[Established caustic stenosis of the esophagus in children. Closed-thorax esophagectomy with in situ colonic esophagoplasty].

Replacement of the oesophagus with a colonic graft brought up to a retrosternal or intrathoracic position is usually performed in children with undilatable oesophageal stenosis resulting from caustic burns. However, incidents associated with the oesophagus remaining in situ regularly raise the problem of concomitant or delayed oesophagectomy and therefore thoracotomy. A one-stage surgical technique is described, which includes closed-thorax oesophagectomy and colonic oesophagoplasty in situ in the oesophageal bed. The 3 patients operated upon by this method made an uneventful recovery.

[Caustic diaper dermatitis and encephalitis secondary to the application of talcum contaminated with hexachlorophene].

Five observations of infants with transcutaneous intoxication by 6,3 p. 100 hexachlorophene contaminated talcum powder are reported. The diaper dermatitis is particular because of its topography (red pants shape), of its sudden occurring, of its papyraceous aspect evoking caustic origin, and of its association with severe encephalopathy. The neurological signs start with epileptic fits leading rapidly to coma. Prognosis is serious leading either to death or to paraplegia. Enquiry on that epidemic shows that mortality raised up to 18 p. 100. The neurological signs with oedematous degeneration of myelin are characteristic of hexachlorophene toxicity. Plasma levels of toxics range up to 15,94 mg/ml i.e. 30 times more than the rates observed by Curley in a premature washed with a commercial solution containing 3 p. 100 hexachlorophene. During a toxic neurological syndrome, the existence of diaper dermatitis with red plants shape must lead to an aetiological diagnosis of the possibility of transcutaneous intoxication even if the product seems as harmless as talcum powder.

[Anterothoracic colonic replacement in cicatricial caustic strictures of the esophagus in a 1-stage procedure].

Five cases of oesophageal stricture secondary to caustic ingestion are presented. The colon, as a total bypass graft, was applied in all patients; two with right hemicolon and three with isoperistaltic transverse colon. The colon was placed subcutaneously, with the operation being performed as a one-stage procedure. A good functional result of the colon bypass, could be achieved in all persons.

Caustic cicatrization of the pharynx associated with dysphagia and premalignant mucosal changes.

Although esophageal strictures with attendant malignant degeneration are well known sequelae of caustic ingestion, fixed pharyngeal structural lesions and development of oral leukoplakia are not well appreciated consequences of such ingestion. We present a patient 12 yr after lye ingestion who displayed rapidly progressive dysphagia suggestive of esophageal carcinoma. Instead, adhesions bisecting the cricopharyngeal inlet and an upper esophageal stricture were found endoscopically. In addition, an area of hypopharyngeal leukoplakia with marked acanthosis, parakeratosis, and dysplasia was noted and resected. The management of this patient's dysphagia and subsequent follow-up for his potentially precancerous oral lesion form the basis of this report.

[Caustic digestive stenosis involving the hypopharynx. Esophagopharyngoplasty using a right ileocolic graft].

The authors describe an oesophago-laryngoplastic technique using a retrosternal ileocolic graft and involving anterior pharyngotomy above the hyoid bone. The technique was applied to five cases of major destructive lesions of the hypopharynx with fibrotic stenosis, due to severe burns of the upper digestive tract by ingestion of caustic substances. Continuity between the oral cavity and the digestive tract was re-established in all cases.

[Esophagopharyngoplasty by transplantation of a right ileocolic graft in the treatment of sequelae of esophageal and hypopharyngeal caustic stenosis].

Esophagopharyngoplasty was employed to treat 7 cases of severe stenosis from scarring due to caustic burns. Emphasis is placed on the advantages of retrosternally applied right ileocolic transplants using the supraphyoidal anterior pharyngeal approach. Results were considered as satisfactory in 4 cases, exclusively oral alimentation being possible, though bougierage, sometimes repeated, was necessary. The 3 unsuccessful cases were due to loss of oropharyngeal muscle peristaltism in one case, progression of scar retraction in the second patient, and the psychological behavior and lack of essential active cooperation in the third case.

[Preventive treatment in caustic stenosis of the oesophagus (author's transl)].

The authors review the varioux forms of treatment used experimentally and clinically in an attempt to prevent oesophageal stenosis following ingestion of caustic substances. Corticosteroid therapy is still used by certain authors though its effectiveness is far from obvious. However, promising results have been obtained experimentally using substances such as amino-nitriles, which alter the properties of newly formed collagen.

[Assessment of a thirty year long experience in treatment of esophageal caustic stenosis (author's transl)].

Among 457 esophageal stenosis treated between 1954 and 1977, 258 (56%) were caustic stenosis. The treatment of those is the most difficult. Functionnal healding was satisfactory in 93% of the treated cases by dilatations. Four strict rules ought to be observed in the management of this cases. Immediate and long terme antibiotic treatment of the corrosive esophagitis. X rays are the only means to be used during evolution of corrosive esophagitis. No instrumental performance until sufficient cicatrization. Retrograde dilatations after gastrostomy shall be prefered in serious cases. Gastro-esophageal reflux may complicate this evolution and require surgery. Total esophageal replacement can be averted in most cases.

Caustic soda burns to the extremities: difficulties in management.

Alkali burns to the extremities are unusual, but they are also a cause of significant morbidity, often because of their late presentation. Their management centres around water lavage, burn debridement and split-skin grafting; there is controversy over the timing of surgery. Three unusual cases of caustic soda burn in adults, and the difficulties encountered in their management, are described, and the literature is reviewed.

The effect of corticosteroids and pentoxifylline in caustic esophageal burns. A prospective trial in rats.

BACKGROUND: Caustic Esophageal Burns (CEB) usually results in scatritial tissue and stricture formation. Management requires preventing the massive inflammatory process that ensues in its early phase and decreasing bacterial complications. METHODS: An animal model was created to investigate the effect of corticosteroids and pentoxifylline in CEB using 52 male Wistar rats. The injury was produced using an indwelling esophageal catheter through which 3N of 12% sodium hydroxide was infused. The rats were grouped as control, CEB, CEB and ceftazidime (CEB-C, 100 mg/kg/day im. bid. 10 days), CEB and ceftazidime plus dexamethasone (CEB-CD, 0.1 mg/kg/day im. bid. 4 weeks) and CEB and ceftazidime plus pentoxifylline (CEB-CP, 50 mg/kg/day im. tid. 4 weeks). The groups were evaluated making use of esophagograms, hydroxyproline (OH-P) contents and histologic examination of the specimens 28 days after injury. RESULTS: No significant statistical differences were observed among the dexamethasone (CEB-CD), pentoxifylline (CEB-CP), antibiotics (CEB-C) and the untreated CEB groups.
