Comparison of surface finish of new aesthetic restorative materials.

This study compared the surface finish of eight different types of aesthetic restorative materials. The materials included resin-modified (Fuji II LC [FL], GC) and highly viscous (Fuji IX GP Fast [FN], GC) glass ionomer cements, a compomer (F2000 [FT], 3M-ESPE), minifilled (Z100 [ZO], 3M-ESPE) and microfilled (A110 [AO], 3M-ESPE) composites and materials based on recently introduced ormocer (Admira [AM], Voco), nanomer (Filtek Supreme Translucent [FST], 3M-ESPE) and nanocluster technology (Filtek Supreme [FS], 3M-ESPE). Sixteen specimens (3-mm long x 3-mm wide x 2-mm deep) of each material were divided into two equal groups. Specimens in Group 1 received no further treatment after polymerization against a matrix strip, while the specimens in Group 2 were roughened with 320 grit grinding paper using a lapping device and were finished/polished with a graded abrasive disk system (Super-Snap, Shofu). The mean roughness (Ra, microm) of materials was determined using a surface profilometry. Data was analyzed by ANOVA/Scheffe's test at significance level 0.05. Mean Ra values ranged from 0.04 to 0.16 microm for Group 1 specimens and 0.15 to 0.68 microm for Group 2 specimens. Results of statistical analysis were as follows: Group 1-FS, FST, FL, FN, AM > FT, AO, ZO; Group 2-FN, FT, FL > AO, FS, ZO, AM, FST (> indicates significantly greater Ra values). For the finished/polished composite materials, Ra values observed for AM and FST were significantly lower than for AO and FS. The surface finish of glass ionomers and compomer was significantly poorer than composites. Composite materials based on ormocer and nanomer technology were significantly smoother than those based on microfillers and nanoclusters.

Finishing/polishing of composite and compomer restoratives: effectiveness of one-step systems.

This study investigated the surface texture of composite (Z100, 3M ESPE) and compomer (F2000, 3M ESPE) restoratives after treatment with different one-step finishing/polishing systems (One-Gloss [OG], Shofu; PoGo [PG], Dentsply; Sof-Lex Brush [SB], 3M ESPE). The surface roughness obtained was compared to that using a matrix strip [MS], a two-step rubber abrasive (CompoSite [CS], Shofu) and a graded abrasive disk (Super Snap [SS], Shofu) system. Eight specimens (3-mm long x 3-mm wide x 2-mm deep) of each material were made according to manufacturer's instructions. With exception of the MS group, all groups were roughened with 320 grit grinding paper using a lapping device prior to finishing/polishing with the different systems. The mean surface roughness (microm) was measured with a profilometer. Data was subjected to ANOVA/Scheffe's tests and independent samples t-test at significance level 0.05. Mean Ra ranged from 0.22 to 0.32 microm for Z100 and 0.45 to 0.68 for F2000. For both materials, the smoothest surfaces were obtained with MS. The roughest surfaces were observed after treatment with SS and OG for Z100 and F2000, respectively. The effectiveness of the finishing/polishing systems was material dependent. The surface finish produced by PG and SB was superior or comparable to that obtained with CS, SS and OG.

Polish retention of new aesthetic restorative materials over time.

New aesthetic restorative materials claiming superior polish retention have recently been introduced. This study compared polish retention of eight different types of aesthetic restorative materials including ormocer and nanofill composites over a 6-month ageing period in distilled water. Eight specimens of each material were fabricated and roughened with 320 grit grinding paper using a lapping device and finished/polished with a graded abrasive disk system. Mean roughness was then determined using a surface profilometer. The specimens were then stored in distilled water at 37 degrees C and roughness measurements were repeated after 3 and 6 months of ageing. Mean surface roughness values ranged from 0.15 to 0.68 microm for immediate readings and 0.17 to 1.02 microm after ageing for 6 months. With the exception of the highly viscous glass ionomer cement, the surface polish of most materials did not deteriorate much over time. At all time intervals, composite materials based on ormocer and nanomer technology were significantly smoother than all other materials evaluated.

Urine microalbumin/creatinine ratios in Singapore children.

The random urine albumin:creatinine (Ua:Uc) ratio was measured in 139 healthy children from newborn to age 4 years using the immunonephelometric method. This was shown to vary with age, the mean for neonates being 5.24mg/mmol (2SD range: 0.54-14.95mg/mmol) decreasing to 1.34mg/mmol (2SD range: 0.55-3.29mg/mmol) for the 2-4 years age range. The values for newborn to 6 months was significantly higher than the older age group (p less than 0.000001). 14 children with structural renal disease were then studied to determine the value of the random Ua:Uc ratio in detecting glomerular injury. This was found to be abnormal in children with glomerular filtration rate (GFR) less than 80ml/min/1.73m2 as well as in some children with complex bilateral renal structural abnormalities. Further longitudinal studies are required to determine the usefulness of this ratio in predicting progressive glomerular injury in this group of patients.

High-dose methotrexate for advanced breast cancer.

High-dose infusions of methotrexate with citrovorum factor rescue were evaluated in 27 patients with advanced recurrent breast cancer who had previously been treated with various Adriamycin-containing regimens. Eight of 27 patients (29%) achieved objective tumor regression with a median duration of response of 26 weeks. Nineteen patients had previously received standard doses of methotrexate (less than 50 mg/m2/dose), while eight patients had had no prior exposure to methotrexate. The response rates observed in these two groups of patients were similar. Except for two drug-related deaths, toxic effects were acceptable. Myelosuppression was mild, transient, and noncumulative. Gastrointestinal toxic effects did not appear to be dose-related and were mild in most instances. Central nervous system dysfunction with lethargy, fatigability, confusion, and disorientation was the most significant toxic effect of this high-dose methotrexate therapy and was observed in six (22%) of the patients. In two patients treatment with this program was discontinued because of the development of renal dysfunction. High-dose methotrexate with citrovorum factor rescue appears to be an effective regimen in patients with advanced refractory breast cancer. However, in view of the enormous cost necessitated by this treatment approach, we do not feel further studies would be worthwhile.

Traumatic esophageal injuries: 12-year experience at Henry Ford Hospital.

We reviewed our 12 years' experience and the literature on traumatic and nontraumatic esophageal injuries. A total of 28 patients were diagnosed and managed at Henry Ford Hospital. Cervical esophageal injuries should be explored surgically as soon as possible but, in our experience, a negative contrast swallow has not been reliable. In thoracic esophageal injuries, however, we rely more on contrast swallow to avoid routine exploratory thoracotomy and its morbidity. Abdominal esophageal injuries are readily identifiable during exploratory laparotomy performed for other abdominal injuries. Early diagnosis and treatment are the key to a successful outcome.

Bisantrene, an active new drug in the treatment of metastatic breast cancer.

Forty-four patients with metastatic breast cancer who had previously received extensive conventional systemic therapy, including combination chemotherapy with doxorubicin, were treated with Bisantrene, a new anthracene derivative. The dose schedule was 250 to 300 mg/sq m body surface administered as a 1- to 2-hr i.v. infusion. Of 40 evaluable patients, there were nine partial responses, and 18 patients had stable disease. Responses were seen in all major sites of organ involvement with a median time to progression of 28 weeks. Moreover, responses were seen among patients who had either failed to respond or had demonstrated refractoriness to prior therapy with doxorubicin, suggesting an apparent lack of cross-resistance between doxorubicin and Bisantrene. Except for myelosuppression and one incidence of acute anaphylactoid reaction, Bisantrene was generally well tolerated by most patients. We believe that Bisantrene may ultimately have a major role in the effective treatment of metastatic breast cancer, and further clinical trials are warranted.

Phase I clinical evaluation of 9,10-anthracenedicarboxyaldehyde[bis(4,5-dihydro-1H-imidazol-2-yl)hydrazone]dihydrochloride (bisantrene).

9,10-Anthracenedicarboxyaldehyde[bis(4,5-dihydro-1H-imidazol-2-y1)-hydrazone ]dihydrochloride (bisantrene) is one of a series of anthracenedicarboxyaldehyde compounds that has significant antitumor activity similar to that of doxorubicin in experimental mouse tumor systems. A phase I clinical study was conducted using a weekly x 3 schedule. Twenty-six patients with a wide variety of advanced malignant solid tumors received a total of 39 courses of the drug at doses ranging from 40 to 180 mg/m2/week x 3. Leukopenia was the dose-limiting toxic effect. Other toxic effects included malaise, which was observed in 49% of the courses, nausea and vomiting in 36%, fever in 21%, and phlebitis in 21%. No responses were observed in this study. For phase II studies, using this schedule, a starting dose of 150 mg/m2/week x 3 repeated every 4-5 weeks is recommended. In patients who have received minimal or no therapy, the starting dose should be 180 mg/m2.

CSF carcinoembryonic antigen in meningeal carcinomatosis from breast cancer.

Levels of CSF carcinoembryonic antigen (CEA) were determined in 23 patients with breast cancer and meningeal carcinomatosis. Levels greater than 1.5 ng/mL were observed in 16 patients; seven patients had undetectable levels. The meningeal disease of seven patients with elevated CSF CEA levels completely responded to treatment, and their CSF CEA levels concurrently decreased. Two patients whose meningeal disease responded poorly to treatment had persistently elevated levels of CSF CEA. In two other patients, a rising CSF CEA level was detected before there was any other evidence of meningeal relapse. Serial measurement of CSF CEA may play an important role in the clinical management of meningeal carcinomatosis in patients with breast cancer.

Spasmus nutans.

Spasmus nutans is classically described as a triad of nystagmus, head nodding and torticollis. It occurs in children, beginning in infancy and usually disappears in childhood. It is uncommon but its prompt recognition by ophthalmologists, paediatricians and neurologists can provide reassurance to the family of its benign nature. It is important to distinguish it from more serious conditions like gliomas which may present with signs mimicking spasmus nutans. We describe a case of a 3-year-old boy with acquired horizontal pendular nystagmus associated with head nodding and torticollis. The diagnosis of spasmus nutans, its implications and its management are discussed.

The long-term outcome of central serous chorioretinopathy.

OBJECTIVE: To assess the long-term outcome of central serous chorioretinopathy (CSR) among a group of patients who previously participated in a prospective argon laser photocoagulation study of CSR. DESIGN: Thirty-eight of 41 surviving patients with CSR participating in an earlier study were invited to participate in a follow-up study that included history taking, ophthalmoscopy, biomicroscopy, and fundus photography. RESULTS: Thirty-seven (38 eyes) of 38 surviving patients (97%) were available for follow-up between 11 and 15 years after participation in the earlier study. There were no clinically documented or historical recurrences of CSR among the six eyes previously treated by direct laser photocoagulation. There were 13 clinically documented recurrences and four historical recurrences among the 32 eyes not treated with direct laser photocoagulation. The difference in recurrences was statistically significant (P = .02). Pigment changes indistinguishable from age-related macular degeneration frequently occurred in eyes with CSR. The difference in the development of such pigment changes between eyes with CSR (33 of 38) and nonaffected fellow eyes (12 of 35) was significant (P = .001). CONCLUSIONS: The decreased rate of CSR recurrence after direct laser photocoagulation reported in an earlier study was sustained with follow-up beyond 10 years. Pigmentary changes in the fundus indistinguishable from those associated with age-related macular degeneration developed in eyes affected with CSR, probably as a consequence of the presence of subretinal fluid accompanying the CSR rather than from early age-related macular degeneration.

Lymphocytic infiltration in undifferentiated nasopharyngeal cancer.

BACKGROUND: Undifferentiated nasopharyngeal carcinoma (NPC) is characterized by prominent lymphocytic infiltration. Although the lymphoid infiltrate in NPC has been examined extensively in morphologic and immunocytochemical studies, the significance of this lymphoid infiltrate and its correlation with prognosis has been a subject of controversy for years. OBJECTIVE: To elucidate the significance of lymphoid infiltration in undifferentiated NPC. DESIGN: Evaluation of the relationship between lymphocytic infiltration in NPC and cervical lymph node status, ultrastructural examination of the lymphoid infiltrate, and assessment of lymphocytic infiltration as an independent prognosticator of regional node metastasis. MATERIALS AND METHODS: Lymphocytic infiltration was evaluated quantitatively in 20 cases of undifferentiated NPC using light microscopy. Four cases of undifferentiated NPC were processed for conventional electron microscopy. The effects of degree of lymphocytic infiltration, age, and tumor stage on cervical nodal metastasis were analysed using the logistic regression model. RESULTS: The degree of lymphoid infiltration correlated with cervical nodal metastasis (P<.001). Ultrastructural evidence of lymphocytes destroying cancer cells was seen. Lymphocytic infiltration was found to be an independent factor affecting cervical nodal metastasis (P =.02, univariate analysis; P =.03, multivariate analysis). CONCLUSIONS: The lymphoid infiltrate is beneficial in undifferentiated NPC, and its presence may deter regional metastasis of cancer cells to the cervical nodes.

Glutathione S-transferase pi expression in nasopharyngeal cancer.

BACKGROUND: Glutathione S-transferase pi (GST-pi) is an enzyme that catalyzes the conjugation of electrophilic substrates and prevents oxidative damage. Although GST-pi expression has been analyzed in many cancers, the significance of GST-pi expression in nasopharyngeal cancer (NPC), a tumor with a high treatment failure rate, is still unclear. OBJECTIVE: To elucidate the significance of GST-pi expression in NPC. DESIGN: Evaluation of GST-pi expression in NPC tissue specimens and determination of its relationship with tissue iron (a pro-oxidant) and clinicopathological factors in NPC. MATERIALS AND METHODS: Immunohistochemical expression of GST-pi was carried out in 55 NPC and 4 normal nasopharyngeal tissue sections. Eleven nasopharyngeal biopsy specimens (4 normal and 7 NPC) were analyzed for tissue iron levels. The expression of GST-pi in NPC was correlated with corresponding tissue iron levels. The relationships between GST-pi expression with sex, race, tumor stage, cervical nodal status, and clinical staging were also analyzed. RESULTS: Glutathione S-transferase pi immunoreactivity was observed in all NPC sections, with the percentage of immunopositive cells ranging from 1.0% to 72.0%. Tissue iron levels were significantly higher in the NPC tissues compared with normal tissues (P =.001). A direct correlation was observed between GST-pi expression and total and nuclear iron levels in NPC (P =.01 and P =.047, respectively). A significant association was also observed between GST-pi expression and cervical nodal disease (P =.007). CONCLUSIONS: Nasopharyngeal tumor cells may respond to pro-oxidant conditions by modulating intracellular antioxidant defense. Glutathione S-transferase pi expression appears to be associated with lymphogenous metastasis in NPC.

Improvement in lupus nephritis following treatment with a Chinese herbal preparation.

OBJECTIVE: To study the effect of a Chinese herbal decoction (CM), which contained 21 different herbs, on clinical remission in a patient with lupus nephritis and chronic nephrotic syndrome. DESIGN: Case report describing the clinical and laboratory markers of lupus activity in the patient before and after treatment with CM. We also studied the in vitro effect of CM and its hydrophobic extract on spontaneous IgG production by peripheral blood mononuclear cells (PBMCs) from 12 patients with systemic lupus erythematosus (SLE) compared with 9 healthy control subjects. RESULTS: Spontaneous PBMC IgG production was significantly higher in patients with SLE (mean +/- SD, 20.4+/-10.6 x 10(-5) g/L) compared with controls (4.7+/-1.9 x 10(-5) g/L) (P<.001). Addition of CM and its hydrophobic extract to PBMCs from patients with SLE resulted in significant suppression of spontaneous IgG production. CONCLUSIONS: The CM may contain some active pharmacological compound with immunosuppressive properties useful in the treatment of SLE. Further controlled studies are important to evaluate the efficacy of this medicine, potential toxic effects, and the possible immunosuppressive mechanisms of the active component(s).
