[Bradykinin levels in BAL fluid in patients with sarcoidosis].

In 14 patients with sarcoidosis and in 9 healthy controls the bradykinin level was estimated in broncho-alveolar lavage fluid. The statistically significantly higher bradykinin concentrations was found in sarcoid patients than in controls. No relationship was found between level of bradykinin in BAL and lymphocytes % and no relationship was found between bradykinin level and pulmonary function parameters.

Alveolar cells in cyclophosphamide-induced lung injury. The effect of pentoxifylline on BAL-isolated cells.

The present study provides the evaluation of pentoxifylline (PTXF) effect on quantitative and morphological changes of cells isolated from the lungs by BAL method in animals given cyclophosphamide (CP). CP was administered once, intraperitoneally, on the 7th day of the experiment, in a dose of 150 mg/kg b.w. PTXF was administered in drinking water in a dose of 30 mg/kg b.w./24 h for 14 days of the experiment. Morphological examinations of the isolated cells were performed using the scanning electron microscope (SEM). An attempt to identify type II alveolar epithelial cells was also undertaken. A single CP dose caused a significant rise in the total number of cells isolated from the lungs of the animals 7 days following CP administration. At the same time, a fall was noted in the percentage of neutrophilic granulocytes. No significant quantitative changes were observed in the population of cells isolated after 1, 3 and 28 days following CP administration. PTXF, in doses applied, did not significantly affect quantitative changes and morphological picture of the isolated cells. Our observations suggest that none of the methods applied for identification of type II pneumocytes are completely specific. At the same time they stain type II cells and subpopulations of alveolar macrophages heavily loaded with phagocytized surfactant. However, those methods are simple and enable an approximate evaluation of damage and regeneration processes in the extracellular alveolar lining layer and epithelium.

Sensitivity and specificity of indirect immunofluorescence and Grocott-technique in comparison with immunocytology (alkaline phosphatase anti alkaline phosphatase = APAAP) for the diagnosis of Pneumocystis carinii in broncho-alveolar lavage (BAL).

The purpose of the study was to compare the sensitivity and specificity of the indirect method of immunofluorescence with the immunocytological technique of alkaline phosphatase anti alkaline phosphatase complex (APAAP) for the detection of Pneumocystis carinii by bronchoalveolar lavage (BAL) in HIV-1 positive patients. - 83 HIV-1 positive patients with clinical presentations suggestive of Pneumocystis carinii pneumonia (PcP) were included in the study. 28 samples were found Pc-positive by immunofluorescence (IFT), 26 by Grocott and 29 by APAAP. In comparison to the lab results 33 patients were diagnosed as PcP according to the clinical course (i.e. therapeutic outcome, drugs used, and therapy changes). Compared to the clinical diagnoses, the following lab tests proved to be false positive and false negative: false positive: IF = 1, Grocott = 0, APAAP = 4 (3F6). false negative: IF = 5, Grocott = 7, APAAP = 4 (3F6). - Grocott stain shows insufficient correlation to the clinical diagnoses (p = 0.0156, McNemar-Test, two-tailed). - The two different detection methods (IFT and APAAP) showed no significant statistical difference with regard to their sensitivity (p = 0.3438, McNemar-Test, two tailed) and specificity. Considering cost and time the immunofluorescence technique seems to be the most suitable for the diagnosis of PcP in HIV-1 positive patients.
