Instead, 78% of these cases were pathologically confirmed as dysplastic nodules. For the 16 lesions undetectable by CTHA/CTAP, four of eight (50%) hypointense nodules turned out to be dysplastic nodules and one hyperintense lesion was HCC. Signal intensity by ferumoxides-enhanced MRI showed a strong correlation with the increase or decrease of Kupffer cells assessed by immunohistochemistry. Assessment of Kupffer cells by ferumoxides-enhanced MRI is beneficial for the accurate diagnosis of primary hepatocellular nodules that are considered borderline or early stage HCC by their hemodynamic profile.

Prediction of regional functional recovery after acute myocardial infarction with low dose dobutamine stress cine MR imaging and contrast enhanced MR imaging.

PURPOSE: Previous studies demonstrated that low-dose dobutamine stress cine magnetic resonance imaging (MRI) and delayed contrast-enhanced MRI can provide assessments of myocardial viability. The purpose of this study was to evaluate the comparative diagnostic values of dobutamine cine MRI and delayed contrast-enhanced MRI for predicting functional recovery of myocardial contraction in patients with acute reperfused myocardial infarction. METHODS: Twenty-three patients with myocardial infarction after percutaneous coronary interventions were studied. All patients underwent steady-state cine MRI covering the entire left ventricle at rest and during low-dose dobutamine stress (10 micrograms/kg/min). Delayed contrast-enhanced MR images were acquired to determine transmural extent of hyperenhancement. Second cine MR images in the resting state were obtained 3 to 11 months after revascularization. RESULTS: On the first cine MR images in the resting state, 278 (20%) of 1380 segments demonstrated abnormal, regional contraction (systolic wall thickening < 40%). Of the 175 segments showing functional recovery on the following cine MRI, 156 (89%) segments were recognized as reversible by dobutamine cine MRI and 146 (83%) segments by delayed contrast-enhanced MRI. The sensitivity, specificity, and accuracy of dobutamine stress cine MRI was 89%, 80%, and 86%, respectively. These values of contrast-enhanced MRI were 83%, 72%, and 79%, respectively. The area under the receiver operating curve (ROC) was 0.87 by dobutamine cine MRI and 0.78 by delayed contrast-enhanced MRI (p < 0.05). CONCLUSIONS: The current results using quantitative segmental analysis indicated that low-dose dobutamine stress cine MRI can predict recovery of myocardial contractility with significantly higher diagnostic performance in comparison with contrast-enhanced MRI in patients with myocardial infarction who underwent revascularization.

Fluorescence lifetime imaging provides enhanced contrast when imaging the phase-sensitive dye di-4-ANEPPDHQ in model membranes and live cells.

We apply fluorescence lifetime imaging to the membrane phase-sensing dye di-4-ANEPPDHQ in model membranes and live cells. We show that the 1700 ps lifetime shift between liquid-disordered and liquid-ordered phases offers greater contrast than the 60 nm spectral shift previously reported. Detection of cholesterol-rich membrane microdomains is confirmed by observation of the temperature dependence of membrane order and by cholesterol depletion using methyl-beta-cyclodextrin.

Gadobenate dimeglumine (BOPTA) enhanced MR imaging: patterns of enhancement in normal liver and cirrhosis.

To determine whether gadobenate dimeglumine (BOPTA) will adequately enhance cirrhotic liver parenchyma, and to document the enhancement patterns in cirrhosis, 14 cirrhotic and 20 non-cirrhotic patients were evaluated before and 60-120 minutes after gadolinium-BOPTA. Proof of liver cirrhosis was biopsy (6), surgical resection (3), and clinical follow-up (5). Enhancement effects were compared quantitatively by determining the liver signal-to-noise ratio (SNR) and signal enhancement in both populations. Qualitatively assessment of the liver enhancement was performed and classified as homogeneous or heterogeneous. Quantitative analysis: cirrhotic liver parenchyma presented a higher increase in SNR values, relative to non-cirrhotic liver parenchyma, on postcontrast images. Likewise the signal enhancement of cirrhotic liver parenchyma was superior to non-cirrhotic liver on T1-weighted SE images (P = .02) and in-phase GRE images (P < .001). There was no statistical difference on out-of-phase GRE images. Qualitative analysis: on T1-weighted SE postcontrast images, cirrhotic liver parenchyma showed a homogeneous enhancement in 7 patients and heterogeneous in 7. Whereas on GRE images, cirrhotic parenchyma showed heterogeneous enhancement in 9 patients and homogeneous in 5 patients. The heterogeneous enhancement was due to the presence of hypointense nodules in 7 patients and hyperintense nodules in 2 patients. In conclusion, our study has shown that the hepatobiliary contrast agent Gd-BOPTA is effective in the cirrhotic liver, demonstrating an increased liver enhancement compared with non-cirrhotic patients.

[Screening for tracheobronchial diseases with digital storage phosphor radiography].

Digital storage phosphor radiography (FCR: Fuji computed radiography) has a wide dynamic range and unique postprocessing capabilities. This study was designed to test whether chest imaging with FCR and its image processing would increase the accuracy of and confidence in the diagnosis of tracheobronchial abnormalities. In a phantom study, the performance of digital images having the appearance of a conventional chest radiograph was compared with that of a conventional system in detecting simulated tracheobronchial nodules. The digital images of lower kilovoltage (ROC area = 0.647 +/- 0.035) were equivalent to the conventional radiographs (ROC area = 0.620 +/- 0.028). On the other hand, nodule detectability was significantly improved in the digital images of higher kilovoltage (ROC area = 0.826 +/- 0.020). The author also compared the impact of five postprocessing algorithms (standard image, wide latitude image, enhanced image, reversed image, and subtraction image). ROC analysis indicated that the default standard image (ROC area = 0.826 +/- 0.020) was as good as an image with a linear rather than a sigmoid gradation curve (ROC area = 0.843 +/- 0.020), an image with strong enhancement of high frequencies (ROC area = 0.804 +/- 0.020), and an image with reversed gray scale polarity (ROC area = 0.775 +/- 0.015). Therefore these specific algorithms had no effect on the detection of tracheobronchial nodules. However, digital subtraction soft tissue images (ROC area = 0.961 +/- 0.030) were significantly better than the control images. Clinical study also indicated that subtraction images improve diagnostic accuracy in tracheobronchial diseases.

Wilms tumor and nephroblastomatosis: imaging characteristics at gadolinium-enhanced MR imaging.

Twelve children with Wilms tumors were prospectively evaluated with 1.5-T magnetic resonance (MR) imaging. Spin-echo images were obtained before and after administration of gadopentetate dimeglumine. Nine of the 12 children had histologically proved nephroblastomatosis or nephrogenic rests involving a total of 14 kidneys. MR imaging depicted involvement in eight of these kidneys, including five of five kidneys with lesions contralateral to the primary Wilms tumor. Overall sensitivity of detection of nephrogenic rests was 43% on nonenhanced images and 58% when gadolinium-enhanced images were added. Nephrogenic rests admixed with Wilms tumors less than 4 mm in diameter were not depicted at MR imaging. On gadolinium-enhanced T1-weighted images, Wilms tumors and hyperplastic were hypointense to normal renal tissue. On T2-weighted images, Wilms tumors and hyperplastic nephrogenic rests were iso- or slightly hyperintense to renal cortex, while sclerotic nephrogenic rests were hypointense to renal cortex. On all images, including gadolinium-enhanced T1-weighted images, the signal intensity of Wilms tumors was inhomogeneous, while the signal intensity of nephrogenic rests was homogeneous.

Quantification of tissue plasma volume in the rat by contrast-enhanced magnetic resonance imaging.

Magnetic resonance imaging enhanced with a macromolecular contrast medium (MMCM), albumin-Gd-DTPA, was used to estimate the plasma volume in vivo in the myocardium, lung, liver, and skeletal muscle of 10 normal rats. The plasma volumes of the same tissues in a parallel group of six rats were estimated in vitro by a conventional radioisotopic technique (111In-transferrin). Plasma volumes of myocardium, lung, liver, and skeletal muscle estimated by the MR technique (microliter plasma cc-1 of tissue) were 101, 109, 163, and 11.0, respectively, while plasma volumes measured by the 111In-transferrin radioisotope technique (mg plasma g-1 of tissue) were 78.6, 215, 143, and 11.2, respectively. Assuming a ratio of densities of aerated lung to blood of 0.45 and of other tissues to blood of 1.0, correlation between the methods was excellent (R2 = 0.99) indicating that MR imaging enhanced with MMCM permits reliable in vivo estimation of tissue plasma volume in the rat.

Hepatic lesion detection and characterization: value of nonenhanced MR imaging, superparamagnetic iron oxide-enhanced MR imaging, and spiral CT-ROC analysis.

PURPOSE: To determine the accuracy for detection and characterization of focal hepatic lesions of nonenhanced, superparamagnetic iron oxide (SPIO)-enhanced, or a combination of nonenhanced and SPIO-enhanced MR imaging and contrast-enhanced spiral computed tomography (CT). MATERIALS AND METHODS: Spiral CT and T2-weighted SPIO-enhanced (ferucarbotran-enhanced) MR imaging were performed in 35 patients within 2 weeks before surgery for malignant hepatic lesions. Only malignant lesions with histopathologic proof were considered. A total of 875 images with and 800 images without focal lesions were presented to five readers, who were asked to assess the presence and characterization of lesions by using a five-point confidence scale. Receiver operating characteristic analysis was performed. RESULTS: Nonenhanced and SPIO-enhanced images together and SPIO-enhanced images alone yielded the best performance for lesion detection. No differences were found among all imaging techniques with regard to lesion characterization (benign vs malignant). The combined approach resulted in larger area under the ROC curve (A(z) = 0.9062) and accuracy (85.3%) (P < 0.02), as compared with SPIO-enhanced MR imaging (A(z) = 0.8667; accuracy, 73.1%). CONCLUSION: SPIO-enhanced T2-weighted MR imaging was more accurate than nonenhanced T1-weighted and T2-weighted MR imaging and contrast-enhanced spiral CT for the detection of focal hepatic lesions. The combined analysis of nonenhanced and SPIO-enhanced images was more accurate in the characterization of focal hepatic lesions than was review of SPIO-enhanced images alone.

Diagnosis and staging of pancreatic cancer: comparison of mangafodipir trisodium-enhanced MR imaging and contrast-enhanced helical hydro-CT.

OBJECTIVE: The purpose of this study was to compare mangafodipir trisodium-enhanced MR imaging performed with a phased array coil and contrast-enhanced single-detector helical CT for accuracy in the detection and local staging of pancreatic adenocarcinoma and in the differentiation between cancer and focal pancreatitis. SUBJECTS AND METHODS: Forty-two patients with suspected pancreatic masses underwent contrast-enhanced helical CT and mangafodipir trisodium-enhanced MR imaging at 1.5 T. The images were assessed for the presence or absence of tumors; characterization of masses; and presence of vascular invasion, lymph node metastases, or liver metastases. Imaging findings were correlated with findings at laparotomy, laparoscopy, biopsy, or follow-up. RESULTS: Focal masses were present in 36 patients (cancer, n = 26; focal pancreatitis, n = 7; other, n = 3). The sensitivity for lesion detection of MR imaging was 100% and of CT, 94%. Two small malignant lesions were missed on CT. For the diagnosis of tumor nonresectability, the sensitivity of MR imaging and CT was 90% and 80%, respectively. Liver metastases were missed on MR imaging in one of the eight patients and on CT in four. For differentiation between adenocarcinoma and nonadenocarcinoma, the sensitivity of MR imaging was 100% (positive predictive value, 90%; negative predictive value, 100%), and the sensitivity of CT was 92% (positive predictive value, 80%; negative predictive value, 67%). Receiver operating characteristic analysis revealed that the mean area under the curve for MR imaging was 0.920 and for CT, 0.832 (not significant). CONCLUSION: Mangafodipir trisodium-enhanced MR imaging is as accurate as contrast-enhanced helical CT for the detection and staging of pancreatic cancer but offers improved detection of small pancreatic metastases and of liver metastases compared with CT.

Brain metastases--comparison of gadodiamide injection-enhanced MR imaging at standard and high dose, contrast-enhanced CT and non-contrast-enhanced MR imaging.

The aim was to compare the abilities of contrast-enhanced CT, non-contrast-enhanced MR imaging and contrast-enhanced MR imaging using standard (0.1 mmol/kg b.w.) and high (0.3 mmol/kg b.w.) doses of Gadodiamide injection to detect brain metastases (i.e. blood-brain barrier damage). Sixteen patients with at least 2 metastases found by CT were evaluated by MR imaging using non-contrast-enhanced spin-echo, T1-weighted, T2-weighted sequences, and contrast-enhanced spin-echo T1-weighted sequences at 2 dose levels. Gadodiamide injection was first given at the dose of 0.1 mmol/kg b.w. After imaging, another 0.2 mmol/kg b.w. was given, yielding a cumulative dose of 0.3 mmol/kg b.w. No contrast media-related adverse events were recorded. The images were evaluated openly by one and blindly by 2 investigators and the number of metastases, size, delineation (open study) and diagnostic certainty (blind study) of each individual metastasis noted. High-dose MR imaging showed significantly more and smaller metastases than any other examination, and gave a higher diagnostic certainty. All high-dose images were superior to those with the standard dose MR imaging when compared blindly in pairs. We conclude that spin-echo MR imaging with a high dose of Gadodiamide injection is an efficient way to improve the detection of brain metastases, in particular of small ones.

Hyperintense uterine leiomyoma at T2-weighted MR imaging: differentiation with dynamic enhanced MR imaging and clinical implications.

PURPOSE: To evaluate the role of dynamic enhanced magnetic resonance (MR) imaging in differentiating leiomyomas with high signal intensity at T2-weighted imaging. MATERIALS AND METHODS: Histologic features of 34 uterine leiomyomas that showed hyperintensity on T2-weighted images were compared with those of ordinary hypointense leiomyomas (n = 74). The results of treatment with gonadotropin-releasing hormone analogue were also analyzed in relation to pretreatment signal intensity. RESULTS: Degenerated leiomyomas (n = 13) had higher signal intensity and heterogeneous architecture, but there was considerable overlap in signal intensity with cellular leiomyomas (n = 21). At dynamic enhanced MR imaging, cellular leiomyomas had marked contrast enhancement in the early phase, while degenerated leiomyomas showed slight or irregular enhancement. Cellular leiomyomas were reduced significantly in size after treatment (P = .02). The response in degenerated leiomyomas was minimal. CONCLUSION: Evaluation of signal intensity at T2-weighted imaging and degree of contrast enhancement at dynamic enhanced MR imaging provide useful information on the nature of leiomyomas.

[Assessment of image display of contrast enhanced T1W images with fat suppression].

UNLABELLED: The effects of imaging conditions and measures for their improvement were examined with regard to recognition of the effects of contrast on images when T1-weighted imaging with selective fat suppression was applied. METHOD: Luminance at the target region was examined before and after contrast imaging using phantoms assuming pre- and post-imaging conditions. A clinical examination was performed on tumors revealed by breast examination, including those surrounded by mammary gland and by fat tissue. RESULTS: When fat suppression was used and imaging contrast was enhanced, the luminance level of fat tumors with the same structure as the prepared phantoms appeared to be high both before and after contrast imaging, and the effects of contrast were not distinguishable. This observation is attributable to the fact that the imaging conditions before and after contrast imaging were substantially different. To make a comparison between pre- and post-contrast images, it is considered necessary to perform imaging with fixed receiver gain and to apply the same imaging method for pre- and post-contrast images by adjusting post-contrast imaging conditions to those of pre-contrast imaging.

Bladder tumor staging: comparison of contrast-enhanced CT, T1- and T2-weighted MR imaging, dynamic gadolinium-enhanced imaging, and late gadolinium-enhanced imaging.

PURPOSE: To evaluate the accuracy of contrast material-enhanced computed tomography (CT), unenhanced T1- and T2-weighted magnetic resonance (MR) imaging, and dynamic gadolinium-enhanced spoiled gradient-echo MR imaging in the staging of bladder cancer. MATERIALS AND METHODS: Thirty-six patients with histologically proved bladder cancer were prospectively examined with CT and MR imaging before tumor resection. Images were evaluated in a blinded fashion. RESULTS: All methods depicted tumors comparably well. Staging was correct in 16 of 29 patients (55%) with CT and 27 of 36 patients (75%) overall with MR imaging. Overstaging was the most common error, and both CT and MR imaging were more accurate for higher-staged tumors. CONCLUSION: CT and MR imaging perform well in the detection of tumors but are less accurate in staging. Dynamic gadolinium-enhanced MR imaging is better than other techniques, but the difference is not statistically significant.

Evaluation of focal pancreatic masses: comparison of mangafodipir-enhanced MR imaging and contrast-enhanced helical CT.

The detection and characterization of pancreatic tumors as well as the reliable staging of pancreatic cancer are important tasks for radiologic evaluation. Contrast-enhanced helical CT has been the standard modality for pancreatic imaging in many institutions, but MR imaging has gained a considerable role in the evaluation of patients with equivocal CT findings. Recently, the first organ-specific MR contrast agent targeted to the liver and pancreas, mangafodipir trisodium, has been registered in the European Union (EU) for use in MR imaging of the pancreas. This paper reviews technical considerations and characteristic imaging findings of mangafodipir-enhanced MR imaging in the assessment of focal pancreatic lesions. Contrast-enhanced MRI has proven to be very helpful in the detection of small tumors or the identification of tumor-simulating lesions in patients with equivocal CT findings. Mangafodopir may improve the staging of pancreatic cancer by increasing the sensitivity of MRI in the detection of liver metastases. This review summarizes the potential of contrast-enhanced MRI and the limitations compared with contrast-enhanced helical CT.
